RESUMO

PURPOSE: To perform a comparative analysis of clinical and functional results and efficiency of corneal astigmatism correction after femtosecond laser-assisted cataract surgery (FLACS) with implantation of toric intraocular lens (TIOL) and in combination with arcuate keratotomy (FL-AC). MATERIAL AND METHODS: The examination included 60 patients (60 eyes). The first group consisted of 30 patients (30 eyes) who underwent FLACS with implantation of TIOL (Acrysof IQ Toric, Alcon, U.S.A.); the second group consisted of 30 patients (30 eyes) who underwent FLACS in combination with FL-AC. The examination was carried out before the operation, on the 3rd day and 3 months after surgery. Corneal astigmatism correction efficiency was analyzed using the Alpins method and graphical vector analysis. RESULTS: Uncorrected visual acuity (UCVA) indices increased to 0.70±0.24 and 0.66±0.31 in the FLACS with TIOL and FLACS with FL-AC groups, respectively, without statistically significant differences between the groups (p=0.03). The value of the residual cylinder was significantly lower in the FLACS with TIOL group (-0.77±0.58 and -0.80±0.39) compared with FLACS with FL-AC (-1.08±0.81 and -1.18±0.63) both on day 3 (p=0.02), and 3 months after surgery (p=0.02). In the FLACS with TIOL group, a higher correction index (0.99±0.18) and a smaller difference vector (0.77±0.64) were achieved compared to the FLACS with FL-AC group (0.78±0.35 and 1.05±0.76, respectively). Corneal higher-order aberrations (HOA) significantly differed on the 3rd day after surgery in the 3.0 mm (p=0.04) and 5.0 mm (p=0.04) zones between the groups. Endothelial cells density was 2460.50±328.40 and 2526.125±196.74 cl/mm2 after 3 months of observation in the FLACS with TIOL and FLACS with FL-AC groups, respectively (p=0.61). CONCLUSION: Both methods had comparable visual acuity results (p=0.03). FLACS with TIOL provided more effective correction of the cylindrical component of refraction to 3.0 D.

PURPOSE: To evaluate clinical results of posterior lamellar keratoplasty performed with Russian femtosecond laser «Vizum¼ for treating patients with primary and secondary corneal endothelial dystrophies. MATERIAL AND METHODS: The study included 30 operated eyes: 10 with Fuch's primary endothelial dystrophy of the cornea and 20 with secondary endothelial dystrophy. Patients were examined on the 7th day, as well as at 3, 6, 12 and 24 months after the surgery. All 28 patients (30 eyes) were followed-up for 12 months, 12 patients (14 eyes) were followed-up for 24 months. RESULTS: In 3 months after the surgery, transparent engraftment was observed in most cases. In 1 case, its primary failure was diagnosed and repeated keratoplasty was performed. Postoperative haze in host-donor interface was observed in 1 case. Central corneal thickness (CCT) was 554±43 µm. Transplant thickness in the central zone was 80±13 µm. The amount of eyes with Best Spectacle-Corrected Visual Acuity (BSCVA) of 0.3 was 43%. Endothelial cells density (ECD) was 1538±500 cells/mm2. Endothelial cells loss amounted to 30.4%. By the 12th month, recipient's cornea and the transplant remained transparent in all cases. Haze in the host-donor interface was still observed in 1 case. CCT was 581±47 µm, transplant thickness was 80±16 µm. The amount of eyes with BSCVA of 0.3 was 53%. Keratometry astigmatism was 2.08±1.47 D, refractive astigmatism - 1.72±0.83 D. ECD was 1324±383 cells/mm2. Endothelial cells loss amounted to 40.1%. At 24th months after the surgery (12 patients, 14 eyes) the recipient's cornea and the transplant remained clear in all cases. CCT was 591±48 µm, transplant thickness was 81±15 µm. Amount of eyes with BSCVA of 0.3 was 71.4%. ECD was 1426±422 cells/mm2. Endothelial cells loss - 35.1%. CONCLUSIONS: The predictability of applanation and high quality of cutting of the «Vizum¼ femtosecond laser assure the predictability of the process of preparing an ultrathin cornel graft from the endothelial side. Clinical results have demonstrated its effectiveness for rehabilitation of visual functions (up to BSCVA 1.0).

PURPOSE: To study the effect of insulin therapy on the concentration of vascular endothelial growth factor A (VEGF-A) in the intraocular fluid of rats with alloxan model of diabetes mellitus. MATERIAL AND METHODS: The experiment was conducted on 80 mongrel rats. In 65 rats, the alloxan model of diabetes mellitus was simulated by a single intraperitoneal injection of 100 mg/kg alloxan hydrate saluted in 0.4 ml of citrate buffer. 72 hours after intraperitoneal administration of alloxan monohydrate, these animals were divided into 2 groups. The main group (group 1) consisted of animals with alloxan model of diabetes mellitus, who started daily single intraperitoneal administration of prolonged-acting insulin at a therapeutic dose of 0.9 U/kg Body weight. The comparison group (group 2) consisted of animals with alloxan model of diabetes mellitus who did not receive specific therapy. 15 healthy rats constituted the control group (group 3). Experimental animals were removed from the study on day 31 of insulin therapy. The concentration of VEGF-A was determined in 80-90 µl of intraocular fluid collected from both eyes of each animal. RESULTS: In the main group, the median of VEGF-A concentration [25th; 75th percentiles] in the intraocular fluid was 140 [136; 210] pg/ml, which is 1.94 times higher than in the comparison group (72 [58; 86] pg/ml) and 1.84 times higher than in the control group (76 [62.5; 88] pg/ml). The concentration of VEGF-A in the intraocular fluid in the main group was statistically significantly higher, as compared with the comparison group (pm-u<0.0004), and compared with the control group (pm-u=0.0045). The comparison group had no statistically significant differences when compared with the control group (pm-u=0.9979). CONCLUSION: Insulin therapy for 31 days increases the concentration of VEGF-A in the intraocular fluid of rats with alloxan model of diabetes mellitus.

PURPOSE: To perform a comparative analysis of ultrasound and hydrodynamic parameters, and the outcomes of femtosecond laser-assisted cataract surgery (FLACS) and conventional phacoemulsification cataract surgery (CPCS). MATERIAL AND METHODS: This prospective cohort study included 246 eyes. The first group consisted of 138 patients who underwent FLACS, second group - 108 patients after CPCS. RESULTS: Total ultrasound time was 95.36±47.93 and 113.3±97.71 seconds (p=0.04) in the 1st and the 2nd groups, respectively. The duration of torsional ultrasound was 84.72±50.03 seconds in 1st group (p=0.04) and 113.3±97.71 seconds in the 2nd group (p=0.04). Aspiration time was 208.3±95.86 seconds in the 1st group (p=0.04) and 258.43±158.81 seconds in the 2nd group (p=0.04). On the 3-4th day after the surgery, uncorrected distance visual acuity (UDVA) and corrected distance visual acuity (CDVA) were significantly better in the 1st group - 0.62±0.21 (LogMAR 0.20±0.19) and 0.53±0.15 (LogMAR 0.30±0.21) (p=0.01), respectively, compared to the 2nd group with UDVA of 0.75±0.18 (LogMAR 0.10±0.16) and CDVA of 0.69±0.19 (LogMAR 0.20±0.15) (p=0.04). Central corneal thickness (CCT) was 573.41±33.12 and 632.43±58.30 µm in the 1st and 2nd groups, respectively (p=0.020). At 1 month post-op there were no statistically significant differences in UDVA (p=0.17), CDVA (p=0.40) or CCT (p=0.50) between the groups. CONCLUSION: Compared with CPCS, total ultrasound time (p=0.04), torsional ultrasound time (p=0.04) and aspiration time (p=0.04) were significantly lower in the FLACS group. CCT was significantly lower (p=0.02), while UDVA (p=0.04) and CDVA (p=0.01) were significantly higher in the FLACS group in the early post-operative period (3-4 days after surgery). There were no statistically significant differences in the parameters between the groups 1 month after the surgery.

PURPOSE: To analyze the state of cornea using 'Pentacam' device data before and five years after femtolaser-assisted in situ keratomileusis (FS-LASIK) in hyperopic children. MATERIAL AND METHODS: The study included 40 children aged 6-15 years examined before and five years after FS-LASIK. RESULTS: Central corneal thickness (CCT) amounted to 548.88±37.24 µm, at 6.5 mm zone - 631.58±48.23 µm, at 8 mm zone - 723.61±57.7 µm. Five years after the surgery corneal thickness at main influence zone (6.5-8.0 mm from the center of the cornea) decreased in average by 6%. Pachymetry at the center showed 545.06±48.11 µm, at 6.5 mm zone - 592.54±36.55 µm, at 8 mm zone - 683.54±50.89 µm. Mean posterior corneal elevation was +21±3.75 µm. Keratotopographic indices ISV and IVA increased 3.6 and 2.7 times compared to initial data and amounted to 83.86±30 mm and 0.58±0.4 mm respectively. Other indices were IHA - 22.8±9.82 µm, Rmin - 6.95±0.38 mm, KI - 1.1±0.09 and CKI - 1.05±0.03. CONCLUSION: Monitoring of the cornea state with 'Pentacam' before and 5 years after hyperopic FS-LASIK in children on the background of stable refraction revealed changes of keratotopographic indices associated to corneal thickness change in the laser ablation zone and the absence of pathologic posterior corneal surface elevation. These changes reflect the corneal profile after laser correction of hyperopia and should be perceived as topographic 'pseudo keratectasia'.

Studying morphological changes of cornea in the remote postoperative period is becoming increasingly relevant with refractive surgeries being adapted for children. PURPOSE: comparative analysis of keratocyte density and hystomorphologic changes of cornea after laser in situ keratomieleusis (LASIK) and femtosecond laser-assisted LASIK (FS-LASIK) in children with hyperopia. MATERIAL AND METHODS: 109 patients aged 6-17 years in 2 groups were examined with 'Confoscan-4' confocal microscope. RESULTS: In comparison with initial data, keratocyte density decreased in the immediate postoperative period after FS-LASIK and LASIK in average by 17.09%, 64.31% and 12.2% in the corneal flap, directly in the laser influence zone (interface) and in the retroablation zone respectively. After 5 years, keratocyte density decreased in the corneal flap, interface zone and retroablation zone by 12.01%, 48.71% and 8.06% respectively in comparison with initial data. A circular scar at the flap edge was left after FS-LASIK; keratocyte density in the corneal flap and in the interface zone was in average 8.9% and 15.28% higher respectively, and twice more subepithelial nerves were noted compared to LASIK. CONCLUSIONS: In the remote postoperative period keratocyte density in the corneal decreased in average by 43% after FS-LASIK and by 46% after LASIK in comparison with initial data. No changes were seen in the morphologic state of the cornea outside the influence zone. Both the technologies are safe, however keratocyte density and the number of subepithelial nerves are higher in the cornea flap and in the interface zone after FS-LASIK; corneal stability is assured by a circular scar formed at the flap edge.

AIM: to conduct a comparative experimental study of the effect of the use of intracorneal implants during femtosecond laser-assisted intrastromal pocket (IP) and intrastromal tunnel (IT) formation on the strength properties of the cornea. MATERIAL AND METHODS: The experimental study was performed on 24 corneas of 12 rabbit eyes enucleated 1 month after slaughtering. All eyes were divided into 6 groups according to the method of operation. IP and IT were created with IntraLase FS 60 kHz femtolaser. Group 1 (control group) consisted of rabbit eyes with intact corneas. In group 2 corneas, intrastromal tunnels were formed, in group 3 - intrastromal pockets, and in group 4 - intrastromal tunnels with 2 intracorneal segments (ICS). In groups 5 and 6, intrastromal pockets, each supplemented with an intrastromal ring, were created at the depth of 62-72% and 80% of pachymetry readings from the 5.0-mm optical zone, respectively. RESULTS: Higher voltage had to be applied to stretch the samples from group 2 as compared to those from groups 3 and 6. Distribution of the Young's modulus of the test corneas agreed with voltage distribution in the study groups. CONCLUSION: IP was associated with greater reduction in corneal biomechanical stability as compared with IT. Ring implantation into an IP yielded a more pronounced increase in the strength properties of the cornea than ICS implantation into an IT. The result was also more favorable at greater depths of intrastromal ring implantation.

AIM: to comparatively analyze corneal aberrations after intrastromal corneal segments (ICS) and MyoRing implantation using femtosecond laser in patients with stage II-III keratoconus (KC) by Pentacam examination. MATERIAL AND METHODS: We have operated on 135 patients (145 eyes) with stage II or III KC. Depending on surgical technique, all patients were divided into two groups. Group I consisted of patients with KC, who received optimized femtosecond laser-assisted MyoRing implantation, group II - of those, who received femtosecond laser-assisted ICS implantation. Depending on the stage of the disease, each group was further divided into two subgroups, for stage II and stage III KC, respectively. Before and after surgery, all patients underwent visometry, biomicroscopy, corneal topography, and corneal aberrations measurement under photopic and mesopic conditions on Pentacam. The follow-up period averaged 30 months (from 6 to 34 months). RESULTS: In both groups corneal aberrations significantly decreased after surgery, which was confirmed by an improvement in parameters of refraction and keratometry readings. CONCLUSION: Implantation of intrastromal MyoRings leads to a greater reduction in the total corneal aberration, higher-order aberrations under photopic and mesopic conditions, and spherical aberration under photopic conditions as compared to ICS in patients with stage III KC; a comparable reduction in the total corneal, higher-order, and spherical aberrations under photopic conditions as compared to ICS in patients with stage II KC; a comparable increase in spherical aberration under mesopic conditions as compared to ICS in patients with stage III KC.

AIM: to conduct a comparative analysis of the value of information provided by anterior segment optical coherence tomography (AS-OCT) and in vivo confocal laser scanning microscopy (CLSM) for identifying the palisades of Vogt (POV) in normal limbus. MATERIAL AND METHODS: POV were studied in 15 healthy participants (30 eyes) without any anterior segment disease. Two examination methods were used: AS-OCT En Face imaging (RTVue XR Avanti, 3D Cornea protocol) and CLSM (HRT3). Concordance of the results was then analyzed. RESULTS: Structural features of POV were distinguishable by both methods. CLSM was able to visualize POV in both the superior and inferior limbi of all the 15 patients (30 eyes). Within the inferior corneal hemisphere, POV had the appearance of well-differentiated hyperreflective double-contoured structures organized in parallel lines at the level of basal epithelium. The structure of the superior limbus, as judged from CLSM data, varied significantly: «classic¼ palisades were seen in only 13 eyes (43%), while in the other 17 cases (57%) POV were atypical. On OCT, POV structure was seen in detail in 29 eyes. By comparing the abilities of CLSM and OCT to visualize POV, a strong positive correlation (rS=0.99, p<0.05) was revealed between the results obtained in both the superior and inferior limbi. CONCLUSION: OCT data provide a detailed understanding of POV structure and strongly correlate with CLSM results. Taking into consideration the noninvasiveness, demonstrativeness, and repeatability of AS-OCT En Face imaging as well as the high value of provided information, the method cay be recommended as an alternative in POV diagnostics.

AIM: To perform a comparative analysis of the following cross-linking techniques: standard cross-linking (SCXL), local transepithelial femto cross-linking (LTF) and transepithelial intrastromal femto cross-linking with MyoRing implantation (TIF+MyoRing) in experimental animals and patients with progressive stage II--III keratoconus (KC), paying particular attention to the changes in biomechanical stability of the cornea. MATERIAL AND METHODS: The experimental series was performed on 20 eyes of 10 rabbits. The animals were divided into 4 groups of 5. Group 1 served as the control, group 2 consisted of animals that underwent SCXL, group 3 -- of those after LTF, and group 4 -- of those after TIF+MyoRing. Femto step was performed using the IntraLase FS 60 kHz femtosecond laser, cross-linking - with the Evolution machine. The follow-up period was 1 month. Clinical study enrolled 48 patients (45 eyes) with stage II-III KC. Depending on the surgical technique all the patients were divided into 3 groups. In group I (14 eyes, 9 patients), TIF+MyoRing was performed, in group II (16 eyes, 20 patients) --SCXL, in group III (15 eyes, 19 patients) -- LTF. The follow-up period was 6 months. RESULTS: The experiment showed a significant increase in rupture resistance of the cornea in all three groups. Six months after surgery, patients from group I demonstrated a 1.0±0.22 mmHg higher corneal resistance factor (CRF) and a 1.0±0.12 mmHg higher corneal hysteresis (CH). In group II, the said parameters increased by 0.8±0.10 mmHg and 0.6±0.16 mmHg, respectively; in group III -- by 0.8±0.25 mmHg and 0.6±0.26 mmHg, respectively. In neither group the density of endothelial cells has reduced significantly over the follow-up period. C-ONCLUSION: All three methods (LTF, TIF and SCXL) equally increase the biomechanical strength of the cornea and are safe. In patients with progressive stage II-III KC, TIF+MyoRing surgery has yielded an even higher improvement of biomechanical parameters in a 6-month follow-up period, which is due to the combination of mechanical and chemical mechanisms of corneal reinforcement.

Over the last decade, corneal collagen cross-linking (CXL) has become a conventional treatment method for progressive keratoconus. Laboratory studies have shown that CXL increases the diameter of collagen fibers and also the number of intra- and interfibrillar cross-links, thus, increasing biomechanical strength of the irradiated cornea. As confirmed by a series of clinical and randomized controlled trials, CXL is able to slow down and, perhaps, to stop the progression of keratoconus. In most post-CXL patients visual acuity improves, while keratometric readings, spherical equivalent, and higher order aberrations reduce. Although published results prove CXL effective in the treatment of progressive keratoconus, its late consequences are yet unknown. This article reviews the stages of CXL development and results of published experimental and clinical studies. Prospects for CXL modifications that do not require epithelial debridement are discussed.

RESUMO

The review discusses literature data on the treatment of keratoconus with intrastromal Myoring implantation into a corneal pocket, formed with a Pocket Maker microkeratome and a femtosecond laser alone and in combination with cross-linking of corneal collagen. The method shows high effectiveness as to refractive and functional outcomes and stabilization of keratoconus over the whole follow-up period (more than 3 years).

Experimental rationale is presented for novel model of artificial iris-lens diaphragm (ILD) that provides a better adaptation of diaphragm to anterior segment structures variable in size and reduction of pressure on ciliary body due to modifications in design of supporting elements. Presented model is good in self-centration, allows sutureless fixation in lens capsule or the other supporting structures in aniridia.

RESUMO

Results of 5 Akhmed valve implantations in patients with secondary glaucoma various time after artificial iridolenticular diaphragm implantation after concomitant iris and lens trauma were analyzed. Surgery allowed to achieve reliable IOP decrease when severe scar deformation of drainage zone of the anterior chamber prevents traditional hipotensive procedures because of their ineffectiveness. Besides visual acuity stabilization a safe performance of subsequent reconstructive procedures can become possible as well.

RESUMO

Due to original Russian technique of frontal photopolimerization of liquid oligomers in form the use of threefocal intraocular lense (IOL) with square relief of diffractive surface has become possible. The first 36 implantations of MIOL-Record showed good clinical and functional results. This lens provides high quality of intermediate vision without decrease of distant and near visual acuity. 94% of patients completely refused spectacle correction for all distances. The vast majority of patients are satisfied with their intermediate vision, 94% of them don't use spectacles for computer work. Incidence of disphotopsias is similar to that after other MIOLs implantation.

RESUMO

An original MIOL-23 multifocal elastic intraocular lens (IOL) was used to operate 5 eyes with acquired lens dislocation and traumatic cataract. By making self-sealing tunnel incision, ILO was implanted into the capsular sac and sutured to the iris. MIOL-23 implantation caused an increase in mean visual acuity. The IOL took up a correct position. Elastic IOL implantation with iris suture fixation is an efficient and safe mode of additional ILO support.

RESUMO

The effectiveness, safety, and stability of multimodality treatment for hyperopia, hyperopic and mixed astigmatism complicated by amblyopia and anisometropia were studied in 117 patients (117 eyes) aged 9 to 16 years, by using the new laser units "Lik-100" and "Glasser" at 1.54 microm. The patients were divided into 3 groups: 1) 43 patients (43 eyes) with hyperopia, spheric anisometropia and amblyopia; 2) 38 patients (38 eyes) with hyperopia, simple and complicated hyperopic astigmatism, astigmatic anisometropia, and amblyopia; 3) 36 patients (36 eyes) with hyperopia, simple and complicated hyperopic astigmatism, mixed anisometropia, and amblyopia. All the groups underwent multimodality treatment involving laser thermokeratoplasty and drug therapy for amblyopia. In children and adolescents, the refraction effect was 2.99 and 3.61 (mean 3.37 +/- 0.60) diopters, respectively. Astigmatism diminished by 2.01 diopters (63%) in children and by 2.62 diopters (79%) in adolescents (mean 2/35 diopters). The predictability of a refraction effect in the range of +/- 0.5 diopters averaged 77% in all the groups. Anisometropia diminished by an average of 2.88 +/- 0.8 diopters, which was 85% of the baseline data (the upper range of residual refraction was not more than 1.5 diopters. In all the groups, uncorrectable visual acuity increased by an average of 0.36 diopters (0.43 and 0.4 diopters in children and adolescents, respectively); correctable visual acuity increased by an average of 0.22 diopters (0.36 and 0.31 diopters in children and adolescents, respectively). Loss of correctable visual acuity lines did not greater than 2.7% (5 eyes). That of endothelial cells was not more than 6-8%. The angle of squint strabismus could be decreased or corrected in 79% after treatment. Binocular vision restored in 57%.

RESUMO

A new-design artificial iridolens stop (ILS) has been constructed, which is monolithic, elastic, universal by possible implantation modalities in relation to the degree of a damage to the anatomy of the anterior segment of the eye, adapted to surgery of minor incisions, and alleviating intra- and postoperative complications. The color spectrum of the haptic part of an ILS yields a good cosmetic effect. The ILS is made from a hydrophobic spatially sutured polymer based on polyoxypropylene. The experimental findings suggest that the proposed ILSs show a chemical and biomedical safety. The stops exert no locally irritant, sensitizing, and toxic effects, they are sterile, apyrogenic and meet the specifications of the items permanently contacting the internal environment of the eye.
