RESUMO

Thromboangiitis obliterans or Buerger's disease is an episodic and segmental inflammatory and thrombotic process of the medium and small arteries of the lower extremities. Even though the disease was described 90 years ago, the etiopathogenesis is still under consideration. Afflicted patients are mostly young male cigarette smokers without signs of atherosclerosis or other risk factors for peripheral arterial occlusive disease. This indicates that hereditary thrombophilic factors could play a role in the etiopathogenesis. Recently, increasing evidence shows that platelet receptor polymorphisms (HPA-1 polymorphism of beta3 subunit of alphaIIbbeta3 and 807 C/T polymorphism alpha2beta1) are associated with early onset of arterial thrombosis (myocardial infarction, stroke). This case-control study was designed to assess whether the 807 C/T polymorphism or the HPA-1 polymorphism is involved in the pathogenesis of Buerger's disease or has any influence on the clinical course of Buerger's disease. Eighteen patients with Buerger's disease and 81 (sex and age matched) healthy control subjects (mean age 44 +/- 10 vs 45 +/- 8 years, respectively) were genotyped for platelet receptor HPA-1 and GPIa 807 C/T polymorphism. The gene frequency of HPA-1 and GPIa 807 C/T polymorphisms was identical in both groups. Prevalence of hetero- and homozygous carriers of the HPA-1b allel (1a1b and 1b1b genotype) as well as the prevalence of the 807 C/T and 807 T/T carriers did not differ significantly between the two groups, p >0.05. The grade of clinical disease manifestation as well as disease progression did not reveal any significant relationship with HPA-1 and 807 C/T polymorphisms. A relationship between the age at onset of the disease and HPA-1 polymorphism was not found. Otherwise analysis of the GPIa 807 C/T platelet receptor polymorphism showed that the average age of patients who are carriers of the T allele at early onset of disease was 32 +/- 6 years (range 27-48 years) compared to 42 +/- 6 years (range 34-53 years) of the C/C carriers (p <0.05). This indicates that the GPIa 807 C/T polymorphism does not represent a risk factor for Buerger's disease itself, but could be associated with premature onset of this disorder in predisposed individuals.

AIM: To asses the intermediate rehabilitation outcome of patients with war-related below-knee amputations and compare it with the patients with other causes of amputation. METHOD: The study comprised 74 patients with below-knee stumps admitted for rehabilitation at the Department of Physical Therapy and Rehabilitation, Split University Hospital, Croatia, in 1994. They were fitted with a preliminary prosthesis, a donation from the Finish Red Cross. The rehabilitation was performed by a professional team and included regular bandaging of the stump, exercises to prevent knee and hip joint contracture, general fitness exercises, standing-up, falling and walking exercises, and electrostimulation of the thigh muscles. The time to reach each rehabilitation phase (walking with 2 crutches, walking with 1 crutch, walking with no crutches) was measured. The satisfaction of the patients with the prosthesis was also assessed at the end of rehabilitation. RESULTS: Among 74 patients with below knee amputation, war trauma was the cause for amputation in 31 patients, and in 6 of them the amputations were bilateral. Patients with war-related below-knee amputations were younger than the patients with amputations related to vascular disease, including diabetes. The rehabilitation time was significantly shorter in patients with war-related amputations (61.1+/-11.4 days to walking with no crutches) compared with patients with vascular disease-related amputations (80.9+/-8.1 days; p<0.001). The satisfaction with the prosthesis was more variable in patients with war-related amputations than in other patients. CONCLUSION: Early physical rehabilitation and replacement of the lost extremity with a preliminary prosthesis is an optimal intervention in below-knee amputations due to war-injury. Special attention should be paid to the psychological support to these patients during rehabilitation therapy.

The aim of this paper is to present the results of the operative treatment of the coracoclavicular ligament. According to Tossy, the injuries are classified into thre degrees. Younger patients and sportsmen (57) with the injury of the third degree were operated in the period from 1987-2000. The diagnostic methods were clinical examination and x-ray in the frontal and sagital plane. The patients were operated by Bosworth method. The operation technique which is described is slightly modified as it facilitates the surgeon's work and gives excellent post-operative results. The immobilisation was not used in the post-operative treatment. During the first week after operation a patient does static isometric exercises using the muscles of shoulders and arms, breathing exercises and passive exercises of the arm up to pain limit. The screw was removed under the local anesthesia after minimum of 6 weeks. Upon the removal of the osteosynthetic material we applied kynetic, hydro, magnetic and electrotherapy. The results of the operation were estimated after 2.5 months. Taking into consideration the functions of coracoclavicular ligament (the regulation of angle between clavicle and shoulder-blade, the limitation of rotation and elevation of clavicle, the support in bearing arm weight, the relaxation of joint between clavicle and shoulder-blade and the lateral reinforcement of acromioclavicular joint) its complete reconstruction is very important, and with the appropriate fixation of clavicle and shoulder-blade, accompanied by physical therapy, makes the basis for good results after the operation.

The coronary sinus is an anatomic landmark and conduit for many diagnostic and therapeutic procedures. We studied 40 human and 40 canine coronary sinuses of the heart in order to ascertain various functional anatomic features of the coronary sinus and its tributaries. We performed a comparative analysis of human and canine cardiac veins. We also studied the formation, tributaries, length, diameter and the ostium of the coronary sinus. The tributaries of the coronary sinus were highly variable in human and dog. The main and constantly present tributaries of both the human and canine coronary sinus were the great and the middle cardiac veins. We also found a rare case of a small cardiac vein connecting two cardiac venous systems--the coronary sinus and its tributaries and the anterior cardiac veins. In two cases we found a common trunk of the small cardiac vein and middle cardiac vein which opened into the coronary sinus. An intramyocardial course of the anterior interventricular vein was found in 2 cases and in 1 case the vein curved twice around the anterior interventricular branch of the left coronary artery. The Thebesian and Vieussens valves and ostial valves of other cardiac veins were also found.

The aim of the study was to determine the diagnostic value of carcinoembryonic antigen (CEA) and ferritine in malignant and tuberculous non-bloody pleural effusion. The etiology of diseases was determined by cytologic, histologic and microbiologic methods. CEA concentration above 5 ng/ml and ferritine concentration above 200 ng/ml were considered to be positive. There was significant difference in the value of CEA measured in malignant and in tuberculous pleural effusion (P < 0.005) as well as in the sera (P < 0.01) of these two groups. There was no correlation between concentration of CEA and ferritine in malignant pleural effusion. Ratio between CEA and ferritine in effusions and sera was of no help in discriminating malignant from tuberculous effusions. No correlation between examined markers and physical status of patients was observed. The sensitivity and specificity of CEA assay in malignant pleural effusion was 65% and 90%, respectively, and for ferritine 67% and 80%, respectively. A high correlation was observed between the CEA concentration in malignant pleural effusion and sera patients (r = 0.95). Combined sensitivity and specificity of CEA and ferritine was 65.9% and 85%. Bayes theorem was used to calculate the positive predictive values for CEA and ferritine, which were 53% and 37%, respectively. Results obtained in the study show the relatively good diagnostic potential of CEA.

RESUMO

The lumbar ache syndrome is more and more getting sign of a social disease because of its rate. Beside the conservative treatment very often the operative treatment is used too. Except the classical discectomy, a percutaneous lumbar discectomy had been used since the last year. In the work we gave the basic anatomical characteristics of the spiral column, stages of intervertebral disk dislocations and general characteristics of the acute ache syndrome. According to Aesculaps prospect we described the operating technique of percutaneous lumbar discectomy as well as the rehabilitation treatment of classical and percutaneous lumbar discectomy.
