RESUMO

The traditional, subject-based medical curriculum in Colombia has been mainly focused on the biomedical model proposed by Flexner in 1910. This means learning outcomes or competences are framed on curative care and the specialization of physicians. Students are mainly trained to work in highly complex hospitals in urban centers and encouraged to enroll (as soon as possible) in residencies. This curriculum lacks pertinence to implement the new Colombian Primary Health Care Model as the focus is a shift toward the promotion of health and prevention of illness. Recommendations to provide light on how to implement a change for ensuring pertinence of medical education in this context are discussed.

Fundamento: la fibrilación auricular es la arritmia más frecuente en la práctica clínica y conlleva implicaciones clínicas y pronósticas muy importantes.Objetivo: caracterizar los aspectos clínicos de la fibrilación auricular paroxística en los pacientes atendidos en el servicio de cardiología del Hospital General Docente Dr Ernesto Guevara de la Serna de Las Tunas, desde enero de 2013 a diciembre de 2014.Método: se realizó un estudio observacional descriptivo de corte transversal, en el lugar y periodo de tiempo ya definidos. El universo de estudio estuvo conformado por 211 pacientes que representaron el total de casos con diagnóstico de fibrilación auricular paroxística, atendidos en el Hospital General Docente Dr. Ernesto Guevara de la Serna en el periodo estudiado; la muestra estuvo representada por los 136 pacientes que ingresaron en el servicio de cardiología. Para la recolección de la información se utilizaron los datos provenientes de las historias clínicas y fueron procesados por el método estadístico porcentual.Resultados: predominó el grupo de edad entre 60 y 75 años con un 36 por ciento; el sexo masculino estuvo representado en el 61,8 por ciento de los casos; dentro de las enfermedades asociadas, la hipertensión arterial, la cardiopatía isquémica y la insuficiencia cardíaca fueron las que mayor prevalencia tuvieron, para un 69,1 por ciento, 35,3 por ciento y 27,2 por ciento respectivamente; y como factor etiológico predominante se obtuvo la hipertensión arterial descompensada en el 32,3 por ciento de la muestra de estudio.Conclusiones: se caracterizaron algunas variables clínicas en los pacientes con fibrilación auricular paroxística (AU)


Background: atrial fibrillation is the most frequent arrhythmia in clinical practice and it bears important clinical and prognostic implications.Objective: to characterize the clinical aspects of paroxysmal atrial fibrillation in patients seen at the Cardiology Service of Dr. Ernesto Guevara de la Serna General Teaching Hospital of Las Tunas, from January 2013 to December 2014.Method: a descriptive observational study was carried out at the place and time herein mentioned. The study group consisted of 211 patients representing the whole of the cases with a diagnosis of paroxysmal atrial fibrillation seen at Dr. Ernesto Guevara de la Serna Teaching General Hospital during the study period and the sample was made up of 136 patients admitted to the Cardiology Service. The information was gathered from the data recorded in the medical histories and they were processed by using the percentage statistical method.Results: the 60 to 75 age group prevailed in 36percent; the male sex was represented in 61.8percent of the cases; among the associated diseases hypertension, ischemic heart disease and heart failure were the most prevailing ones for 69.1percent, 35.3percent and 27.2percent, respectively; and uncontrolled hypertension was the prevailing etiologic factor for 32.3percent of the study sample.Conclusions: some clinical variables were characterized in patients with paroxysmal atrial fibrillation (AU)
