RESUMO

The aim of the study was to define the spectrum of genetic risk factors of chronic pancreatitis (CP) development in patients living in the European part of the Russian Federation. Materials and Methods: The study group included 105 patients with CP, with the age of the disease onset under 40 years old (the average age of onset was 26.9 years). The control group consisted of 76 persons without clinical signs of pancreatitis. The diagnosis of chronic pancreatitis in patients was made on the basis of clinical manifestations and the results of laboratory and instrumental investigations. Genetic examination of patients was conducted using the next-generation sequencing (NGS) technology and included targeted sequencing of all exons and exon-intron boundaries of the PRSS1, SPINK1, CTRC, CFTR, and CPA1 genes. The genotyping of the rs61734659 locus of the PRSS2 gene was also conducted. Results: Genetic risk factors of the CP development were found in 61% of patients. Pathogenic and likely-pathogenic variants associated with the risk of CP development were identified in the following genes: CTRC (37.1% of patients), CFTR (18.1%), SPINK1 (8.6%), PRSS1 (8.6%), and CPA1 (6.7%). The frequent gene variants in Russian patients with CP were as follows: CTRC gene - c.180C>T (rs497078), c.760C>T (rs121909293), c.738_761del24 (rs746224507); cumulative odds ratio (OR) for all risk alleles was 1.848 (95% CI: 1.054-3.243); CFTR gene - c.3485G>T (rs1800120), c.1521_1523delCTT (p.Phe508del, rs113993960), and c.650A>G (rs121909046); OR=2.432 (95% CI: 1.066-5.553). In the SPINK1, PRSS1, and CPA1 genes, pathogenic variants were found only in the group of patients with CP. The frequent variants of the SPINK1 gene include c.101A>G (p.Asn34Ser, rs17107315) and c.194+2T>C (rs148954387); of the PRSS1 gene - c.86A>T (p.Asn29Ile, rs111033566); of the CPA1 gene - c.586-30C>T (rs782335525) and c.696+23_696+24delGG. The OR for the CP development for the c.180TT genotype (rs497078) CTRC according to the recessive model (TT vs. CT+CC) was 7.05 (95% CI: 0.86-263, p=0.011). In the CTRC gene, the variant c.493+49G>C (rs6679763) appeared to be benign, the c.493+51C>A (rs10803384) variant was frequently detected among both the diseased and healthy persons and did not demonstrate a protective effect. The protective factor c.571G>A (p.Gly191Arg, rs61734659) of the PRSS2 gene was detected only in the group of healthy individuals and confirmed its protective role. 12.4% of the patients with CP had risk factors in 2 or 3 genes. Conclusion: Sequencing of the coding regions of the PRSS1, SPINK1, CTRC, CFTR, and CPA1 genes allowed to identify genetic risk factors of the CP development in 61% of cases. Determining the genetic cause of CP helps to predict the disease course, perform preventive measures in the proband's relatives, and facilitate a personalized treatment of the patient in future.

The purpose of the work is to identify the most optimal ways to solve the problem of home parenteral nutrition for patients with short bowel syndrome.Basics. Discussion of the current situation will help optimize the work of the Orphan Diseases Center, lead to a greater introduction of online technology, and aim at the search and implementation of new methods of treating patients in this category.Conclusion. The most optimal strategy has been proposed to improve the interaction between various specialists and patients in need of long-term parenteral nutrition.

We compared the levels of functional activity of cells in each adrenal zone with blood levels of corticosterone, testosterone, and neuropeptide Y in control and hippocampectomized F1(C57BL/6×DBA/2) mice during modeling of metabolic, motivational, and cognitive tension. The morphofunctional state of the adrenal glands was studied using a new morphometric approach. It was found that hippocampectomy changed the testosterone response to neurobiological stimuli; similar changes were observed in the zona reticularis of the adrenal cortex producing dehydroepiandrosterone that is involved in the regulation of testosterone secretion. At the same time, hippocampectomy enhanced the response of the peptide hormone; the index of functional activity of chromaffin cells producing this hormone also increased. These findings allow us to put forward a hypothesis that the hippocampus is involved in the regulation of mutual influences of the studied hormones and that it modulates the sensitivity of testosterone and NPY to metabolic and cognitive factors.

The serum hormone concentrations were studied in a group of male F1 (C57BL/6×DBA/2) mice in different states of food activity (satiety, 24-h food deprivation, and cognitive load against the background of food deprivation). The hormonal response depended on food activity: the content of leptin, triiodothyronine, and testosterone decreased in hungry animals, while during cognitive load (learning), we observed a decrease in the concentrations of ghrelin, leptin, thyroxine, and testosterone. The exceptions were neuropeptide Y (its concentration increased in hungry animals) and corticosterone (its level remained unchanged). The use of hierarchical cluster analysis allowed identifying functional organization of the relationships within the hormonal ensemble that underwent plastic changes depending on the state of the organism. It was shown that the hormonal ensemble was system-organized in the form of a "core" that determines stability of the system and the "field", within which functional interactions of the hormones are preserved.

RESUMO

The psychophysiological effects of aging in mice of the line F1 (C57BL/6×DBA/2) at the age of 3, 7, and 14 months were investigated. It was found that the cognitive abilities, memory and spatial orientation in all animals did not depend on age, they were able to form a 4-link food-getting habit. The instability of behavior found in all mice at the stereotyped stage of in the form of periodic reproduction of false and irrational realizations was determined by the peculiarities of the basic nervous processes - increased excitability and weakness of inhibition. It was possible to establish that the consistent reduction of the excitation phase during the aging process caused the instability of the dominant state and motivational activity, negatively affecting the attention stable and, consequently, the ability to consistently reproduce the optimal variant of habit.

RESUMO

The Russian consensus on exo- and endocrine pancreatic insufficiency after surgical treatment was prepared on the initiative of the Russian "Pancreatic Club" on the Delphi method. His goal was to clarify and consolidate the opinions of specialists on the most relevant issues of diagnosis and treatment of exo- and endocrine insufficiency after surgical interventions on the pancreas. An interdisciplinary approach is provided by the participation of leading gastroenterologists and surgeons.

RESUMO

We study the dependence of the mitogen-induced cytokine secretion by splenocytes from the character of the behavior of F1 (C57BL/6XDBA/2) mice in environments with different degrees of spatial complexity: simple in the form of home box, enriched box and maze. Suppression of in-terleukins IL-2, IL-4, IL-6 and IL-10 expression, regardless of their structural and functional properties, was observed only in an enriched environment when aggressive and neurotic manifestations were increased. The opposite effect in the form of significant increase of the productions in all studied cytokines was observed in the case of the stereotypical goal-directed behavior in a maze. It was supposed that cytokines of the splenocytes are involved in the realization of highly 307 differentiated adaptive response of the organism due to the influence of social and cognitive factors, the expression of which depends on the specific physiological and biochemical systems.

RESUMO

Duodenal dystrophy (DD) is the pathological change in the wall of the duodenum, which is caused by chronic inflammation in its ectopic pancreatic tissue. The most common complications of DD are acute or chronic pancreatitis and impaired duodenal patency, which along with severe pain are an indication for surgical treatment. Pancreaticoduodenal resection is recognized as the operation of choice. The paper describes a clinical case demonstrating the efficiency and safety of minimally invasive (laparoscopic) surgical technologies in this category of patients. Resectional interventions of this volume are also shown to be accompanied by the development of pancreatic insufficiency that necessitates continuous enzyme replacement therapy.

RESUMO

The damage of dorsal hippocampus in mice F1 from DBA/2J and C57BL/6J on the learning in conditions of free choice in a complex multialternative maze was studied. It was revealed that the HPC-mice were able to form a 4-links food-getting habit in the cyclic form. The main differences affected the conjugation of the investigate activity, behavioral efficiency and inhibition of mistakes. While these processes developed by the same exponential types in control, this conjugation were absent in HPC-mice. The main defects were found in stage of habit stabilization: motivational state stability reduced sharply and duration of transition from disorganization to organizing behavior increased. It is supposed that the hippocampus involved in learning and memory indirectly because its main role is organization of the dominant state providing the stability of attention for habit realization.

RESUMO

Preliminary administration of opioid peptide opilong accelerated information perception, but at the same time caused neurotic disorders in rats exposed to information load during training. Under these conditions, suppression of fibrinolysis and hypercoagulation (signs of prethrombosis confirmed by functional thrombin challenge) were detected in the blood. Histochemical analysis of tissues, depots of fibrinolytic enzymes of the anticoagulation system, revealed inadequate weakened tissue response to thrombin in opilong-treated rats in comparison with the control.
