Chagas disease (CD) is a parasitic neglected tropical disease (NTD) caused by the protozoan Trypanosoma cruzi that affects 6 million people worldwide, often resulting in financial burden, morbidity, and mortality in endemic regions. Given a lack of highly efficient and safe treatments, new, affordable, and fit-for-purpose drugs for CD are urgently needed. In this work, we present a hit-to-lead campaign for novel cyanopyridine analogues as antichagasic agents. In a phenotypic screening against intracellular T. cruzi, hits 1 and 2 were identified and displayed promising potency combined with balanced physicochemical properties. As part of the Lead Optimization Latin America consortium, a set of 40 compounds was designed, synthesized, and tested against T. cruzi intracellular amastigotes and relevant human cell lines. The structural modifications were focused on three positions: cyanopyridine core, linker, and right-hand side. The ADME properties of selected compounds, lipophilicity, kinetic solubility, permeability, and liver microsomal stability, were evaluated. Compounds 1-9 displayed good potency (EC50T. cruzi amastigote <1 µM), and most compounds did not present significant cytotoxicity (CC50 MRC-5 = 32-64 µM). Despite the good balance between potency and selectivity, the antiparasitic activity of the series appeared to be driven by lipophilicity, making the progression of the series unfeasible due to poor ADME properties and potential promiscuity issues.

BACKGROUND: Describe complications and clinical outcomes of heavy silicone oil (HSO) Oxane HD® use as an alternative to overcome the challenges of performing vitrectomy to treat tractional and rhegmatogenous retinal detachments with proliferative vitreoretinopathy (PVR). METHODS: A retrospective, observational study was performed on patients from one center from August 2014 to Aug 2023. It was included patients who underwent surgery using HSO Oxane HD® to treat rhegmatogenous retinal detachment with PVR or mixed tractional and rhegmatogenous diabetic retinal detachment. Severely ill patients who could not attend to follow up were excluded. The primary outcome was successful retinal attachment at first postoperative month. A descriptive analysis was performed. RESULTS: Among the 31 patients, 29 (93.5%) underwent surgeries due to rhegmatogenous retinal detachment and two (6.5%) for diabetic retinal detachment. The primary anatomic success was achieved in 27 (87.1%) patients. At the final visit, 17 (56.6%) had vision better than 20/400 (range, 20/30 to light perception). The vision was stable or improved in 22 (76.8%) patients at the end of follow-up. Nineteen (61.3%) patients required hypotensive eye drops after HSO use and twelve (38.7%) still required hypotensive eye drops at the final follow-up; three (9.7%) patients required additional glaucoma surgeries. CONCLUSIONS: HSO is safe and useful for complex retinal detachments cases specially with inferior tears and PVR. Ocular hypertension is frequent and usually clinically controlled with hypotensive eyedrops. Close postoperatively follow-up is advised due to the ocular complications, particularly elevated intraocular pressure and emulsification.

BACKGROUND: Recommendations for different types of bathing to prevent central line-associated bloodstream infections (CLABSI) are still divergent. The objective of this study was to verify whether bed bathing with wipes impregnated with 2% chlorhexidine (CHG) compared to conventional bed bathing is more effective in preventing CLABSI. METHODS: Systematic review of the literature by consulting the electronic databases PubMed/Medline, Embase, CINAHL, Scopus, and Web of Science from the date of inception until July 1, 2023, with no language or time restrictions. RESULTS: A total of 84,462 studies were examined, of which 6 were included in the meta-analysis. Data from 20,188 critical care patients included in primary studies were analyzed. The meta-analysis found that bed bathing with wipes impregnated with 2% CHG reduced the risk of CLABSI by 48% compared to conventional bed bathing (risk ratio 0.52; 95% confidence interval, 0.37-0.73), and this is moderate-quality evidence. The reduction in length of stay in the intensive care unit and length of hospital stay as well as the risk of death were not significantly different between the study groups. Whether bed bathing with 2% CHG-impregnated wipes increases the occurrence of skin reactions is unclear. CONCLUSIONS: This meta-analysis provides moderate-quality evidence that daily bathing with 2% CHG-impregnated wipes is safe and helps prevent CLABSI among adult intensive care unit patients.

RESUMO

The main drawback for the use of heavy silicone oil (HSO) Oxane HD is the difficulties it presents during removal. It differs from conventional silicone oil removal because it progressively concentrates in the posterior pole during the removal because of its heavier than water density. We describe a technique to facilitate proper HSO Oxane HD removal and minimize residual posterior bubbles and intraoperative complications. We describe our pearls for removing HSO to obtain the optimal results. A concern associated with HSO is the difficulty it presents when it is removed. However, this can be overcome by appropriate modification of surgical techniques during removal to limit intraoperative complications.

Erythrina L. is a genus that comprises ca. 120 to 130 species distributed throughout the tropics and subtropics of the world. Linnaeus established the genus in Genera Plantarum (1737) and the first binomial name given to a Brazilian Erythrina was E.crista-galli L., described by himself in Mantissa Plantarum (1767). Vellozo proposed in Florae Fluminensis (1790-1881) the first treatment of the genus in Brazil, where he treated three species from the states of Rio de Janeiro and São Paulo. Martins and Tozzi proposed the most recent treatment in 2018, where the authors recognized 11 valid names and presented three new synonyms. Despite extensive efforts already made in the genus, previous works did not treat all names related to the valid ones for Brazilian Erythrina. The present work is the most comprehensive and up-to-date nomenclatural treatment for the genus in Brazil, covering all 84 related names found on digital nomenclatural databases. Here we analyze 64 protologues, update typification statuses, propose five new synonyms, 13 new lectotypes (11 first-step, two second-step) and one neotype, linking all protologues and type specimens with their corresponding available digital sources, and make additional notes on etymology and vernacular names.

In recent decades, waves of yellow fever virus (YFV) from the Amazon Rainforest have spread and caused outbreaks in other regions of Brazil, including the Cerrado, a savannah-like biome through which YFV usually moves before arriving at the Atlantic Forest. To identify the vectors involved in the maintenance of the virus in semiarid environments, an entomological survey was conducted after confirmation of yellow fever (YF) epizootics at the peak of the dry season in the Cerrado areas of the state of Minas Gerais. In total, 917 mosquitoes from 13 taxa were collected and tested for the presence of YFV. Interestingly, mosquitoes of the Sabethes genus represented 95% of the diurnal captured specimens, displaying a peak of biting activity never previously recorded, between 4:30 and 5:30 p.m. Molecular analysis identified three YFV-positive pools, two from Sabethes chloropterus-from which near-complete genomes were generated-and one from Sa. albiprivus, whose low viral load prevented sequencing. Sa. chloropterus was considered the primary vector due to the high number of copies of YFV RNA and the high relative abundance detected. Its bionomic characteristics allow its survival in dry places and dry time periods. For the first time in Brazil, Sa. albiprivus was found to be naturally infected with YFV and may have played a role as a secondary vector. Despite its high relative abundance, fewer copies of viral RNA were found, as well as a lower Minimum Infection Rate (MIR). Genomic and phylogeographic analysis showed that the virus clustered in the sub-lineage YFVPA-MG, which circulated in Pará in 2017 and then spread into other regions of the country. The results reported here contribute to the understanding of the epidemiology and mechanisms of YFV dispersion and maintenance, especially in adverse weather conditions. The intense viral circulation, even outside the seasonal period, increases the importance of surveillance and YFV vaccination to protect human populations in affected areas.

O Estado Nutricional (EN) exerce grande influência na morbimortalidade de idosos institucionalizados, e a sua avaliação permite definir uma intervenção nutricional individualizada. Esse estudo objetivou avaliar o estado nutricional e o consumo alimentar de idosos residentes de uma instituição de longa permanência. Trata-se de um estudo transversal, realizado com 37 idosos institucionalizados residentes, no ano de 2019. O EN foi avaliado pelo questionário da Mini Avaliação Nutricional (MAN). O consumo alimentar foi obtido através da pesagem total dos alimentos, verificando sua adequação de acordo com as recomendações para idade. As diferenças entre variáveis foram testadas por meio do teste t de Student e o teste de Mann-Whitney. As associações entre variáveis foram investigadas com auxílio dos coeficientes de correlação de Pearson e Spearman. As prevalências de desnutrição e risco de desnutrição nos idosos foram de 21,6% e 73%, respectivamente. O Índice de Massa Corporal (IMC) demonstrou predominância de baixo peso nos homens (38,9%) e sobrepeso nas mulheres (47,4%). Foi encontrado consumo excessivo de carboidratos e proteínas na população, sendo que, a ingestão de energia, proteínas e lipídios foi superior entre o grupo masculino. A circunferência do braço (p= 0,007), necessidade energética estimada (p= 0,049) e IMC (p <0,001) foram associados positivamente com a MAN. Os resultados demonstraram que, esses idosos são caracterizados por alto risco nutricional associado a alterações na composição corporal, e inadequações nutricionais. Sugerindo que, independente do consumo alimentar estar adequado em quantidade, não foi suficiente para prevenir o quadro de desnutrição nos idosos institucionalizados.(AU)


The Nutritional Status (NS) has a great influence on the morbidity and mortality of institutionalized elderly, and its evaluation allows to define of an individualized nutritional intervention. This study aimed to assess the nutritional status and food consumption of elderly residents of a long-term institution. This is a cross-sectional study carried out with 37 institutionalized elderly residents in 2019. The NS was assessed by the Mini Nutritional Assessment (MNA) questionnaire. Dietary intake was obtained through total weighing of the food, checking its adequacy according to the recommendations for age. Differences between variables were tested using the Student's t-test and the Mann-Whitney test. Associations between variables were investigated with the aid of Pearson and Spearman correlation coefficients. The prevalence of malnutrition and risk of malnutrition in the elderly was 21.6% and 73%, respectively. The Body Mass Index (BMI) showed a predominance of low weight in men (38.9%) and overweight in women (47.4%). Excessive consumption of carbohydrates and proteins was found in the population, and the intake of energy, proteins, and lipids was higher among the male group. Arm circumference (p = 0.007), estimated energy requirement (p = 0.049) and BMI (p <0.001) were positively associated with MAN. The results demonstrate that these elderly people are characterized by the high nutritional risk associated with changes in body composition, and nutritional inadequacies. Suggesting that, regardless of whether food intake was adequate in quantity, it was not enough to prevent malnutrition in institutionalized elderly.(AU)

BACKGROUND: The evidence supporting intensive blood glucose control to prevent surgical site infections (SSIs) among liver transplant recipients is insufficient. We aimed to assess the effects of postoperative intensive blood glucose control (IBGC) against standard blood glucose control (SBGC) on the incidence of SSIs among adult liver transplant recipients. METHODS: We performed a randomized controlled trial (ClinicalTrials.gov identifier NCT03474666). The IBGC target was 80 to 130 mg/dL, and the SBGC target was below 180 mg/dL. Analyses were made on an intention-to-treat basis. RESULTS: Of the 41 recipients enrolled onto the trial, 20 were randomly allocated to the IBGC group and 21 to the SBGC group. There were no significant differences in SSIs among recipients allocated to either group (relative risk [RR], 0.78; 95% confidence interval [CI], 0.21-2.88; P = .69). Mean (SD) blood glucose levels were significantly lower in the IBGC group in the 24-hour period after surgery (145.0 [20.7] mg/dL and 230.2 [51.6] mg/dL; P = .001). While there were fewer episodes of hypoglycemia in the IBGC group, this was not statistically significant. There were no episodes of severe hypoglycemia in either group. Hyperglycemia and severe hyperglycemia were significantly more frequent in the SBGC group (RR, 0.70; 95% CI, 0.52-0.93; P = .001 and RR, 0.07; 95% CI, 0.01-0.48; P = .001, respectively). Length of hospital stay was significantly shorter for recipients in the IBGC group (13.1 [5.5] days vs 19.3 [12.1] days; P = .04). CONCLUSIONS: Although this small trial did not find intensive control reduced SSI, it was associated with lower blood glucose levels, fewer episodes of hyperglycemia and severe hyperglycemia, and shorter length of hospital stay.

BACKGROUND: In Brazil, the yellow fever virus (YFV) is maintained in a sylvatic cycle involving wild mosquitoes and non-human primates (NHPs). The virus is endemic to the Amazon region; however, waves of epidemic expansion reaching other Brazilian states sporadically occur, eventually causing spillovers to humans. OBJECTIVES: To report a surveillance effort that led to the first confirmation of YFV in NHPs in the state of Minas Gerais (MG), Southeast region, in 2021. METHODS: A surveillance network was created, encompassing the technology of smartphone applications and coordinated actions of several research institutions and health services to monitor and investigate NHP epizootics. FINDINGS: When alerts were spread through the network, samples from NHPs were collected and YFV infection confirmed by reverse transcription-quantitative polymerase chain reaction (RT-qPCR) and genome sequencing at an interval of only 10 days. Near-complete genomes were generated using the Nanopore MinION sequencer. Phylogenetic analysis indicated that viral genomes were related to the South American genotype I, clustering with a genome detected in the Amazon region (state of Pará) in 2017, named YFVPA/MG sub-lineage. Fast YFV confirmation potentialised vaccination campaigns. MAIN CONCLUSIONS: A new YFV introduction was detected in MG 6 years after the beginning of the major outbreak reported in the state (2015-2018). The YFV strain was not related to the sub-lineages previously reported in MG. No human cases have been reported, suggesting the importance of coordinated surveillance of NHPs using available technologies and supporting laboratories to ensure a quick response and implementation of contingency measures to avoid YFV spillover to humans.

The genus Orthopoxvirus (OPXV) of the family Poxviridae comprises several viruses that are capable of infecting a wide range of hosts. One of the most widespread OPXVs is the Vaccinia virus (VACV), which circulates in zoonotic cycles in South America, especially in Brazil, infecting domestic and wild animals and humans and causing economic losses as well as impacting public health. Despite this, little is known about the presence and/or exposure of neotropical primates to orthopoxviruses in the country. In this study, we report the results of a search for evidence of OPVX infections in neotropical free-living primates in the state of Minas Gerais, southeast Brazil. The sera or liver tissues of 63 neotropical primates were examined through plaque reduction neutralization tests (PRNT) and real-time PCR. OPXV-specific neutralizing antibodies were detected in two sera (4.5%) from Callithrix penicillata, showing 55% and 85% reduction in plaque counts, evidencing their previous exposure to the virus. Both individuals were collected in urban areas. All real-time PCR assays were negative. This is the first time that evidence of OPXV exposure has been detected in C. penicillata, a species that usually lives at the interface between cities and forests, increasing risks of zoonotic transmissions through spillover/spillback events. In this way, studies on the circulation of OPXV in neotropical free-living primates are necessary, especially now, with the monkeypox virus being detected in new regions of the planet.

Low-load resistance training performed to muscle failure or near muscle failure does not promote additional gains on muscle strength, hypertrophy, and functional performance of older adults. J Strength Cond Res 36(5): 1209-1215, 2022-The aim of the present study was to compare the effects of low-load resistance training (RT) protocols performed to failure (FAI), to voluntary interruption (VOL), and with a fixed low repetitions (FIX) on muscle strength, hypertrophy, and functional performance in older adults. Forty-one subjects (60-77 years) were randomized into one of the RT protocols (FAI, VOL, or FIX) and completed 12 weeks of RT at 40% of 1 repetition maximum (1RM), twice a week. The assessments included 1RM test, muscle cross-sectional area (CSA), rate of torque development (RTD), and functional performance (chair stand [CS], habitual gait speed [HGS], maximal gait speed [MGS], and timed up-and-go [TUG]). All protocols significantly increased 1RM values from Pre (FAI: 318.3 ± 116.3 kg; VOL: 342.9 ± 93.7 kg; FIX: 328.0 ± 107.2 kg) to Post (FAI: 393.0 ± 143.1 kg, 23.5%; VOL: 423.0 ± 114.5 kg, 23.3%; FIX: 397.8 ± 94.6 kg, 21.3%; p < 0.0001 for all groups). Regarding CS, all protocols showed significant improvements from Pre (FAI: 11.5 ± 2.4 seconds; VOL: 12.1 ± 2.5 seconds; FIX: 11.3 ± 1.1 seconds) to Post (FAI: 10.5 ± 1.1 seconds, -8.5%, p = 0.001; VOL: 10.3 ± 1.5 seconds, -15.1%, p = 0.001; FIX: 11.0 ± 1.1, -3.2%, p = 0.001). Habitual gait speed values increased significantly from Pre (FAI: 1.3 ± 0.2 m·s-1; VOL: 1.3 ± 0.1 m·s-1; FIX: 1.3 ± 0.1 m·s-1) to Post (FAI: 1.4 ± 0.2 m·s-1, 2.5%, p = 0.03; VOL: 1.4 ± 0.2 m·s-1, 5.2%, p = 0.036; FIX: 1.4 ± 0.1 m·s-1, 5.7%, p = 0.03). No significant differences between protocols were found (p > 0.05). In addition, there were no significant changes in CSA, RTD, MGS, and TUG for any protocols (p > 0.05). In conclusion, low-load RT performed without muscle failure promotes significant improvements in muscle strength and some parameters of functional performance in older adults.

Synthetic hydrogels have been proposed as vitreous substitutes recently. This study aims to evaluate the biocompatibility of polyvinyl alcohol (PVA) crosslinked with trisodium trimetaphosphate (SMTP) hydrogel in rabbit vitrectomized eyes. Seven animals were submitted to pars plana vitrectomy and the vitreous was replaced by PVA/SMTP hydrogel. Optical coherence tomography, fluorescein angiogram, clinical, and electrophysiological (ERG) examinations were analyzed at baseline, on postoperative days 7 and 30. The fellow eye was used as the control group. Hydrogel opacification was observed and ERG recordings were reduced in the hydrogel group in rod response, b-wave cone response and flicker. A histological analysis showed retinal disorganization, presence of multinucleated cells, and intraretinal hydrogel particles. The PVA/SMTP hydrogel showed poor biocompatibility. Novel biomaterials compounds should be analyzed in vivo.

Microscopic wood identification plays a critical role in many economically important areas in wood science. Historically, producing and curating relevant and representative microscopic cross-section images of wood species is limited to highly experienced and trained anatomists. This manuscript demonstrates the feasibility of generating synthetic microscopic cross-sections of hardwood species. We leveraged a publicly available dataset of 119 hardwood species to train a style-based generative adversarial network (GAN). The proposed GAN generated anatomically accurate cross-section images with remarkable fidelity to actual data. Quantitative metrics corroborated the capacity of the generative model in capturing complex wood structure by resulting in a Fréchet inception distance score of 17.38. Image diversity was calculated using the Structural Similarity Index Measure (SSIM). The SSIM results confirmed that the GAN approach can successfully synthesize diverse images. To confirm the usefulness and realism of the GAN generated images, eight professional wood anatomists in two experience levels participated in a visual Turing test and correctly identified fake and actual images at rates of 48.3 and 43.7%, respectively, with no statistical difference when compared to random guess. The generative model can synthesize realistic, diverse, and meaningful high-resolution microscope cross-section images that are virtually indistinguishable from real images. RESUMO

Chagas disease is a neglected tropical disease caused by the protozoan parasite Trypanosoma cruzi that endangers almost 70 million people worldwide. The only two drugs that are currently approved for its treatment, benznidazole and nifurtimox, have controversial efficacy in adults and restricting safety issues, leaving thousands of patients without a suitable treatment. The neglect of Chagas disease is further illustrated by the lack of a robust and diverse drug discovery and development portfolio of new chemical entities, and it is of paramount importance to build a strong research and development network for antichagasic drugs. Focusing on drug discovery programs led by scientists based in Latin America, the main endemic region for this disease, we discuss herein what has been published in the last decade in terms of identification of new antiparasitic drugs to treat Chagas disease, shining a spotlight on the origin, chemical diversity, level of characterization of hits, and strategies used for optimization of lead compounds. Finally, we identify strengths and weaknesses in these drug discovery campaigns and highlight the importance of multidisciplinary collaboration and knowledge sharing.

BACKGROUND: Considering the new SARS-CoV-2 pandemic and the potential scarcity of material resources, the reuse of personal protective equipment such as filtering facepiece respirators (FFRs) for N95 filtering or higher is being discussed, mainly regarding the effectiveness and safety of cleaning, disinfection and sterilization processes. AIM: To analyze the available evidence in the literature on the safety in processing FFRs. METHODS: A systematic review conducted by searching for studies in the following databases: PubMed, CINAHL, LILACS, CENTRAL, EMBASE, Web of Science, and Scopus. RESULTS: Forty studies were included in this review. The disinfectant/sterilizing agents most frequently tested at different concentrations and exposure periods were ultraviolet irradiation, vaporized hydrogen peroxide and steam sterilization. Microbial reduction was assessed in 21 (52.5%) studies. The only disinfectants/sterilizers that did not caused degradation of the material-integrity were alcohol, electric cooker, ethylene oxide, and peracetic acid fogging. Exposure to ultraviolet irradiation or microwave generated-steam resulted in a nonsignificant reduction in filter performance. CONCLUSION: There is a complex relationship between the FFR raw materials and the cycle conditions of the decontamination methods, evidencing the need for validating FFRs by models and manufacturers, as well as the process. Some methods may require additional tests to demonstrate the safety of FFRs for use due to toxicity.

Objetivo: descrever o perfil clínico, principais alterações laboratoriais e hemodinâmicas entre pacientes acometidos por Covid-19 internados na unidade de terapia intensiva de um hospital universitário da região dos Campos Gerais - Paraná. Método: Estudo de coorte retrospectivo. A amostra foi constituída por prontuários de pacientes internados na unidade de terapia intensiva cujo resultado do teste Polymerase Chain Reaction foi positivo para Covid-19. O quadro clínico foi analisado durante 10 dias desde a admissão na unidade. A coleta de dados foi obtida por meio do acesso ao prontuário eletrônico. O estudo foi aprovado por um Comitê de Ética em Pesquisa. Resultados: Predominaram pacientes do sexo masculino com faixa etária de 48 a 57 anos. Destacam-se o uso de ventilação mecânica, relação PaO2 FiO2 menor que 200, caracterizado como síndrome do desconforto respiratório agudo. Conclusão: A caracterização do perfil clínico contribuiu para a compreensão da fisiopatologia da doença e discussão com estudos semelhantes


Objective: to describe the clinical profile, main laboratory and hemodynamic changes among patients with Covid-19 admitted to the intensive care unit of a university hospital in the region of Campos Gerais - Paraná. Method: This is a historical cohort. The sample consisted of medical records of patients admitted to the intensive care unit whose Polymerase Chain Reaction test result was positive for Covid-19. The clinical picture was analyzed for 10 days from admission to the unit. Data collection was obtained through access to the electronic medical record. The study was approved by a Research Ethics Committee. Results: There was a predominance of male patients aged from 48 to 57 years. The use of mechanical ventilation stands out, with a PaO2 FiO2 ratio lower than 200, characterized as acute respiratory distress syndrome. Conclusion: The characterization of the clinical profile contributed to the understanding of the pathophysiology of the disease and discussion with similar studies.

BACKGROUND: Surgical site infections (SSIs) are among the leading health care-associated infections as well as a major problem in the postoperative period of lung transplant recipients. Little is known about the risk factors in this specific population. The objective of this study was to identify the incidence, risk factors, and outcomes of SSI following lung transplant. METHODS: Digital medical records of adult recipients subjected to lung transplant from July 2011 and June 2016 in a large Brazilian referral teaching public center were analyzed in this retrospective cohort follow-up. RESULTS: Among the 121 recipients analyzed, 19 (15.7%) had SSI; of these, 11 (57.8%) had superficial incisional infections, 1 (5.2%) had a deep incisional infection, and 7 (36.8%) had organ/space infection. Recipient-related risk factors for SSI were high body mass index (P = .041), prolonged surgery time (P = .043), and prolonged duration of chest drain placement (P = .009). At the multiple logistic regression was found that each hour elapsed in the surgical time increased the odds of SSI by around 2 times (odds ratio 2.34; 95% CI, 1.46-4.53; P = .002). Donor-related risk factors included smoking status (P = .05) and positive bronchoalveolar lavage (P < .001). Having an SSI was associated with an increased length of stay in intensive care units (P = .003), reoperation (P = .014), and a higher 1-year mortality rate (P = .02). CONCLUSIONS: The identified incidence rate was higher to that observed in the previous studies. The risk factors duration of chest tube placement and donor smoking status are different from those reported in the scientific literature.

BACKGROUND: Surgical Site Infections (SSIs) are common among liver transplant recipients and result in adverse patient outcomes. Standard glycemic control is effective in reducing SSIs. Some studies suggest intensive glycemic control reduces the risk of SSI further. METHODS: For this systematic review, were searched for studies comparing perioperative intensive and standard glycemic control in liver transplant recipients. Clinical trials registries and reference lists of included studies were also searched. No date or language restrictions were applied. Randomized controlled trials (RCTs) were assessed using Cochrane risk of bias tool and GRADE method. Cohort studies were assessed using the Newcastle-Ottawa Scale. RESULTS: Two RCTs and three cohort studies met the inclusion criteria. Low-quality evidence from the two RCTs in a meta-analysis with 264 recipients found it was uncertain whether the risk of SSI was reduced by having intensive glycemic control (Risk Ratio [RR] 1.52, 95% CI 0.66-3.51). However, there was an increased risk of hypoglycemia among recipients having intensive glycemic control (RR 2.34, 95% CI 1.40-3.92) n = 264. Meta-analyses found it uncertain whether secondary outcomes, allograft rejection and death, were reduced among recipients having intensive glycemic control; (RR 0.85, 95% CI 0.48-1.50) and (RR 0.92, 95% CI 0.44-1.95), respectively. The two cohort studies were poor quality and presented conflicting outcomes on the effects of intensive blood glucose control on SSI. CONCLUSION: There is insufficient evidence to recommend the use of intensive glycemic control among liver transplant recipients to reduce SSIs.
