BACKGROUND: Several strategies for identifying biologically implausible values in longitudinal anthropometric data have recently been proposed, but the suitability of these strategies for large population datasets needs to be better understood. This study evaluated the impact of removing population outliers and the additional value of identifying and removing longitudinal outliers on the trajectories of length/height and weight and on the prevalence of child growth indicators in a large longitudinal dataset of child growth data. METHODS: Length/height and weight measurements of children aged 0 to 59 months from the Brazilian Food and Nutrition Surveillance System were analyzed. Population outliers were identified using z-scores from the World Health Organization (WHO) growth charts. After identifying and removing population outliers, residuals from linear mixed-effects models were used to flag longitudinal outliers. The following cutoffs for residuals were tested to flag those: -3/+3, -4/+4, -5/+5, -6/+6. The selected child growth indicators included length/height-for-age z-scores and weight-for-age z-scores, classified according to the WHO charts. RESULTS: The dataset included 50,154,738 records from 10,775,496 children. Boys and girls had 5.74% and 5.31% of length/height and 5.19% and 4.74% of weight values flagged as population outliers, respectively. After removing those, the percentage of longitudinal outliers varied from 0.02% (<-6/>+6) to 1.47% (<-3/>+3) for length/height and from 0.07 to 1.44% for weight in boys. In girls, the percentage of longitudinal outliers varied from 0.01 to 1.50% for length/height and from 0.08 to 1.45% for weight. The initial removal of population outliers played the most substantial role in the growth trajectories as it was the first step in the cleaning process, while the additional removal of longitudinal outliers had lower influence on those, regardless of the cutoff adopted. The prevalence of the selected indicators were also affected by both population and longitudinal (to a lesser extent) outliers. CONCLUSIONS: Although both population and longitudinal outliers can detect biologically implausible values in child growth data, removing population outliers seemed more relevant in this large administrative dataset, especially in calculating summary statistics. However, both types of outliers need to be identified and removed for the proper evaluation of trajectories.

Introdução: O envelhecimento é um fenômeno natural no decorrer da vida e se altera de acordo com cada sujeito. Além das questões naturais do envelhecimento também deve-se considerar os fatores externos que influenciam na saúde da população, revelando-se os Determinantes Sociais de Saúde. Os idosos foram a primeira população elencada na faixa de risco para a COVID-19, e incentivados a realizar o distanciamento social. Estudos sobre as repercussões da pandemia do coronavírus ainda são escassos, pois trata-se de um fenômeno recente. Objetivo: Buscou-se verificar a percepção da saúde psíquica e ocupacional, através da autopercepção de idosos residentes do Município de Agudo, no Rio Grande do Sul, Brasil, baseando-se nos Determinantes Sociais de Saúde. Metodologia: Trata-se de um estudo transversal e analítico com abordagem quantitativa e amostragem por conveniência. Resultados: Participaram do estudo 40 idosos, que revelaram a grande prevalência de Doenças Crônicas Não Transmissíveis, relacionadas aos determinantes sociais de saúde intermediários. A maioria da população tinha como rotina a participação de grupos semanais. Esses grupos influenciavam diretamente na sua socialização, contribuindo para a melhora nas condições de saúde e, consequentemente, na qualidade de vida. Conclusão: Verificou-se a constância de respostas referenciando o distanciamento social como um fator que interferiu nas condições psíquicas dos idosos. Observou-se a ocorrência de limitação do processo de participação ocupacional, identificando-se a privação ocupacional no que tange aos desfechos ocupacionais da pesquisa. Apesar de contar com a avaliação subjetiva, observou-se que a maioria da população apresentou índices negativos quanto à autopercepção dos sentimentos despertados após o distanciamento das ocupações significativas. Introduction: Aging is a natural phenomenon that occurs throughout life and changes according to each subject. In addition to the natural issues of aging, external factors that influence the health of the population should also be considered, revealing the Social Determinants of Health. The elderly were the first population listed in the risk range for COVID-19, and were encouraged to practice social distancing. Studies on the repercussions of the coronavirus pandemic are still scarce, as it is a recent phenomenon. Objective: We sought to verify the perception of mental and occupational health, through the self-perception of elderly residents of the Municipallity of Agudo, Rio Grande do Sul, Brazil, and the Social Determinants of Health. Methodology: This is a cross-sectional and analytical study with a quantitative approach and convenience sampling. Results: Forty elderly people participated in the study, which revealed a high prevalence of Non-Communicable Chronic Diseases, related to intermediate social determinants of health. The majority of the population routinely participated in weekly groups, and these directly influenced their socialization, contributing to an improvement in their health conditions and, consequently, their quality of life. Conclusion: It was verified the constancy of answers that referred to social distancing as a factor that interfered in the psychic conditions of the elderly. It was observed that there was a limitation of the process of occupational participation, identifying occupational deprivation with regard to the occupational outcomes of the research. Despite relying on the subjective assessment, it was observed that the majority of the population had negative indices regarding the self-perception of feelings aroused after distancing from significant occupations.(AU)

Además de las cuestiones naturales del envejecimiento, también se deben considerar los factores externos que influyen en la salud de la población, revelando los Determinantes Sociales de la Salud. Los ancianos fueron la primera población incluida en el rango de riesgo de COVID-19 y se les alentó a practicar el distanciamiento social. Los estudios sobre las repercusiones de la pandemia del coronavirus son todavía escasos, por tratarse de un fenómeno reciente. Objetivo: Buscamos verificar la percepción de salud mental y ocupacional, a través de la autopercepción de ancianos residentes del Municipio de Agudo, Rio Grande do Sul, Brasil, revelando los Determinantes Sociales de la Salud . Metodología: Se trata de un estudio transversal y analítico con enfoque cuantitativo y muestreo por conveniencia. Resultados: Cuarenta ancianos participaron del estudio, que reveló alta prevalencia de Enfermedades Crónicas No Transmisibles, relacionadas con determinantes sociales intermedios de la salud. La mayoría de la población participaba rutinariamente en grupos semanales, y estos influían directamente en su socialización, contribuyendo a la mejora de sus condiciones de salud y, consecuentemente, de su calidad de vida. Conclusión: Se verificó la constancia de respuestas que se refirieron al distanciamiento social como un factor que interfiere en las condiciones psíquicas de los ancianos. Se observó que hubo una limitación del proceso de participación ocupacional, identificándose privación ocupacional con respecto a los resultados ocupacionales de la investigación. A pesar de basarse en la valoración subjetiva, se observó que la mayoría de la población presentaba índices negativos en cuanto a la autopercepción de los sentimientos suscitados tras el alejamiento de ocupaciones significativas.(AU)

OBJECTIVE: To evaluate the quality of anthropometric data of children recorded in the Food and Nutrition Surveillance System (SISVAN) from 2008 to 2017. METHOD: Descriptive study on the quality of anthropometric data of children under five years of age admitted in primary care services of the Unified Health System, from the individual databases of SISVAN. Data quality was annually assessed using the indicators: coverage, completeness, sex ratio, age distribution, weight and height digit preference, implausible z-score values, standard deviation, and normality of z-scores. RESULTS: In total, 73,745,023 records and 29,852,480 children were identified. Coverage increased from 17.7% in 2008 to 45.4% in 2017. Completeness of birth date, weight, and height corresponded to almost 100% in all years. The sex ratio was balanced and approximately similar to the expected ratio, ranging from 0.8 to 1. The age distribution revealed higher percentages of registrations from the ages of two to four years until mid-2015. A preference for terminal digits "zero" and "five" was identified among weight and height records. The percentages of implausible z-scores exceeded 1% for all anthropometric indices, with values decreasing from 2014 onwards. A high dispersion of z-scores, including standard deviations between 1.2 and 1.6, was identified mainly in the indices including height and in the records of children under two years of age and residents in the North, Northeast, and Midwest regions. The distribution of z-scores was symmetric for all indices and platykurtic for height/age and weight/age. CONCLUSIONS: The quality of SISVAN anthropometric data for children under five years of age has improved substantially between 2008 and 2017. Some indicators require attention, particularly for height measurements, whose quality was lower especially among groups more vulnerable to nutritional problems.

OBJECTIVE: To describe the time trends and socio-economic inequalities in infant and young child feeding practices in accordance with the Brazilian deprivation index (BDI). DESIGN: This time-series study analysed the prevalence of multiple breast-feeding and complementary feeding indicators based on data from the Brazilian Food and Nutrition Surveillance System, 2008-2019. Prais-Winsten regression models were used to analyse time trends. Annual percent change (APC) and 95 % CI were calculated. SETTING: Primary health care services, Brazil. PARTICIPANTS: Totally, 911 735 Brazilian children under 2 years old. RESULTS: Breast-feeding and complementary feeding practices differed between the extreme BDI quintiles. Overall, the results were more favourable in the municipalities with less deprivation (Q1). Improvements in some complementary feeding indicators were observed over time and evidenced such disparities: minimum dietary diversity (Q1: Δ 47·8-52·2 %, APC + 1·44, P = 0·006), minimum acceptable diet (Q1: Δ 34·5-40·5 %, APC + 5·17, P = 0·004) and consumption of meat and/or eggs (Q1: Δ 59·7-80·3 %, APC + 6·26, P < 0·001; and Q5: Δ 65·7-70·7 %, APC + 2·20, P = 0·041). Stable trends in exclusive breast-feeding and decreasing trends in the consumption of sweetened drinks and ultra-processed foods were also observed regardless the level of the deprivation. CONCLUSIONS: Improvements in some complementary food indicators were observed over time. However, the improvements were not equally distributed among the BDI quintiles, with children from the municipalities with less deprivation benefiting the most.

RESUMO OBJETIVOS Avaliar a qualidade dos dados antropométricos de crianças registradas no Sistema de Vigilância Alimentar e Nutricional (Sisvan) no período 2008-2017. MÉTODOS Estudo descritivo sobre a qualidade dos dados antropométricos de crianças menores de 5 anos atendidas nos serviços de atenção primária do Sistema Único de Saúde, a partir das bases de dados individuais do Sisvan. A qualidade dos dados foi avaliada anualmente por meio dos indicadores: cobertura, completude, razão entre sexos, distribuição da idade, preferência por dígitos de peso e estatura, valores de escore-z implausíveis, desvio-padrão e normalidade dos escores-z. RESULTADOS No total, 73.745.023 registros e 29.852.480 crianças foram identificados. A cobertura aumentou de 17,7% em 2008 para 45,4% em 2017. A completude da data de nascimento, peso e estatura correspondeu a quase 100% para todos os anos. A razão entre sexos foi equilibrada e aproximadamente similar a razão esperada, variando entre 0,8 e 1. A distribuição da idade revelou maiores percentuais de registros entre as idades de 2 a 4 anos até meados de 2015. Uma preferência pelos dígitos terminais "zero" e "cinco" foi identificada entre os registros de peso e estatura. As porcentagens de escores-z implausíveis excederam 1% para todos os índices antropométricos, com redução dos valores a partir de 2014. Uma alta dispersão dos escores-z, incluindo desvios-padrão entre 1,2 e 1,6, foi identificada principalmente nos índices incluindo estatura e nos registros de crianças menores de 2 anos e residentes das regiões Norte, Nordeste e Centro-Oeste. A distribuição dos escores-z foi simétrica para todos os índices e platicúrtica para estatura/idade e peso/idade. CONCLUSÕES A qualidade dos dados antropométricos do Sisvan para crianças menores de 5 anos melhorou substancialmente entre 2008 e 2017. Alguns indicadores requerem atenção, sobretudo para medidas de estatura, cuja qualidade foi principalmente inferior entre os grupos mais vulneráveis a agravos nutricionais.

RESUMO O objetivo do presente estudo foi mostrar a evolução do percentual de escolas aderidas ao Programa Saúde na Escola (PSE) que realizaram atividade coletiva sobre Promoção da Alimentação Adequada e Saudável (Paas) ao longo dos últimos cinco anos e verificar se há diferença desse indicador segundo adesão ao Programa Crescer Saudável (PCS). Trata-se de um estudo de delineamento ecológico, desenvolvido com dados cadastrados no Sistema de Informação em Saúde para a Atenção Básica (Sisab). Houve incremento na realização de ações coletivas sobre Paas durante o período de 2017 a 2019. Em 2020, houve redução das ações realizadas; e em 2021, houve incremento de ações realizadas em comparação com 2020. Os municípios que receberam recurso financeiro adicional por terem aderido ao PCS apresentaram os maiores percentuais de escolas que realizaram atividades coletivas sobre Paas. A realização das ações de Paas não ocorreu de forma homogênea segundo as macrorregiões brasileiras. Conclui-se que, nos últimos anos, houve o fortalecimento das ações de Paas no âmbito do PSE, com significativa redução dessa ação no contexto da pandemia da Covid-19. O recebimento do recurso adicional do PCS favoreceu o desenvolvimento das ações sobre Paas, o que reforça a importância do repasse financeiro regular.
ABSTRACT The study aims to show the evolution of the percentage of schools adhering to the School Health Program (PSE) that carried out a collective activity on the Promotion of Adequate and Healthy Eating (PAAS) over the last five years and verify if there is a difference in this indicator according to adhesion to the Healthy Growth Program (PCS). Ecological study, developed with data registered in the Health Information System for Primary Care (SISAB). The performance of collective actions on PAAS from 2017 to 2019 increased. In 2020, there was a reduction in the actions carried out; and in 2021 there was an increase in actions carried out compared to 2020. The municipalities that received additional funds from the PCS had the highest percentages of schools carrying out collective activities on PAAS. The implementation of PAAS actions did not occur homogeneously according to the Brazilian macro-regions. It is concluded that there has been a strengthening of PAAS actions within the scope of the PSE, with a significant reduction of this action in the context of the COVID-19 pandemic. Getting additional resources from the PCS favored the development of actions on PAAS, which reinforces the importance of regular financial transfers.
RESUMO
Responsive feeding is currently the recommended feeding style, in which the key principle is reciprocity between the child and the caregiver. The lack of reciprocity characterizes non-responsive feeding styles, which are associated with unhealthy eating behaviors, losses in the regulation of hunger and satiety, and childhood obesity. To assess factors associated with feeding styles between infants aged 6-12 months old, a cross-sectional study with 474 pairs of mothers and infants from 6 to 12 months old was conducted in Public Health Units in the Federal District, Brazil. The questionnaire applied to mothers evaluated four feeding styles from nine subconstructs (Infant Feeding Style Questionnaire), breastfeeding, bottle use, maternal depression and anxiety, household food insecurity (HFI), and prematurity. Multiple linear regression analysis was performed using the automated backward method. Bottle use was positively associated with feeding styles Laissez-faire (Attention) (ß:0.56), Pressuring (Cereal) (ß:0.58) and Restrictive (Quantity) (ß:0.26). Breastfeeding was negatively associated with Laissez-faire (Attention) (ß:-0.29) and Pressuring (Cereal) (ß:-0.50). Higher maternal education was negatively associated with Pressuring (Cereal) (ß:-0.34) and Pressuring (Soothing) (ß:-0.36). HFI is positively associated with Restrictive (Quantity) (ß:0.25), and Prematurity is negatively associated with Laissez-faire (Attention) (ß:-0.33). An important positive association was found between non-responsive styles and both bottle use and HFI, while breastfeeding and higher maternal education exhibited a negative association. Therefore, feeding styles and their associated factors should be considered in the design of interventions that promote a healthy diet for infants.

The Brazilian Early Childhood Friendly Municipal Index (IMAPI) is a population-based approach to monitor the nurturing care environment for early childhood development (ECD) using routine information system data. It is unknown whether IMAPI can be applied to document metropolitan urban territorial differences in nurturing care environments. We used Brasilia, Brazil's capital with a large metropolitan population of 2,881,854 inhabitants divided into 31 districts, as a case study to examine whether disaggregation of nurturing care data can inform a more equitable prioritization for ECD in metropolitan areas. IMAPI scores were estimated at the municipal level (IMAPI-M, 31 indicators) and at the district level (IMAPI-D, 29 indicators). We developed a quantitative prioritization process for indicators in each IMAPI analysis, and those selected were jointly mapped in the socioecological model for the role of indicators in relation to the enabling environment for nurturing care. Out of 28 common nurturing care indicators across IMAPI analysis, only four were prioritized in both analyses: one from the Adequate nutrition, two from the Opportunities for early learning, and one from the Responsive caregiving domains. These four indicators were mapped as enabling policies, supportive services, and caregivers' capabilities (socioecological model) and Effort, Coverage, and Quality (indicator's role). In conclusion, the different levels of nurturing care data disaggregation in the IMAPI can better inform decision-making than each one individually, especially in metropolitan areas where municipalities and districts within metropolitan areas have relative decision-making autonomy.

Selecting indicators to monitor nurturing care (NC) environments that support decision-making and guide the implementation of integrated early childhood development (ECD) programmes has become a priority globally. Several population-based approaches have been attempted to create a set of indicators or a composite index methodology to measure the NC environment using existing secondary data. However, they have not been systematized. Our scoping review aimed to analyse the population-based approaches for monitoring the domains of the NC (e.g. good health, adequate nutrition, responsive caregiving, security and safety, and opportunities for early learning). ECD experts, peer-reviewed, and grey literature were systematically searched with no year or language restrictions. Data extraction used a standard predefined protocol. Thirty-two population-based approaches were identified. Most approaches were composed of a set of indicators (53.1%, n = 17) versus composite indexes (46.9%, n = 15) and had the country as their unit of analysis (68.8%, n = 22). Twenty-seven approaches were applied in middle-income countries (84.4%) and thirteen in low-income countries (40.6%). Four approaches were guided by the NC framework (12.5%), and 56.3% (n = 18) did not include any indicator representing responsive caregiving. NC indicators (n = 867) were sorted into 100 groups of indicators. Twenty of the 32 approaches had some kind of methodological validation (62.5%). We identified six methodological challenges to build a population-based approach. Standardized methods for selecting and validating indicators, and coordinated efforts to share findings/data with stakeholders should be prioritized. Given the great variability in methods and indicators used to measure NC environments, valid approaches should be flexible to work well across different contexts.

The Nurturing Care Framework (NCF) calls for establishing a global monitoring and accountability systems for early childhood development (ECD). Major gaps to build low-cost and large-scale ECD monitoring systems at the local level remain. In this manuscript, we describe the process of selecting nurturing care indicators at the municipal level from existing routine information systems to develop the Brazilian Early Childhood Friendly Index (IMAPI). Three methodological steps developed through a participatory decision-making process were followed. First, a literature review identified potential indicators to translate the NCF domains. Four technical panels composed of stakeholders from federal, state and municipal levels were consulted to identify data sources, their availability at the municipal level and the strengths and weakness of each potential indicator. Second, national and international ECD experts participated in two surveys to score, following a SMART approach, the expected performance of each nurturing care indicator. This information was used to develop analytical weights for each indicator. Third, informed by strengths and weaknesses pointed out in the previous steps, the IMAPI team reached consensus on 31 nurturing care indicators across the five NCF domains (Good health [n = 14], Adequate nutrition [4], Responsive caregiving [1], Opportunities for early learning [7] and Security and safety [4]). IMAPI represents the first attempt to select nurturing care indicators at the municipal level using data from existing routine information systems.

Providing an enabling nurturing care environment for early childhood development (ECD) that cuts across the five domains of the Nurturing Care Framework (i.e., good health, adequate nutrition, opportunities for early learning, security and safety and responsive caregiving) has become a global priority. Brazil is home to approximately 18.5 million children under 5 years of age, of which 13% are at risk of poor development due to socio-economic inequalities. We explored whether the Early Childhood Friendly Municipal Index (IMAPI) can detect inequities in nurturing care ECD environments across the 5570 Brazilian municipalities. We examined the validity of the IMAPI scores and conducted descriptive analyses for assessing sociodemographic inequities by nurturing care domains and between and within regions. The strong correlations between school achievement (positive) and socially vulnerable children (negative) confirmed the IMAPI as a multidimensional nurturing care indicator. Low IMAPI scores were more frequent in the North (72.7%) and Northeast (63.3%) regions and in small (47.7%) and medium (43.3%) size municipalities. Conversely, high IMAPI scores were more frequent in the more prosperous South (52.9%) and Southeast (41.2%) regions and in metropolitan areas (41.2%). The security and safety domain had the lowest mean differences (MDs) among Brazilian regions (MD = 5) and population size (MD = 3). Between-region analyses confirmed inequities between the North/Northeast and South/Southeast. The biggest within-region inequity gaps were found in the Northeast (from -22 to 15) and the North (-21 to 19). The IMAPI distinguished the nurturing care ECD environments across Brazilian municipalities and can inform equitable and intersectoral multilevel decision making.

Third, informed by strengths and weaknesses pointed out in the previous steps, the IMAPI team reached consensus on 31 nurturing care indicators across the five NCF domains (Good health [n = 14], Adequate nutrition [4], Responsive caregiving [1], Opportunities for early learning [7] and Security and safety [4]). IMAPI represents the first attempt to select nurturing care indicators at the municipal level using data from existing routine information systems.

We cross-culturally adapted and validated the Infant Feeding Style Questionnaire (IFSQ) in Brazil. The cross-cultural adaptation and content validity assessment was conducted in five steps: translation, synthesis of translations, back-translation, evaluation by experts, and pre-test. To evaluate reliability, construct validity, and floor and ceiling effects, interviews were conducted with 465 mother-infant pairs at Primary Health Centers in the Federal District, Brazil. The mothers answered the Brazilian Portuguese version of the IFSQ (IFSQ-Br), which evaluated four feeding styles (laissez-faire, pressuring, restrictive, and responsive) from 9 sub-constructs. The indulgent style was not evaluated due to time limitation. We performed reliability analysis using Cronbach's alpha coefficient and construct validity was evaluated through Confirmatory Factor Analysis. Higher means were found in the sub-constructs of the responsive and restrictive styles. The IFSQ-Br presented adequate reliability (α = 0.73) with values for the Cronbach's alpha coefficient of the sub-constructs ranging from 0.42 to 0.75. In the Confirmatory Factor Analysis, the final models presented good fit, with the Comparative Fit Indices (CFI) ranging from 0.86 to 1.0 and the Root-Mean Squared Error of Approximation (RMSEA) between 0.0 and 0.09. The IFSQ-Br was shown to be a valid and reliable questionnaire to evaluate maternal feeding beliefs and behaviors in Brazil. Future studies should evaluate the psychometric properties of the indulgent style and include mother-infant pairs from different cultural contexts in Brazil.

OBJECTIVE: To evaluate the performance and perception of five models of front-of-package nutrition labeling (FOPNL) among Brazilian consumers. METHODS: Cross-sectional study, which applied an online questionnaire to 2,400 individuals, allocated randomly into six study groups: a control group and five others exposed to FOPNL (octagon, triangle, circle, magnifier and traffic light), applied to nine products. We evaluated the understanding of nutritional content, the perception of healthiness, the purchase intention and the perception of Brazilian consumers on the models. RESULTS: All FOPNL models increased the understanding of the nutritional content and reduced the perception of healthiness and purchase intention, when compared with the control group (41.3%). FOPNL warning models - octagon (62.4%), triangle (61.9%) and circle (61.8%) - performed significantly better than the traffic light (55.0%) regarding the understanding of the nutritional content. The performance of the magnifier (59.5%) was similar to the other four tested models, including the traffic light (55.0%), for understanding nutritional content. The individual analysis of the products suggests a better performance of warnings in relation to the magnifier and the traffic light for the perception of healthiness and purchase intention. Consumers were favorable to the presence of FOPNL, perceiving it as reliable to increase the understanding to nutritional information. CONCLUSION: FOPNL must be implemented on food labels in Brazil, considering that it increases the nutritional understanding, reduces the perception of healthiness and the purchase intention of products with critical nutrients. Warnings showed a better performance when compared with other models.

BACKGROUND: Optimal early child development (ECD) is fundamental for every child to achieve their developmental potential and thrive. Household food insecurity (HFI) is a risk factor for suboptimal development, but there is still a need to better understand the pathways that explain this relationship. OBJECTIVE: To evaluate whether maternal mental health and infant's diet mediate the association between HFI and ECD using structural equation modeling. METHODS: Cross-sectional study with 474 mother-infant dyads. Mothers answered a questionnaire that evaluated variables through validated instruments. Structural equation modeling analyses were conducted to obtain standardized effects (ß) and bias-corrected 95% CI, enabling comparisons between the magnitude of the effects. The following observed variables were included: HFI, ECD, consumption of a healthy and diverse diet, the presence of a partner in the household, wanted pregnancy, and current breastfeeding. Latent variables were included to evaluate mental health, poverty, and neonatal health. RESULTS: Poor maternal mental health mediated the relationship between HFI and ECD (ß = -.05), but a healthy and diverse diet was not a mediator in this relationship. Poverty was related to lower development scores directly (ß = .21) and indirectly (ß = .02). Not having a partner (ß = .05) and having an unwanted pregnancy (ß = .02) predicted indirectly lower development scores. CONCLUSIONS: Poor maternal mental health mediates the relationship between HFI and ECD, and ECD is influenced by poverty, single motherhood, and unwanted pregnancy through different pathways. Therefore, public policies and interventions that aim to promote an optimal ECD should also approach these aspects.

Objetivo: Comparar o tempo necessário para realizar escolhas alimentares com diferentes modelos de rotulagem nutricional frontal entre adultos brasileiros. Métodos: Foram realizadas entrevistas com 150 participantes, que foram orientados a escolher o produto mais saudável entre dois alimentos, sendo apresentados oito pares de imagens. Um dos produtos de cada par apresentava advertências (alto em açúcares, sódio ou gordura saturada), testados em formatos diferentes de acordo com o grupo no qual o indivíduo era alocado: lupa, octógono, círculo, triângulo e semáforo. Foram contabilizados o número de acertos do produto mais saudável em cada par e o tempo para escolha correta do produto mais saudável. Resultados: Observou-se menor número de acertos do produto mais saudável entre os pares com semáforo, em comparação com qualquer outro modelo (p<0,001), sendo este também o formato no qual se observou maior tempo necessário para escolher corretamente o produto mais saudável (p<0,001). Não foram observadas diferenças significativas entre os demais formatos. Na análise individual dos produtos, o círculo teve melhor desempenho, sendo observado tempo significativamente menor para escolhas corretas em sete dos oito produtos. Conclusão: Para apoiar escolhas alimentares saudáveis, indica-se o uso de qualquer um dos formatos testados para a rotulagem nutricional frontal, exceto o do semáforo. O círculo aparenta ser uma boa opção para reduzir o tempo do consumidor frente a escolhas saudáveis.
Objective: to compare the time Brazilian adults required to make food choices with different models of front-of-pack nutrition labeling. Methods: Interviews were conducted with 150 participants, who were instructed to select the healthiest product between two foods, with eight pairs of images presented. One of the products in each pair had warnings (high in sugars, sodium, or saturated fat), evaluated in different formats according to the group in which the individual was allocated: magnifying glass, octagon, circle, triangle, and traffic light. The number of correct answers for the healthiest product in each pair and the time to correctly select of the healthiest product were measured. Results: A lower number of correct answers for the healthier product among the pairs occurred with traffic lights than with any other model (p <0.001), which is also the format in which the longest time was necessary to correctly select the healthiest product (p < 0.001). There were no significant differences between the other formats. In the individual analysis of the products, the circle performed better, with significantly less time required to make the correct choices in seven of the eight products. Conclusion: To support healthy food choices, any of the tested formats for frontal nutrition labeling are indicated, except for the traffic light. The circle appears to be a good option to reduce consumer's time to select healthy choices.

RESUMO OBJETIVO: Avaliar o desempenho e a percepção de cinco modelos de rotulagem nutricional frontal (RNF) entre consumidores brasileiros. MÉTODOS: Estudo transversal, com aplicação de questionário online a 2.400 indivíduos, alocados de forma aleatória em seis grupos de estudo, sendo um controle e cinco expostos a modelos de RNF (octógono, triângulo, círculo, lupa e semáforo), aplicados a nove alimentos. Foi avaliado o entendimento do conteúdo nutricional, a percepção de saudabilidade, a intenção de compra e a percepção dos consumidores brasileiros sobre os modelos. RESULTADOS: Todos os modelos de RNF aumentaram o entendimento do conteúdo nutricional e reduziram a percepção de saudabilidade e a intenção de compra, quando comparados ao grupo controle (41,3%). Os modelos de RNF de advertência - octógono (62,4%), triângulo (61,9%) e círculo (61,8%) - apresentaram desempenho significativamente superior ao semáforo (55,0%) quanto ao entendimento do conteúdo nutricional. O desempenho do modelo da lupa (59,5%) não diferiu dos outros quatro modelos testados, inclusive do semáforo (55,0%), para o entendimento do conteúdo nutricional. A análise individual dos alimentos sugere melhor desempenho das advertências em relação à lupa e ao semáforo para a percepção de saudabilidade e a intenção de compra. Os consumidores manifestam-se favoráveis à presença da RNF, percebendo-a como confiável para aumentar o entendimento das informações nutricionais. CONCLUSÃO: A RNF deve ser implementada nos rótulos dos alimentos no Brasil, considerando que aumenta o entendimento nutricional, reduz a percepção de saudabilidade e a intenção de compra de alimentos com nutrientes críticos. As advertências apresentaram melhor desempenho quando comparadas aos demais modelos.

We examined the association between household food insecurity and early child development and whether or not maternal depression and anxiety modifies this association. The cross-sectional study included 468 mother-infant pairs recruited at primary health centers of the Federal District, Brazil. Mothers answered a questionnaire that evaluated early child development (outcome), household food insecurity (independent variable), maternal depression and trait anxiety (effect modifiers). Variables were collected with validated questionnaires for the Brazilian population. Pearson's χ2 test and logistic regression analyses were conducted. Infants who lived in a moderate or severe food insecure household had 2.52 times (95% confidence interval [CI] [1.13, 5.65]) the odds of having early child development delays compared with infants in secure households. Maternal depression and anxiety modified the strength of association between household food insecurity and early child development, which is an innovative finding. Among infants with depressed mothers, those experiencing mild (adjusted odds ratio [aOR] 3.33, 95% CI [1.17, 9.46]) and moderate/severe household food insecurity (aOR 10.13, 95% CI [2.18, 47.10]) had higher odds of having early child development delays, compared with infants in food secure households. Among infants with both anxious and depressed mothers, these associations were even stronger for mild (aOR 4.69, 95% CI [1.41, 15.59]) and moderate/severe household food insecurity (aOR 16.07, 95% CI [2.70, 95.66]). In conclusion, household food insecurity is a risk factor for early child development delays, and this association is modified by maternal depression and anxiety. Future studies should evaluate the impact of intervention packages that address maternal depression and anxiety and household food insecurity on preventing early child development delays.

ABSTRACT Objective To analyze the relationship between the maternal attitudes, beliefs and practices and the children's food consumption and nutritional status. Methods A cross-sectional study of 563 mother-child pairs. Mothers answered an online questionnaire that collected the following information: sociodemographic data, maternal nutritional status, the child's food consumption (evaluated through the Food Frequency Questionnaire and the School Child Diet Index), and maternal attitudes, beliefs and practices about the schoolchildren's eating (assessed through the Child Feeding Questionnaire). Children had their anthropometric measurements evaluated for the interpretation of the nutritional status. Results Mothers who considered their children to be bigger (ORaj=6.60) and who worried more about their children's weight (ORaj=3.47) presented greater chances of having overweight children. Mothers with a greater perception of responsibility (ORaj=0.57) and who put more pressure in their children to eat (ORaj=0.38) presented lower chances of having overweight children. The majority of children consumed high-quality food (77.4%). Mothers who more closely monitored their children's food consumption were 2.79 times more likely to have their children eating high-quality food. Conclusion Nutritional interventions should focus on stimulating the parents' sense of responsibility for the child's nutrition, as well as on promoting the frequent monitoring of the child's food intake.
RESUMO Objetivo Analisar a relação entre as atitudes, crenças e práticas maternas e o consumo alimentar e o estado nutricional da criança. Métodos Estudo transversal com 563 pares mães-crianças. Mães responderam questionário online que coletava as seguintes informações: dados sociodemográficos, estado nutricional materno, consumo alimentar da criança (avaliado a partir do Questionário de Frequência Alimentar e do Índice de Alimentação do Escolar), e atitudes, crenças e práticas maternas sobre a alimentação de crianças em idade escolar (avaliadas a partir do Questionário de Alimentação da Criança). As crianças tiveram suas medidas antropométricas aferidas para interpretação do estado nutricional. Resultados Mães que percebiam os corpos de suas crianças maiores (ORaj=6,60) e que se preocupavam mais com o peso da sua criança (ORaj=3,47) apresentavam maior chance de terem filhos com excesso de peso. Mães com maior percepção de responsabilidade (ORaj=0,57) e que pressionavam mais a criança para comer (ORaj=0,38) apresentavam menor chance de terem crianças com excesso de peso. A maioria das crianças possuía uma alimentação de alta qualidade (77,4%). Mães que monitoravam mais a alimentação de sua criança tiveram a chance 2,79 vezes maior de ter criança com alimentação de alta qualidade. Conclusão As intervenções nutricionais devem focar em estimular o senso de responsabilidade dos pais pela nutrição da criança, bem como em promover o monitoramento frequente do consumo alimentar da criança.

We investigated the prevalence of maternal dissatisfaction with their child's body size and its associated factors among mothers of first- to third-grade elementary school students in private schools in the Federal District, Brazil. This is a cross-sectional study with 548 mother-schoolchildren pairs. We measured children's weight and height, and collected their mother's sociodemographic data and Body Mass Index using an online questionnaire. We also verified maternal body dissatisfaction and maternal dissatisfaction with their child's body size using Shape Scales. Most mothers (50.5%) were dissatisfied with their child's body size. Mothers of boys (Adjusted OR = 2.85) were more likely to want a larger silhouette for their child, while mothers of girls (Adjusted OR = 3.18), overweight (Adjusted OR = 24.83) and obese (Adjusted OR = 189.86) children were more likely to want a thinner silhouette for their child. A positive correlation was observed between maternal dissatisfaction with their own body and maternal dissatisfaction with their children's body size (rs = 0.178). There was a high prevalence of maternal dissatisfaction with their child's body size, particularly among mothers of overweight and obese children. Additional studies should be conducted to better understand the influence of this dissatisfaction on maternal practices and attitudes related to their child's body, food consumption, and lifestyle.
