RESUMO

To determine the role of ultrasonic and transcranial dopplerography in investigation of both arterial and venous systems of children's head in conditions of outpatient clinic were observed 100 children aged 4-14 years with autonomic dysfunction, migraine, neurosis-like and closed cerebrocranial trauma (concussion of the brain). There were estimated the qualitative indices of dopplerography (a form, distributions of the frequencies in spectrum, direction of the blood flow, acoustic characteristics) and quantitative indices (systolic, diastolic, mean rate of blood flow, indices RI and S/D) and alterations in venous circulation. In all the groups of children there were found angiodystonic manifestations in the form of the increase of vessels resistance in 87.5-97.6% of the patients; besides, the signs of venous dysgemia were observed in 28-50% of the cases.

Short- and long-term results of combined treatment of 440 children and adolescents afflicted with lymphogranulomatosis (Hodgkin's disease) were traced. A group of 104 children received treatment in 1964-1973. Complete effect was attained in 72.1% of the patients (in 75 out of 104); the 5-year survival was 72.5%, the 10-year 61.1%, the 15-year 52.75%. The long-term results are described in detail. Analysis of the results allowed basing the necessity of the three-stage combined chemoradiation therapy in children with lymphogranulomatosis. In 1976-1980, the treatment according to the first combined program was provided to 193 children and adolescents aged 2.5 to 16 years suffering from lymphogranulomatosis. In 1979-1983, the treatment according to the second combined program was provided to 143 children and adolescents aged 3 to 15 years suffering from the same disease. In patients treated according to the first and second programs, the 5-year survival amounted to 81.3 and 93.3%, respectively. The total number of lethal outcomes was 20.2 and 4.2%, respectively, during the whole observation period.

RESUMO

The authors analyze the results of the treatment of 366 children afflicted with lymphogranulomatosis. All the patients received treatment from 1976 to 1990 according to three combined programs. The necessity is demonstrated of the individual treatment with regard to the predictors. Immunocorrection by means of taktivin was employed in patients with immunity disorders. The perfection of a complex of treatment measures favoured an increase of the survival to over 5 years in 93.8% of cases.

RESUMO

The paper presented the immediate and long-term therapeutic response in 141 patients (88 males and 53 females) who had suffered from Hodgkin disease at the age between 3 and 15 years and then were followed up from 5 to 25 years. Their physical and sexual development, occupation, family status were considered. In case the pregnancy was not forbidden terms of its successful development were formulated.

A total of 186 children and adolescents aged 2.5 to 16 years with Hodgkin's disease were placed under observation. According to the accepted classification (Ann Harbour, 1971) they were divided into two groups. Exploration laparatomy was applied in the first group of the patients (n-103) as the final diagnostic stage in order to verify the pathomorphological stage of the process. In the second group (n-83) the clinical stage of the disease was only appraised. The pathological foci beneath the diaphragm were discovered, however, with the aid of ultrasonic tomography of the abdominal cavity, scanning with gallium-67 citrate, and computer-aided tomography. The latter technique was employed in case of controversies in the interpretation of the foci of lesion with the aid of the first two diagnostic methods. It is concluded that in view of the high tempo of the so-called silent generalization and dissemination of Hodgkin's disease in children, this condition should be regarded in childhood as a potentially systemic process. With such an attitude towards Hodgkin's disease in children it is advisable to abandon exploration laparotomy with splenectomy depriving the patient of the massive depot of humoral (B cell) immunity and creating the danger of an aggressive course of intercurrent infections in splenectomized children affected with Hodgkin's disease. In the choice of the initial treatment which provides the basis for obtaining the highest effect, preference should be given to polychemotherapy. It is indicated in the presence of "biological activity" of the process, general symptoms, generalization and dissemination of the process (stages III and IV), and in the presence of multiple foci in stage II.(ABSTRACT TRUNCATED AT 250 WORDS)
