We compare the data of three studies of hospitalized infants with cardiac disease performed between 1967 and 1987. The studies were from the New England Regional Infant Cardiac Program (1967-1974), the Brompton Hospital (1973-1982), and the Northern Great Plains Regional Cardiac Program (1982-1987). Considering differences in classification among the studies, the distribution of cardiac anomalies during the first year of life is similar among the three studies. Although about 30% of infants are admitted during the first week of life and nearly 40% between 3 and 6 months, the proportion of infants admitted between 6 and 12 months was larger (25%) in the latest than in the earliest study (10%). There were also differences in the distribution of the diagnoses at various ages, reflecting changes in the patterns of care during the three eras.

The cardiac manifestation usually associated with tuberous sclerosis is rhabdomyoma of the heart. We report a rare association with cyanotic congenital heart disease in the form of double outlet right ventricle with infundibular pulmonary stenosis.

Mitral valve replacement and bypass are high-risk operations in the infant with severe mitral stenosis. A palliative approach is presented that obviates the need for valve replacement in infancy.

Double-outlet right ventricle and severe systemic outflow tract hypoplasia comprises a subset of patients in whom total correction or palliation requires complex surgical procedures in the neonatal period. Our experience with 3 patients illustrates the difficulties associated with treatment and suggests possible surgical options for this otherwise lethal variant of the Taussig-Bing syndrome.

A young woman with double-outlet right ventricle, mitral stenosis, and high systemic pulmonary artery pressure is described. Pulmonary vascular morphometric analysis indicated considerably less pulmonary vascular obstructive disease than might otherwise have been expected. Total repair was performed and resulted in a marked fall in pulmonary artery pressure. Pulmonary vascular morphometry may be indicated in selected patients before palliative management is selected over complete repair.

The successful repair of double-outlet right atrium, tetralogy of Fallot, and common atrioventricular valve is reported. Double-outlet right atrium is reviewed and its pathoembryology and nomenclature are discussed.

Aortic balloon valvuloplasty was performed on a three-year-old girl with severe valvar aortic stenosis. This case demonstrates excellent, nonsurgical relief of stenosis without significant complications. A new technique of simultaneous caval balloon inflow occlusion is described.

To determine the cardiovascular and respiratory effects of intravenous ketamine in the neonatal lamb we studied six unpremedicated chronic neonatal lamb preparations. Each lamb was anaesthetized with ketamine 15 mg [corrected] X kg-1, allowed to recover and then anaesthetized with ketamine 30 mg [corrected] X kg-1. Mean arterial pressure, mean pulmonary artery pressure, mean left atrial pressure, pulmonary bloodflow, heart rate, respiratory rate and arterial blood gases were measured before and at one, two, four, six, eight and ten minutes after the administration of each dose of ketamine. Pulmonary vascular resistance, systemic vascular resistance and the ratio of pulmonary to systemic vascular resistance were also determined. After the administration of either dose of ketamine, only respiratory rate changed significantly (p less than or equal to 0.05), reaching a maximum after two minutes. This change was not associated with any arterial blood gas abnormalities. We conclude that in the presence of adequate ventilation, ketamine produces no significant cardiovascular effects in neonatal lambs.

Hypoplasia of the abdominal aorta is a rare cause of renovascular hypertension. Arteriographic studies of the renal vasculature are presented from an infant with hypoplasia of the abdominal aorta and segmental renal artery stenosis. The renovascular hypertension was cured by partial nephrectomy. There was no difference in the parenchymal histology in the tissue from the congenitally ischemic lower pole of the kidney and the vascularized upper pole. In this unique case with decreased renal blood flow during fetal development there was no evidence that parenchymal ischemia can cause renal parenchymal hypoplasia.
