RESUMO

Magnetic resonance imaging (MRI) is a crucial modality for abdominal imaging evaluation of focal lesions and tissue properties. However, several obstacles, such as prolonged scan times, limitations in patients' breath-hold capacity, and contrast agent-associated artifacts, remain in abdominal MR images. Recent techniques, including parallel imaging, three-dimensional acquisition, compressed sensing, and deep learning, have been developed to reduce the scan time while ensuring acceptable image quality or to achieve higher resolution without extending the scan duration. Quantitative measurements using MRI techniques enable the noninvasive evaluation of specific materials. A comprehensive understanding of these advanced techniques is essential for accurate interpretation of MRI sequences. Herein, we therefore review advanced abdominal MRI techniques.

It is challenging to overcome motion artifacts in diffusion-weighted imaging (DWI) of the abdomen. This study aimed to evaluate the image quality of motion-corrected DWI of the liver using non-rigid registration in comparison with conventional DWI (c-DWI) in patients with liver diseases. Eighty-nine patients who underwent 3-T magnetic resonance imaging (MRI) of the liver were retrospectively included. DWI was performed using c-DWI and non-rigid motion-corrected (moco) DWI was performed in addition to c-DWI. The image quality and conspicuity of hepatic focal lesions were scored using a five-point scale by two radiologists and compared between the two DWI image sets. The apparent diffusion coefficient (ADC) was measured in three regions of the liver parenchyma and in hepatic focal lesions, and compared between the two DWI image sets. Moco-DWI achieved higher scores in image quality compared to c-DWI in terms of liver edge sharpness and hepatic vessel margin delineation. The conspicuity scores of hepatic focal lesions were higher in moco-DWI. The standard deviation values of ADC of the liver parenchyma were lower in the moco-DWI than in the c-DWI. Moco-DWI using non-rigid registration showed improved overall image quality and provided more reliable ADC measurement, with an equivalent scan time, compared with c-DWI.

To evaluate variations in pre-contrast (preT1) and post-contrast (postT1) myocardial T1 values and extracellular volume fraction (ECV) according to left ventricular (LV) segments and to find correlations between them and cardiovascular risk factors. The 233 asymptomatic subjects (210 men, 23 women; aged 54.1 ± 6.0 years) underwent cardiac magnetic resonance imaging with preT1 and postT1 mapping on a 1.5-T scanner. T1 values and ECVs were evaluated according to LV segments, age, sex, and estimated glomerular filtration rate (eGFR). Based on the presence of hypertension (HTN) and diabetes mellitus (DM), subjects were subdivided into the control, HTN, DM, and HTN and DM (HTN-DM) groups. T1 values and ECV showed significant differences between septal and lateral segments at the mid-ventricular and basal levels (p ≤ 0.003). In subgroup analysis, the HTN-DM group showed a significantly higher ECV (0.260 ± 0.023) than the control (0.240 ± 0.021, p = 0.011) and HTN (0.241 ± 0.024, p = 0.041) groups. Overall postT1 and ECV of the LV had significant correlation with eGFR (r = 0.19, p = 0.038 for postT1; r = - 0.23, p = 0.011 for ECV). Septal segments show higher preT1 and ECV but lower postT1 than lateral segments at the mid-ventricular and basal levels. ECV is significantly affected by HTN, DM, and eGFR, even in asymptomatic subjects.

BACKGROUND: Several noninvasive tools are available for the assessment of nonalcoholic fatty liver disease (NAFLD) including clinical and blood biomarkers, transient elastography (TE), and magnetic resonance imaging (MRI) techniques, such as proton density fat fraction (MRI-PDFF) and magnetic resonance elastography (MRE). In the present study, we aimed to evaluate whether magnetic resonance (MR)-based examinations better discriminate the pathophysiologic features and fibrosis progression in NAFLD than other noninvasive methods. METHODS: A total of 133 subjects (31 healthy volunteers and 102 patients with NAFLD) were subjected to clinical and noninvasive NAFLD evaluation, with additional liver biopsy in some patients (n=54). RESULTS: MRI-PDFF correlated far better with hepatic fat measured by MR spectroscopy (r=0.978, P<0.001) than with the TE controlled attenuation parameter (CAP) (r=0.727, P<0.001). In addition, MRI-PDFF showed stronger correlations with various pathophysiologic parameters for cellular injury, glucose and lipid metabolism, and inflammation, than the TE-CAP. The MRI-PDFF and TE-CAP cutoff levels associated with abnormal elevation of serum alanine aminotransferase were 9.9% and 270 dB/m, respectively. The MRE liver stiffness measurement (LSM) showed stronger correlations with liver enzymes, platelets, complement component 3, several clinical fibrosis scores, and the enhanced liver fibrosis (ELF) score than the TE-LSM. In an analysis of only biopsied patients, MRE performed better in discriminating advanced fibrosis with a cutoff value of 3.9 kPa than the TE (cutoff 8.1 kPa) and ELF test (cutoff 9.2 kPa). CONCLUSION: Our results suggest that MRI-based assessment of NAFLD is the best non-invasive tool that captures the histologic, pathophysiologic and metabolic features of the disease.

We investigated the feasibility of free-breathing modified Look-Locker inversion recovery (MOLLI) sequence for measuring hepatic T1 values in children and young adults. To investigate the accuracy and the reproducibility of the T1 maps, a phantom study was performed with 12 different gadoterate meglumine concentrations and the T1 relaxation times of phantoms measured with the MOLLI sequence were compared against those measured with three different sequences: spin-echo inversion recovery, variable flip angle (VFA), and VFA with B1 correction. To evaluate the feasibility of free-breathing MOLLI sequence, hepatic T1 relaxation times obtained by free-breathing and breath-hold technique in twenty patients were compared. The phantom study revealed the excellent accuracy and reproducibility of MOLLI. In twenty patients, the mean value of hepatic T1 values obtained by free-breathing (606.7 ± 64.5 ms) and breath-hold (609.8 ± 64.0 ms) techniques showed no significant difference (p > 0.05). The Bland-Altman plot between the free-breathing and breath-hold revealed that the mean difference of T1 values was - 3.0 ms (- 0.5%). Therefore, T1 relaxation times obtained by MOLLI were comparable to the values obtained using the standard inversion recovery method. The hepatic T1 relaxation times measured by MOLLI technique with free-breathing were comparable to those obtained with breath-hold in children and young adults.

OBJECTIVE: To evaluate quantitative magnetic resonance imaging (MRI) parameters for differentiation of cysts from and solid masses in the anterior mediastinum. MATERIALS AND METHODS: The development dataset included 18 patients from two institutions with pathologically-proven cysts (n = 6) and solid masses (n = 12) in the anterior mediastinum. We measured the maximum diameter, normalized T1 and T2 signal intensity (nT1 and nT2), normalized apparent diffusion coefficient (nADC), and relative enhancement ratio (RER) of each lesion. RERs were obtained by non-rigid registration and subtraction of precontrast and postcontrast T1-weighted images. Differentiation criteria between cysts and solid masses were identified based on receiver operating characteristics analysis. For validation, two separate datasets were utilized: 15 patients with 8 cysts and 7 solid masses from another institution (validation dataset 1); and 11 patients with clinically diagnosed cysts stable for more than two years (validation dataset 2). Sensitivity and specificity were calculated from the validation datasets. RESULTS: nT2, nADC, and RER significantly differed between cysts and solid masses (p = 0.032, 0.013, and < 0.001, respectively). The following criteria differentiated cysts from solid masses: RER < 26.1%; nADC > 0.63; nT2 > 0.39. In validation dataset 1, the sensitivity of the RER, nADC, and nT2 criteria was 87.5%, 100%, and 75.0%, and the specificity was 100%, 40.0%, and 57.4%, respectively. In validation dataset 2, the sensitivity of the RER, nADC, and nT2 criteria was 90.9%, 90.9%, and 72.7%, respectively. CONCLUSION: Quantitative MRI criteria using nT2, nADC, and particularly RER can assist differentiation of cysts from solid masses in the anterior mediastinum.

PURPOSE: The purpose was to evaluate the clinical value of PETRA sequence for the diagnosis of internal derangement of the knee. MATERIALS AND METHODS: The major structures of the knee in 34 patients were evaluated and compared among conventional MRI findings, PETRA images, and arthroscopic findings. RESULTS: The specificities of PETRA with 2D FSE sequence were higher for meniscal lesions than those obtained when using 2D FSE alone. CONCLUSION: Using PETRA images along with conventional 2D FSE images can increase the accuracy of assessing internal derangements of the knee and, specifically, meniscal lesions.

OBJECTIVE: The purpose of this study was to evaluate the usefulness of radially sampled 3D fat-suppressed T1-weighted gradient-echo sequences (radial volumetric interpolated breath-hold examination [radial VIBE]) for contrast-enhanced brain MRI of children through comparison with a magnetization-prepared rapid-acquisition gradient-echo (MP-RAGE) sequence. MATERIALS AND METHODS: Sixty-five consecutive contrast-enhanced brain MRI examinations performed with axial MP-RAGE and radial VIBE sequences were included. For quantitative analysis, coefficients of variation of gray matter and white matter and CSF and relative contrast between tissue types (gray matter and white matter, gray matter and CSF, and white matter and CSF) for each sequence were calculated. For qualitative assessment, motion, pulsation artifacts, overall image quality, and lesion conspicuity were retrospectively scored on a 5-point scale. Quantitative and qualitative subgroup analyses were performed for patients with serious motion artifacts. RESULTS: Images obtained with the radial VIBE sequence had fewer motion and pulsation artifacts than those obtained with the MP-RAGE sequence (MP-RAGE vs radial VIBE motion score, 3.57 ± 1.00 vs 4.52 ± 0.51; pulsation score, 3.57 ± 0.60 vs 4.91 ± 0.21; all p < 0.001). Among 25 images with serious motion artifacts, radial VIBE images had significantly higher scores for all qualitative parameters, including overall image quality, than did MP-RAGE images (overall image quality for MP-RAGE vs radial VIBE, 2.63 ± 0.82 vs 3.42 ± 0.55, p < 0.001). CONCLUSION: The study showed that a radial fat-suppressed T1-weighted gradient-echo sequence is a viable alternative to conventional cartesian acquisition for contrast-enhanced brain imaging of restless children.

PURPOSE: The aim of this study was to compare intraindividual single and multiple arterial phase acquisitions and evaluate which acquisition method was more advantageous for obtaining high-quality hepatic arterial phase in gadoxetic acid-enhanced liver magnetic resonance imaging (MRI). MATERIALS AND METHODS: Sixty-seven patients who underwent gadoxetic acid-enhanced liver MRIs and had all 3 kinds of acquisitions (single, dual, and triple arterial phases) were retrospectively included. For hepatic arterial phase imaging, controlled aliasing in parallel imaging results in higher acceleration (CAIPIRINHA) with or without time-resolved imaging with interleaved stochastic trajectories (TWIST) was used. The adequacy of optimal hepatic arterial timing was assessed and respiratory motion artifacts were rated using a 5-point scale, with the highest score indicating the worst image quality. Optimal timing and respiratory motion artifacts among 3 different acquisitions were compared using Fisher exact test and repeated measures one-way analysis of variance with multiple comparisons. RESULTS: Optimal timing of hepatic arterial phase was observed in 89.6% (60/67) of single arterial phase acquisitions and 98.5% (66/67) of both dual and triple arterial phase acquisitions (P = 0.015). Respiratory motion artifact was significantly lower in single and dual arterial acquisitions than in triple arterial acquisition (mean score, 1.70 vs 1.90 vs 2.49; P < 0.001), although there was no significant difference between single and dual arterial acquisitions (P = 0.091). CONCLUSIONS: A 15-second breath-hold dual arterial phase acquisition during gadoxetic acid-enhanced MRI reliably offers well-timed hepatic arterial phase with less respiratory motion artifact. However, a 13-second breath-hold single arterial phase acquisition was most effective in reducing respiratory motion artifact.

OBJECTIVE: The purpose of this study was to compare free-breathing radially sampled 3D T1-weighted gradient-echo acquisitions (radial volumetric interpolated breath-hold examination [VIBE]) with a T1-weighted turbo spin-echo (TSE) sequence for contrast-enhanced spinal imaging of children with CNS tumors. MATERIALS AND METHODS: Twenty-eight consecutively registered children with CNS tumors underwent evaluation of leptomeningeal seeding with 1.5-T MRI that included both radial VIBE and T1-weighted TSE sequences. For qualitative analysis, overall image quality; presence of motion, CSF flow, and radial artifacts; and lesion conspicuity were retrospectively assessed with scoring systems. The signal-intensity uniformity of each sequence was evaluated for quantitative comparison. The acquisition times for each sequence were compared. RESULTS: Images obtained with the radial VIBE sequence had a higher overall image quality score than did T1-weighted TSE images (3.61 ± 0.73 vs 2.80 ± 0.69, p < 0.001) and lower motion artifact (0.82 ± 0.43 vs 1.29 ± 0.56, p = 0.001) and CSF flow artifact (0 vs 1.68 ± 0.67, p < 0.001) scores. Radial artifacts were found only on radial VIBE images (1.36 ± 0.31 vs 0, p < 0.001). In 13 patients with spinal seeding nodules, radial VIBE images showed greater lesion conspicuity than did T1-weighted TSE images (4.23 ± 0.52 vs 2.47 ± 0.57, p = 0.005). Radial VIBE images had diminished signal-intensity variation compared with T1-weighted TSE images in air, spine, and muscle (p < 0.01). The mean acquisition times were not significantly different between the two sequences (p = 0.117). CONCLUSION: For pediatric spinal imaging, radial VIBE images had better image quality and lesion conspicuity and fewer CSF and respiratory motion artifacts than did T1-weighted TSE images in a similar acquisition time.

OBJECTIVES: To validate 4D flow MRI in a flow phantom using a flowmeter and computational fluid dynamics (CFD) as reference. METHODS: Validation of 4D flow MRI was performed using flow phantoms with 75 % and 90 % stenosis. The effect of spatial resolution on flow rate, peak velocity and flow patterns was investigated in coronal and axial scans. The accuracy of flow rate with 4D flow MRI was evaluated using a flowmeter as reference, and the peak velocity and flow patterns obtained were compared with CFD analysis results. RESULTS: 4D flow MRI accurately measured the flow rate in proximal and distal regions of the stenosis (percent error ≤3.6 % in axial scanning with 1.6-mm resolution). The peak velocity of 4D flow MRI was underestimated by more than 22.8 %, especially from the second half of the stenosis. With 1-mm isotropic resolution, the maximum thickness of the recirculating flow region was estimated within a 1-mm difference, but the turbulent velocity fluctuations mostly disappeared in the post-stenotic region. CONCLUSION: 4D flow MRI accurately measures the flow rates in the proximal and distal regions of a stenosis in axial scan but has limitations in its estimation of peak velocity and turbulent characteristics. KEY POINTS: • 4D flow MRI accurately measures the flow rate in axial scan. • The peak velocity was underestimated by 4D flow MRI. •4D flow MRI demonstrates the principal pattern of post-stenotic flow.

OBJECTIVES: To determine whether multislice T1 mapping of the liver using a modified look-locker inversion recovery (MOLLI) sequence on gadoxetic acid-enhanced magnetic resonance imaging (MRI) can be used as a quantitative tool to estimate liver function and predict the presence of oesophageal or gastric varices. METHODS: Phantoms filled with gadoxetic acid were scanned three times using MOLLI sequence to test repeatability. Patients with chronic liver disease or liver cirrhosis who underwent gadoxetic acid-enhanced liver MRI including MOLLI sequence at 3 T were included (n = 343). Pre- and postcontrast T1 relaxation times of the liver (T1liver), changes between pre- and postcontrast T1liver (ΔT1liver), and adjusted postcontrast T1liver (postcontrast T1liver-T1spleen/T1spleen) were compared among Child-Pugh classes. In 62 patients who underwent endoscopy, all T1 parameters and spleen sizes were correlated with varices. RESULTS: Phantom study showed excellent repeatability of MOLLI sequence. As Child-Pugh scores increased, pre- and postcontrast T1liver were significantly prolonged (P < 0.001), and ΔT1liver and adjusted postcontrast T1liver decreased (P < 0.001). Adjusted postcontrast T1liver and spleen size were independently associated with varices (R (2) = 0.29, P < 0.001). CONCLUSIONS: T1 mapping of the liver using MOLLI sequence on gadoxetic acid-enhanced MRI demonstrated potential in quantitatively estimating liver function, and adjusted postcontrast T1liver was significantly associated with varices. KEY POINTS: • T1 mapping using MOLLI sequence can be achieved within a breath-hold. • T1liver measured by MOLLI sequence provided excellent short-term repeatability. • Precontrast and postcontrast T1liver were significantly prolonged as Child-Pugh scores increased. • Adjusted postcontrast T1liver and spleen size were independently associated with varices.

OBJECTIVE: To assess the feasibility of T2*-corrected fat-signal fraction (FF) map by using the three-echo volume interpolated breath-hold gradient echo (VIBE) Dixon sequence to differentiate between malignant marrow-replacing lesions and benign red marrow deposition of vertebrae. MATERIALS AND METHODS: We assessed 32 lesions from 32 patients who underwent magnetic resonance imaging after being referred for assessment of a known or possible vertebral marrow abnormality. The lesions were divided into 21 malignant marrow-replacing lesions and 11 benign red marrow depositions. Three sequences for the parameter measurements were obtained by using a 1.5-T MR imaging scanner as follows: three-echo VIBE Dixon sequence for FF; conventional T1-weighted imaging for the lesion-disc ratio (LDR); pre- and post-gadolinium enhanced fat-suppressed T1-weighted images for the contrast-enhancement ratio (CER). A region of interest was drawn for each lesion for parameter measurements. The areas under the curve (AUC) of the parameters and their sensitivities and specificities at the most ideal cutoff values from receiver operating characteristic curve analysis were obtained. AUC, sensitivity, and specificity were respectively compared between FF and CER. RESULTS: The AUCs of FF, LDR, and CER were 0.96, 0.80, and 0.72, respectively. In the comparison of diagnostic performance between the FF and CER, the FF showed a significantly larger AUC as compared to the CER (p = 0.030), although the difference of sensitivity (p = 0.157) and specificity (p = 0.157) were not significant. CONCLUSION: Fat-signal fraction measurement using T2*-corrected three-echo VIBE Dixon sequence is feasible and has a more accurate diagnostic performance, than the CER, in distinguishing benign red marrow deposition from malignant bone marrow-replacing lesions.

INTRODUCTION: This study aimed to evaluate the feasibility of non-contrast-enhanced 4D magnetic resonance angiography (NCE 4D MRA) with signal targeting with alternative radiofrequency (STAR) spin labeling and variable flip angle (VFA) sampling in the assessment of dural arteriovenous fistula (DAVF) in the transverse sinus. METHODS: Nine patients underwent NCE 4D MRA for the evaluation of DAVF in the transverse sinus at 3 T. One patient was examined twice, once before and once after the interventional treatment. All patients also underwent digital subtraction angiography (DSA) and/or contrast-enhanced magnetic resonance angiography (CEMRA). For the acquisition of NCE 4D MRA, a STAR spin tagging method was used, and a VFA sampling was applied in the data readout module instead of a constant flip angle. Two readers evaluated the NCE 4D MRA data for the diagnosis of DAVF and its type with consensus. The results were compared with those from DSA and/or CEMRA. RESULTS: All patients underwent NCE 4D MRA without any difficulty. Among seven patients with patent DAVFs, all cases showed an early visualization of the transverse sinus on NCE 4D MRA. Except for one case, the type of DAVF of NCE 4D MRA was agreed with that of reference standard study. Cortical venous reflux (CVR) was demonstrated in two cases out of three patients with CVR. CONCLUSION: NCE 4D MRA with STAR tagging and VFA sampling is technically and clinically feasible and represents a promising technique for assessment of DAVF in the transverse sinus. Further technical developments should aim at improvements of spatial and temporal coverage.
