RESUMO

Efficiency of bioactive osteoplastic composition HA-phage at a surgical stage of combined treatment of chronic generalized periodontitis was examined. The results obtained in the control and experimental groups proved that HA-phage allows reducing the time of treatment and to achieve remission of the disease. Out of 92 patients with chronic generalized periodontitis 52 undertook microplasmic scalpel-irradiator in complex treatment. The treatment course comprises 3-5 procedures. The quantity of manipulations is determined by the pathological process. Before and after the treatment the radiographic investigations were carried out in persons of both groups. 18 months later after the operation a bone regeneration was shown in 87,8% cases in the investigated group and 60,1%--in the control group patients. The therapy has shown positive results. So, the new method of treatment of chronic generalized periodontitis may prove to be more practical in clinical dentistry. Currently, it is possible to isolate highly purified BMSSCs from bone marrow using STRO-1. The management of periodontal defects has been an ongoing challenge in clinical periodontics. This is mainly a result of the fact that the tissues which comprise the periodontium, the periodontal ligament, and the cementum and alveolar bone represent three unique tissues in their own right. Thus, reconstruction of the periodontium is not just a simple matter of regenerating one tissue but involves at least three quite diverse and unique tissues. Taking into account the results of recent studies we hypothesize that mesenchymal stem cells transplantation should be a perspective approach in the treatment of periodontal defects. For this purpose we plan to perform an experimental investigations which will include an induction of periodontitis, purification of the bone marrow stem cells and their transplantation.

The aim of our research was to analyze general and local complications during the restoration-reconstruction operations in patients with epidermoid cancer on a lower lip. We have analyzed 61 patients' histories who were treated at the department of head and neck tumors of A. Gvamichava National Center of Oncology during 1990-2000. Based on the spread of the disease at the certain stage of treatment surgical operation becomes necessary that is broad excision of the tumor. Unfortunately it is impossible to restore the lip with the help of the local tissues that demands to perform restoration-reconstruction surgery with the use of rotated or free skin-muscular or mucilaginous-muscular scraps and consequently we considered it expedient to estimate functional and cosmetic efficiency of the used restoration-reconstruction surgery. The most attention was paid to analyze the local and general complication that might significantly influence cosmetic and functional results of the treatment. Based on our investigations it may be concluded that after the surgical treatment of the cancer of the lower lip the local complications is detected in 23,1%, among which the most frequent is secondary wound repair along with formation of oral fistula. After the performed restoration-reconstruction surgery the scrap necrosis was detected in 5 cases (8, 2%). The mentioned complication mostly was developed in patients for whom the rotated scrap was taken from upper lipt.

RESUMO

A new medicine--Curiosin--was applied for treatment of inflammatory processes in parodontal tissues. Curiosin has no analogies by the composition and effects. It is the organotrophic medication, which includes Zinc and Hyaluronic acid. It is the glue-like, limpid solution, its pH=5-6. The Hyaluronic acid of Curiosin gets into contact with the fibrinous net and makes transitional matrix. It stipulates activity and migration of granulocytes, macrophages and fibroblasts, also - the proliferation of epithelial cells. The effectiveness and success of complex treatment was shown in 130 persons. The patients were divided into two groups: control group, n=59 persons and the study group, n=71 patients. The traditional treatment course was applied in patients of the control group. This course includes removing of gingival plaque, curette of periodontal recesses, medication of periodontal tissues with antiseptic solutions. After these preparations, periodontal tissues treated by heparin ointment and trichopol, also by periodontal bounds. In this case antibiotic-therapy was introduced. In the patients from the second-group Curiosin and plasma-therapy was applied. Before and after treatment the cytological examination was carried out in patients of both groups. For examination we got contents of periodontal recesses. Before the treatment the inflammatory processes in parodontal tissues was shown. In presence of ulcero-necrotic gingivitis, on 18-month post-operative cytogramms we saw the following results: after the treatment there was no any more processes of inflammatory-destructive character; the number of leucocytes and lymphocytes sharply decreased, while the number of undamaged cells increased. This fact can prove discontinuance of inflammation and induction of regenerative processes in periodontal tissues. We have seen the same results in groups of patients with periodontitis. Beside, there were the large number of undamaged epitheliocytes and increased number of plasmocytes.

The effectiveness of the bioactive osteoplastic composition--Cerasorbe--at a surgical stage of combined treatment of chronic generalized periodontitis was examined. The results obtained in the study and the control groups prove, that Cerasorbe allows reduction of the treatment duration and achieves remission of the disease. From 127 patients with chronic generalized periodontitis 65 were operated by microplasmatic scalpel-irradiator as a part of a complex treatment. The treatment course comprised of 3-5 procedures. The quantity of manipulations was determined by the severity of the pathological process. Before and after the treatment the radiographical investigation was carried out in both groups. At the 18th month after the surgical intervention bone regeneration, confirmed by radiography, was shown in 87,8% cases in the study group and in 60,1% in the control group. The work presents experimental and morphological studies of regeneration of damaged areas of maxillo-facial bones. Time course of healing of defects in the lower jaw bone filled with Cerasorb was studied in experiment on rabbits. On days 7, 14, 21 and 28 four rabbits from each group were killed and the defects were investigation by X-ray and histological methods. We have stained the micropreparations by hematoxilin-eosine. In experiments with Cerasorb, bone regenerations replaced up to one half of the defect area by the day 28. We have shown high efficiency of Cerasorb in experiment and during complex treatment. Cerasorb stimulated reparative osteogenesis and can be recommended for using in the clinical practice.
