Paracoccidioidomycosis (South American blastomycosis) successfully treated with terbinafine: first case report.

We describe a 63-year-old man who presented with painful malodorous lesions in the perianal, perineal and scrotal regions. Following definitive diagnosis of paracoccidioidomycosis, he was treated initially with trimethoprim/sulphamethoxazole, but there was no clinical improvement. He then received terbinafine (Lamisil) 250 mg twice daily for 6 months. There was rapid resolution of all lesions and complete relief of symptoms, without any associated side-effects. The patient remains clinically well and without any evidence of infection 2 years after discontinuation of terbinafine treatment.

Malignant epithelioid schwannoma of the skin. A low-grade neurotropic malignant melanoma?

We studied a polypoid cutaneous neoplasm excised from the left arm of a 23-year-old man by light microscopy, immunohistochemistry, and electron microscopy. Histologically, the tumor was composed of a proliferation of round to oval epithelioid cells that were arranged in fascicles and showed a prominent perineural distribution. Immunohistochemistry revealed the presence of S-100 protein and vimentin within tumor cells. Electron microscopy demonstrated reduplication of basal lamina and abundant long-spacing collagen in intercellular spaces. The findings are consistent with a primary malignant epithelioid schwannoma of the skin. We discuss the relationship of this lesion with other neoplasms in the spectrum of tumors showing schwannian and melanocytic lines of differentiation.

Human gnathostomiasis in Ecuador (nodular migratory eosinophilic panniculitis). First finding of the parasite in South America.

Fifteen patients with human gnathostomiasis are reported from Ecuador. The name "nodular migratory eosinphilic panniculitis" is proposed because of the histopathologic and clinical changes found in two cases in which the parasite was found on biopsy.

We present the first case of lymphomatoid papulosis described in Ecuador. The clinicopathologic aspects of the disease are characteristic and make this entity different deserving its individualization . We discuss the different factors that may have some influence in clinical evolution and prognosis . The case presented is interesting because the lesions have only appeared once. Complete healing was observed after twenty days, and no relapse has been noted since then.

Reporting 4 cases of gnathostomiasis, a clinical review of this disease is given by the authors. In one of these cases the diagnosis could be established by evidencing the parasite. The name: nodular migratory eosinophilic panniculitis is suggested for this disease.
