INTRODUCTION: Disparity in healthcare is an important and timely topic. One example of such disparity appears to be the disproportionate use of lighter skin tone illustrations in medical and surgical educational material.1 Visual representation of pathology in instructional material is one key element that informs decision making in surgical disease and could contribute to disparity in outcomes in underrepresented tonal groups. Our hypothesis is that visual examples (illustrations) of clinical pathology in major surgical texts are biased in that they are heavily weighted to Caucasian skin tones and therefore fail to accurately represent the racial distribution of patients in the United States population. METHODS: Images from 4 commonly used general surgery textbooks were screened independently by 2 reviewers from Maimonides Medical Center and SUNY (State University of New York) Downstate College of Medicine. Human photographic and cartoon images (where skin tone could be determined), with adequate skin shown, were included. These images were assigned a Fitzpatrick skin photo type (FP) score (1-6). The distribution of images among the 6 FP categories were compared to the expected distribution of images in the United States population, as described from a previous National Health and Nutrition Examination Survey (NHANES). Differences in distribution were compared using a chi-squared test, with p-value <0.05 considered as statistically significant. RESULTS: There were 556 total images deemed adequate for assessment from the 4 textbooks chosen. 169 from Sabiston, 61 from Mulholland and Greenfield, 177 from Schwartz, and 149 from ACS. About 539 of these images (96.9%) were depictions of patients with light skin tone (FP scores 1-3.) while less than 4% of images were of dark-skinned individuals (FP score between 4 and 6.) An FP score 1 accounted for most images, comprising 477 images (86.1%). There was a 1.8% analytical discrepancy noted between the textbook reviewers. The distribution on the general US population (NHANES) is: FP score 1: 1.6%, FP score 2: 33.1%, FP score 3: 47.8%, FP score 4: 4.9%, FP score 5: 3.6%, FP score 6: 9.0%. CONCLUSIONS: Screening of commonly used general surgery textbooks showed a significant lack of diversity in image-based skin tone representation when compared to the United States population at large. The overwhelming majority of images were of light skin tones. Improving diversity of imagery in educational material, such as basic textbooks, might help reduce observed disparities in outcomes among surgical patients in the future.

We present a patient with a 16 cm adrenocortical carcinoma that underwent a left adrenalectomy en bloc with resection of the involved segment of the left renal artery. A splenectomy and splenorenal bypass was performed to revascularize the left kidney. To our knowledge, this is the first instance in the literature of a splenorenal arterial bypass being reported for renal revascularization during an extirpative oncologic procedure. A 64-year-old male patient, with history significant for adrenocortical carcinoma, status post prior right adrenalectomy with partial right nephrectomy, presented for an elective left adrenalectomy. Preoperative work-up revealed an 11.4 × 13.2 × 16 cm left adrenal mass, most consistent with an adrenocortical carcinoma. At the time of surgery, the mass was found to be intimately adherent to the aorta at the takeoff of the left renal artery. Moreover, the left renal artery appeared to be coursing directly through the mass. The involved segment of the left renal artery was resected en bloc with the tumor. Because of concerns for a small and likely poorly functioning right renal remnant, a decision was made to attempt to salvage the left kidney. This was accomplished by performing a splenectomy and constructing a splenorenal bypass. Serial Duplex Doppler renal ultrasound studies were obtained over the first three postoperative days and demonstrated improved arterial waveforms. Serum creatinine reached a peak level of 3.76 mg/dL on postoperative day 3, and then began to slowly trend down to 3.37 mg/dL on the day of discharge (postoperative day7).

In a study of 57 actual or impending pathologic fractures of the humerus in 52 patients with inoperable cancer treated between 1972 and 1982, we retrospectively reviewed the charts for analysis and comparison of the functional result and pain relief afforded by the various treatments used. Function of the extremity and relief of pain were each graded as excellent, good, fair, or poor using a modification of Perez's rating system. Seven pathologic fractures were treated nonoperatively. These patients generally had only fair pain relief and a poor functional result. Forty-six pathologic fractures were treated with intramedullary fixation using a Rush rod (n = 16), a Küntscher rod (n = 29), or an Ender rod (n = 1); the Neer endoprosthesis was used in four patients. Thirty-one patients received radiation to the humerus. There were seven operative complications, the most common (n = 3) being prominence of an intramedullary rod at the insertion site which required a second minor procedure for advancement of the rod. From this series, we conclude that any patient who has a pathologic fracture or impending fracture of the humerus and a predicted survival of six weeks or more is likely to benefit from rigid internal fixation with an appropriately selected device, adjunctive use of methylmethacrylate, and postoperative local irradiation therapy as needed.

RESUMO

The use of computed tomography (CT) in the postoperative evaluation of 19 congenitally dislocated hips is presented so that appropriate indications for reduction can be determined and radiologic criteria can be defined for judging whether reduction is satisfactory. The radiologic technique is presented. CT is an accurate and cost-effective modality that is capable of demonstrating subtle changes in femoral head position.

Two siblings, one male and one female, were noted to have a distinct skeletal dysplasia. The clinical and radiographic features resemble those observed in Kniest dysplasia and Rolland-Desbuquois syndrome, but important differences were noted. Specifically, these two patients have microstomia, "pursed" lips, and ectopia lentis, and their radiographs reveal no coronal clefts. Chondro-osseous features also differ from those observed in either of the other disorders. Scattered dense patches consisting of collagen fibers 10 to 30 times broader than normal are seen scattered throughout the cartilage matrix; the "Swiss cheese" appearance characteristic of Kniest dysplasia is not observed. These patients appear to have a new skeletal dysplasia, most likely inherited in an autosomal recessive fashion.

The neurological complications of segmental sublaminar stabilization that have been reported by other authors led us to perform a cineradiographic study of the pathways in the spinal canal that were taken by wires as they were being removed. The single wires were removed by pulling on the wire while keeping the wire perpendicular to the lamina; by winding the wire on the wire-extractor, with the wire being kept as nearly parallel with the lamina as possible (the roll-up technique); or by pulling on the wire while keeping the wire parallel with the lamina. During removal, thirty-four single wires conformed to the lamina and forty-one single wires compressed the dura. The roll-up technique caused the most erratic pathways. Double wires, although they were removed together, assumed independent pathways unless a wire-extractor guide was used. These findings suggest that the removal of sublaminar wires may cause dural compression in the clinical situation.

Cyclosporin A not only has improved renal allograft survival but it also has caused a renewed interest in cardiac and hepatic transplantation. The South Texas Organ Bank was sensitive to the need for multiple organ donors and also concerned that the recovery of multiple organs could have a negative impact on the quantity or quality of kidneys recovered. From July 1983 through March 1985 cadaveric kidneys were obtained from 43 renal donors and multiple vital organs were obtained from 11 additional donors. There was no statistical difference between the renal and multiple organ donors in the incidence of renal contamination, post-transplant acute renal failure or renal discard rate (p greater than 0.3 in each comparison). No donor family rescinded permission for renal donation because other vital organs were requested. Urologists involved in cadaveric renal recovery are encouraged to view every cadaver donor as a potential multiple organ donor.

Two cases of aneurysmal bone cyst of the dorsal lumbar spine are presented with four- and seven-year follow-up periods. The lesion in each case involved the vertebral bodies and posterior elements and extended to contiguous bony structures. Both anterior and posterior staged procedures were required for maximal excision, rendering the spine extremely unstable at the involved levels. Anterior strut grafting with autogenous iliac bone was done during the first stage after the vertebral bodies had been curetted. Three weeks later the remaining dorsal elements and associated tumor were removed through a posterior approach, and the spine was stabilized with the Harrington compression-distraction system and posterior autogenous iliac crest grafts. Both patients were neurologically normal after operation, and the spines in both were stable and solidly fused. Both have remained symptom-free and have had no evidence of recurrence.

A case of fracture-separation and dislocation of the proximal humeral epiphysis, a rare injury, is reported. The patient, a 32-month-old girl suspected of being an abused child, was reported to have fallen from her crib 2 days earlier. She had a painful, swollen, slightly ecchymotic left shoulder, which she held in the neutral position and refused to move. Radiographs showed a healing, nondisplaced radial fracture and fracture-separation of the proximal humeral epiphysis, which was dislocated into an anterior subglenoid location. Surgery under general anesthesia showed the humeral head to lie in a subglenoid position and the distal fragment of the separated epiphysis to have penetrated the posterior lateral capsule. The reduction was stabilized with a single, percutaneous, smooth Kirschner wire and shoulder immobilized in a Velpeau cast for 3 weeks. At 2 year follow-up, the patient has painless, full range of motion, the epiphysis is open, and the length of the humerous appears normal.

Experience with the posterolateral approach to total hip replacement arthroplasty in 229 patients is reported. The technic has the advantages of providing excellent exposure and decreased operative time, minimal blood loss, and absence of the complications associated with removal and replacement of the greater trochanter. Capsulectomy may be done or not, as desired. Classification of results was based on absence of pain and increased range of motion. Of the 229 patients, 95% were classified as having a good to excellent result; 2%, a fair result; and 3%, a poor result. There was no statistically significant correlation between the postoperative formation of ectopic bone and whether or not capsulectomy was done.

A preliminary study of the effects of Surgical Simplex, mixed for various time periods, was performed on S. aureus growth rate, and phagocytosis and glucose metabolism of macrophage in vitro. Quantitatively, no significant effects of the mixture on S. aureus growth rate were observed. The cement mixed for less than 4 min tended to cause adverse effects on phagocytosis rate and hexose monophosphate shunt activity of macrophage. This information may not be directly applicable to in vivo, but in vitro the cement mixture in a certain situation can cause decreased phagocytosis and hexose monophosphate shunt activity of macrophage.

The clinical use of inert materials for internal fixation and joint replacement devices is increasing rapidly; studies on the effect of these materials on bacteria and host resistance to infection have not kept apace. Any substance placed within the body may interfere with host-parasite interaction, either by its physical presence or by physiochemical activity at its interface with the surrounding tissue. Either mechanism, by altering the normal host defense mechanism, may promote or retard bacterial growth. Bacterial growth may also be altered if a trace element essential to or inhibitory to the bacteria or an element that antagonizes or potentiates humoral antibacterial systems is leached from the substance. We have tested bacterial growth and inhibition in vitro in the presence of substances used as implant materials: surgical silver, iron, zinc-coated galvanized iron, aluminum alloy, stainless steel, Vitallium, and methyl methacrylate. Our results showed: 1) metals that have low grades of tissue reactivity and little bacteriotoxic effect provided a framework along which bacterial growth and propagation occurred; 2) metals that have higher levels of cellular reactivity, specifically aluminum and galvanized wire, caused selective bacterial toxicity. The in vitro response of bacteria to methyl methacrylate was similar to that of Vitallium and other inert substances, i.e., growth and propagation were abundant adjacent to methyl methacrylate. These studies led us to speculate that, if similar phenomena occur in vivo, bacterial growth and dissemination might be increased when an inert implant material was used and decreased when the implant material was more reactive.
