Biochar aging may affect the interactions of biochar with water and thus its performance as soil amendment; yet the specific mechanisms underlying these effects are poorly understood. By means of FTIR, N2 adsorption, Hg intrusion porosimetry, thermogravimetric analysis, 13C solid state nuclear magnetic resonance (NMR) and 1H NMR relaxometry, we investigated changes in the chemistry and structure of biochar as well as its interaction with water after biochar aging, both physical (simulated by ball-milling) and biochemical (simulated by co-composting). Three different porosities of biochar were examined: <5 nm, 1 µm and 10 µm diameter sizes. Physical aging caused the disappearance of the porosity at 10 µm. With biochemical aging, biochar underwent an enrichment of oxygenated functional groups either as a result of surface functionalisation processes or by the deposition of fresh organic matter layers on the surface and pores of biochar. 1H NMR relaxometry revealed that the proportion of water strongly interacting with biochar increased with both physical and biochemical aging. Although biochemical aging significantly altered the composition of biochar surface and modulates its interaction with water, 1H NMR relaxometry proved that physical aging had a relatively stronger influence on water mobility and dynamics in biochar, lowering both T1 and T2 relaxation times in the initial contact times of biochar and water.

Wolfram syndrome 1, a rare autosomal recessive neurodegenerative disease, is caused by mutations in the WFS1 gene. It is characterized by diabetes insipidus, diabetes mellitus, optic atrophy, and deafness (DIDMOAD), and other clinical manifestations such as urological and neurological disorders. Here we described the case of a patient with an atypical late-onset Wolfram syndrome 1 without DI. Our WS1 patient was a c.1620_1622delGTG (p.Trp540del)/c.124 C > T (p.Arg42*) heterozygous compound. The p.Arg42* nonsense mutation was also found in heterozygosity in his sister and niece, both suffering from psychiatric disorders. The p.Arg42* nonsense mutation has never been found in WS1 and its pathogenicity is unclear so far. Our study underlined the need to study a greater number of WS1 cases in order to better understand the clinical significance of many WFS1 variants.

Hemp (Cannabis sativa L.) seedlings were used to remove from water the fungicide metalaxyl-M and the endocrine disruptor (EDC) bisphenol A (BPA) at concentrations ranging from 2 to 100 µg mL-1. In 7 days of exposure, despite the phytotoxicity of each compound that reduced elongation and biomass, the seedlings were able to remove between 67 and 94% of metalaxyl-M and between 86 and 95% of BPA. The amounts of metalaxyl-M and BPA extracted from plant dry biomass were in the range of 106-3861 µg g-1 and 16-101 µg g-1, respectively, and resulted positively correlated to both the dose of compound added (P ≤ 0.01) and the amount removed by the plants (P ≤ 0.01). Plant uptake and transformation were the main mechanisms involved in the removal of the compounds. In another set of experiments, hemp was used to remove a mixture of two pesticides, metalaxyl-M and metribuzin, and three EDCs, BPA, 17ß-estradiol (E2), and 4-tert-octylphenol (OP), at concentrations of 10, 10, 10, 10, and 1 µg g-1, respectively, from soil column not added and added with 2.5% (w/w) of a green compost (CM) or a wood biochar (BC). In 25 days, plants did not alter considerably the distribution of the compounds along the soil profile and were capable of removing, on average, 12, 11, 10, 9, and 14% of metalaxyl-M, metribuzin, BPA, E2, and OP, respectively. During growth, hemp transformed the compounds and accumulated part of them (except OP) mainly in the shoots. CM and, especially, BC significantly protected the plants from the toxicity of the compounds and enhanced the retention of the latter in soil, contrasting leaching. Thus, the single or synergistic use of hemp and amendments deserves attention being a very low-cost and eco-sustainable strategy to remediate water and soil.

AIMS: Diabetic retinopathy remains asymptomatic until its late stages but remains a leading cause of vision impairment and blindness. We studied quality of life and the ability to deal with the discomfort deriving from the presence of a chronic disease in patients with type 1 diabetes and different stages of retinopathy. METHODS: Multicenter collaborative observational study involving nine centers screening for retinopathy in different areas of Italy. The National Eye Institute 25-item visual functioning questionnaire and the locus of control tool were administered to 449 people with type 1 diabetes between February 2016 and March 2018. Socio-demographic and clinical data were collected. RESULTS: On multivariable analysis, severe retinopathy is associated with worse scores for general vision, ocular pain, near vision activities, distance vision activities, driving, color vision, peripheral vision and lower values of internal control, independently of visual acuity. Women had a perception of worse general health, distance vision activities and driving, and lower internal control and trust in others. Worse scores for visual-specific social functioning, visual-specific mental health, visual-specific role difficulties, visual-specific dependency and peripheral vision were associated with higher HbA1c levels. Fatalism increased with rising HbA1c levels. CONCLUSIONS: These results confirm that a gap exists between patients' knowledge and expectations on retinopathy and providers' expertise and assumptions. To bridge this gap, patient-centered education and engaging approaches may be more effective than simple information given during consultations.

AIMS: This study aimed to investigate the relationship between cardio-ankle vascular index (CAVI) and diabetic retinopathy (DR) in Caucasian patients with type 2 Diabetes Mellitus (T2DM). METHODS: This was a cross-sectional study of 299 T2DM patients admitted to Endocrine Unit of Foggia. DR was diagnosed using the International Clinical Disease Severity Scale of American Academy of Ophthalmology. The VaSera VS-1500N was used to measure CAVI. Because age is the most powerful determinant of arterial stiffness and affects the progression of DR, we divided the whole sample into two subgroups: above (older) and below (younger) 70 years. RESULTS: The mean age of patients was 60.4 ±â€¯12.6 years and the mean CAVI value was 8.6 ±â€¯1.7. In the whole population DR was diagnosed in 74 (24.7%) patients. CAVI value was clearly higher in patients with DR (9.5 ±â€¯1.6) than in those without (8.7 ±â€¯1.7) (P = 0.001) although this difference was not any more significant when adjusted by age and gender (P = 0.067). In the multivariate model taking into account several possible confounders, the correlation between DR and CAVI remained significant only in younger subjects. In the same subgroup we found a significant association between the stages of DR and CAVI (p = 0.019 adjusted by age and gender). CONCLUSIONS: This study shows that CAVI is significantly higher in younger patients with DR than in those without, with a relationship between the stages of DR and CAVI in the same subgroup. Physicians should pay attention to sub-clinical macroangiopathy in younger T2DM patients who have DR.

OBJECTIVE: Primary pigmented nodular adrenocortical disease (PPNAD), an uncommon cause of Cushing's syndrome, is frequently associated with a wider clinical spectrum, the Carney complex (CC), a multiple endocrine neoplasia syndrome. DESIGN: We evaluated a low-dose mitotane regimen for treating severe hypercortisolism in a 27-year-old woman with CC. She presented with severe hypercortisolism and a history of surgeries for breast ductal adenoma, atrial cardiac myxomas with cerebral and peripheral arterial embolism, and near-total thyroidectomy because of an oxyphilic adenoma. The patient refused further surgery for adrenalectomy. RESULTS: During the first 7 months of mitotane (Lysodren, HRA Pharma, Paris, France), the daily oral dose was progressively increased from 0.5 to 4 g/day and then stopped because of the appearance of sustained signs of hypoadrenalism, that required a replacement therapy with 5 mg of prednisone o.d. A 10-month mitotane off-therapy follow-up was performed and when an increase in urine free cortisol (UFC) was noted, the mitotane regimen was restarted at lower doses (0.750-1 g/day). Serum morning cortisol levels and UFC were then maintained within the normal range, with plasma mitotane ranging between 2 and 4 mg/L. A sustained regression of Cushing's features without inducing hypoadrenalism was achieved, which still persists after 122 months of follow-up. Minimal initial gastric discomfort was the only side effect of which the patient complained and only during the first higher dose mitotane course. CONCLUSIONS: Long-term administration of a low maintenance dose of mitotane may be suggested as treatment for hypercortisolism in CC patients who refuse or are at high risk for surgical adrenalectomy.

OBJECTIVE: To evaluate lymphocyte subset distribution in the secretory endometrium from infertile patients with polycystic ovary syndrome (PCOS), and the expression of the cytokines known to play a role in determining the endometrial lymphocyte pattern. DESIGN: Experimental clinical study. SETTING: Outpatient clinic in a university hospital. PATIENT(S): Twenty-eight patients with PCOS (PCOS group) and 6 fertile patients (control group). INTERVENTION(S): On days 22-26 of a spontaneous cycle, subjects underwent endometrial biopsies. MAIN OUTCOMES MEASURE(S): In 19 of 28 patients with PCOS and 6 controls with a late secretory endometrium, the percentage and phenotype of lymphocyte subsets were analyzed by flow cytometry. In the late secretory endometrium of 11 patients with PCOS and 3 controls, the expression of interleukins 15 and 18 and of chemokine ligand 10 was also analysed by polymerase chain reaction. RESULT(S): In patients with PCOS the percentage of CD56+/CD16- and of CD56bright/CD16- cells was significantly lower (median [confidence interval]: 38% [31%-52.7%] vs. 63.7% [57.7%-69%] and 17.4% [8%-41.6%] vs. 52% [43%-60%], respectively), whereas the percentage of CD3+ was significantly higher (45% [33.3%-64%] vs. 26.1% [21%-32%]) as compared with controls. Accordingly, polymerase chain reaction analysis revealed a significantly lower expression of interleukins 15 and 18 and of chemokine ligand 10 in patients with PCOS than in controls. CONCLUSION(S): Results demonstrated an abnormal percentage of endometrial lymphocyte subsets, associated with an impaired cytokine network in patients with PCOS. This could explain the poor reproductive potential in these patients.
