RESUMO

The purpose of this study is to analyze the literature on the role of optical coherence tomography (OCT) and optical coherence tomography angiography (OCTA) in the diagnosis of glaucoma. This review considers the structural and functional correlations observed during the progression of glaucomatous optic neuropathy, as well as the capabilities of the method in late glaucoma, describes the strengths and weaknesses of OCT and OCTA, and pays particular attention to the role of OCT in assessing the effectiveness of treatment. Optical coherence tomography is the main method for determining the progression of glaucoma, which plays a key role in the choice of treatment algorithm. However, the use of OCT in far advanced glaucoma has certain particularities and limitations. OCTA can be helpful in overcoming this problem.

RESUMO

This paper reviews the literature on the role of optical coherence tomography (OCT) and optical coherence tomography angiography (OCTA) in the diagnosis of glaucoma and considers the significance of evaluating retinal nerve fiber layer and ganglion cell complex in assessment of glaucoma progression, variability and reproducibility of the method, as well as the influence of age-related retinal changes on the results, analyzes the role of OCTA in glaucoma monitoring. Optical coherence tomography is a modern standard for glaucoma diagnosis and monitoring, and OCTA shows high potential as an auxiliary diagnostic tool.

The work is devoted to analysis of various methods of assessment and control of house dust mites population and mite allergens in city dwellings using own results and the literature data. Data about in-house dust mites biology and ecology, mechanisms of their migration and circulation in modern city conditions are presented. The comparison of several methods of mites number assessment and mite allergens exposure (classical acarological analysis, colorimetric method of guanine detection in house dust, immunochemical methods) has been performed and their advantages and disadvantages analyzed. As choice of adequate avoidance measures is one of the key question, various such measures (mechanical, physical and chemical) have been compared.

Twenty-seven mite species belonging to 23 genera, 13 families, and 2 orders were identified. The mites of the family Pyroglyphidae, amounting to 91.1% of the total number of mites, were found to dominate in the acarocomplex of Penza dwellings. Pyroglyphid mites were represented by 3 species with Dermatophagoides pteronyssinus and Dermatophagoides farinae absolutely prevailing in both their abundance (20.0 and 68.5%) and frequency (39 and 75%). The maximum number of D. pteronyssinus and D. farinae reached 2.867 and 3.400 mites per g of dust, respectively. Mites of the families Acaridae and Glycyphagidae were registered in 27 and 15% of the apartments, respectively; and their abundance was about 4%. G. destructor is most frequently encountered. The specific abundance and detection rate of other mites were not high. The influence of various socioeconomic factors on the number of D. pteronyssinus and D. farinae was analyzed using statistical methods. A significant correlation was found between the number of mites, the air moisture of a premise, the building material used for the construction of a house, the type of a housing estate (a private or tenement house) and heating (central or self-contained). No significant relationship was established between the number of mites, the ventilation of premises, the presence of domestic animals and plants or flowers, and the number of dwellers. Out of 81 patients with allergic diseases (allergic rhinitis (73%), bronchial asthma (4%), and allergic rhinitis concurrent with asthma (23%)), 44 and 25% patients had positive skin tests for D. pteronyssinus and allergens of D. farinae, respectively. There was no positive correlation between the number of pyroglyphid mites, positive skin tests, and the diagnosis.

RESUMO

In this work information on the biology and ecology of synanthropic mites and mold fungi, proved to be the sources of indoor allergens in different premises, is presented. The authors propose the algorithm of the ecological monitoring of dwellings, developed on the basis their investigations. The types of dwellings, the time and situations when ecological monitoring should be carried out are analyzed. Grounds for the necessity of controlling the number of mites and the concentration of Micromyces spores, as well as their elimination from the environment, are given.

The present study revealed that 73% of surveyed apartments in Moscow whose residents included children with the atopic form of bronchial asthma and sensitization to Dermatophagoides pteronyssinus allergens were infested with the pyroglyphid mites D. pteronyssinus and D. farinae. The number of mites in the surveyed apartments varied between 0 and 154 mites/g of dust for D. pteronyssinus and between 0 and 162 mites/g of dust for D. farinae. The levels of mite allergens in these apartments ranged from 0.5 to 165.8 micrograms/g for Der p I and from 0.3 to 91.3 micrograms/g of dust for Der f I. The Der p I allergen was found to predominate, and its concentration in one-third of the apartments was more than 10-fold greater than that of Der f I. Correlation between the number of pyroglyphid mites and the concentration of group I allergens was established for both D. pteronyssinus (r = 0.4932; P < 0.01) and D. farinae (r = 0.6748; P < 0.01). In most of the apartments, high and moderate levels of Der I allergens were detected.

The review contains an of the literature on sensitivity and the clinical manifestations of allergy in human being to various parasitic and free-living mites and ticks (Sarcoptidae, Psoroptidae, Demodicidae, Ixodidae, Argasidae, Gamasina, Trombiculidae, Cheyletidae, Pygmephoridae, Tarsonemidae, Tetranychidae). The epidemiology and clinic of the allergic reactions of man to the penetration of mites and tick allergens through skin at their constant, temporal and occasional parasitizing are considered. The allergic reactions to different mite and tick allergens take both delayed and immediate type. However, the mechanism of these reactions sometimes is unknown. The mites or ticks infestations produce, as a rule, dermal allergic reactions in the atopic patients. In some cases the asthmatic component may be present. The cross-reactivity between mite/tick antigens and pyroglyphid mite antigens complicates the clinic, diagnosis and treatment of allergic diseases. New data on the nature and specificity of mite and ticks allergens are presented.

RESUMO

Paper is the second part of the literature review on house dust mites (including 1985). It deals with the diagnosis of the dust mite allergy, the nature of the mite allergens, distribution and population density of the mites in various premises, seasonal population dynamics and population age structure of the dermatophagoid mites, life cycle and breeding of Dermatophagoides pteronyssinus and D. farinae on different food substrates. The methods of the house dust mite control are discussed. The original data on the house dust mite distribution in Moscow is shown. A comparative estimation of the results of mite antigens in the house dust discovering by the acarological and three immunological methods are given.

The acarological+ study of dust samples from the homes of allergic patients and normal persons in Moscow mite antigens were detected, respectively, in 66.7% and 38.1% of homes. The indirect mast cell degranulation test and the brain gliacyte volume change test in white rats gave similar results. The occurrence of Dermatophagoides pteronyssinus was approximately twice as great of D. farinae, and the numerical prevalence of the former species over the latter one was 1.3-fold. Dermatophagoides mites occurred in the homes of allergic patients 1.8-2.3 times as frequently as in the homes of normal persons.

RESUMO

The literary data on the study of the house-dust mites during the period from 1864 till 1983 are given. The methods of the house dust samples collection in flats and the methods of the house-dust mites isolation from the house dust are evaluated. The house-dust mites fauna including more than 130 species is described. The synantropic mites-pyroglyphides are the main part of the fauna, the stored products mites and predatory cheyletid and gamasid mites occur regularly. The rest part of the fauna consists of the different mites got into house by chance. The literary data on the influence of temperature and relative humidity on the development and growth of the mite population are given. The house-dust mites feeding and digestion processes bearing relation to the formation of the mite allergen are reviewed. The original data complete the literary data on the fauna and distribution of the house-dust mites in Moscow.
