RESUMO

BACKGROUND: Nipple-sparing mastectomy (NSM) offers superior cosmetic outcomes and has been gaining wide acceptance. It has always been difficult to objectively quantify the risk of nipple-areola complex involvement (NACi). The goal was to develop a prediction model for clinical application. METHODS: Patients who had a total mastectomy (TM) between January 2016 and January 2020 at a single institute formed the development cohort (n = 578) and those who had NSM + immediate breast reconstruction (IBR) between January 2020 and January 2021 formed the validation cohort (n = 112). The prediction model was developed using univariate and multivariate logistic regression studies. Based on NACi risk variables identified in the development cohort, a nomogram was created and evaluated in the validation cohort. Meanwhile, stratified analysis was performed based on the model's risk levels and was combined with intraoperative frozen pathology (IFP) to optimize the model. RESULTS: Tumor central location, clinical tumor size (CTS) > 4.0 cm, tumor-nipple distance (TND) ≤ 1.0 cm, clinical nodal status positive (cN +), and KI-67 ≥ 20% were revealed to be good predictive indicators for NACi. A nomogram based on these major clinicopathologic variables was employed to quantify preoperative NACi risk. The accuracy was verified internally and externally. The diagnostic accuracy of IFP was 92.9%, sensitivity was 64.3%, and specificity was 96.9% in the validation group. Stratified analysis was then performed based on model risk. The diagnostic accuracy rates of IFP and NACiPM in low-risk, intermediate-risk, and high-risk respectively were 96.0%, 93.3%, 83.9%, 61.3%, 66.7%, and 83.3%. CONCLUSION: We created a visual nomogram to predict NACi risk in breast cancer patients. The NACiPM can be used to distinguish the low, intermediate, and high risk of NAC before surgery. Combined with IFP, we can develop a decision-making system for the implementation of NSM.

RESUMO

Persulfate (PS)-based advanced oxidation processes (AOPs) have been promoted as alternatives to H2O2-based AOPs. To gauge the potential of this technology, the PS/Fe(II) and Fenton (H2O2/Fe(II)) processes were comparatively evaluated using formate as a simple target compound and nanofiltration concentrate from a municipal wastewater treatment plant as a complex suite of contaminants with the aid of kinetic modeling. In terms of the short-term rate and extent of mineralization of formate and the nanofiltration concentrate, PS/Fe(II) is less effective due to slow Fe(II)/Fe(III) cycling attributable to the scavenging of superoxide by PS. However, in the concentrate treatment, PS/Fe(II) provided a sustained removal of total organic carbon (TOC), with ∼81% removed after 7 days with SO4•- consistently produced via homolysis of the long-life PS. In comparison, H2O2/Fe(II) exhibited limited TOC removal over ∼57% after 10 h due to the futile consumption of H2O2 by HO•. PS/Fe(II) also offers better performance at transforming humic-like moieties to more biodegradable compounds as a result of chlorine radicals formed by the reaction of SO4•- with the matrix constituents present in the concentrate. The application of PS/Fe(II) is, however, subject to the limitations of slow oxidation of organic contaminants, release of sulfate, and formation of chlorinated byproducts.

RESUMO

The use of animal medicine has a long history in China, it has the characteristics of high curative effect, strong activity, wide application and great potential. However, the circulation of animal medicine in current market mixed counterfeit variety and complex. Molecular identification technology of DNA barcoding is an emerging molecular biotechnology in recent years, it is a powerful supplement to traditional identification methods. This method can well identify animal species at the molecular level and has high accuracy, it can identify animal medicines quickly and monitor the medicine market effectively. This article summarizes the research process of molecular identification of DNA barcoding, the application of DNA barcoding in medicinal animals identification in recent years, and the limitations of DNA barcoding technology.

RESUMO

OBJECTIVES: The purpose of this study was to determine the sonographic performance in pathologically proven cases of nasopharyngeal carcinoma and its involvement in the parapharyngeal space. METHODS: The study included 58 patients being treated for suspected nasopharyngeal carcinoma detected by routine nasopharyngoscopy who underwent pathologic biopsy. Sonography was performed immediately thereafter with a convex array transducer in both the B-mode and color mode. Forty-five of the 58 patients (90 parapharyngeal spaces) in whom nasopharyngeal carcinoma was proved by both sonography and pathologic biopsy underwent preradiotherapy magnetic resonance imaging (MRI). The sonographic findings were compared to the pathologic findings. The sonographic findings of parapharyngeal space involvement were correlated with the MRI findings. RESULTS: The normal anatomy of the nasopharynx and parapharyngeal space, nasopharyngeal carcinoma, and its relationship with the parapharyngeal space were well shown on sonography. The sensitivity of sonography for detection of nasopharyngeal carcinoma was 97.8%, and the specificity was 41.7%. The sonographic findings of parapharyngeal space involvement had a high degree of agreement with MRI (κ = 0.757; P < .001). CONCLUSIONS: These promising initial data indicate that sonography may be a useful tool for diagnosing nasopharyngeal carcinoma and defining the relationship between the tumor and the parapharyngeal space.
