[Changes of the thymus gland in patients operated for myasthenia in dependence on the blood grouping and Rh-factor].

In 1974-2001 yrs there were examined 250 patients with various forms of myasthenia, of them in 95 (38%) the dependence of thymus gland changes from the blood grouping and Rh-factor presence was studied. The blood group in 39 (41%) of patients was A (II) Rh+ and in 22% - 0 (I).

[Application of videothoracoscopy in surgical treatment of the anterior mediastinum diseases].

In 1994-2000 yrs there were observed 248 patients with various mediastinal diseases, including 105--with primary diagnosis: mediastinal tumor. In 102 (41.8%) patients operative intervention was performed using videothoracoscopy method, in 146--conventional methods: left-sided partial oblique sternotomy, longitudinal and longitudinal-transversal sternotomy, thoracotomy. Indications and contraindications for performance of operations on the anterior mediastinum organs, using videothoracoscopy, were determined for the first time.

[Activity of the thyroid and sex hormones in patients with myasthenia after surgical intervention].

In 1995-2003 yrs there were operated on 55 women for various forms of myasthenia. In 20 of them myasthenia coexisted with thymoma, in 35--have been combined with the thymus hyperplasia. In all the patients the hormonal misbalance was revealed, depending on changes in the thymus structure present.

In 1998-2003 yrs in the clinic there were treated 360 patients with the thyroid gland diseases, 147 (40.8%) of them were operated. In 19 (12.9%) of patients various kinds of complications had occurred.

In clinic of thoracic surgery were operated on 2108 patients for various forms of pulmonary cancer, 218 (10.3%) of them--using videothoracoscopy (VTHS). In 141 (64.7%) patients the method of VTHS was applied for diagnostical purposes and in 55 (25.2%)--for treatment. Histological investigations of excised specimen showed planocellular cancer in 38 patients, adenocarcinoma--in 9, nonsmall cell carcinoma--in 6, anaplastic cancer--in 2. The operation cost while applying VTHS had constituted at average 648.65 US dollars, of thoracotomy--1283.5 US dollars.

[Five-year experience of surgical treatment of the thyroid gland tumors in regional oncological dyspensary]. / Piatiletnii opyt khirurgicheskogo lecheniia opukholei shchitovidnoi zhelezy v usloviiakh oblastnogo onkologicheskogo.

In 1998-2003 in dispensary there were treated 39,085 patients for different oncologic diseases, of them 360 (0.92%)--for diseases of the thyroid gland. There were 147 (40.8%) of patients operated on.

[Application of video-thoracoscopy in surgical treatment of myasthenia gravis]. / Primenenie videotorakoskopii v khirurgicheskom lechenii miastenii.

In 39 patients with various forms of myasthenia operation thymectomy was performed, including those 9 (20.5%), to whom it was done using method of videothoracoscopy. There were no postoperative complications. Good cosmetic effect was noted, the duration of hospital treatment of the patients had reduced more than twice, causing economic effect.

From 1992 to 2000 yr 79 patients were operated on for the thymoma, including I-II stage--33.3%, III stage--41.7%, IV stage--25%. Completed course chemotherapy was conducted in 12 (15.2%) patients, index of five-year survival in I-II stage had constituted 94%, in III stage--85% and IV stage--32%.

Of 4715 patient, who were present in 1995-2000 yrs in the clinic, in 708 (15%) operation, using videothoracoscopy method, was performed. In 539 (76.1%) of patients the method was applied for diagnosis and in 169 (23.9%)--for treatment. After the operation 365 patients were observed, complications have occurred in 14, including the early--in 10 (2.7%) and the late--in 4 (1.1%). All the patients have survived.

Analysis of results of surgical treatment of 72 patients with thymoma, including 49 in combination with clinical signs of myasthenia and 23 without them, is presented. In group of patients with symptoms of myasthenia the women aged from 30 to 39 yrs prevailed and among those who had not--the men, aged from 40 to 49 yrs. Histologic investigation in 29 (59.1%) of patients with symptoms of myasthenia showed lymphocytic thymoma, in 10 (43.4%) of patients without signs of myasthenia--benign thymoma was revealed.

For period from 1992 to 2000 years 72 patients with thymoma were operated. In 29 (40.2%) patients lymphocytic thymoma was revealed, in 10 (13.8%)--"benign". Postoperative mortality was 4.1%. In term from 1 to 1.5 years 5 (6.9%) patients died.

In 1995-2000 yrs 480 patients were operated on in the clinic using videothoracoscopy (VTHS). In 45 of them various forms of myasthenia were diagnosed. In 9 (20%) patients thymectomy was performed. There were no complications. The VTHS application had permitted to shorten the stationary treatment of the patients twice and to reduce the analgetics dosage in postoperative period by 2-2.5 times.

Histological investigation of thymic gland was performed in 155 patients with various forms of myasthenia. Two variants of histological changes in thymic gland were revealed. In presence of the first variant performance of thymectomy was effective in 81.3% and in presence of the second--in 51%.
