AIM: The aim of this study was to evaluate the usefulness of the trial umbilical cord blood sampling bag for unrelated cord blood transplantation. MATERIAL AND METHODS: Data were obtained from 100 vaginal deliveries. In 50 cases, umbilical cord blood (UCB) was taken with the traditional Kawasumi type UCB sampling bag. In another 50 cases, UCB were taken with trial UCB sampling bag offered by NIPRO Co. We compared the sampling volume between the two groups. Furthermore, 10 cases in each group were matched by sampling volume; we examined the quality of UCB on the number and concentration of nucleated cells, mononuclear cells, CD34+ cells and colony-forming unit granulocyte macrophage and the numbers tested positive for bacteria. RESULTS: Whereas there were no significant differences in gestational weeks at sampling, the ratio of primipara women to multipara women, maternal age, and neonatal weight between the two groups, the sampling UCB volumes with the trial sampling bag were significantly higher than those with traditional sampling bags (P < 0.05). In addition, this phenomenon was more significant in the latter part of the study period (P < 0.05). On the other hand, there were no significant differences in the quality of UCB between the two groups. CONCLUSION: Once clinicians have become accustomed to the trial UBC sampling bag, this method might be a useful method for collecting UCB for unrelated cord blood transplantation.

In this research, we used near-infrared spectroscopy (NIRS) as an alternative technique for mental state analysis, and compared its performance with other conventional techniques such as electroencephalography (EEG) and peripheral arterial tonometry (PAT) during stress and healing tasks. We measured biological signals simultaneously in our experiments using these techniques for comparison. NIRS results showed that the level of total hemoglobin in the frontal cortex increased during a stress task and decreased during a healing task for all subjects whose blood volume change was properly recorded. EEG, however, showed inconsistent results due to task variation. Only PAT gave consistent results in many of the subjects. Taken together, the results suggest that NIRS might be suitable for mental state evaluation, with PAT as an alternative.

RESUMO

The frontal cortex is the part of the brain that relates to higher brain functions, such as logical thinking and emotion. As part of the development of a brain-computer interface, we tested whether the frontal cortex reacts differently in people as they performed three different cognitive tasks. The reaction of the cortex was tested using near-infrared spectroscopy. Our preliminary research results showed a difference in blood flow volume occurred before and after two assigned tasks: a math task and a word task. However, after a naming task there was no particular reaction. We believe that mental stress might be the cause of the difference in frontal cortex activity.

OBJECTIVE: To investigate the common carotid blood flow volume and velocity in men with chronic low thoracic and lumbar spinal cord injury (SCI), using duplex sonography. DESIGN: Experimental control study. SETTING: University laboratory in a department of rehabilitation medicine. PARTICIPANTS: Twenty men with SCI between the T7 and L1 segments and 15 age- and sex-matched control subjects. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Common carotid artery flow volume and velocity. RESULTS: The mean carotid artery time-averaged flow velocity of subjects with SCI was significantly lower than that of control subjects. Moreover, the mean carotid flow volume of subjects with SCI was significantly lower than that of control subjects (right side: SCI, .52+/-.14 L/min; controls, .67+/-.10 L/min; P <.05; left side: SCI, .54+/-.15 L/min; controls, .72+/-.14 L/min; P <.05). CONCLUSIONS: There are reduced common carotid artery flow volumes and flow velocities in men with chronic SCI. The reduction may result from disturbances of blood distribution induced by reduced vasoconstriction tone below the level of SCI lesion.

OBJECTIVE: To investigate a part of the immune homeostasis in recreational athletes with spinal cord injury (SCI) during and after a wheelchair half-marathon race. DESIGN: Case-control study in an actual race. SETTING: The half-marathon division of an international wheelchair marathon race in Japan. PARTICIPANTS: Seven male wheelchair racers with SCI between T7 and L1. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Changes in the number and function of natural killer (NK) cells and the endocrine effects, including plasma catecholamines and cortisol in blood samples obtained the day before the race, immediately after it, and 1 day after the race. RESULTS: Both the percentage and absolute number of peripheral NK cells did not change significantly throughout the experiment. Mean NK cell cytotoxic activity +/- standard deviation increased significantly, from 45.5%+/-7.5% to 56.1%+/-5.1% (P<.01) immediately after the race and remained increased until the next day. Plasma adrenaline levels were increased (P<.05) immediately after the race and recovered 1 day later; the plasma concentration of cortisol did not change throughout the experiment. CONCLUSIONS: The wheelchair half-marathon race induced activation of NK cell function in recreational athletes with SCI between T7 and L1.

OBJECTIVE: To investigate cardiovascular regulation and endocrine responses during the cold pressor test in patients with chronic spinal cord injury (SCI). DESIGN: Experimental and control study. SETTING: University laboratory, department of rehabilitation medicine, in Japan. PARTICIPANTS: Eight quadriplegic subjects with complete spinal cord transection at the C6 to C8 level and 6 age-matched healthy subjects. INTERVENTIONS: Cardiovascular and endocrine responses were examined during 2 minutes of control, 3 minutes of ice-water immersion of the foot, followed by a 3-minute recovery. MAIN OUTCOME MEASURES: Blood pressure, heart rate, the Borg 15-point Rating of Perceived Pain Scale, and blood samples for measurement of plasma norepinephrine, epinephrine, plasma renin activity, plasma aldosterone, and arginine vasopressin. RESULTS: The rise in the mean arterial blood pressure during the cold pressor test in patients with SCI (baseline, 81.6+/-3.7mmHg; increased by 30%+/-6.1%) was significantly (P<.05) higher than that in healthy subjects (baseline, 101.2+/-4.5mmHg; increased by 20%+/-4.5%). The SCI subjects had no change in heart rate throughout the test, in contrast to the tachycardia noted in normal subjects. Baseline plasma norepinephrine in SCI subjects (63.0+/-18.3pg/mL) was significantly lower than in normal subjects (162.3+/-19.6pg/mL) and plasma norepinephrine increased significantly during the cold pressor test in both groups. CONCLUSIONS: In the SCI subjects, a reflex sympathetic discharge through the isolated spinal cord results in a more profound rise in mean blood pressure during ice-water immersion. This response was free of inhibitory impulses from supraspinal center and baroreceptor reflexes, either of which might restrain the increase in blood pressure.

BACKGROUND: Alzheimer's disease (AD) is an important problem that should be solved in the 21st century. Prior to treatment, a simple and easy diagnostic method using biological markers should be available. As a method to attain this goal, we detected and determined tau protein in oral mucosal epithelium. METHODS: Oral epithelium was exfoliated from 34 patients with AD or 29 patients with vascular dementia, and 33 young and 34 age-matched controls. Western blot was performed for determining the molecular weight of oral tau protein. The tau protein level was determined with an enzyme-linked immunosorbent assay (ELISA) kit for cerebrospinal fluid (CSF). CSF tau was also measured and compared with oral tau. RESULTS: Western blot analysis using an anti-non-phosphorylated tau-protein antibody showed two bands, one at 65 Kd and the other at 110 Kd. The tau-protein level in oral epithelia showed a significant positive correlation with those in the CSF (p <.05). The patients with AD had significantly higher levels of tau protein than the patients with vascular dementia and the controls (p <.01). AD patients with a younger age at onset of the study showed a higher level of the tau protein than the patients with later age at onset (p <.05). CONCLUSIONS: Like other nonneural tissues, oral epithelium contains small tau and big tau. The tau protein in oral epithelium reflects the pathological changes, as does the CSF tau. Individuals who develop AD may have had high levels of the tau protein in oral mucosal epithelium since early childhood. The tau-protein level in oral epithelia could be helpful in diagnosing AD.
