RESUMO

La polimialgia reumática (PMR) es una enfermedad inflamatoria de las articulaciones que se presenta en pacientes mayores de 50 años con dolor y rigidez matutina prolongada en las cinturas del hombro y la cadera y en el cuello. La falta de hallazgos clínicos específicos, signos de laboratorio, biomarcadores y métodos de imagen establecidos dificulta el diagnóstico de los pacientes con esta enfermedad. La 18F-FDG PET/TC es una técnica de imagen funcional que constituye una herramienta consolidada en Oncología y que también ha demostrado su utilidad en el campo de las enfermedades inflamatorias. El objetivo de este trabajo es presentar evidencia bibliográfica sobre el uso de métodos de imagen molecular como la PET/TC para el diagnóstico precoz, la evaluación de la actividad de la enfermedad y la respuesta terapéutica en la PMR. Al mismo tiempo, se consideran las ventajas, las desventajas y las contraindicaciones de otros métodos (AU)

objetivo Analizar la literatura disponible sobre el valor pronóstico de los parámetros metabólicos de la 18F-FDG PET/TC preoperatoria y su utilidad en la estratificación de riesgo en pacientes con cáncer de endometrio (CE). Material y métodos En búsquedas de Pubmed se utilizó «(endometr* OR uter*) AND (PET OR FDG)» como palabras clave desde enero-2000 hasta junio-2020. Se revisaron referencias en los artículos incluidos buscando posibles publicaciones no incluidas en la primera búsqueda. Se incluyeron los estudios que evaluaron el valor pronóstico de la 18F-FDG PET/TC preoperatoria y su papel para la estratificación del riesgo en pacientes con CE. Los artículos no originales (revisiones, editoriales, cartas, casos legales, entrevistas, informes de casos, etc.) no fueron incluidos. Resultados Veintiséis estudios (1.918 pacientes) fueron elegidos según los criterios de inclusión en esta revisión. Se incluyeron 13 estudios (939 pacientes) relacionados con el papel pronóstico de la 18F-FDG PET/TC preoperatoria y 14 estudios (1.036 pacientes) relacionados con su papel en la estratificación de riesgo. Parámetros como SUVmáx, volumen tumoral metabólico (VTM) y glicólisis total de lesión (GTL) del tumor primario fueron analizados. Conclusiones El SUVmáx preoperatorio es útil para realizar diagnósticos no invasivos y decidir la estrategia terapéutica adecuada, ya que podría utilizarse como marcador pronóstico independiente de recurrencia y supervivencia en el CE. Además, tanto VTM como GTL preoperatorios podrían ser factores pronósticos independientes para predecir recurrencia y supervivencia, pero, aun no existe suficiente evidencia científica. La utilidad del SUVmáx para la estratificación del riesgo es limitada (no existe literatura suficiente acerca de que la 18F-FDG PET/TC pueda sustituir la estadificación quirúrgica), aunque el VTM y la GTL sí que son más precisos y tienen un valioso papel en la estratificación del riesgo del CE (AU)


Objective To analyse the available literature on the prognostic value of preoperative 18F-FDG PET/CT metabolic parameters and their usefulness in risk stratification in patients with endometrial cancer (EC). Material and methods Pubmed searches used «(endometr* OR uter*) AND (PET OR FDG)» as keywords from January-2000 to June-2020. References in included articles were checked for possible publications not included in the first search. Studies evaluating the prognostic value of preoperative 18F-FDG PET/CT and its role for risk stratification in patients with EC were included. Non-original articles (reviews, editorials, letters, legal cases, interviews, case reports, etc.) were not included. Results Twenty-six studies (1918 patients) were selected according to the inclusion criteria in this review. Thirteen studies (939 patients) related to the prognostic role of preoperative 18F-FDG PET/CT and 14 studies (1036 patients) related to its role in risk stratification were included. Parameters such as SUVmax, metabolic tumour volume (MTV) and total lesion glycolysis (TLG) of the primary tumour were analysed. Conclusions Preoperative SUVmax is useful for non-invasive diagnosis and for deciding the appropriate therapeutic strategy, as it could be used as an independent prognostic marker for recurrence and survival in EC. In addition, both preoperative VTM and GTL could be independent prognostic factors for predicting recurrence and survival, but there is still insufficient scientific evidence. The usefulness of SUVmax for risk stratification is limited (there is insufficient literature that 18F-FDG PET/CT can replace surgical staging), although VTM and GTL are more accurate and have a valuable role in risk stratification of EC. However, larger multicentre studies with adequate follow-up time are needed to confirm these findings (AU)

Un diagnóstico de cáncer es perturbador a cualquier edad, pero especialmente cuando el paciente es un niño. Solo se puede diagnosticar lo que se sospecha, solo se sospecha lo que se conoce o, al menos, se conoce su existencia, pero los tumores musculoesqueléticos son infrecuentes y, por tanto, muy difíciles de diagnosticar o tratar. Probablemente por su infrecuencia, por la complejidad de las imágenes radiológicas, por la apariencia histopatológica, así como por las serias consecuencias debido a biopsias y tratamientos inadecuados, la patología tumoral musculoesquelética necesita un manejo clínico que debe ser llevado a cabo por un grupo de especialistas con formación específica, que permita un adecuado diagnóstico, la introducción de terapias adyuvantes, así como un tratamiento quirúrgico, haciéndose hoy imprescindible un tratamiento multidisciplinar. Los estudios de imagen proporcionan información esencial sobre la naturaleza de cada lesión, su tamaño, su localización anatómica, el efecto sobre el hueso o los tejidos blandos circundantes y la afectación de las articulaciones adyacentes y las estructuras neurovasculares. En este artículo los autores analizan los avances de las técnicas de medicina nuclear (gammagrafías, PET/TC y SPECT/TC) y su utilidad en la estadificación de tumores musculoesqueléticos malignos pediátricos, así como en la valoración de la respuesta, el seguimiento y el diagnóstico de recidiva (AU)

Bone metastases are very common complications associated with certain types of cancers that frequently negatively impact the quality of life and functional status of patients; thus, early detection is necessary for the implementation of immediate therapeutic measures to reduce the risk of skeletal complications and improve survival and quality of life. There is no consensus or universal standard approach for the detection of bone metastases in cancer patients based on imaging. Endorsed by the Spanish Society of Medical Oncology (SEOM), the Spanish Society of Medical Radiology (SERAM), and the Spanish Society of Nuclear Medicine and Molecular Imaging (SEMNIM) a group of experts met to discuss and provide an up-to-date review of our current understanding of the biological mechanisms through which tumors spread to the bone and describe the imaging methods available to diagnose bone metastasis and monitor their response to oncological treatment, focusing on patients with breast and prostate cancer. According to current available data, the use of next-generation imaging techniques, including whole-body diffusion-weighted MRI, PET/CT, and PET/MRI with novel radiopharmaceuticals, is recommended instead of the classical combination of CT and bone scan in detection, staging and response assessment of bone metastases from prostate and breast cancer.

Objetivo: Evaluar el efecto de los problemas técnicos y de las características de las pacientes, en la detección gammagráfica del ganglio centinela (GC), y el éxito del mapeo en estadios precoces del cáncer endometrial (CE).Material y métodos: Se incluyeron pacientes con estadio precoz de CE (IA-IB) sometidos a mapeo de GC empleando nanocoloides de 99mTc entre septiembre de 2011 y febrero del 2020. Fueron excluidos los casos con problemas técnicos, 92 pacientes se incluyeron en el análisis para valorar la rentabilidad diagnostica y la relación del mapeo linfático (pélvico unilateral o no detección del GC) con las características del paciente (edad, índice de masa corporal, enfermedad o cirugía pélvica previas) y de la enfermedad (histología, grado, invasión miometrial, infiltración del espacio linfovascular, tamaño tumoral y riesgo de infiltración linfática).Resultados: La tasa de detección global fue del 79%. En 7 pacientes se detectaron metástasis ganglionares. La edad (p=0,01), profundidad de invasión miometrial ≥ 50% (p=0,04) y el alto riesgo de infiltración linfática (p=0,02) se asociaron de forma positiva con el fallo linfático. En el análisis multivariante, la edad se asoció significativamente con el fallo del mapeo [odds ratio=1,63; IC:95%: 1,06-2,50; p=0,027].Conclusiones: La edad, profundidad de invasión miometrial y el alto riesgo de infiltración linfática fueron los factores asociados con mayor fallo de mapeo. Una técnica de inyección individualizada, optimizando la metodología, podría minimizar los fallos de detección (AU)

An individualized injection technique, optimizing the methodology, could minimize the detection failures (AU)

Objetivo: Como hay poca literatura sobre el tema, nos propusimos comparar la utilidad diagnóstica del análisis semicuantitativo versus el análisis visual en la escintigrafía de glóbulos blancos etiquetados (WBCS) para la infección osteoarticular. Se evaluaron protocolos de uno y dos días, en particular en los dispositivos ortopédicos.Material y métodos: Estudio prospectivo de 79 pacientes consecutivos con sospecha de infección osteoarticular. En todos los pacientes, la SCBM se realizó a los 30 min, 4 h, 8 h y 24 h. Las imágenes se analizaron agrupándolas en dos protocolos: protocolo de un día (los expertos evaluaron imágenes planas de 30 min, 4 h y 8 h) y protocolo de dos días (los expertos evaluaron imágenes planas de 30 min, 4 h y 24 h). Las imágenes planas se interpretaron cualitativa y semicuantitativamente y también se compararon agrupando a los pacientes con y sin dispositivos ortopédicos. Para determinar qué valor de corte de la variación porcentual podía predecir la infección osteoarticular, se calcularon múltiples valores de corte en ambos protocolos a partir del índice de Youden. Tres lectores ciegos analizaron las imágenes.Resultados: Comparando el diagnóstico final, el análisis visual del protocolo de un día proporcionó mejores resultados con una sensibilidad del 95,5%, una especificidad del 93% y una precisión diagnóstica del 93,7% (p < 0,01) que el protocolo de dos días con valores del 86,4%, 94,7% y 92,4%, respectivamente (p < 0,01). Para el análisis semicuantitativo, el protocolo de un día también obtuvo mejores resultados con una sensibilidad del 72,7%, una especificidad del 78,9% y una precisión del 77,2% (p < 0,01) que el protocolo de dos días (sin resultados significativos; p = 0,14), especialmente en el grupo de pacientes con aparatos ortopédicos (sensibilidad del 100%, especificidad del 79,5% y precisión del 82,7%; p < 0,01)

To find which cut-off value of the percentage variation could predict of osteoarticular infection, multiple cut-off values were calculated in both protocols from the Youden index. Three blinded readers analysed the images.Results: Comparing final diagnosis visual analysis of the one-day-protocol provided better results with sensitivity of 95.5%, specificity of 93% and diagnostic accuracy of 93.7% (P<.01) than the two-day-protocol with values of 86.4%, 94.7% and 92.4%, respectively (P<.01). For semi-quantitative analysis, the one-day-protocol also obtained better results with sensitivity of 72.7%, specificity of 78.9% and accuracy of 77.2% (P<.01) than two-day-protocol (no significant results; P=.14), especially in the group of patients with orthopaedic devices (sensitivity of 100%, specificity of 79.5% and accuracy of 82.7%; P<.01).Conclusions: Most accurate approach in the diagnosis of osteoarticular infection corresponded to visual analysis in one-day-protocol that showed greater sensitivity and specificity than semi-quantitative analysis. Semi-quantitative analysis only could be useful when visual analysis is doubtful

Planar images were interpreted qualitative and semiquantitatively and also were compared grouping patients with and without orthopaedic devices. To find which cut-off value of the percentage variation could predict of osteoarticular infection, multiple cut-off values were calculated in both protocols from the Youden index. Three blinded readers analysed the images. RESULTS: Comparing final diagnosis visual analysis of the one-day-protocol provided better results with sensitivity of 95.5%, specificity of 93% and diagnostic accuracy of 93.7% (P<.01) than the two-day-protocol with values of 86.4%, 94.7% and 92.4%, respectively (P<.01). For semi-quantitative analysis, the one-day-protocol also obtained better results with sensitivity of 72.7%, specificity of 78.9% and accuracy of 77.2% (P<.01) than two-day-protocol (no significant results; P=.14), especially in the group of patients with orthopaedic devices (sensitivity of 100%, specificity of 79.5% and accuracy of 82.7%; P<.01). CONCLUSIONS: Most accurate approach in the diagnosis of osteoarticular infection corresponded to visual analysis in one-day-protocol that showed greater sensitivity and specificity than semi-quantitative analysis. Semi-quantitative analysis only could be useful when visual analysis is doubtful. In patients with joint prostheses, an increase in percentage variation above 9% obtained maximum sensitivity and negative predictive value.

Non-tumour musculoskeletal pathology in children represents a high percentage of the nuclear medicine studies requested by paediatricians. As these are immature bones and joints, in constant growth and remodelling, they present their own physiological and anatomical peculiarities that require a specific management for the paediatric age group. Using different clinical scenarios frequently observed in medical appointments (limping, back pain or fever, among others), we have summarised the possible findings or artefacts that can be obtained in the different Nuclear Medicine explorations, including bone scintigraphy and hybrid images such as PET/CT. To obtain high quality images requires careful attention to technique and positioning in children. Bone scintigraphy is a common paediatric nuclear medicine procedure and plays an important role in the diagnosis of musculoskeletal pathologies and can be complemented by techniques such as SPECT to improve localisation and diagnostic accuracy. In addition, 18F-FDG PET/CT is increasingly applied in the evaluation of children. This article reviews the usual indications of, mainly, bone scintigraphy and 18F-FDG PET/CT in paediatric non-tumour musculoskeletal diseases, how to interpret them properly, being essential to know the normal physiological distribution of each radiopharmaceutical, as well as the common variants of paediatric growth that can simulate disease, implying possible misinterpretations between normal and pathological structures.

Clinical problems in the human spine are still common in our society, often causing pain and can also limit movement. Back pain is a very common clinical entity, although difficult to diagnose due to its multifactorial nature. There are multiple processes that can alter the structure of the spine, injure vertebrae and/or the surrounding tissue. For the study of the spine, image diagnosis is essential, and within this, molecular hybrid techniques play an important role by providing us with an image of functional and morphological fusion. Among these, SPECT/CT is key in the diagnosis of traumatic and stress pathology, allowing us to locate hidden vertebral fractures, and is also very useful in degenerative and post-surgical pathology. On the other hand, PET/CT with 18F-FDG also plays an important role in the management and monitoring of infectious and oncological processes. This review describes the application of these hybrid techniques in the different pathologies of the spine and the findings of their images, being very useful for the diagnostic assessment and therapeutic management of the patient.

Las enfermedades inflamatorias e infecciosas osteoarticulares pueden tener graves consecuencias para el paciente si no se diagnostican a tiempo. En las últimas décadas, diferentes modalidades de la Medicina Nuclear han permitido estudiar la fisiopatología de estos procesos y desempeñan un importante papel hoy en día en el diagnóstico, caracterización y monitorización de las enfermedades infecciosas musculoesqueléticas. Por lo tanto, es esencial que todos los especialistas de Medicina Nuclear tengan una visión de las ventajas y desventajas de cada método y saber cómo usarlos correctamente en el diagnóstico del paciente. Este artículo destaca el papel de la Medicina Nuclear en la estandarización del abordaje diagnóstico en pacientes con enfermedades infecciosas/inflamatorias, en particular en osteomielitis periférica, artritis séptica, infecciones de prótesis articulares, pie diabético infectado e infecciones de la columna vertebral. Los autores presentan las pruebas con radionúclidos más comunes, con sus ventajas e indicaciones clínicas, para lograr un diagnóstico adecuado de la infección y la inflamación

Introducción: Resulta difícil determinar la infección osteoarticular y diferenciar entre inflamación e infección mediante procedimientos de laboratorio e imagen (TC, RM, US). La gammagrafía con leucocitos marcados (GLM) constituye la prueba de medicina nuclear de elección, pero su duración es de dos días, y a veces es difícil diferenciar entre tejido blando e infección, por lo que se produce una variabilidad interobservador que hace disminuir su especificidad. Objetivo: Demostrar la utilidad del protocolo de un día de GLM con corrección por decaimiento del tiempo de adquisición para diagnosticar la infección osteoarticular y reducir la variabilidad interobservador. También se evaluó la función de SPECT/TC en GLM en la localización del foco de infección. Métodos: Se estudiaron prospectivamente 110 pacientes con sospecha de infección osteoarticular. Se obtuvieron imágenes planares con corrección por decaimiento del tiempo de adquisición a 30min, 4h, 8h y 24h. Las imágenes planares de GLM se agruparon en dos protocolos: Protocolo de un día: los expertos evaluaron imágenes de 30min, 4h y 8h. Protocolo de dos días: los expertos evaluaron imágenes de 30min, 4h y 24h. Ambos protocolos fueron clasificados como: Negativos: ausencia de migración leucocitaria. Positivos: persistencia o incremento de la captación con el tiempo. Inflamación aséptica: disminución de la captación con el tiempo. La SPECT/TC se realizó en 72 pacientes. Se calculó el índice kappa para evaluar la variabilidad interobservador. Resultados: Se confirmó infección en 34 casos. Los valores de sensibilidad, especificidad, valor predictivo positivo, valor predictivo negativo y precisión diagnóstica fueron del 97,1, del 97,4, del 94,3, del 98,7 y del 97,3% para el protocolo de un día, y del 94,1, del 97,4, del 94,1, del 97,4 y del 96,4 para el protocolo de dos días, respectivamente. SPECT/TC contribuyó al diagnóstico en 45 de 50 pacientes con GLM planar positiva. Índice kappa: 0,8 para el protocolo de un día y 0,79 para el protocolo de dos días. Conclusión: El protocolo de un día de GLM con corrección por decaimiento del tiempo de adquisición y SPECT/TC permite el diagnóstico precoz y preciso de la infección osteoarticular

La patología del pie y tobillo es una de las más frecuentes del sistema musculoesquelético y de gran repercusión en la calidad de vida de los pacientes. El diagnóstico preciso supone un desafío clínico importante debido a que la compleja anatomía y la función del pie dificultan la localización del origen del dolor por un examen clínico de rutina. En el estudio de la patología del pie se han utilizado técnicas anatómicas (radiografía, resonancia magnética [RM], ultrasonido y tomografía computarizada [TC]) y funcionales (gammagrafía ósea [GO], tomografía de emisión de positrones [PET] y RM). La imagen híbrida combina las ventajas de los estudios morfológicos y funcionales de forma sinérgica, ayudando al clínico en la gestión de problemas complejos. En este artículo profundizamos en la anatomía y en la biomecánica del pie y tobillo y describimos las indicaciones potenciales de las técnicas hibridas actuales disponibles para el estudio de la patología del pie y tobillo


Disorders of the foot and ankle are some of the most frequent ones affecting the musculoskeletal system and have a great impact on patients’ quality of life. Accurate diagnosis is an important clinical challenge because of the complex anatomy and function of the foot, that make it difficult to locate the source of the pain by routine clinical examination. In the study of foot pathology, anatomical imaging (radiography, magnetic resonance imaging [MRI], ultrasound and computed tomography [CT]) and functional imaging (bone scan, positron emission tomography [PET] and MRI) techniques have been used. Hybrid imaging combines the advantages of morphological and functional studies in a synergistic way, helping the clinician manage complex problems. In this article we delve into the anatomy and biomechanics of the foot and ankle and describe the potential indications for the current hybrid techniques available for the study of foot and ankle disease
