RESUMO

Congenitally corrected transposition of the great arteries (CCTGA) is a rare congenital heart disease which may present sudden cardiac death presumably due to malignant ventricular tachycardia (VT). In patients with congenital heart disease, knowledge of arrhythmogenic substrate is crucial for planning an ablation procedure. We present the first description of the arrhythmogenic endocardial substrate of a non-iatrogenic scar-related VT in a patient with CCTGA.

Atrial cycle length (CL) is an important feature for the analysis of electrogram (EGM) characteristics acquired during catheter ablation (CA) of atrial fibrillation (AF), the commonest cardiac arrhythmia. Nevertheless, a robust ACL estimator requires the precise detection of local activation waves (LAWs), which still remains a challenge. This work aims to compare the performance in (CL) estimation, especially under fractionated EGMs, of three different LAW detection methods relying on different operation strategies. The methods are based on the hyperbolic tangent (HT) function, an adaptive amplitude threshold (AAT) and a (CL) iteration (ACLI), respectively. For each method, LAW detection has been assessed with respect to manual annotations made by two experts and performance has been estimated by confusion matrix and mean and individual (CL) error calculation by EGM types of fractionation. The influence of EGM length on the individual (CL) error has been additionally considered. For the HT method, accuracy, sensitivity and precision were 92.77-100%, while for the AAT and ACLI methods they were 78.89-99.91% for all EGM types. The CL error on the HT method was lower than AAT and ACLI methods (up to 12 ms versus up to 20 ms), with the difference being more prominent in complex EGMs. The HT method also showed the lowest dependency on EGM length, presenting the lowest and least variable error values. Therefore, the HT method achieves higher performance in (CL) estimation in comparison with previous LAW detection techniques. The high robustness and precision demonstrated by this method suggest its implementation on CA mapping devices for a more successful location of ablation targets and improved results during CA procedures.

Local activation waves (LAWs) detection in complex fractionated atrial electrograms (CFAEs) during catheter ablation (CA) of atrial fibrillation (AF), the commonest cardiac arrhythmia, is a complicated task due to their extreme variability and heterogeneity in amplitude and morphology. There are few published works on reliable LAWs detectors, which are efficient for regular or low fractionated bipolar electrograms (EGMs) but lack satisfactory results when CFAEs are analyzed. The aim of the present work is the development of a novel optimized method for LAWs detection in CFAEs in order to assist cardiac mapping and catheter ablation (CA) guidance. The database consists of 119 bipolar EGMs classified by AF types according to Wells' classification. The proposed method introduces an alternative Botteron's preprocessing technique targeting the slow and small-ampitude activations. The lower band-pass filter cut-off frequency is modified to 20 Hz, and a hyperbolic tangent function is applied over CFAEs. Detection is firstly performed through an amplitude-based threshold and an escalating cycle-length (CL) analysis. Activation time is calculated at each LAW's barycenter. Analysis is applied in five-second overlapping segments. LAWs were manually annotated by two experts and compared with algorithm-annotated LAWs. AF types I and II showed 100% accuracy and sensitivity. AF type III showed 92.77% accuracy and 95.30% sensitivity. The results of this study highlight the efficiency of the developed method in precisely detecting LAWs in CFAEs. Hence, it could be implemented on real-time mapping devices and used during CA, providing robust detection results regardless of the fractionation degree of the analyzed recordings.

Objetivos: Exponer en base a un caso clínico una revisión de literatura reciente sobre Proteinosis alveolar pulmonar (PAP). Presentación del caso: Revisión de ficha clínica electrónica de paciente de sexo masculino de 76 años con antecedente de linfoma no Hodgkin (LNH) mesentérico, estirpe B de tipo folicular, quien acude en forma reiterada a servicios de urgencia por cuadro de dos meses de evolución de fiebre, compromiso del estado general y tos. Al examen físico destaca crépitos en hemitórax derecho. Se realizó Tomografía computada (TC) de tórax que mostró opacidades pulmonares en vidrio esmerilado periféricas, con engrosamiento septal liso y algunas bandas retráctiles subpleurales. Se manejó ambulatoriamente con Azitromicina por una semana. Sin respuesta, evoluciona con baja de peso y diaforesis nocturna. Nueva TC de tórax en enero 2021, muestra nuevos focos de "empedrado" periféricos extensos, descrito como "crazy paving", focos de vidrio esmerilado difusos extensos, sin condensación y con resolución de bandas retráctiles. Estudio infeccioso negativo. Se realiza lavado broncoalveolar (LBA) con estudio histológico de líquido que muestra proceso inflamatorio crónico con abundantes macrófagos y material proteináceo. Discusión: Tras el descarte de patología infecciosa, se orientó el estudio hacia otras causas de enfermedad parenquimatosa pulmonar. Así, resulta fundamental la descripción correcta del patrón imagenológico tomográfico y el LBA que resultaron compatibles con PAP. Conclusión: La PAP es una patología infrecuente, pero una historia clínica adecuada, el planteamiento de diagnósticos diferenciales de neumonía de lenta resolución, asociado el reconocimiento del patrón radiológico característico y el estudio histológico con LBA permiten realizar un diagnóstico certero, con gran implicancia terapéutica.


Objective: To present a review of recent literature on pulmonary alveolar proteinosis (PAP) based on a clinical case. Presentation of the case: Review of electronic clinical record of a 76 years-old masculine patient with history of mesenteric Non-Hodgkin Lymphoma (NHL) follicular-type lineage B, who repeatedly attended the emergency services due to a two-month history of symptoms of fever, compromised general condition and cough. Physical examination revealed crepitus in the right hemithorax. Chest computed tomography (CT) was performed, which showed peripheral ground-glass pulmonary opacities, with smooth septal thickening and some subpleural retractile bands. He was managed on an outpatient basis with Azithromycin for one week. No response; evolves with weight loss and night diaphoresis. New chest CT in January 2021, shows new extensive peripheral "cobblestone" foci, described as "crazy paving", extensive diffuse ground glass foci, without condensation and with resolution of retractile bands. Negative infectious study. Bronchoalveolar lavage (BAL) was performed with a histological study of the fluid showing a chronic inflammatory process with abundant macrophages and proteinaceous material. Discussion: After ruling out infectious pathology, the study was oriented towards other causes of pulmonary parenchymal disease. Thus, the correct description of the tomographic imaging pattern and the BAL that were compatible with PAP are essential. Conclusion: PAP is an infrequent pathology, but an adequate clinical history, the approach to differential diagnoses of slowly resolving pneumonia, associated with the recognition of the characteristic radiological pattern and the histological study with BAL allow an accurate diagnosis to be made, with great therapeutic implications.

Abstract Objective: To determine the effectiveness of a program of cognitive stimulation of the elderly residents in Social Protection Centers in Cartagena, 2014. Methods: Quasi-experimental study with pre and post tests in control and experimental groups. A sample of 37 elderly residents in Social Protection Centers participated: 23 in the experimental group and 14 in the control group. A survey and a mental evaluation test (Pfeiffer) were applied. The experimental group participated in10 sessions of cognitive stimulation. Results: The paired t-test showed statistically significant differences in the Pfeiffer test, pre and post intervention, compared to the experimental group (P=.0005). The unpaired t-test showed statistically significant differences in Pfeiffer test results to the experimental and control groups (P=.0450). The analysis of the main components showed that more interrelated variables were: age, diseases, number of errors and test results; which were grouped around the disease variable, with a negative association. Conclusions: The intervention demonstrated a statistically significant improvement in cognitive functionality of the elderly. Nursing can lead this type of intervention. It should be studied further to strengthen and clarify these results.


Resumen Objetivo: Determinar la efectividad de un programa de estimulación cognitiva de los adultos mayores residentes en Centros de Protección Social de Cartagena en 2014. Métodos: Estudio cuasi-experimental, con pre y post-test en grupos control y experimental. Participó una muestra de 37 adultos mayores residentes en Centros de Protección Social: 23 en el grupo experimental y 14 en el grupo control. Se aplicaron una encuesta y el test de valoración mental de Pfeiffer. El grupo experimental participó en 10 sesiones de estimulación cognitiva. Se tuvieron en cuenta las consideraciones éticas. Resultados: La prueba de la t apareada mostró diferencias estadísticamente significativas en el test de Pfeiffer antes y después de la intervención en el grupo experimental (p=0,0005). La prueba de la t no apareada mostró diferencias estadísticamente significativas en los resultados del test de Pfeiffer entre los grupos (p = 0,0450). El análisis de componentes principales mostró que las variables más relacionadas entre sí fueron: edad, enfermedades, número de errores cometidos y resultado de la prueba, las cuales se agruparon en torno a la variable enfermedad con una asociación negativa. Conclusiones: La intervención demostró una mejoría estadísticamente significativa en la funcionalidad cognitiva de los adultos mayores. Enfermería puede liderar este tipo de intervenciones. Se debe continuar investigando para fortalecer y aclarar estos resultados.

INTRODUCCIÓN: El varicocele es una causa frecuente de infertilidad en el hombre, encontrándose en el 40 por ciento de los hombres que consultan por infertilidad, existiendo diversas teorías que explican esta situación. Numerosos estudios apuntan a que la reparación del varicocele se asociaría a una mejoría en los parámetros seminales e incluso en las tasas de fertilidad. Entre las técnicas reparativas la varicocelectomía microquirúrgica (VM) se ha posicionado como la técnica gold standard dado sus mejores resultados en cuanto a éxito, recidiva y seguridad. OBJETIVO: evaluar el rol de la VM en los parámetros seminales de varones que consultan por infertilidad y que son sometidos a VM bilateral en comparación con aquellos en que se realiza VM unilateral. Materiales y método: se seleccionó a pacientes con varicocele clínico que consultaron por infertilidad y que tenían el espermiograma alterado, a los que se realizó VM uni o bilateral según tuvieran la enfermedad en uno o ambos lados. Se tomó el promedio de los parámetros seminales de 2 espermiogramas pre-operatorios y otras variables clínicas y demográficas y se comparó con los parámetros seminales a 6 meses post-cirugía. Además se comparó los resultados del grupo de VM bilateral con el unilateral. RESULTADOS: no hubo diferencias entre los grupos en el pre-operatorio, evidenciándose una mejoría en ambos grupos al analizar los resultados a los 6 meses post-VM. Esta mejoría fue significativamente mayor en el grupo de VM unilateral en cuanto a la concentración espermática (mediana de 6 mill/ml, comparado con 0.75 mill/ml en el grupo de pacientes sometidos a VM bilateral. p Value = 0.02)...


Introduction: Varicocele is a common cause of infertility in men, present in 40 percent of men with infertility, there are several theories that explain this situation. Numerous studies suggest that varicocele repair would be associated with an improvement in semen parameters and even fertility rates. Among the reparative techniques microsurgical varicocelectomy (VM) has positioned itself as the gold standard technique given the best results in terms of success, recurrence and security. Objective: To evaluate the role of the VM in semen parameters of men consulting for infertility and who undergo bilateral VM compared to those VM is performed unilaterally. Materials and Methods: We enrolled patients with clinical varicocele who consulted for infertility and had altered semen analysis, in which VM was performed unilateral or bilateral depending if they had the disease in one or both sides. The average of 2 pre-operative semen parameters was used and other clinical and demographic variables and they were compared with semen parameters at 6 months post-surgery. We also compared the results of the group with unilateral versus that with bilateral VM. Results: There were no differences between groups in the pre-operative, evidence-dose improvement in both groups in analyzing the results at 6 months post-VM. This improvement was significantly greater in the group of unilateral VM regarding sperm concentration (median of 6 mill / ml, compared with 0.75 million / ml in the group of patients undergoing bilateral VM. P Value = 0.02).Discussion: VM improves seminal parameters in patients with unilateral or bilateral varicocele, being unilateral VM group that benefits the most. This could be explained by a greater cumulative damage spermatogenesis in the case of bilateral varicocele. Randomized controlled prospective studies are needed, allowing to determine the real effect of unilateral vs. bilateral VM.

Las lesiones focales del hígado en niños incluyen neoplasias, lesiones metastásicas, masas inflamatorias y quistes (congénitos o adquiridos); las neoplasias primarias —tanto benignas como malignas— representan del 1% al 2% de todos los tumores pediátricos. La mayoría de niños con tumores hepáticos benignos o malignos se presentan al examen físico con masa palpable. Dentro del estudio por imágenes de ésta se encuentran incluidas múltiples modalidades, como ultrasonido, tomografía computarizada, resonancia magnética, angiografía y medicina nuclear. Debido a que la resección quirúrgica es la principal opción de tratamiento para muchas de dichas lesiones, la descripción detallada de la extensión de la masa y sus relaciones anatómicas es esencial. En este artículo realizamos una revisión de las características imaginológicas de algunas lesiones focales hepáticas en niños.

RESUMO

Objetivos: establecer de la concordancia entre la colangiotomografía con multidetectores, con reconstrucciones MPR y MinIP, y la colangiopancreatografía endoscópica retrógrada (CPER), la colangiografía transparietohepática (CTPH) o la cirugía, para identificar el sitio y la causa de la obstrucción, en pacientes con dilatación biliar. Material y métodos: estudio de concordancia entre pruebas diagnósticas, descriptivo y prospectivo. Población de referencia: durante 24 meses se estudiaron 34 pacientes, con dilatación de la vía biliar, diagnosticada con colangiotomografia, en el hospital Universitario san vicente de paúl, y a quienes posteriormente se les realizó un estudio confirmatorio, como CPER, CTPH o cirugía. Resultados: de los 34 pacientes, 16 presentaban litiasis biliar, y de ellos 6 tuvieron como prueba confirmatoria la cirugía, y 10, la CPER. los 18 pacientes restantes tenían malignidad como la causa de la obstrucción y el diagnóstico fue confirmado por cirugía en 11, 3 con CPER y 4 con CTPH. se obtuvo un 91% de concordancia con la colangiografía TC para determinar el sitio y la causa de la obstrucción. Conclusión: nuestros resultados muestran muy buena concordancia de la colangiotomografía con multidetectores para identifir el sitio y la causa de la obstrucción, en pacientes con dilatación de la vía biliar; así mismo, puede ser una alternativa a la colangiorresonancia.

Leakage and fistula are feared complications after gastro-intestinal anastomosis. A 36-year-old female underwent an one-anastomosis gastric bypass. The 24-h routine radiological study before oral intake showed a tiny leak, which was treated by NPO and I.V. fluids. After 5 days, despite output reduction, total parenteral nutrition was commenced. After 8 days, the leak remained with reduced output. It was then occluded endoscopically by fibrin glue. To our surprise, we found the drain that we had left behind the anastomosis, inside the gastric pouch. We began withdrawing the drain and occluded the defect with 4 ml Tissucol. After 48-h of no output, a repeat radiological study showed persistence of the leak. 6 days later, a radiological study demonstrated total closure of the leak.

BACKGROUND: One-Anastomosis Gastric Bypass (OAGB) by laparoscopy consists of constructing a divided 25-ml (estimated) gastric pouch between the esophago-gastric junction and the crow's foot level, parallel to the lesser curvature, which is anastomosed latero-laterally to a jejunal loop 200 cm distal to the ligament of Treitz. METHODS: The results of our first 209 OAGB patients operated from July 2002 to June 2004 are reported. Mean age was 41 years (14-66), BMI 48 (39-86) and mean excess body weight 66 kg (35-220). In 144 patients, OAGB was the only operation performed, and in 61 patients it was accompanied by other surgery (18 cholecystectomies, 5 incisional hernia repairs, and 38 adhesiolysis), and in 4 patients a restrictive bariatric operation had been performed previously. RESULTS: 2 patients (0.9%) were converted to open surgery due to uncontrollable bleeding. 3 patients (1.4%) needed re-operation in the immediate postoperative period. 5 patients (2.3%) needed prolonged hospital stay due to acute pancreatitis in 1 and anastomotic leakage in 4, all resolving with conservative treatment. 2 patients died (0.9%), 1 from fulminant pulmonary thromboembolism and 1 from nosocomial pneumonia. Long-term complications have occurred in only 2 patients who developed clinically significant iron-deficiency anemia. Mean excess weight loss was 75% after 1 year and >80% at 2 years. CONCLUSION: OAGB is a simple, safe and effective operation with less perioperative risk than conventional gastric bypass, quicker return to normal activities, and better quality of life.

Spinner-flask bioreactors have been used for the production of articular cartilage in vitro. The dynamic environment within bioreactors is known to significantly affect the growth and development of the tissue. The present research focuses on the experimental and numerical characterization of the flow field within a spinner flask operating under conditions used to produce cartilage. Laboratory experiments carried out in a scaled-up model bioreactor employ particle-image velocimetry (PIV) to determine velocity and shear-rate fields in the vicinity of the construct closest to the stir bar, in addition to turbulence properties. Numerical computations calculated using FLUENT, a commercial software package, simulate the flow field in the same model bioreactor under similar operating conditions. In the computations, scaffolds were modeled as both solid and porous media with different permeabilities and flow rates through various faces of the construct nearest the stir bar were examined.

Se realiza el estudio de 48 pacientes con lesiones traumáticas del codo atendidos en el hospital de Niños "Baca Ortiz" (H.B.O) de Quito que ingresaron para tratamiento durante el período entre septiembre de 1991 a abril de 1992. Se analiza: el área de cobertura de atención del H.B.O.; el fenómeno de la frecuencia de tratamiento empírico (fregador) antes de acudir para tratamiento especializado, su relación con el tipo de tratamiento realizado y las características de las lesiones traumáticas. El 52 por ciento de los pacientes atendidos recibieron tratamiento empírico previo al definitivo que fue finalmente la reducción abierta en el 80 por ciento de los de este grupo de pacientes que no fue atendido por empíricos, en el 43 por ciento, se realizó cirugía abierta, con una diferencia significativa entre los grupos (Ji=6.82 p 0.01).

RESUMO

En este trabajo se presentan resultados del estudio de 23 pacientes con luxación traumática de la cadera, atendidos en el Hospital Baca Ortiz de Quito, en el período de 1980 a 1989; 11 pacientes fueron descartados por falta de seguimiento; la relación hombres-mujeres fue de 3 a 1 y el grupo de edad más afectado fue de 5 a 10 años (58,32 por ciento) de los casos. Todos los casos fueron luxaciónes posteriores sin lesiones asociadas aparentes y el tiempo transcurrido entre la lesión y el tratamiento fue alto en el 83,3 por ciento de los casos con una medida de 2,3 meses, lo que derivó en un alto índice de complicaciones, 83,3 por ciento de los casos. La complicación más frecuente fue la necrosis aséptica 50 por ciento (6 casos), seguido de subluxación, 16,6 por ciento (2 casos) y la recidiva y rigidez 1 caso cada una 8,35 por ciento, respectivamente. El tipo de tratamiento no influyó en el pronóstico de estas caderas, sino que tuvo más importancia el tiempo transcurrido entre lesión y tratamiento, y las lesiones asociadas subdiagnosticadas con radiología simple.
