RESUMO

SLAP tears are a common cause of shoulder pain in overhead athletes. The benefits of in-office nano-arthroscopy include the ability to diagnosis and treat biceps tendinopathy, quicker patient recovery, reduced health care costs, and improved patient satisfaction. This technique can be particularly advantageous in the management of SLAP tears given that magnetic resonance imaging has poor sensitivity without the use of an invasive arthrogram. The purpose of this technical report is to describe our technique for performing in-office nano-arthroscopy for SLAP tears with special consideration of the technique for obtaining adequate local anesthesia, proper indications, and adequate visualization, as well as the advantages of performing these procedures in the office rather than the operating room.

Many polyprenylated acylphloroglucinols with fascinating chemical structures and intriguing biological activities have been identified as key to phytochemicals isolated from Garcinia, Hypericum, and related genera. In the present work, two chiral, tautomeric, highly-oxygenated polyprenylated acylphloroglucinols tethered with acyl and prenyl moieties on a bicyclo[3.3.1]nonanetrione core were isolated from the 95% ethanolic extract of Garcinia gummi-gutta fruit. The structures of both compounds were elucidated based on the NMR and MS data with ambiguity in the exact position of the enol and keto functions at C-1 and C-3 of the core structure. The structures of both polyprenylated acylphloroglucinols were established as a structurally revised guttiferone J and the new iso-guttiferone J with the aid of gauge-independent atomic orbital NMR calculations, CP3 probability analyses, specific rotation calculations, and electronic circular dichroism calculations in combination with the experimental data. The structures of both compounds resemble hyperforin, a potent activator of the human pregnane X receptor. As expected, both compounds showed strong pregnane X receptor activation at 10 µM [7.1-fold (guttiferone J) and 5.0-fold (iso-guttiferone J)], explained by a molecular docking study, necessitating further in-depth investigation to substantiate the herb-drug interaction potential of G. gummi-gutta upon co-administration with pharmaceutical drugs.

The significant impact of plastics on Earth's environments has transformed from being a symbol of modern innovation to a major ecological concern. This perspective paper explores the integration of plastics into geological contexts, emphasizing their role in contemporary sedimentary processes. It examines the lifecycle of plastics, from production to disposal, and their subsequent interaction with natural sedimentary cycles. The production and usage of plastics have led to considerable environmental repercussions. One of these, is their incorporation into geological systems and the formation of novel geological materials. Such a phenomenon challenges traditional geological concepts and necessitates a multidisciplinary approach encompassing geology, chemistry, and environmental science.

RESUMO

A 42-year-old male with no past medical history presented to an emergency room with increasing pain and swelling of his left lower extremity over 48 hours with no preceding trauma. A computed tomography scan demonstrated a hematoma (20 cm × 4 cm × 10 cm) present within the gastrocnemius-soleus complex. Acute compartment syndrome (ACS) was diagnosed clinically, confirmed intraoperatively with an arterial line transducer, and treated with emergent fasciotomy. Extensive workup found no evidence of coagulopathy or source of bleeding. This case presents a patient with ACS secondary to an atraumatic gastrocnemius hematoma discovered in the emergency room with no history of coagulopathies or anticoagulation.

Coastal environments, essential for about half of the world's population living near coastlines, face severe threats from human-induced activities such as intensified urbanization, aggressive development, and particularly, coastal sewage pollution. This type of pollution, comprising untreated sewage discharging nutrients, pathogens, heavy metals, microplastics, and organic compounds, significantly endangers these ecosystems. The issue of sewage in coastal areas is complex, influenced by factors like inadequate sewage systems, septic tanks, industrial and agricultural runoff, and natural processes like coastal erosion, further complicated by oceanic dynamics like tides and currents. A global statistic reveals that over 80 % of sewage enters the environment without treatment, contributing significantly to nitrogen pollution in coastal ecosystems. This pollution not only harms marine life and ecosystems through chemical contaminants and eutrophication, leading to hypoxic zones and biodiversity loss, but also affects human health through waterborne diseases and seafood contamination. Additionally, it has substantial economic repercussions, impacting tourism, recreation, and fisheries, and causing revenue and employment losses. Addressing this issue globally involves international agreements and national legislations, but their effectiveness is hindered by infrastructural disparities, particularly in developing countries. Thus, effective management requires a comprehensive approach including advanced treatment technologies, stringent regulations, regular monitoring, and international cooperation. The international scientific community plays a crucial role in fostering a collaborative and equitable response to this pressing environmental challenge.

Sachets and plastic sticks, single-use packaging primarily constructed from polyethylene terephthalate (PET), have proliferated globally for their convenience and multilayered construction that ensures product integrity. Especially prominent in emerging markets and amplified by pandemic-driven demand for hygiene products, these formats contribute significantly to fossil fuel industry revenue, aligning closely with petrochemical infrastructure developments such as fracking. While providing producers risk mitigation and cost-effective branding opportunities, these packaging types impose significant environmental tolls. The multimaterial layered composition of these materials hampers recycling efforts, and incineration releases toxins, exacerbating pollution. The plastics industry thus becomes an economic support for fossil fuel sectors facing declining oil demand. The growth of this sachet-stick economy represents a precarious balance between immediate economic benefits and long-term environmental ramifications. As global attention increasingly turns toward sustainability and pollution reduction, it becomes crucial to analyze the true environmental and socioeconomic costs of sachet and stick packaging.

The past decade has seen a rapid development in metal halide perovskite nanocrystals (NCs), which has been witnessed by their potential applications in nanotechnology. The inimitable chemical nature behind their unique photoluminescence characteristics has attracted a growing body of researchers. However, the low intrinsic stability and surface defects of perovskite NCs have hampered their widespread applications. Therefore, numerous techniques such as doping and encapsulation (polymer matrices, silica coating, salt matrix, etc.) have been examined for the surface modification of perovskite NCs and to increase their efficiency and stability. In this study, we demonstrated the self-passivation method for surface defects by introducing potassium (K) or rubidium (Rb) during the colloidal fabrication of NCs, resulting in the much-improved crystallinity, photoluminescence, and improved radiative efficiency. In addition, K-doped NCs showed a long-term colloidal stability of more than 1 month, which indicates the strong bonding between the NCs and the smaller-sized potassium cations (K+). We observed the enhancement of the radiative lifetime that can also be explained by the prevention of "Frenkel defects" when K+ stays at the interstitial site of the nanocrystal structure. Furthermore, our current findings signify the importance of surface modification techniques using alkali metal ions to reduce the surface traps of perovskite nanocrystals (PeNCs). Comparable developments could be applied to polycrystalline perovskite thin films to reduce the interface trap densities. The findings of this study have several important implications for future light-emitting applications.

Pregnane X receptor (PXR), extensively expressed in human tissues related to digestion and metabolism, is responsible for recognizing and detoxifying diverse xenobiotics encountered by humans. To comprehend the promiscuous nature of PXR and its ability to bind a variety of ligands, computational approaches, viz., quantitative structure-activity relationship (QSAR) models, aid in the rapid dereplication of potential toxicological agents and mitigate the number of animals used to establish a meaningful regulatory decision. Recent advancements in machine learning techniques accommodating larger datasets are expected to aid in developing effective predictive models for complex mixtures (viz., dietary supplements) before undertaking in-depth experiments. Five hundred structurally diverse PXR ligands were used to develop traditional two-dimensional (2D) QSAR, machine-learning-based 2D-QSAR, field-based three-dimensional (3D) QSAR, and machine-learning-based 3D-QSAR models to establish the utility of predictive machine learning methods. Additionally, the applicability domain of the agonists was established to ensure the generation of robust QSAR models. A prediction set of dietary PXR agonists was used to externally-validate generated QSAR models. QSAR data analysis revealed that machine-learning 3D-QSAR techniques were more accurate in predicting the activity of external terpenes with an external validation squared correlation coefficient (R2) of 0.70 versus an R2 of 0.52 in machine-learning 2D-QSAR. Additionally, a visual summary of the binding pocket of PXR was assembled from the field 3D-QSAR models. By developing multiple QSAR models in this study, a robust groundwork for assessing PXR agonism from various chemical backbones has been established in anticipation of the identification of potential causative agents in complex mixtures.

A series of polythiophenes with varying side chain density was synthesized, and their electrical and thermoelectric properties were investigated. Aligned and non-aligned thin films of the polymers were characterized in the neutral and chemically doped states. Optical and diffraction measurements revealed an overall lower order in the thin films with lower side chain density, also confirmed using polarized optical experiments on aligned thin films. However, upon doping the non-aligned films, a sixfold increase in electrical conductivity was observed for the polythiophene with the lowest side chain density compared to poly(3-hexylthiophene) (P3HT). We found that the improvement in conductivity was not due to a larger charge carrier density but an increase in charge carrier mobility after doping with 2,3,5,6-tetrafluoro-7,7,8,8-tetracyanoquinodimethane (F4TCNQ). On the other hand, doped aligned films did not show the same trend; lower side chain density instead led to a lower conductivity and Seebeck coefficient compared to those for P3HT. This was attributed to the poorer alignment of the polymer thin films with lower side chain density. The study demonstrates that optimizing side chain density is a synthetically simple and effective way to improve electrical conductivity in polythiophene films relevant to thermoelectric applications.

The lateral patellofemoral joint, composed of multiple soft-tissue structures, balances the knee by aiding patella tracking, stability, and force distribution. Arthroscopic lateral release is a well-described procedure that addresses patellofemoral knee pain and, in cases with patellar instability, may be combined with medial stabilization. In-office needle arthroscopy is an up-trending technique that simultaneously diagnoses and treats patellofemoral pathology in the office, leading to a quicker patient recovery, reduced cost, and improved patient satisfaction. The purpose of this Technical Note is to describe in-office needle arthroscopy technique to address patellofemoral pain and lateral patellar mal-tracking, with special consideration for achieving adequate local anesthesia, proper indications, adequate visualization, and the advantages of performing these procedures in the office rather than the operating room.

The Anthropocene, defined by human-induced environmental transformations, presents a critical challenge: plastic pollution. This complex problem, particularly prominent in coastal and marine environments, requires integrated and adaptive responses. This opinion paper examines global efforts across policy interventions, scientific innovations, and public education, highlighting both advancements and hurdles in managing this problem. These include enforcement limitations in policy implementation, scalability and cost issues in scientific innovations, and challenges in effecting large-scale behavioral change through public education. The complexities inherent in managing plastic litter in coastal and marine environments are further discussed, emphasizing the necessity for an integrated approach. This approach involves interdisciplinary collaboration, adaptive management, stakeholder engagement, policy integration, sustainable financing, resilience building, capacity enhancement, technological innovation, policy reform, ecosystem-based management, disaster risk reduction, and advocacy. The management of plastic pollution in the Anthropocene requires strategic planning, innovative thinking, and unified global efforts, ultimately providing an opportunity to redefine our relationship with the planet and steer toward a more sustainable future.

The Anthropocene, the most recent geologic time division, marks humanity's profound impact on Earth. Amidst debates, the Anthropocene Working Group recommended its inclusion in the International Chronostratigraphic Chart (ICC). This period is characterized by the mid-20th century Great Acceleration Event Array (GAEA), which includes widespread presence of pollutants such as radionuclides, organochlorine pesticides, PCBs, and plastic production. The Anthropocene concept should raise public awareness of these threats, with plastic pollution being a primary concern. Plastics are now pervasive and serve as a marker for the Anthropocene Epoch or Age. Understanding their entry into the geological record requires considering the "Plastic Geological Cycle," which encompasses extraction, production, use, disposal, degradation, fragmentation, accumulation, and lithification. This cycle reveals the transformation of plastics into new forms of pollution characteristic of the Anthropocene. With 91 % of discarded plastics never recycled, they accumulate in the environment and enter the geological record through processes like photodegradation, thermal stress, and biodegradation. The proposed Plasticene stage within the Anthropocene is defined by the post-World War II surge in plastic production and their incorporation into sedimentary processes and rocks. The study of plastics in the geologic record serves as a warning of their negative impacts and highlights the urgency of addressing plastic pollution for a sustainable future.

The findings emphasize the need for effective monitoring, management, and interdisciplinary research to address the multifaceted challenges posed by sediment pollution. As the global population grows and human activities expand, it is essential to prioritize sustainable practices and policies to minimize anthropogenic impacts on coastal and marine ecosystems. By advancing collective knowledge and sharing best practices, we can work towards ensuring a healthier and more resilient future for these crucial ecosystems and the lives they support.
