OBJECTIVE: To describe the clinical trajectories of patients discharged directly from a critical unit to a postacute care facility. METHODS: This was a retrospective cohort study of patients who were transferred from an intensive care unit or intermediate care unit to a postacute care facility between July 2017 and April 2023. Functional status was measured by the Functional Independence Measure score. RESULTS: A total of 847 patients were included in the study, and the mean age was 71 years. A total of 692 (82%) patients were admitted for rehabilitation, while 155 (18%) were admitted for palliative care. The mean length of stay in the postacute care facility was 36 days; 389 (45.9%) patients were discharged home, 173 (20.4%) were transferred to an acute hospital, and 285 (33.6%) died during hospitalization, of whom 263 (92%) had a do-not-resuscitate order. Of the patients admitted for rehabilitation purposes, 61 (9.4%) had a worsened functional status, 179 (27.6%) had no change in functional status, and 469 (63%) had an improved functional status during hospitalization. Moreover, 234 (33.8%) patients modified their care goals to palliative care, most of whom were in the group that did not improve functional status. Patients whose functional status improved during hospitalization were younger, had fewer comorbidities, had fewer previous hospitalizations, had lower rates of enteral feeding and tracheostomy, had higher Functional Independence Measure scores at admission to the postacute care facility and were more likely to be discharged home with less complex health care assistance. CONCLUSION: Postacute care facilities may play a role in the care of patients after discharge from intensive care units, both for those receiving rehabilitation and palliative care, especially for those with more severe illnesses who may not be discharged directly home.

Objective: The objective of this study is to estimate the prevalence of chronic Chagas disease (CCD) in Brazil: in the general population, in women, and in women of childbearing age. Methods: A meta-analysis of the literature was conducted to extract data on the prevalence of CCD in municipalities in Brazil in the 2010-2022 period: in the general population, in women, and in women of childbearing age. Municipal-level CCD indicators available in health information systems were selected. Statistical modeling of the data extracted from the meta-analysis (based on data obtained from information systems) was applied to linear, generalized linear, and additive models. Results: The five most appropriate models were selected from a total of 549 models tested to obtain a consensus model (adjusted R2 = 54%). The most important predictor was self-reported CCD in the primary health care information system. Zero prevalence was estimated in 1 792 (32%) of Brazil's 5 570 municipalities; in the remaining 3 778 municipalities, average prevalence of the disease was estimated at 3.25% (± 2.9%). The number of carriers of CCD was estimated for the Brazilian population (~3.7 million), for women (~2.1 million) and for women of childbearing age (~590 000). The disease reproduction rate was calculated at 1.0336. All estimates refer to the 2015-2016 period. Conclusions: The estimated prevalence of CCD, especially among women of childbearing age, highlights the challenge of vertical transmission in Brazilian municipalities. Mathematical projections suggest that these estimates should be included in the national program for the elimination of vertical transmission of Chagas disease.

Foram selecionados os cinco modelos mais adequados de um total de 549 modelos testados para obtenção de um modelo de consenso (R2 ajustado = 54%). O preditor mais importante foi o cadastro autorreferido de DCC do sistema de informação da Atenção Primária à Saúde. Dos 5 570 munícipios brasi- leiros, a prevalência foi estimada como zero em 1 792 (32%); nos 3 778 municípios restantes, a prevalência média da doença foi estimada em 3,25% (± 2,9%). O número de portadores de DCC foi estimado na popu- lação brasileira (~3,7 milhões), mulheres (~2,1 milhões) e mulheres em idade fértil (~590 mil). A taxa de reprodução da doença foi calculada em 1,0336. Todas as estimativas se referem ao intervalo 2015–2016. Conclusões. As prevalências estimadas de DCC, especialmente entre mulheres em idade fértil, evidenciam o desafio da transmissão vertical em municípios brasileiros. Tinnitus is a conscious attended awareness perception of sourceless sound. Widespread theoretical and evidence-based neurofunctional and psychological models have tried to explain tinnitus-related distress considering the influence of psychological and cognitive factors. However, tinnitus models seem to be less focused on causality, thereby easily misleading interpretations. Also, they may be incapable of individualization. This study proposes a Conceptual Cognitive Framework (CCF) providing insight into cognitive mechanisms involved in the predisposition, precipitation, and perpetuation of tinnitus and consequent cognitive-emotional disturbances. The current CCF for tinnitus relies on evaluative conditional learning and appraisal, generating negative valence (emotional value) and arousal (cognitive value) to annoyance, distress, and distorted perception. The suggested methodology is well-defined, reproducible, and accessible, which can help foster future high-quality clinical databases. Perceived tinnitus through the perpetual-learning process can always lead to annoyance, but only in the clinical stage directly cause annoyance. In the clinical stage, tinnitus perception can lead indirectly to distress only with experiencing annoyance either with (" I n d - 1 C " = 1.87; 95% CI 1.18-2.72)["1st indirect path in the Clinical stage model": Tinnitus Loudness â Attention Bias â Cognitive-Emotional Value â Annoyance â Clinical Distress]or without (" I n d - 2 C "= 2.03; 95% CI 1.02-3.32)[ "2nd indirect path in the Clinical stage model": Tinnitus Loudness â Annoyance â Clinical Distress] the perpetual-learning process. Further real-life testing of the CCF is expected to express a meticulous, decision-supporting platform for cognitive rehabilitation and clinical interventions. Furthermore, the suggested methodology offers a reliable platform for CCF development in other cognitive impairments and supports the causal clinical data models. It may also enhance our knowledge of psychological disorders and complicated comorbidities by supporting the design of different rehabilitation interventions and comprehensive frameworks in line with the "preventive medicine" policy.

ABSTRACT Objective The present study aimed to investigate the association of quadriceps muscle thickness, measured by ultrasound, with the nutritional status of critically ill patients in a referral high-complexity trauma care hospital. Methods A cross-sectional observational study was conducted in the intensive care units in a tertiary hospital in Brazil. The sample comprised 30 critically ill trauma patients admitted between February and March 2022. The methodology involved evaluating muscle mass and comparing nutritional status through mid-upper arm circumference measurements and ultrasound assessments. Specifically, the quadriceps muscle thickness was quantified using ultrasound at a predefined site between the iliac crest and the proximal border of the patella. Results: The Kruskal-Wallis test indicated variability in quadriceps muscle thickness between the nutritional status groups, with statistical significance reached after excluding the overweight group (H(2) = 7.532, p=0.023). The moderate malnutrition group exhibited notably lower quadriceps muscle thickness. A positive correlation was found between quadriceps muscle thickness and mid-upper arm circumference adequacy (p<0.05), demonstrating fair to moderately strong correlation (rs=0.531). Conclusion Significant changes in quadriceps muscle thickness were detected by ultrasound assessment in moderate malnutrition patients compared to patients of other nutritional statuses. Ultrasound may be a valuable technique for monitoring muscle integrity in critically ill patients.
RESUMO Objetivo O presente estudo teve como objetivo investigar a associação da espessura do músculo quadríceps, medida por ultrassom, com o estado nutricional de pacientes críticos em um hospital de referência de alta complexidade para o cuidado de traumas. Métodos Um estudo observacional transversal foi conduzido nas unidades de terapia intensiva de um hospital terciário no Brasil. A amostra compreendeu 30 pacientes críticos de trauma admitidos entre fevereiro e março de 2022. A metodologia envolveu a avaliação da massa muscular e a comparação do estado nutricional por meio de medições da circunferência do braço e avaliações por ultrassom. Especificamente, a espessura do músculo quadríceps foi quantificada usando ultrassom em um local pré-definido entre a crista ilíaca e a borda proximal da patela. Resultados O teste de Kruskal-Wallis indicou variabilidade na espessura do músculo quadríceps entre os grupos de estado nutricional, com significância estatística alcançada após a exclusão do grupo com sobrepeso (H(2)=7.532, p=0,023). O grupo com desnutrição moderada exibiu um resultado menor em espessura do músculo quadríceps. Foi encontrada uma correlação positiva entre a espessura do músculo quadríceps e a adequação da circunferência do braço médio-superior (p<0,05), demonstrando uma correlação de razoável a moderadamente forte (rs=0,531). Conclusão Mudanças significativas na espessura do músculo quadríceps foram detectadas pela avaliação de ultrassom em pacientes com desnutrição moderada em comparação com pacientes de outros estados nutricionais. O ultrassom pode ser uma técnica valiosa para monitorar a integridade muscular em pacientes críticos.

RESUMO
A Residência Multiprofissional é uma importante estratégia de Educação Permanente (EPS) no Sistema Único de Saúde (SUS), sendo a primeira oportunidade para muitos recém-graduados de exercer sua profissão e adquirir experiência. O objetivo do presente estudo foi conhecer o perfil de profissionais egressos de um programa de Residência Multiprofissional em Cardiopneumologia no Ceará e sua inserção no mercado de trabalho. No estudo, transversal e quantitativo, coletaram-se dados socioeconômicos, sociodemográficos e sobre histórico no mercado de trabalho, entre junho e outubro de 2022, via formulário eletrônico enviado aos egressos. A grande maioria estava empregada, atuando no SUS como terceirizados, vinculados como celetistas ou cooperados, em áreas de atuação diversas, destacando o potencial de contribuição na saúde, na educação e na gestão pública. Estudos com profissionais egressos de outros programas hospitalares são necessários para melhor elucidar as potencialidades, contribuições e lacunas do processo formativo à inserção dos profissionais no mercado de trabalho.
Multiprofessional Residency is an important strategy for Continuing Education (PDE) in the Unified Health System (SUS), being the first opportunity for many recent graduates to exercise their profession and acquire experience. The aim of this study was to know the profile of professionals graduating from a Multiprofessional Residency in Cardiopneumology program in Ceará and their insertion in the labor market. In this cross-sectional and quantitative study, socioeconomic, sociodemographic and labor market history data were collected between June and October 2022, via an electronic form sent to the graduates. The vast majority were employed, working in the SUS as outsourced, contracted, or cooperative workers, in diverse areas, highlighting the potential to contribute to health, education, and public management. Studies with professionals graduating from other hospital programs are necessary to better elucidate the potentialities, contributions, and gaps of the formative process to the insertion of the professionals in the labor market.
La Residencia Multiprofesional es una importante estrategia de Educación Continuada (PDE) en el Sistema Único de Salud (SUS), siendo la primera oportunidad para muchos recién graduados de ejercer su profesión y adquirir experiencia. El objetivo de este estudio fue conocer el perfil de los profesionales egresados de un programa de Residencia Multiprofesional en Cardoneumología en Ceará y su inserción en el mercado de trabajo. En este estudio transversal y cuantitativo, se recogieron datos socioeconómicos, sociodemográficos y de historia laboral entre junio y octubre de 2022, a través de un formulario electrónico enviado a los graduados. La gran mayoría estaba empleada, trabajando en el SUS como tercerizados, contratados o cooperativistas, en diversas áreas, destacando el potencial de contribución a la salud, educación y gestión pública. Estudios con profesionales egresados de otros programas hospitalarios son necesarios para dilucidar mejor las potencialidades, contribuciones y lagunas del proceso formativo para la inserción de los profesionales en el mercado de trabajo.

The cryopreservation reduces ram sperm quality, decreasing the pregnancy rate of ewes inseminated with thawed sperm. Hence, we aimed to improve the post-thaw quality of ram sperm replacing egg yolk on Tris-Glucose extender with different concentrations of LDL (2 or 8%), associated with the addition of 10 mM non-enzymatic antioxidants (ascorbic acid, hydroxytoluene butylate, ascorbyl palmitate, and trehalose). Semen samples were collected from six rams, split into different treatments, and frozen. After thawing, kinematic (CASA), structural (propidium iodide and carboxyfluorescein diacetate) and functional (hypoosmotic test) sperm membrane integrity was assessed. Total motility, VCL, and LIN were also assessed in thawed samples during 3 h of incubation (38 °C). The results showed that hydroxytoluene butylate at 10 mM in Tris-Glucose extender with 8% LDL improved velocity parameters immediately post-thaw compared with Tris-Glucose egg yolk extender, as well as prevented the reduction of total motility and VCL after incubation. There was no benefit of adding ascorbic acid and trehalose. Moreover, for the first time, it was shown the motility impairment promoted by ascorbyl palmitate to ram sperm.

BACKGROUND: Low back pain is a very common symptom frequently characterized as a biopsychosocial problem. This study aims to investigate the effectiveness of education to keep the abdomen relaxed versus contracted during Pilates exercises in patients with primary chronic low back pain. METHODS: Two-group randomised controlled trial with allocation of parallel groups and intention-to-treat-analysis. This study will be conducted in Lavras, MG, Brazil. A total of 152 participants will be randomised into two groups that will be treated with Pilates exercises for 12 weeks (twice a week for 60 minutes). Recruitment began in May 2022. The control group will receive guidance on the specific activation of the center of strength (the powerhouse), while the experimental group will receive guidance to perform the exercises in a relaxed and smooth way. Primary outcomes will be pain intensity (Numeric Pain Rating Scale) and disability (Rolland-Morris Questionnaire) 12 weeks post randomisation. Secondary outcomes will be global improvement (Perception of Global Effect Scale) and specific functionality (Patient-specific Functional Scale). The outcomes will be analyzed using repeated-measure linear mixed models. The assessors were not considered blinded because the participants were not blinded, and outcomes were self-reported. DISCUSSION: The findings of this study will help in clinical decision-making concerning the need to demand abdominal contraction during the exercises, understanding if it's a fundamental component for the effectiveness of the Pilates method for this population. TRIAL REGISTRATION: This trial was prospectively registered in the Clinical Trials (NCT05336500) in April 2022.

To find effective silver nanoparticles (AgNPs) for control of phytopathogens, in this study, two strains of actinomycetes isolated from the soil of the Brazilian biome Caatinga (Caat5-35) and from mangrove sediment (Canv1-58) were utilized. The strains were identified by using the 16S rRNA gene sequencing as Streptomyces sp., related to Streptomyces mimosus species. The obtained AgNPs were coded as AgNPs 35 and AgNPs58 and characterized by size and morphology using dynamic light scattering, zeta potential, transmission electron microscopy, and Fourier transformed infrared (FTIR). The antifungal activity of the AgNPs35 and AgNPs58 was evaluated in vitro by the minimal inhibitory concentration (MIC) assay on the phytopathogens, Alternaria solani, Alternaria alternata, and Colletotrichum gloeosporioides. The phytotoxic effect was evaluated by the germination rate and seedling growth of rice (Oryza sativa). AgNPs35 and AgNPs58 showed surface plasmon resonance and average sizes of 30 and 60 nm, respectively. Both AgNPs presented spherical shape and the FTIR analysis confirmed the presence of functional groups such as free amines and hydroxyls of biomolecules bounded to the external layer of the nanoparticles. Both AgNPs inhibited the growth of the three phytopathogens tested, and A. alternate was the most sensible (MIC ≤ 4 µM). Moreover, the AgNPs35 and AgNPs58 did not induce phytotoxic effects on the germination and development of rice seedlings. In conclusion, these AgNPs are promising candidates to biocontrol of these phytopathogens without endangering rice plants.

The cryopreservation reduces ram sperm quality, decreasing the pregnancy rate of ewes inseminated with thawed sperm. Hence, we aimed to improve the post-thaw quality of ram sperm replacing egg yolk on Tris-Glucose extender with different concentrations of LDL (2 or 8%), associated with the addition of 10 mM non-enzymatic antioxidants (ascorbic acid, hydroxytoluene butylate, ascorbyl palmitate, and trehalose). Semen samples were collected from six rams, split into different treatments, and frozen. After thawing, kinematic (CASA), structural (propidium iodide and carboxyfluorescein diacetate) and functional (hypoosmotic test) sperm membrane integrity was assessed. Total motility, VCL, and LIN were also assessed in thawed samples during 3 h of incubation (38 °C). The results showed that hydroxytoluene butylate at 10 mM in Tris-Glucose extender with 8% LDL improved velocity parameters immediately post-thaw compared with Tris-Glucose egg yolk extender, as well as prevented the reduction of total motility and VCL after incubation. There was no benefit of adding ascorbic acid and trehalose. Moreover, for the first time, it was shown the motility impairment promoted by ascorbyl palmitate to ram sperm.(AU)

One case was allochthonous and the others autochthonous, showing a dissemination behavior from a focus in the eastern area of the capital. Study limitations The present study was based on data from only one hospital. Studies that include data from other hospitals and other regions must be carried out to obtain a complete picture of this disease. Conclusions As in other regions of the country, zoonotic sporotrichosis presents itself as an endemic disease with an increase in the number of cases. The findings highlight epidemiological characteristics of great importance so that public health policies can contain disease progression.

OBJECTIVE: Military personnel has a large prevalence of back pain, especially those involved in patrolling routines, as they wear heavy protective equipment. Patrolling includes long periods of sustaining the protective equipment in a sitting or in a motor vehicle (motorcycle or car). Thus, understanding spinal loading of military police officers after patrolling by car (CAR; n = 14), motorcycle (MOT; n = 14), and administrative (ADM; n = 14) routines is relevant to establish preventive strategies. METHODS: The torque of the trunk and working and anthropometric characteristics were assessed to explain spinal loading using stature variation measures. Precise stature measures were performed before and after a 6 h journey (LOSS) and 20 min after a resting posture (RECOV). The trunk extensor (PTE BM-1 ) and flexor (PTF BM-1 ) muscles' isometric peak torque were measured before the working journey. RESULTS: The LOSS was similar between CAR and MOT (4.8 and 5.8 mm, respectively) after 6 h of patrolling. The ADM presented the lowest LOSS (2.8 mm; P < .05). No changes in RECOV between groups were observed (P > .05). Vibration may explain the greater spinal loading involved in patrolling in comparison to the ADM. A GLM analysis revealed that BMI was the only explanatory factor for stature loss. No independent variables explained RECOV. The ability of the trunk muscles to produce force did not influence LOSS or RECOV. CONCLUSIONS: Military police officers involved in patrolling may require greater post-work periods and strategies designed to reduce the weight of the protective apparatus to dissipate spinal loading. The external load used in patrolling is a relevant spinal loading factor.

Silver nanoparticles (AgNP) have been extensively applied in different industrial areas, mainly due to their antibiotic properties. One of the environmental concerns with AgNP is its incorrect disposal, which might lead to severe environmental pollution. The interplay between AgNP and plants is receiving increasing attention. However, little is known regarding the phytotoxic effects of biogenic AgNP on terrestrial plants. This study aimed to compare the effects of a biogenic AgNP and AgNO3 in Sorghum bicolor seedlings. Seeds were germinated in increasing concentrations of a biogenic AgNP and AgNO3 (0, 10, 100, 500, and 1000 µM) in a growth chamber with controlled conditions. The establishment and development of the seedlings were evaluated for 15 days. Physiological and morpho-anatomical indicators of stress, enzymatic, and non-enzymatic antioxidants and photosynthetic yields were assessed. The results showed that both AgNP and AgNO3 disturbed germination and the establishment of sorghum seedlings. AgNO3 released more free Ag+ spontaneously compared to AgNP, promoting increased Ag+ toxicity. Furthermore, plants exposed to AgNP triggered more efficient protective mechanisms compared with plants exposed to AgNO3. Also, the topology and connectivity of the correlation-based networks were more impacted by the exposure of AgNO3 than AgNP. In conclusion, it is plausible to say that the biogenic AgNP is less toxic to sorghum than its matrix AgNO3.
