BACKGROUND:  Adolescence is a unique and distinct stage of development that involves changes in the physical, psychological and social aspects of adolescents. It is a critical transition into adulthood whereby heightened risk-taking and sensation-seeking takes place, such as substance abuse. In a South African context, this transition sometimes occurs under economic stress, poverty, unemployment, high levels of crime and political instability. This can place adolescents at risk of substance abuse. OBJECTIVES:  To explore and describe the lived experiences of adolescents abusing substances in the Greater Giyani Municipality in the Limpopo province, South Africa. METHOD:  A qualitative, exploratory, descriptive and contextual research design with a phenomenological approach was used. Data were collected through individual, in-depth, phenomenological interviews and field notes. Thematic coding was utilised to analyse the collected data, and literature was reviewed to support the findings. Moreover, measures to ensure trustworthiness and ethical principles were applied throughout the research process. RESULTS:  Five themes were identified: substance abuse behaviour among adolescents, adolescents' motivation for continuing substance abuse, the effects of substance abuse on the lives of adolescents, factors affecting adolescents' discontinuation of substance abuse and a need to discontinue substance abuse. CONCLUSION:  The study concluded that adolescents abusing substances in the Greater Giyani, Limpopo province, experience loss of control, broken relationships, poor academic performance, stigma attached to mental illness and negative emotions. The adolescents foresaw their future as uncertain and without direction. It is recommended that mental healthcare professionals introduce and implement interventions that will assist the adolescents who abuse substances in the Greater Giyani, Limpopo province.Contribution: The findings in this study could add knowledge in developing and implementing of strategies for psychiatric nurses to support adolescents abusing substances in the Greater Giyani, Limpopo province.

BACKGROUND:  A national tragedy occurred between October 2015 and June 2016 when psychiatric patients with profound intellectual disabilities were transferred from psychiatric care centres to non-governmental organisations (NGOs). The process of transferring psychiatric patients had severe consequences for psychiatric patients and psychiatric nurses. OBJECTIVES:  The study's objective was to explore and describe psychiatric nurses' experiences after the closure of Life Esidimeni psychiatric care centres. METHOD:  A qualitative, exploratory, descriptive and contextual research design was employed. Semi-structured interviews were utilised to collect data. Purposive sampling was utilised to select eight psychiatric nurses to participate in the study. Data were analysed using Tesch's thematic method of coding. RESULTS:  The analysis of data revealed the following themes: With the closure of the care centres participants experienced (1) shock, dismay and life interruption; (2) trauma related to the disintegration, of psychiatric patients' lives, their own families and work-life and (3) sense of resilience. CONCLUSION:  From the findings, it is clear that the psychiatric nurses needed support as evidenced by the challenges they experienced. The healthcare professionals in mental health and mental health nursing post-graduate students could conduct further research focusing on the experiences and the impact that the closure of Life Esidimeni psychiatric care centres have on the psychiatric nurses' mental health.Contribution: This study contributes to the body of knowledge in psychiatric nursing by highlighting the impact of hospital closure on psychiatric nurses.

BACKGROUND:  Caring for mental healthcare users (MHCUs) with a comorbid disorder of human immunodeficiency virus (HIV) and schizophrenia has always been challenging and requires expertise, skill, intuition and empathy. OBJECTIVES:  The objective of this study was to explore and describe the experiences of psychiatric nurses caring for MHCUs with a comorbid disorder of HIV and schizophrenia. METHOD:  A qualitative, exploratory, descriptive and contextual research design was used. Eight participants were selected through purposive sampling for individual in-depth interviews to collect data. Thematic analysis was used to analyse data. RESULTS:  Three themes emerged from this study. The first theme is that the psychiatric nurses experienced deep frustration because they were capable but unable to manage MHCUs with HIV and schizophrenia because of poor infrastructure and other contributing barriers. The second theme identified that the psychiatric nurses experienced discrimination against MHCUs compromising their holistic recovery. Lastly, the psychiatric nurses identified the need for health care workers in general hospitals and communities and families of MHCUs with a comorbid disorder to be educated in mental health issues to ensure continuous medical care. CONCLUSION:  The results of this study showed that psychiatric nurses became exhausted when trying to cope with difficult nursing situations. The challenges they faced had negative consequences for the mental health of the psychiatric nurses and compromised patient care.Contribution: This study adds knowledge to nursing practice, nursing education and nursing research by implementing recommendations to mitigate the challenges of psychiatric nurses caring for MHCUs with HIV and schizophrenia.

BACKGROUND:  Mental illness is a serious condition affecting the diagnosed individual and all family members. Family members caring for a sibling with mental illness encounter severe challenges, which, if unresolved, becomes a burden they carry for the rest of their lives. OBJECTIVES:  The purpose of the study was to describe family members' experiences caring for a sibling with mental illness in Giyani. METHOD:  A qualitative, exploratory, descriptive and contextual research design was used. Eight family members caring for a sibling with mental illness were purposively sampled. Data were collected through in-depth phenomenological interviews, and data were analysed using thematic coding, and an external coder was consulted. RESULTS:  The study's findings revealed that family members caring for a sibling with mental illness experienced significant challenges. The three themes that emerged after data analysis were the following: caring for a sibling with mental illness was an overwhelming experience; family members experience emotional instability as a result of caring for their sibling with mental illness and family members need support in caring for their sibling with mental illness. CONCLUSION:  The findings indicated that family members need support in caring for a sibling with mental illness. Family members would cope better with support from extended family, healthcare professionals in mental health and the community; this will reduce the burden of caring for a sibling with mental for family members. Furthermore, the availability of resources can assist in facilitating family members' mental health.Contribution: This study could make an impact in psychiatric nursing practice, nursing education in promotion of support for family members for a sibling with mental illness.

BACKGROUND:  When there is a lack of resources in the community to support deinstitutionalisation, family members of a relative diagnosed with substance-induced psychosis disorder (SIPD) are the most affected and vulnerable. Nevertheless, family members' care is still largely unacknowledged in the mental health sector in low- and middle-income countries. Furthermore, no prior research could be found on family members' experiences caring for a relative with SIPD in Giyani, Limpopo province, South Africa. OBJECTIVES:  To explore and describe family members' experiences caring for a relative with SIPD. METHOD:  The study employed a qualitative research design using interpretative phenomenological analysis as the research method. Telephonic interviews were conducted and analysed. Eight family members were selected to participate in the study using a purposive sampling technique. RESULTS:  The analysis of data led to the emergence of the following themes: family members experienced caring for a relative with SIPD as a destabilising responsibility; they experienced acceptance and support from significant others and the community and solace in prayer. Participants also expressed they experienced a need for support from government structures in order to care for a relative with SIPD. CONCLUSION:  The study's findings highlighted the family members' experiences of caring for a relative with SIPD and the role of the family, community and government structures in caring for an individual with SIPD. It is evident from the challenges experienced that the family members need external interventions to develop healthy coping strategies.Contribution: This study adds knowledge to nursing practice, nursing education and nursing research by promoting effective coping amongst family members caring for a relative with SIPD.

Objective: This article aimed to present a developed model for nurse anaesthesia practice in South Africa and then to evaluate the model by national and international anaesthetist experts. Methods: For the model development, a theory-generative research design was used as guided by Chinn & Kramer. The study included four steps to develop and evaluate the model: step one, concept analysis; step two, placing the concepts into relationships; step three, description and critical reflection of the model; step four, evaluation of the model. For evaluation of the model, data were collected using individual, in-depth interviews. Four South African medical specialist anaesthesiologists and four international nurse anaesthetists were interviewed. Transcribed interviews were analyzed using Tesch's eight steps of thematic data analysis. Results: The model was described in four phases: relationship phase, working phase, termination phase, and independent phase. The model shows a development process of a student nurse anaesthetist by a nurse anaesthetist facilitator to the mastery of the scope of practice of a nurse anaesthetist. Two themes (six categories) emerged through thematic analysis: the model and guidelines for implementation were found to be appropriate (model and guidelines for implementation are important because of the need for nurse anaesthetist, the model is applicable because it is described as comprehensive, the relationship phase was found to be important and the interdependence between disciplines is imperative, the affirmation that mastery has to be attained by nurse anaesthetists due to the complexity of services rendered), anticipated limitations to the South African nurse anaesthetist (the need to identify complex patients and limitations on the setting and refer to higher level of care, limitations due to red tape). Conclusions: This model provides fertile soil to inform and outline the education (curriculum) and practice (scope of practice) and research for nurse anaesthesia practice in South Africa.

BACKGROUND: Caring is described as the innermost core of nursing which occurs in a relationship between the patient and the care provider. Although caring in nursing is associated with maintaining and strengthening of the patient's sense of dignity and being a person, there seems to be a gap between caring theories in nursing, healthcare policies and caring for patients by professional nurses in primary health care clinics. Developing strategies that will facilitate effective caring for patients by professional nurses in primary health care clinics within an ethical and mindful manner became an area of focus in this study. OBJECTIVES: To develop strategies to facilitate effective caring for patients by professional nurses in primary health care clinics in South Africa. METHOD: Strategies were developed based on the conceptual framework developed in Phase 2, which was derived from synthesis of the results of Phase 1 of the previously conducted study and supported by literature. The conceptual framework reflects the survey list of Dickoff, James and Wiedenbach's practice theory. RESULTS: Three strategies were developed: 1) facilitating maintaining of the empowering experiences; 2) facilitating addressing the disempowering experiences by professional nurses, and 3) facilitating addressing of the disempowering primary health care clinic systems. CONCLUSION: The developed strategies, being the proposed actions, procedures and behaviours, could facilitate effective caring for patients by professional nurses in primary health care clinics.

PURPOSE: The purpose of this article is to propose a definition for the concept "scope of practice" as it applies to a nurse anesthetist in South Africa. DESIGN: Concept analysis. METHODS: Walker and Avant's procedure of concept analysis was followed. The actions included "Select a concept"; "Determine the purpose of analysis"; "Identifying uses of the concept"; "Determining the defining attributes"; "Identifying antecedents and consequences"; "Define empirical referents"; "Identify model case"; and "Identifying additional cases." FINDINGS: The concept 'scope of practice' as it applies to the nurse anesthetist in South Africa can be defined as the individual's competence, accountability, and responsibility as a health professional. The nurse anesthetist is (1) competent: ready to use skills and judgement in practice; (2) accountable: able to be registered as a nurse anesthetist and willing to abide by the regulations; and (3) responsible: upholding professionalism and demanding recognition from the public and peers. CONCLUSIONS: A definition (revealing the concept's structure) and it's uses (revealing the concept's function) for "scope of practice" of a South African nurse anesthetist is proposed for the consideration of introducing nurse anesthesia to provide safe and affordable anesthesia services in South Africa. This article forms part of a larger study titled "A Model for Nurse Anaesthesia Practice in South Africa."

BACKGROUND: Parents of adolescents with intellectual disability experienced stress caused by challenges that come from having such adolescents. Those challenges affected the parents physically and emotionally, depending on the severity of the adolescent's intellectual disability. Having an adolescent with an intellectual disability becomes a burden if the challenges were not resolved. AIM: This study aimed to explore and describe the experiences of parents of adolescents with intellectual disability in Giyani. SETTING: This study was conducted at the participants' homes in Giyani, Limpopo province, South Africa. METHODS: A qualitative, exploratory, descriptive and contextual design was utilised. The main question was 'How is it to have an adolescent with intellectual disability?' Eight purposively sampled parents participated, and data were collected through in-depth interviews, observations and field notes. Data were analysed by means of thematic coding and an independent coder was consulted. RESULTS: Four themes were identified. The study revealed that parents of adolescents with intellectual disability experienced negative emotional responses. Most parents reported a lack of support from family members and the community. They also reported caring challenges, yet some showed positive coping mechanisms. CONCLUSION: Parents of adolescents with intellectual disability experienced various challenges in caring for these children. A collaborative approach from relevant stakeholders could have a positive impact in supporting the parents of adolescents with intellectual disabilities.

BACKGROUND: Caring in nursing helps patients feel better, whilst the absence of caring will affect patients psychologically, emotionally and physically. AIM: The aim of this article was to explore and describe primary health care (PHC) professional nurses' and PHC nurse managers' perceptions of the developed strategies to facilitate effective caring for patients in PHC clinics. SETTING: This study was conducted in two PHC clinics in Ekurhuleni, an area east of the Gauteng province, South Africa. METHODS: The study used a qualitative, exploratory, descriptive and contextual design. In-depth individual phenomenological interviews were conducted with eight purposively selected PHC professional nurses working in PHC clinics and two PHC nurse managers supervising PHC clinics in Ekurhuleni. Giorgi's coding method was used to analyse the data. RESULTS: Three themes were identified from the results of the exploration and description of PHC professional nurses' and PHC nurse managers' perceptions of the developed strategies. The use of active listening skills, showing interest in what is being said, asking questions and providing constructive feedback that focuses on the issue were the most effective strategies in improving effective communication between PHC nurse managers and PHC professional nurses. The PHC professional nurses were encouraged to put patients' interests first whilst adhering to the ethical principles of nursing. CONCLUSION: Although caring is considered as the core of nursing practice, PHC professional nurses and PHC nurse managers perceive that rendering effective caring for patients needs to be reinforced through the use of strategies that will enable change and improve clinical practice in PHC clinics.

BACKGROUND: Borderline personality disorder (BPD) is characterised by emotional dysregulation, feelings of worthlessness, impulsivity, suicidality and poor relationships. As a result of the challenges in the treatment of women living with BPD and the lack of skills from the psychiatric nurse, there was a need to develop a model for psychiatric nurses to facilitate the mental health of women living with BPD. OBJECTIVES: To describe the process that was followed in developing, describing and evaluating a model that could be used as a framework of reference for psychiatric nurses to facilitate the mental health of women living with BPD. METHOD: A theory-generative, qualitative, exploratory, descriptive and contextual study design was used to develop the model. The central concept of the model was derived from a previous study: 'The experiences of women living with borderline personality disorder'. The process entailed the identification of the central concept and other essential criteria, the classification of the central concepts and describing the relationships between the concepts. RESULTS: The central concept was identified as 'facilitation of self-empowerment' of women living with BPD. The concepts 'facilitation' and 'self-empowerment' were defined and classified. The identified and defined central concepts were placed into interrelated statements. The model to facilitate self-empowerment of women living with BPD was developed, described and evaluated. The model has not been implemented. CONCLUSION: The model provides a framework of reference for psychiatric nurses to facilitate self-empowerment of women living with BPD.

BACKGROUND: Family members face the burden of adult females living with depression who do not comply with psychiatric medication. Discomfort, tension, anxiety, frustration, and related feelings of hopelessness and dysfunction were identified by family members. There have also been records of financial problems, physical ill-health, limitations on social and recreational opportunities and a general deterioration in their quality of life. There is a shortage of published literature and information on the reasons for non-compliance to psychiatric medications. The existing body of information needs to be strengthened and future approaches encouraged. The study aimed to improve compliance of adult females dealing with depression to psychiatric medical treatment and the effect it has on family members caring for adult females living with depression. OBJECTIVES: To describe family members' lived experiences of non-compliance to psychiatric medication by adult females living with depression. METHOD: A qualitative, exploratory, descriptive and contextual study design was used. A purposive sample of family members aged between 20 and 45 years was made. Data were collected by conducting eight in-depth, phenomenological interviews, and field notes were taken. The interviews focused on the central question: 'Tell me your experiences of living with your wife, mother, sister and daughter living with depression and not taking their medication as ordered by the doctor?' Tesch's method for data analysis was used, and an independent coder analysed the data and met with the researcher for a consensus discussion of the results. Measures to ensure trustworthiness were applied and ethical principles were adhered to. RESULTS: The three themes identified were: experienced psycho-social effects, experienced treatment refusal and experienced challenges in caring for adult females living with depression who are non-compliant to psychiatric medication. As a result, the absence of social help, disturbance of family working, shame, separation and troublesome conduct of the adult females who are non-compliant to psychiatric medication developed as principal subjects. CONCLUSIONS: The results demonstrated that family members experienced debilitation because they needed information about their relatives living with depression who are non-compliant to psychiatric medication. More information about the management of non-compliance of psychiatric medication was needed; a comprehensive awareness of the ramifications of the findings, treatment and care are required from mental health care professionals and service providers.

RESUMO

PURPOSE: To elicit the importance of caring with awareness in order to facilitate student nurses' caring toward patients. HYPOTHESIS: There is a positive relationship between student nurses acting with awareness and caring. METHOD: A quantitative, descriptive, correlational, and contextual research design with a purposive sampling method was used in the study. Respondents (n = 56) were student nurses in their third- and fourth-year level of study. Data were collected by use of a self-administered questionnaire. FINDINGS: There was a negative correlation between acting with awareness and caring. There was no difference in caring among third- and fourth-year students. CONCLUSION: Student nurses may be caring for patients, but it does not necessarily mean they act with awareness in caring. Awareness should be cultivated among student nurses for them to be present in caring and provide holistic caring.

BACKGROUND: The authors developed a psycho-educational model as a conceptual framework of reference for university lecturers to facilitate the constructive management of experienced aggression. The model must be implemented in a workshop and in practice to confirm the if the model is effective. AIM: This article describes the implementation of a psycho-educational model in a workshop and in practice, as well as the evaluation of the effectiveness of the psychoeducational model. SETTING: This study was conducted in a specific college at a university in Johannesburg in South Africa. METHOD: This study followed a qualitative, exploratory, descriptive, contextual and theory-generating research design. The psycho-educational model was implemented in three phases during a workshop and then for three months in practice by university lecturers. A purposive sample of university lecturers was applied. The effectiveness of the psycho-educational model was evaluated during and directly after the workshop, 1 week after the workshop and 3 months journal entries. Final evaluation was in a focus group after 3 months of implementation of the model in the workplace. RESULTS: The participating university lecturers found the implementation of the psycho-educational model, as a conceptual framework of reference to constructively manage experiences of aggression, effective, helpful and important. The model increased their understanding of aggression in their places of work and increased their effectiveness to constructively manage experiences of aggression in their workplace. CONCLUSION: The implementation and evaluation of the psycho-educational model underscored the need for affective and effective facilitative support for university lecturers to be able to constructively manage experienced aggression.

BACKGROUND: Experienced workplace aggression is a lingering phenomenon and comprises an extensive body of knowledge. Experiences of workplace aggression require constructive intervention and support. These interventions and support must aim to assist university lecturers to constructively manage experiences of aggression. AIM: The aim of this article is to describe the process followed to develop, describe and evaluate a psycho-educational model for university lecturers that could be used as a conceptual framework of reference to facilitate the constructive management of experienced aggression. SETTING: The model is applicable in universities where the university lecturers work and experience aggression. This study was conducted in a specific college at a university in Johannesburg in South Africa. METHOD: A theory-generating, qualitative, exploratory, descriptive and contextual design was applied to develop a psycho-educational model. The process included four steps: concept analysis, relationship statements, description of the model and evaluation of the developed model. The criteria of clarity, simplicity, generality, accessibility and importance were used during the evaluation of the model. RESULTS: The central concept in this study was to facilitate constructive management. The psycho-educational model as the conceptual framework of reference for facilitating the constructive management of experienced aggression was described and evaluated. University lecturers' destructive management of experienced aggression formed the basis for the psycho-educational model. CONCLUSION: The psycho-educational model provides a conceptual framework of reference for university lecturers that may assist them to constructively manage experiences of aggression in their places of work.

BACKGROUND: Students are sometimes subjected to difficult circumstances to achieve success. Studying under these circumstances could cultivate aggression towards self and others, and the environment. Little, if any, published research is available dealing with students being orientated autonomous versus being submissive and perceptions of aggression. OBJECTIVES: To explore and describe the perceptions of groups of students being orientated autonomous versus students being submissive and perceptions of aggression. Recommendations concerning these two groups of students are made. METHOD: An exploratory quantitative research design that is descriptive and inferential in nature was applied. A questionnaire was electronically distributed to students in a faculty. The questionnaire consisted of items on biographic, personality and aggression. In the statistical analysis Cronbach's alphas, principal component analysis were done and hypotheses were tested on differences between students orientated autonomous and those being submissive. RESULTS: Of the 266 completed questionnaires used, 177 were received from females and 89 from males. Eighty-two (82) of these were honours, masters or doctoral students. Findings reflected graded differences between students being autonomous and students being submissive orientated concerning Overt verbal aggression (means below 1.82), Overt physical aggression (means below 2.53) and aggressive inclination towards others (means below 3.01). The implications are that students are to be sensitised to be reflective of their levels of aggression. University management should help students to be reflective concerning their aggression, and to establish congruency between self-perception and reality. CONCLUSION: Students should be helped to understand and manage their aggressive inclination.

BACKGROUND: Substance abuse by adolescents may be a problem that contributes to their mental illness. Substance abuse does affect not only the individual who is abusing it but also friends, family and the whole community. The adolescent abusing substances may be mentally unstable and have unpredictable behaviour. There is no research on the experiences of parents with adolescents abusing substances in Giyani, South Africa. OBJECTIVE: The purpose of the study was to explore and describe the experiences of parents with adolescents abusing substances admitted to a mental health institution in Giyani. METHOD: A qualitative, exploratory, descriptive and contextual research design was used. Data were collected by means of conducting individual, in-depth, phenomenological interviews, observations and field notes. The following central question was asked to the participants: 'How is it for you to have an adolescent who is abusing substances'. Data were analysed by using a thematic method of coding. An independent coder analysed data together with the researcher, and consensus was reached. RESULTS: Four themes emerged from the data: parents experienced uncontrolled thoughts regarding their adolescent abusing substances, not being able to control their adolescent abusing substances through discipline, negative feelings regarding their adolescent abusing substances and negative consequences regarding their adolescents abusing substances. CONCLUSION: From the study result, it is clear that parents with adolescent abusing substances need professional assistance and support as evidenced by the challenges faced in terms of promoting, maintaining and restoring their mental health. Psychiatric nurses should take responsibility to educate the community about substance abuse, for example offering school health programmes. Further research studies can also be conducted in other villages to gain a greater understanding of those parents' experiences with an adolescent abusing substances.
