In this work, we investigate the correlation between ragweed pollen concentration and conjunctival, nasal, and asthma symptom severity in patients allergic to ragweed pollen using ambient pollen exposure in the Milan area during the 2014 ragweed season We calculate the pollen/symptom thresholds and we assess the effectiveness of ragweed allergen immunotherapy (AIT). A total of 66 participants allergic to ragweed (Amb a 1) were enrolled in the study and divided into two groups: AIT treated (24) and no AIT treated (42). Pollen counts and daily symptom/medication patient diaries were kept. Autoregressive distributed lag models were used to develop predictive models of daily symptoms and evaluate the short-term effects of temporal variations in pollen concentration on the onset of symptoms. We found significant correlations between ragweed pollen load and the intensity of symptoms for all three symptom categories, both in no AIT treated (τ = 0.341, 0.352, and 0.721; and ρ = 0.48, 0.432, and 0.881; p-value < 0.001) and in AIT treated patients ([Formula: see text]= 0.46, 0.610, and 0.66; and ρ = 0.692, 0.805, and 0.824; p-value < 0.001). In both groups, we observed a positive correlation between the number of symptoms reported and drug use. Mean symptom levels were significantly higher in no AIT treated than in AIT treated patients (p-value < 0.001) for all symptom categories. Pollen concentration thresholds for the four symptom severity levels (low, medium-low, medium-high and high) were calculated. Ragweed pollen concentration is predictive of symptom severity in patients with a ragweed (Amb a 1) allergy. Patients treated with AIT had significantly reduced mean symptom levels compared to those without AIT.

On 4 November 2020 the Italian government introduced a new policy to address the second wave of COVID-19. Based on a battery of indicators, the 21 administrative regions of Italy were assigned a risk level among yellow, orange, red, and, starting on 6 November 2020, different type of restrictions were applied accordingly. This event represents a natural experiment that allows the evaluation of the effects of non-pharmaceutical interventions, free from those nuisance factors affecting cross-national studies. In this work, we extract the daily growth rate of new cases, hospitalizations and patients in ICU from official data using an unobserved components model and assess how the different restrictions had different impacts in reducing the speed of spread of the virus. We find that all the three packages of restrictions have an effect on the speed of spread of the disease, but while the mildest (yellow) policy leads to a constant number of hospitalizations (zero growth rate), the strictest (red) policy is able to halve the number of accesses to regular wards and intensive care units in about one month. The effects of the intermediate (orange) policy are more volatile and seem to be only slightly more effective than the milder (yellow) policy.

Most urban areas of the Po basin in the North of Italy are persistently affected by poor air quality and difficulty in disposing of airborne pollutants. In this context, the municipality of Milan started a multi-year progressive policy based on an extended limited traffic zone (Area B). Starting on 25 February 2019, the first phase partially restricted the circulation of some classes of highly polluting vehicles on the territory, in particular, Euro 0 petrol vehicles and Euro 0 to 3 diesel vehicles, excluding public transport. This is the early-stage of a long term policy that will restrict access to an increasing number of vehicles. The goal of this paper is to evaluate the early-stage impact of this policy on two specific vehicle-generated pollutants: total nitrogen oxides (NO x ) and nitrogen dioxide (NO 2 ), which are gathered by Lombardy Regional Agency for Environmental Protection (ARPA Lombardia). We use a statistical model for time series intervention analysis based on unobservable components. We use data from 2014 to 2018 for pre-policy model selection and the relatively short period up to September 2019 for early-stage policy assessment. We include weather conditions, socio-economic factors, and a counter-factual, given by the concentration of the same pollutant in other important neighbouring cities. Although the average concentrations reduced after the policy introduction, this paper argues that this could be due to other factors. Considering that the short time window may be not long enough for social adaptation to the new rules, our model does not provide statistical evidence of a positive policy effect for NO x and NO 2 . Instead, in one of the most central monitoring stations, a significant negative impact is found.

Poker playing and responsible gambling both entail the use of the executive functions (EF), which are higher-level cognitive abilities. This study investigated if online poker players of different ability showed different performances in their EF and if so, which functions were the most discriminating for their playing ability. Furthermore, it assessed if the EF performance was correlated to the quality of gambling, according to self-reported questionnaires (PGSI, SOGS, GRCS). Three poker experts evaluated anonymized poker hand history files and, then, a trained professional administered an extensive neuropsychological test battery. Data analysis determined which variables of the tests correlated with poker ability and gambling quality scores. The highest correlations between EF test results and poker ability and between EF test results and gambling quality assessment showed that mostly different clusters of executive functions characterize the profile of the strong(er) poker player and those ones of the problem gamblers (PGSI and SOGS) and the one of the cognitions related to gambling (GRCS). Taking into consideration only the variables overlapping between PGSI and SOGS, we found some key predictive factors for a more risky and harmful online poker playing: a lower performance in the emotional intelligence competences (Emotional Quotient inventory Short) and, in particular, those grouped in the Intrapersonal scale (emotional self-awareness, assertiveness, self-regard, independence and self-actualization).

The aim of this article was to evaluate the time course of paroxysmal positioning vertigo (PPV) and to investigate correlations with environmental and seasonal factors through a retrospective statistical analysis spanning 4 years (2001-2004). Applying rigorous diagnostic criteria, we selected 575 patients (429 women and 146 men; age range, 17-94 years; mean age, 55 years for men and 56 years for women). Statistical analysis included events per month and per year. We conducted a descriptive statistical analysis to investigate the correlation between vertigo events and main environmental factors: air pollution as expressed by daily concentration of nitric monoxide and ultrafine particles; air pressure; mean temperature and sun radiation; and humidity. We referred the environmental data, collected by Regione Lombardia (the regional government of Lombardy), to the greater Milan homogeneous area. We performed an analysis of variance test and observed that PPV is more frequent in middle-aged women (in or around their fifties) and on the right side. PPV is clearly negatively correlated with temperature, and frequency of attacks depends on temperature variations. The role of air pollution, especially particles, is suspected, but it is not yet clearly identified. Factors that link climate and otoconia metabolism require further investigation.

BACKGROUND: The primary objective of this study was to make the first step in the modelling of pharmaceutical demand in Italy, by deriving a weighted capitation model to account for demographic differences among general practices. The experimental model was called ASSET (Age/Sex Standardised Estimates of Treatment). METHODS AND MAJOR FINDINGS: Individual prescription costs and demographic data referred to 3,175,691 Italian subjects and were collected directly from three Regional Health Authorities over the 12-month period between October 2004 and September 2005. The mean annual prescription cost per individual was similar for males (196.13 euro) and females (195.12 euro). After 65 years of age, the mean prescribing costs for males were significantly higher than females. On average, costs for a 75-year-old subject would be 12 times the costs for a 25-34 year-old subject if male, 8 times if female. Subjects over 65 years of age (22% of total population) accounted for 56% of total prescribing costs. The weightings explained approximately 90% of the evolution of total prescribing costs, in spite of the pricing and reimbursement turbulences affecting Italy in the 2000-2005 period. The ASSET weightings were able to explain only about 25% of the variation in prescribing costs among individuals. CONCLUSIONS: If mainly idiosyncratic prescribing by general practitioners causes the unexplained variations, the introduction of capitation-based budgets would gradually move practices with high prescribing costs towards the national average. It is also possible, though, that the unexplained individual variation in prescribing costs is the result of differences in the clinical characteristics or socio-economic conditions of practice populations. If this is the case, capitation-based budgets may lead to unfair distribution of resources. The ASSET age/sex weightings should be used as a guide, not as the ultimate determinant, for an equitable allocation of prescribing resources to regional authorities and general practices.
