RESUMO

Prostatic hyperplasia (PH) is an androgen-dependent condition associated with increased prostatic size that is common in intact dogs, and similar to the condition in men. In dogs, the increase in prostatic size is most prominent the first years, and after approximately four years (in beagles), a plateau is reached, and further growth is slower. Why the prostate continues to grow more in some individuals is not clear. Most testosterone in the circulation is bound to albumin or sex hormone binding globulin (SHBG) and only a minor part is unbound and biologically active. The binding to SHBG has higher affinity than that to albumin. In addition, SHBG has own biological functions, modifying testosterone action. The aim of the present study was to investigate if there is an association between relative prostatic size and the variables total testosterone concentration, SHBG concentration, an estimation of bioavailable testosterone: the ratio between testosterone and SHBG (free androgen index, FAI), estradiol concentration, the estradiol/testosterone ratio, dog age and dog weight. Hormone concentrations were measured in serum from 79 intact male dogs aged ≥ four years, weighing ≥ five kg. The size of the prostate was estimated using ultrasonography, and relative prostate size, Srel, was calculated as the estimated size related to the normal size for a 4-year-old dog of the same weight. There as a negative correlation between testosterone concentration and age (ρ = -0.27, P = 0.018) and a positive correlation between age and Srel (ρ = 0.27, P = 0.016) and between SHBG and weight (ρ = 0.38, P = 0.001). The FAI was negatively correlated with dog weight (ρ = -0.32, P = 0.004). There were no significant correlations between Srel and SHBG or FAI or between estradiol or estradiol/testosterone and Srel, age or weight. A multiple regression analysis showed significant associations between log Srel and log testosterone concentration, log age and log weight of the dog, with an adjusted R2 of 9.5%. Although the variables total testosterone concentration, age and weight of the dog were all significantly associated with Srel, the coefficient of determination was low, indicating that they only explained a minor part of the prostatic size. The results support the analysis of total testosterone in studies of prostatic growth in the dog.

This study aimed to develop a method for performing accurate, high-resolution, three-dimensional (3D) Fricke gel dosimetry measurements of high dose rate (HDR) brachytherapy dose distributions using optical computed tomography (CT). A multi-needle brachytherapy gel phantom was purpose-built to contain four stainless-steel brachytherapy needles and a sample of Fricke Xylenol gel. A Paris-style HDR brachytherapy treatment was planned and delivered to the gel, which was then read out using a novel optical CT scanning method; all the brachytherapy needles were removed prior to scanning and replaced with a refractive index matched fluid. The removal of the stainless-steel needles during pre- and post-irradiation scanning minimised the potential for artefacts caused by missing ray-sum data. Results showed good agreement between measured and calculated doses (within 1%) at all positions greater than 0.1 cm from each needle. This study demonstrated that 3D Fricke gel phantoms may be valuable tools in verifying HDR brachytherapy treatments. The phantom construction and optical CT scanning method proposed in this work has the potential to enable routine quality assurance measurements of complex HDR brachytherapy treatment deliveries via accurate and detailed three-dimensional dose measurements.

The use of a non-water-equivalent personalised mould for gynaecological brachytherapy treatments can result in a substantial dose reduction at the treatment site, compared to calculated dose, in lieu of a dose calculation algorithm capable of modelling non-water-equivalent materials. This study describes the characterisation of the radiological properties of a brachytherapy applicator moulding material. Simple line source correction factors for an 192Ir source are obtained through Monte Carlo simulations and verified by film measurements. The dwell position corrections are used to estimate aggregate correction factors for dose deliveries that involve multiple dwell positions, in terms of treatment length, applicator radii and depth of reference dose. For the Fricotan moulding material used locally, the dose reductions varied from 1% for an applicator radius of 0.5 cm to > 4% for radii exceeding 2 cm. The method described in this paper could be used to develop correction factors for other non-water-equivalent moulding materials, in a TG-43UI dose calculation environment.

INTRODUCTION: The traditional use of two-dimensional geometric prescription points in intracavitary brachytherapy planning for locally advanced cervical cancer is increasingly being replaced by three-dimensional (3D) planning. This study aimed to directly compare the two planning methods to validate that CT planning provides superior dosimetry for both tumour and organs at risk (OARs) in our department. METHODS: The CT planning data of 10 patients with locally advanced cervical cancer was audited. For each CT dataset, two new brachytherapy plans were created, comparing the dosimetry of conventional American Brachytherapy Society points and 3D-optimised volumes created for the high-risk clinical target volume (HR CTV) and OARs. Total biologically equivalent doses for these structures were calculated using the modified EQD2 formula and comparative dose-volume histogram (DVH) analysis performed. RESULTS: DVH analysis revealed that for the 3D-optimised plans, the prescription aim of D90 ≥ 100% was achieved for the HR CTV in all 10 patients. However, when prescribing to point A, only 50% of the plans achieved the minimum required dose to the HR CTV. Rectal and bladder dose constraints were met for all 3D-optimised plans but exceeded in two and one of the conventional plans, respectively. CONCLUSIONS: This study confirms that the regionally relevant practice of CT-based 3D-optimised planning results in improved tumour dose coverage compared with traditional points-based planning methods and also improves dose to the rectum and bladder.

This feasibility study aims to determine if a low-cost 3D printer (BitsFromBytes 3D Touch) with ABS plastic can print custom mould structures and catheter channels defined in a brachytherapy treatment planning system (Nucletron Oncentra) for patient-specific treatment. Printer accuracy was evaluated through physical measurement, and print quality was investigated by adjusting print parameters (print speed, layer thickness, percentage infill). Catheter positioning and reproducibility were measured over repeated insertions. ABS plastic water equivalency was investigated by comparing Ir-192 HDR source dose distributions, measured with radiochromic film, in ABS plastic and in water. Structures and catheter channels were printed accurately to within 0.5 mm laterally and 1 mm in the vertical print direction. Adjusting print parameters could reduce print time, albeit with reduced print quality. 3.5 mm channel diameters allowed for easy catheter insertion. Catheter positioning was reproducible to within 0.5 mm but, because of catheter flex within the channel, was on average 1 mm offset from defined TPS positions. This offset could be accounted for by repeating the treatment planning CT scan with the printed mould positioned on the patient. Dose attenuation in ABS plastic and in water was equivalent to within the measurement limitations. While clinical uses for this particular low-cost printer and ABS plastic are limited by print size restrictions and non-certification for biocompatibility, it has been demonstrated that a low-cost 3D printer set-up can accurately create custom moulds and catheter channels potentially acceptable for clinical use.

PURPOSE: Patients with cervical and vaginal cancer sometimes have a less straightforward approach for choice of brachytherapy treatment owing to the tumor's location and clinical presentation. The staff at Royal Brisbane & Women's Hospital in Queensland, Australia, is trying to solve this problem by the use of an old technique in a new approach called vaginal molds. With a patient-specific vaginal mold, the appearance of the applicator and the dose distribution can be customized to provide an optimal treatment for each patient. METHODS AND MATERIALS: The technique used at the Royal Brisbane & Women's Hospital uses a flexible two-part putty, moulded to the shape of the vagina, in which standard catheters (flexible implant tubes) are incorporated, in a pattern designed to permit a dose distribution more conformal to the target volume. RESULTS: The presented technique is efficient and improves the accuracy of a homogeneous target cover and sparing of organs at risk for vaginal mold brachytherapy treatments at our institution. CONCLUSION: This technique offers a customizable option when traditional cylindrical- or dome-type applicators cannot be used, or provide inadequate dose coverage. Molds to match the patient anatomy can be created quickly, while allowing flexibility in positioning of catheters to achieve the desired dose distribution.
