RESUMO

The aim of this study was to describe the clinical and immunological profile of patients infected with HIV after initiation of antiretroviral therapy. Sociodemographic characteristics, clinical and immunological patients were recorded. Chi square test and Mann-Whitney were used to compare variables. The multivariate regression model identified risk factors. So that, 936 (56.2%) patients were in stages III and IV of the WHO and 65.2% at an advanced stage of the disease. Factors associated with initiation at an advanced stage, were male sex (p = 0.007) and time to diagnosis (p = 0.005). In 2/3 cases, treatment is started at an advanced stage of disease. It is therefore important to intensify awareness campaigns for early detection and encourage patients to ensure regular medical follow-up screening.

In the framework of implementation of his national program for control and prevention of cardiovascular diseases, Cameroonian government has set up a cardiac surgery project. We report in this manuscript results of one year follow up of the patients operated during the pilot phase. From September 22 till 26, 2008, 11 patients have been operated in Cameroun. Surgical procedures were 5 mitral mechanic valve replacement, 2 aortic mechanic valve replacement, 1 atrial septal defect closure, 2 pace maker implantation. No intrahospital death was observed. One patient died at 11th month after the operation due to mitral valve thrombosis and attributed to lack of compliance. One patient presented low cardiac output, pneumonia and a pleural effusion. 2 patients presented 2 minor complications consisting of pericarditis and superficial wound infection. The results of the pilot phase of cardiac surgery in Cameroon are effective. However, the sustainability of the program require human, material capacity building, and funding mechanism as well.

RESUMO

Le but de ce travail etait de situer la place qu'occupe le prolapsus valvulaire mitral (PVM) idiopathique dans la pathologie cardio-vasculaire de la region. Nos objectifs etaient de savoir si les sujets qui etaient porteurs (24 en tout) presentaient des retentissements cliniques et electrocardiographiques qui les distinguaient des sujets normaux (30). Les signes fonctionnels cardiovasculaires; le souffle systolique mitral et le click systolique se retrouvent avec une plus grande frequence dans les cas de PVM idiopathique. Sur le plan electrocardiographique 10 des 24 porteurs de PVM presentaient des signes de HVG. Les indices de performance VG a l'echo sont meilleurs chez les temoins. Cependant le diagnostic meme du PVM ne fait pas encore l'unanimite; en particulier de nouvelles etudes restent indispensables a partir d'effectifs plus larges avec des criteres diagnostiques plus precis
