RESUMO

Peculiarities of reactive hypertension after phacoemulsi- fication cataract with patients of pseudoexfoliation syndrome. Research was carried out on 87 designed for the surgical treatment of cataract patients (112.eyes) who were grouped according to the initial intraocular pressure (IOP), the degree of pseudoexfoliative syndrome (PES), the presence of concomitant vascular pdthology. IOP level was estimated in the first, the third day, and a month after catarditphacoemulsiication with intraocular lens implantation. It was found that the incidence of reactive hypertension in the group. ranged from 10,7 to 18,7% and correlated with the severity of the manifestations of the PES, baseline IOP and the presence of systemic cardiovascular disease. At the same time dominated by vascular factors. The results allow to identify patients with RPE included in the group of high risk of developing hypertension, reactive, and thus improve the quality -of their care and treatment.

RESUMO

The expression of immune response gene mRNA in the umbilical and venous blood were compared in newborns of the first day of life with and without signs of infection. The expression of il1b, il6, il8, il10, il12a, il15, il18, tnfa, tgfb1, tbx21, gata3, foxp3, rorc2, cd45, cd68, cd69, tlr2, tlr4, tlr9, and mmp8 mRNA was evaluated in umbilical and venous blood cells of newborns by reverse transcription real time PCR. In full-term newborns without signs of infection, the expression of il8, tlr2, tlr4, and mmp8 in venous blood was higher than in umbilical blood, while in preterm newborns, the levels of mmp8 transcript were elevated while the levels of tlr9, cd45, and gata3 were reduced. The expression of some markers differed in the umbilical and venous blood and in newborns with congenital infectious disease and without signs of infection.

RESUMO

Proteomic analysis of the urine was used for noninvasive diagnostics of abnormalities in newborns treated in the neonatal intensive care unit. This approach can be used to differentiate between infectious and noninfectious respiratory disorders.

RESUMO

A decision regarding adjuvant chemotherapy in early (operable) breast cancer in the past was made entirely on the basis of clinical and pathological features. However with the growing awareness of tumor biology and the possibility of the genomic analysis to determine the molecular subtypes of breast cancer it is getting real to identify patients whose tumors are resistant to chemotherapy or vice versa benefit from its addition. Despite the fact that genomic analysis allows some patients avoiding chemotherapy (especially patients with localized breast cancer), such studies do not indicate the most appropriate chemotherapy regimens. Therefore treatment decisions should be based on a combination of biological features of the tumor, its stage and signs that characterize the patient such as age and tolerance to the side effects of therapy.

UNLABELLED: It is common knowledge that arterial hypotension is a one of most prevalent pathology of early neonatal period and it causes severe neurological complications. Purpose of the prospective randomized open study was to assess the efficacy of HES 130/0.4 (6% Voluven) as a start medicine for arterial hypotension in early neonatal period in comparison with normal saline solution (0.9% NaCl). MATERIALS AND METHODS: the study was held from January 2010 to September 2011. Newborns of the ICU with arterial hypotension on the first day of life were included into the study. Acute haemorrhage was an exclusion criterion. Routine monitoring of arterial pressure, heart rate and diuresis was carried out in all newborns. pH, pCO2, pO2 blood glucose, lactate and BE levels were measured before and after the treatment. Echocardiography, examination of regional blood flow in anterior cerebral artery, renal artery and mesenteric artery was held HES 130/0.4 (6% Voluven) and saline solution (0.9% NaCl) were used Parameters of haemostasis, biochemical and haematological analysis monitored after the solutions use. Time of arterial pressure normalization and duration of the treatment positive effect were recorded Episodes of hypotension, amount of cardiotonics per day and per 7 days and duration of cardiotonics administration were recorded as well. RESULTS: 6% Voluven infusion both to 0.9% NaCl infusion normalizes the regional circulation. Furthermore it increases the blood pH and diuresis. Lactate level normalization occurred only after 6% Voluven infusion. There was not change of serum sodium level. Liver enzymes, C-protein, prothrombin index were same in both groups of patients. 6% Voluven use in newborns with extremely low birth weight was accompanied with increasing of creatinine level, prolongation of activated partial thromboplastin time and increasing of intraventricular hemorrhage rate. Newborns with weight over 1000 gram did not have these complications. Voluven advantage in comparison with saline solution is a decreasing of cardiotonics administration in patients with weight over 1000 gram. CONCLUSIONS: 6% Voluven is more effective than 0.9% NaCl for increasing of cardiac output, myocardial contractile ability and diuresis, normalization of liquid balance and lactate level, decreasing of cardiotonics amount. 6% Voluven can be recommended for arterial hypotension treatment in newborns with weight over 1000 gram both to normal saline solution especially in case of shock.

RESUMO

Recently, the rates of early stage breast cancers involving breast-conserving therapy have increased due to use of mammography screening. Morever, a patient's desire to keep the breast is now considered to be a factor which determines the surgery tactics. However, there has been so far no consensus on optimal extent of surgery capable of preventing local recurrence. In order to precisely estimate such risk it is essential to take into account factors such as tumor size, surgical margin status and age. Besides, correlations between certain subtypes of breast cancer and various associated risks of local recurrence are to be established. Another important factor is to devise the necessary subsequent radiotherapy.

RESUMO

L-thyroxine action on GPI and phosphatidylinositol (PI) metabolism in the liver have been investigated in 3- and 24-month-old Wistar rats. PI and GPI were labeled by [14C]acetate Na in vivo and [14C]glucose in vitro. Aging caused a significant decrease in basal PI and GPI levels and reduced [14C]glucose incorporation into GPI of liver. The addition of exogenous PI stimulated the [14C]GPI formation (about 2-3 fold) in 24-month-old rat liver. Thyroxine injection (200 micrograms/100 g weight) to young rats induced triphasic alteration in GPI content in the liver. We observed the marked violation in the thyroxine-mediated GPI-metabolism in the old rats liver. These results indicate that thyroid hormones regulate GPI metabolism in rat liver.

In 60 miners working in a deep coal mine the blood plasma level of endoteline-1 (E-1) was measured by the immunoenzyme technique immediately after working shift. Those in the mining where the working conditions are especially harsh were found to have the highest level of E-1 exceeding the control values. In studying the age-related content of E-1 in blood plasma of coal miners the highest levels of E-1 were recordable in workers 20 to 30 years old, declining and differing in age groups 20-30 and 41-50 years old. The level of E-1 was at its greatest in those workers with minimum length of service in the underground conditions, declining with the service more than 10 years in duration.

Evidence is presented for the chromosome localization of seven silver fox genes by the use of a panel of fox x Chinese hamster somatic cell hybrids. AK1, GOT2, and ALDOC are assigned to chromosome VFU2, PGP to chromosome VFU38, BLVR to chromosome VFU5, ACP1 to chromosome VFU8, and ITPA to chromosome VFU14. The genetic map of 29 fox genes is compared with those reported for man and other mammals. The results we obtained support and extend our previous suggestion that the formation of the Canidae branch of the Carnivora phylogenic tree was associated with a great increase in the rate of reorganization of the ancestral karyotype.
