RESUMO

The paper covers clinical, morphological and functional aspects of gastrointestinal tract condition in patients with psoriasis accompanied by chronic opisthorchosis (CO). The authors examined 150 patients with psoriasis accompanied by CO, 100 patients having psoriasis without helminthiasis, 100 patients with CO and 30 healthy people. The gastric secretion was evaluated by means of the fractional test (both phases) with histamine stimulation; other diagnostic procedures included carbohydrate absorption evaluation (5-gram D-Xylose absorption test) and Kamer test of fat absorption. The morphological condition of the gastric and intestinal mucosa was investigated by means of light and electron microscopy. The study revealed gastric secretory dysfunction and malabsorption in small and large intestines in patients with psoriasis and CO, clarified the relation between the duration of psoriasis and opisthorchosis and gastric secretory dysfunction and determined dependence of small intestine malabsorption on such factors as stage, severity, degree of skin involvement and duration of psoriasis. The authors also established interrelation between the above malfunctions and gastric and intestinal structural abnormalities.

RESUMO

The authors present results of manifold evaluation of stomach in 109 patients with vibration disease caused by various vibration characteristics. The evaluation covered clinical signs, laboratory studies, endoscopic and pathomorphologic methods with light and electronic microscopy, in vitro radioautography and morphostereologic analysis. Features of gastric disease in vibration disease are asymptomatic course, depressed gastric secretion, prevalent atrophic changes of mucosa on endoscopy. Morphologic analysis of gastric biopsies revealed marked dystrophy and atrophy of the lining, glandular atrophy, diffuse stromal sclerosis and in most cases no signs of gastritis. The findings are termed vibration gastropathy.

RESUMO

A clinical-morphological study of bronchobiopsies from 78 patients with endoscopic picture of atrophic endobronchitis was carried out. Two forms of morphogenesis of chronic atrophic process in the bronchial mucosa are distinguished: primary dystrophic and primary inflammatory. Primary dystrophic bronchopathy is a clinical manifestations of the first form which presents structurally and functionally as primary dystrophy and atrophy of the bronchial mucosa in combination with sclerosis of the lamina propria in a sharp decrease of synthetic processes in bronchial epitheliocytes as indicated by the data of autoradiography. The primary inflammatory form of atrophic process has clinical symptoms typical for inflammatory process and is characterized by gradual reorganization in the bronchial wall and by maintaining sufficiently high level of metabolic reactions in bronchial epitheliocytes.

RESUMO

Qualitative and quantitative morphologic analysis of myocardium of Wistar rats were carried out when they have been transferred to high latitudes (being in latitude 69 degrees north during 37 days). It was shown, that already in a day after transferring to high latitudes pronounced disorders of lymph and blood circulations developed and heterogeneity of cardiomyocyte lesions was noted. By the end of the experiment atrophic and necrobiotic cardiomyocytes resorbed by macrophages were registered. In the group of animals, whose were transported in middle latitudes morphologic changes in myocardium gradually increased by the end of the experiment. Stereologic analysis revealed stereotype dynamics of tissue reorganization of myocardium in both animal groups. A key event in tissue reorganization of myocardium was the significant decrease of volume and surface densities of capillaries at 12th day of experiment and as consequence of these changes the volume and surface-to-volume ratios of capillaries to cardiomyocytes were decreased. The decrease of these parameters was more pronounced in animals whose were transported to high latitudes.

Combined morphological study of 714 bronchial biopsies and 363 samples of bronchoalveolar lavage (BAL) from 560 patients with chronic inflammatory diseases of nonspecific and tuberculous etiology was carried out. By light microscopic, electron microscopic and radioautographic investigation it was shown, that more important criteria of the dynamics of pathologic process, the stages of compensatory-adaptive reorganization of the lungs and the prognosis of disease were the structural-metabolic transformation of surface epithelium (metaplasia to stratified squamous epithelium or its atrophy) and the quantitative-qualitative characteristics of alveolar macrophages. A correlation was revealed between metabolic activity of BAL cells and those of bronchial epithelial cells, which determined the structural-functional interaction between the airways and respiratory part of the lungs under chronic inflammatory processes and promoted the right interpretation of clinical and morphological data. Morphological changes mainly of degenerative, but noninflammatory genesis often observed in the last years during chronic inflammatory processes in the lungs, are considered in terms of regenerative-plastic insufficiency of cells and intracellular structures.

Bronchial biopsies and bronchoalveolar lavage (BAL) in 226 patients with chronic inflammatory pulmonary diseases of different genesis were studied using light and electron microscopies and radioautography. 76 patients were treated with endobronchial laser therapy. Two types of structural-functional reorganization of epithelium in large bronchi were established: atrophy and squamous metaplasia both accompanied by the decrease of metabolic activity of epitheliocytes, but differing in DNA synthesis level. The most essential index of the lung respiratory compartment state was revealed according to BAL data, i.e. quantitative and qualitative characteristics of alveolar macrophages. Correlation between metabolic activity of BAL cells and bronchial epithelial cells characterizing structural-function interaction of air-way and respiratory compartments of lungs in chronic inflammatory processes was found. After endobronchial laser therapy the processes was found. After endobronchial laser therapy the regeneratory processes in lungs were stimulated, their important property was structural-functional value of regenerating tissue.

By methods of immunohistochemistry, light and electron microscopies gastrobiopsies from regions of acute and chronic erosions and border zones of gastric mucosa were studied in 36 patients with erosive gastritis. There was determined the decrease of amount of IgA-producing cells against a background of the increase of IgG-producing plasmocytes in the zone of mucosal defect. IgG-precipitations and fixation of heterogeneous complement in fibrinoid necrosis of erosive defect were revealed. Mucosal microcirculatory disturbances stipulated by immunocomplex injuries are found to be mosaic. There is given proof of the role of second local immunity disturbances in origin and chronization of erosive defects of gastric mucosa.
