The urinary tract infection (UTI) is one of the most common bacterial infections in childhood. An adequate diagnosis is essential to be able to carry out a rational, efficient and effective treatment, however, there is great heterogeneity in diagnostic methods, specifically in the study of antimicrobial susceptibility. The aim of these recommendations is to provide tools to homogenize the diagnosis criteria, susceptibility study and antimicrobial treatment of urinary tract infection in the pediatric population, with a rational use of antibiotics approach. In the first part, the recommendations regarding diagnosis were presented, such as sampling and cut-off points, as well as microbiological considerations for susceptibility study and management of UTI in pediatrics. This second part details the management of complications, UTI in special situations, and pharmacokinetic and pharmacodynamic considerations of antimicrobials to be prescribed in UTI.

Resumen La infección del tracto urinario (ITU) es una de las infecciones bacterianas más frecuentes en la infancia. Un adecuado diagnóstico es esencial para poder realizar un tratamiento racional, eficiente y eficaz; sin embargo, existe gran heterogeneidad en los métodos diagnósticos, específicamente en el estudio de la susceptibilidad antimicrobiana. El objetivo de estas recomendaciones es entregar herramientas para uniformar los criterios diagnósticos, el estudio de susceptibilidad bacteriana in vitro y el tratamiento antimicrobiano de la ITU en la población pediátrica, con un enfoque de uso racional de los antimicrobianos. En esta primera parte, se presentan las recomendaciones en cuanto a cómo obtener una adecuada muestra de orina, el diagnóstico de laboratorio incluyendo puntos de corte -unidades formadoras de colonias/mL de orina-, además de consideraciones microbiológicas para el estudio de susceptibilidad y finalmente, el manejo de la ITU en pediatría. En la segunda parte se detalla el tratamiento antimicrobiano de sus complicaciones, el manejo de ITU en situaciones especiales y consideraciones farmacocinéticas y farmacodinámicas de los antimicrobianos a indicar en ITU.

OBJECTIVE: To describe the clinical and epidemiological characteristics of hospitalized children with multisystem inflammatory syndrome in children (MIS-C) in Santiago, Chile. METHODS: This was an observational study of children with MIS-C (May 1 to June 24, 2020), in three pediatric hospitals in Santiago. Demographic characteristics and epidemiological data, medical history, laboratory tests, cardiology evaluations, treatment, and clinical outcomes were analyzed. RESULTS: Twenty-seven patients were admitted (median age 6, range 0-14 years). Sixteen of the 27 (59%) required intensive care unit admission; there were no deaths. Seventy-four percent had no comorbidities, and the median number of days of symptoms before admission was 4 (range 2-9 days). Gastrointestinal symptoms were the most frequent, and inflammatory markers were increased at admission. A recent severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection was detected in 82% of cases. The severe group showed significantly lower hemoglobin and albumin levels, decreased platelet counts, and higher d-dimer during disease evolution. Echocardiography showed abnormalities (myocardial, pericardial, or coronary) in 12 patients (46%) during their hospital stay. Anti-inflammatory treatment (immunoglobulin and/or corticosteroids) was prescribed in 24 patients. MIS-C appeared in clusters weeks after the peak of SARS-CoV-2 cases, especially in the most vulnerable areas of Santiago. CONCLUSIONS: This study describes the first series (n = 27) of children with MIS-C in a Latin American country, showing favorable clinical outcomes. Education and alerts are required for clinical teams to establish an early diagnosis and prompt treatment.

Warfarin and acenocoumarol are used in various cardiovascular disorders to improve the prognosis of patients with thromboembolic disease. However, there is a lack of substantial efficacy and safety data on antithrombotic prophylaxis in several countries, particularly in Latin America. The aim of this study was to provide information about the efficacy of anticoagulants in Chilean patients. Data were collected from databases of the Western Metropolitan Health Service, Santiago, Chile. We identified 6280 records of patients receiving anticoagulant treatment. The three most common diagnoses were rhythm disorder (43.7%), venous thrombosis (22%), and valvular prosthesis (10.7%). The majority of patients (98.5%) received acenocoumarol while 1.5% of patients received warfarin, at weekly therapeutic doses of 13.6 mg and 30.4 mg, respectively. For total diagnoses, the median time in the therapeutic range was 50%. However, better results, 66.7%, were observed when a telemedicine strategy was used only in Santiago Province. Our findings emphasize that in Chile, where the number of patients receiving anticoagulant treatment increases every year, telemedicine, by committed teams, improves the use of oral anticoagulants and is able to increase quality indicators of anticoagulant treatment care.

Resumen Las quinolonas constituyen una familia de antimicrobianos de amplio uso y si bien son consideradas segura para los pacientes, el conocimiento del perfil de seguridad es necesario para que los profesionales estén alertas a lo que deben vigilar. Sobre el sistema músculo-esquelético, las quinolonas tienen el potencial de dañar cartílagos, provocando incluso muy excepcionalmente rotura de tendón. A nivel endocrino se ha observado hipoglicemia/hiperglicemia, por lo que en pacientes diabéticos se recomienda el control cuidadoso de la glicemia. Las reacciones adversas cardiovasculares son poco frecuentes, pero pueden ir desde alteraciones del ECG como prolongación del QT sin traducción clínica a graves arritmias que pueden ser de riesgo vital. En el sistema nervioso, destaca la aparición de alteraciones del sistema nervioso central y la neuropatía periférica. Durante la evaluación de la seguridad de las quinolonas es importante considerar las potenciales interacciones con otros medicamentos. En niños se prefiere no usar las fluoroquinolonas debido al potencial riesgo de daño a los cartílagos de crecimiento, efectos que no parecen ser tan dramáticos a la luz de la evidencia actual. A pesar del optimismo se debe evaluar la seguridad del tratamiento de estos antimicrobianos en todo paciente pediátrico.

In children it is preferred not to use fluoroquinolones because of the potential risk of cartilage damage and growth, effects that do not seem to be so dramatic in the face of new evidence. Despite optimism, the safety of the treatment of these antimicrobials should be evaluated in every pediatric patient.

INTRODUCTION: CMV pp65-antigenemia (antigenemia) has been used for monitoring CMV viremia in allogeneic hematopoietic stem cell transplant (aHSCT) recipients. Recently, real time quantitative PCR (RT-qPCR) has been used as a better approach than antigenemia for CMV diagnosis. The objective of this study was to assess the correlation of CMV viremia between RT-qPCR and antigenemia in aHSCT patients. MATERIAL AND METHODS: Observational prospective study of all aHSCT patients during 10 months in our center. CMV RT-qPCR in whole blood was performed weekly from day +7 to +100 after aHSCT. Simultaneous antigenemia was performed from engrafment to day +100. Concordance between both assays was evaluated. RESULTS: Eighteen patients were included. In 120 simultaneous samples, 96 were concordant by both methods (80%). Kappa coefficient was 0.583. In 42% of cases without concordant results, patients were on antiviral therapy. Thirteen patients (72%) developed CMV infection (20 episodes). In 17 episodes, both the antigenemia and CMV RT-qPCR were positive. CMV RT-qPCR was detectable 1-2 weeks before antigenemia in 45% of the episodes. CONCLUSION: Both methods had a moderate concordance and CMV RT-qPCR detects CMV reactivations earlier than antigenemia, especially in neutropenic patients.

Introducción: La antigenemia pp65 (antigenemia) ha sido utilizada para monitorizar viremia de citomegalovirus (CMV) en pacientes sometidos a trasplantes alogeneicos de precursores hematopoiéticos (TPHa). Recientemente, la reacción de polimerasa en cadena cuantitativa en tiempo real (en inglés RT-qPCR) se ha usado como una mejor aproximación al diagnóstico de infección por CMV. El objetivo de este estudio fue evaluar la correlación de viremia por CMV a través de RT-qPCR con antigenemia, en pacientes que han recibido TPHa. Material y Métodos: Estudio prospectivo, observacional, de los pacientes sometidos a TPHa durante 10 meses. Se realizó RT-qPCR de CMV en sangre total semanalmente desde el día +7 al+100 después del trasplante y antigenemia en forma simultánea desde el prendimiento hasta el día +100. Se evaluó la concordancia entre ambos ensayos. Resultados: Dieciocho pacientes fueron incluidos. En 120 muestras simultáneas, 96 fueron concordantes por ambos métodos (80%). El coeficiente Kappa fue 0,583. En los casos no concordantes, el 42% se encontraba en terapia antiviral. Trece pacientes (72%) desarrollaron infección por CMV (20 episodios). En 17 episodios, ambos ensayos fueron positivos. La carga viral fue detectable 1-2 semanas antes que la antigenemia en 45% de los episodios. Conclusión: Existe una buena correlación entre ambas técnicas y la RT-qPCR detecta más precozmente que la antigenemia las reactivaciones de CMV, especialmente en pacientes neutropénicos.
