RESUMO

Addition of immunomodulating therapy with Thymalin, a thymus agent, to the multimodality therapy of burnt patients contributes to a rapid normalization of immunological parameters. The most marked immune response was observed 7-10 days after a therapy course initiated in the early periods of burn disease in patients with severely depressed T-system immunity and a high sensitivity to the drug. Inclusion of a donor blood leukomass transfusion course activates the cellular link of the immune system just after the treatment course, but fails to favour a stable normalization of thymus-dependent lymphocyte ratios.

Wound pieces taken from 3-6 sites at a time during surgical treatment of the wound in 10 patients with thermal burns involving 25-50% of the body surface area, were examined by electron-microscopic radioautography and bacteriologically. The burn wounds were found to contain not only live (RNA-synthesizing) bacteria but also replicating (DNA-synthesizing) bacteria, and to be heterogeneous in terms of bacterial seeding (the largest numbers being present in the scab and the smallest, in the granulation tissue), but not of bacterial species; these were represented in all layers by mixed populations (Pseudomonas aeruginosa, pathogenic Staphylococcus, Proteus), although the deep layers contained more pathogenic and more virulent subpopulations than the more superficial ones. Septic patients had considerably more organisms in all layers of their wounds than nonseptic patients, and contained proliferating organisms in the granulation tissue. The main steps in the penetration of individual microbial species into the circulation were traced out by means of electron-microscopic radioautography.

The dynamics of burn wound healing was studied on biopsies from the wound area in 30 patients treated by open methods (treatment under conditions of abacterial environment and in a cabinet with laminar air). Twenty patients were treated by applications on the wound of ointment bandages. Under the bandages, the crust formed on the burn surface was infiltrated with polymorphonuclear leukocytes. When patients with burns over 10-25% of the body surface were treated in abacterial environment, a thin dry crust developed on the burn wound surface with small accumulations of microflora in its external layers. The granulation tissue filling the wound after early necrectomy contains numerous vessels and cell elements. In extensive burns (35-50% of the body surface) the granulation tissue fills the wound cavity incompletely, as separate islets.

RESUMO

A study of pyoimmunogen, a new immunological preparation recommended for the prophylaxis and treatment of Pseudomonas aeruginosa infection, was carried out. The immunization of 20 patients with traumas caused by burns showed that the preparation induced no postvaccinal reactions and had no pathological effect on the hemogram and blood proteins, while normalizing temperature and producing the statistically significant increase of the specific antibody titer. The injection of pyoimmunogen improved the general state of the patients and the state of the burn wounds and reduced the period of hospitalization 1.5 times.
