RESUMO

In the article, on the example of two demonstrative clinical cases, the case histories of patients are described, some phenomenon of which at first glance may be perceived as symptoms of a dental and maxillofacial surgical pathology, but in fact are phenomenon of a mental disorder. In the most severe cases, patients, in order to alleviate their condition, stubbornly insist and achieve from the surgeons multiple operative interventions, which should be considered as the formation of a special type of addiction - pathological sectiomania. Summarizing these observations, the authors come to the conclusion about the clinical expediency of isolating this type of addiction and justify the need to refer these patients for a consultation with a psychiatrist.

RESUMO

OBJECTIVE: to identify and give an objective assessment of all changes in the periorbital region in paralysis orbicularis oculi muscle. METHODS: With physical, laboratory and instrumental methods of research, we analyzed the survey of 33 patients with paralytic lagophthalmos. This survey has allowed us to identify the symptoms of periorbital changes characteristic of paralysis orbicularis oculi involved in lagophthalmos development. RESULTS: After analyzing the results of a survey of 33 patients with paralytic lagophthalmos, we identified 15 symptoms of changes in the periorbital region. CONCLUSION: In our opinion, lagophthalmos should be considered as a syndrome, including certain symptom in a particular case. For adequate closure of the optic fissure and obtaining a good aesthetic result is necessary to simultaneously remove more of the symptoms.

RESUMO

Objective of the study was to determine representative cephalometric signs for the patients with TMJ anterior disk displacement without reduction (ADWoR). Twenty one female patients with ADWoR, which was confirmed by MRI, were included in the study. Cephalometric parameters described by Arnett-McLaughlin and Jarabak were measured by means of lateral cephalograms and computed tomograms (CT) to determine the growth pattern of the facial skeleton. All the data obtained was evaluated statistically. The investigation has revealed reduction in vertical measurements of facial skeleton and several signs of hyperdivergent growth pattern in the sample.

RESUMO

The aim of the study was to evaluate the effectiveness of mandibular condyle fractures fixation via intraoral approach with endoscopic assistance. Thirty-nine patients with low mandibular condyle fractures with lateral (34 patients) or medial (5 patients) displacement were included in the study. The paper contains detailed description of operation technique considering displacement type. This minimally invasive procedure resulted in stable fixation of the fracture so that maxillomandibular splinting was limited to 9-14 days after operation. The patients were discharged 4 to 7 days postoperatively. The study revealed no cases of facial palsies and all patients were highly satisfied with the absence of visible scars.

RESUMO

OBJECTIVE: To evaluate an efficiency of TMJ open disc repositioning surgery with use of bone anchors. MATERIAL AND METHODS: TMJ open disc repositioning surgery was indicated for patients with TMJ anterior disc displacement without reduction and skeletal class II or asymmetric forms of dysgnathia related to condyle resorption. Surgery was performed on 9 patients divided into two groups: the first one (5 patients) consisted of patients that were seeking help for bite problems, the second one (4 patients) consisted of patients with signs of TMJD, such as decreased mouth opening and pain in TMJ area. RESULTS: The first group patients underwent open disc repositioning and orthognathic surgery with counterclockwise rotation of occlusal plane. All the patients showed stability in skeletal and occlusal elements at 6 months follow-up period. The second group patients underwent disc-repositioning surgery only. In all the cases pain was completely eliminated but there was no sufficient improvement in MIO at 6 months follow-up period. Open TMJ disc repositioning surgery can be used for prevention of relapse after orthognathic surgery in patients with TMJ internal derangements such as anterior disc displacement without reduction.

RESUMO

The article describes the method of surgical treatment of patients with huge tumors of pharyngeal processus of parotid gland saving ramus of n. facialis. The method allows avoiding mandibular osteotomy without relapse risk it supposes the total excision of the gland.

RESUMO

The incidence of plexiform neurofibromas (PN) is 1:2500-3000, the disease affects maxilla-facial area in 31% of cases. The current paper describes results of treatment of 23 patients as well as preliminary data on using laser coagulation as treatment option for PN, namely on preoperative stage of treatment.

RESUMO

In spite of the complications after expander use the advantages of local plastics predetermine constant return to the method. Systemic use of hypoxic training produces stimulating effect on the skin microcirculation and can be used widely in clinical surgical practice in cases of local plastic operations to prevent possible complications reducing consequences of blood supply disturbance and tissue oxygen provision. The method of hypoxic adaptation let us receive as non-medicamentous mean to prepare for shorter period of time more vital flap.

RESUMO

Deformations after mandibular resection can be eliminated by single stage defects replacement by reconstructive materials of different origin. Prior to microsurgical methods of jaws reconstruction the patient rehabilitation with defects of the jaws had very limited success due to a number of drawbacks of traditional techniques (transplant tendency to resorption and seizure, complexity in surrounding tissues recovery) that makes impossible the use of the dental implantation techniques. In case of interdisciplinary planning of the treatment dental implants can be successfully placed in reconstructed mandible providing the conditions for aesthetic and functional rehabilitation of patients. The article described a clinical case of carrying out microsurgical mandible reconstruction with consequent placing of 7 dental implants and rational prosthetic appliance making.

RESUMO

Methods of reconstructive maxillofacial operations bringing about the best functional and esthetic results are analyzed. Surgical methods for repair of maxillofacial bone defects and deformations with the use of transplants and different methods of their fixation depending on the tissues at the site of defect (deformation) in patients with acquired and congenital abnormalities of the facial skull have been developed and are used at Central Institute of Dentistry. Osteosynthesis using titanium constructions and compression-distraction devices is acknowledged as an effective method of bone repair operations on the face. It notably reduces the duration of operation and prevents the relapse of the deformation. Analysis was carried out on extensive clinical data (more than 1000 patients), which allowed an objective evaluation of remote results of plastic repair of bones and helped define the indications and contraindications for such operations.

RESUMO

Obstructive respiratory disorders during sleep present an important medical and social problem. Serious dysfunctions of cardiovascular, nervous, endocrine and other vital systems of the body reduce longevity and life quality. On the other hand, load nocturnal snore and abnormal diurnal sleepiness cause great damage to family life, reduce working capacity and induce accidents. X-ray visualization of the upper airways is essential in diagnosing obstructive upper airway states and selecting patients for surgical treatment. The paper presents the authors' own experience in using various X-ray diagnostic methods in patients with chronic snore and obstructive sleep apnea-hypopnea syndrome.

RESUMO

Late results of repair surgery are analyzed in 40 patients with segmented mandibular defects of different origin. Titanium constructions were used for repair of defects in 21 patients and in 19 others combined endoprostheses were used, consisting of the crest of the patient's iliac bone and a titanium carcass. Check-ups 1.5-2 years after repair showed a higher reliability of combined endoprostheses.

Combinations of arterialized flaps with other grafts have to be used for repair of extensive perforating defects caused by removal of local disseminated malignant tumors of the head and neck. A total of 635 plastic surgeries were carried out during the latest 15 years at Department for Tumors of the Upper Respiratory Tract, Cancer Research Center, Russian Academy of Medical Sciences. Of these, combined plasty was carried out in 79 cases; in 28 patients the tumor was localized in the median area of the face, in 37 in the lower part, and in 14 on the neck. Primary combined plasty was carried out in 41 patients, delayed repair in 38. Musculocutaneous flaps were used in 42 cases, fatty-cutaneous ones in 37. A working schematic classification of variants of primary and delayed combined plasty in patients operated on for tumors of the head and neck has been developed.

Variants of nerve neurotization are described. Indications for their use are defined and potentialities of nerve neurotization assessed. Clinical results demonstrate its efficacy both in cases with facial paralysis and with partial ptosis of the upper eyelids or paralysis of the tongue.
