RESUMO

Binding of a panel of eight monoclonal antibodies (mAbs) with the C domain of angiotensin converting enzyme (ACE) to human testicular ACE (tACE) (corresponding to the C domain of the somatic enzyme) was studied and the inhibition of the enzyme by the mAb 4E3 was found. The dissociation constants of complexes of two mAbs, IB8 and 2H9, with tACE were 2.3 +/- 0.4 and 2.5 +/- 0.4 nM, respectively, for recombinant tACE and 1.6 +/- 0.3 nM for spermatozoid tACE. Competition parameters of mAb binding with tACE were obtained and analyzed. As a result, the eight mAbs were divided into three groups, whose binding epitopes did not overlap: (1) 1E10, 2B11, 2H9, 3F11, and 4E3; (2) 1B8 and 3F10; and (3) IB3. A diagram demonstrating mAb competitive binding with tACE was proposed. Comparative analysis of mAb binding to human and chimpanzee ACE was carried out, which resulted in revealing of two amino acid residues, Lys677 and Pro730, responsible for binding of three antibodies, 1E10, 1B8, and 3F10. It was found by mutation of Asp616 located close to Lys677 that the mAb binding epitope 1E10 contains Asp616 and Lys677, whereas mAbs 1B8 and 3F10 contain Pro730.

RESUMO

The paper presents the results of clinical and labour examinations of 51 drivers with cardiac arrhythmia. Long-term postinfarctional ECG monitoring was attempted in 30 drivers in the course of driving. The frequency of arrhythmia episodes correlated with the presence of extrasystole in acute and subacute period of the disease, anginal functional class, the size and time of myocardial infarction. Single episodes of ventricular extrasystole registered in the course of driving did not interfere with professional activities because such extrasystoles did not imply the risk of more severe arrhythmia. Labour prognosis in drivers with frequent, polytopic ventricular extrasystoles or fibrillation was much worse.

RESUMO

When planning the average number of bed occupancy days per year at a hospital providing emergency hospitalization one should take into account the demurrage of reserve beds which are needed for urgent hospitalization of patients. The influence of emergency demurrage of reserve beds on occupancy rate is not determined by the absolute number of these beds and their share in the structure of hospital bed fund. The number of reserve beds depends on the number of emergency patients hospitalized and the average length of hospital stay.

Cases of late applications for medical aid and refusals from hospital admission depend on the multiplicity of factors: the nature of pathological process, personal peculiarities of patients, sex, age and participation in the working process that determine their social status. As severity of disease increases, the role of other factors diminishes.

RESUMO

Cases of sudden death most frequent among coronary patients are becoming more common in the practice of therapeutists accounting for the one-fourth of the overall death rate in adult population. Most cases of sudden death take place outside hospital setting and terminate rapidly hindering effective resuscitation. Annual routine mass health examinations in accordance with current instructions fail to detect a great number of individuals at high risk for sudden death. This urges on further studies in this field aimed at development and introduction into wide outpatient practice of up-to-date methods of effective prevention of sudden death.
