Importance: Kindler epidermolysis bullosa is a genetic skin-blistering disease associated with recessive inherited pathogenic variants in FERMT1, which encodes kindlin-1. Severe orofacial manifestations of Kindler epidermolysis bullosa, including early oral squamous cell carcinoma, have been reported. Objective: To determine whether hypoplastic pitted amelogenesis imperfecta is a feature of Kindler epidermolysis bullosa. Design, Settings, and Participants: This longitudinal, 2-center cohort study was performed from 2003 to 2023 at the Epidermolysis Bullosa Centre, University of Freiburg, Germany, and the Special Care Dentistry Clinic, University of Chile in association with DEBRA Chile. Participants included a convenience sampling of all patients with a diagnosis of Kindler epidermolysis bullosa. Main Outcomes and Measures: The primary outcomes were the presence of hypoplastic pitted amelogenesis imperfecta, intraoral wounds, gingivitis and periodontal disease, gingival hyperplasia, vestibular obliteration, cheilitis, angular cheilitis, chronic lip wounds, microstomia, and oral squamous cell carcinoma. Results: The cohort consisted of 36 patients (15 female [42%] and 21 male [58%]; mean age at first examination, 23 years [range, 2 weeks to 70 years]) with Kindler epidermolysis bullosa. The follow-up ranged from 1 to 24 years. The enamel structure was assessed in 11 patients, all of whom presented with enamel structure abnormalities. The severity of hypoplastic pitted amelogenesis imperfecta varied from generalized to localized pitting. Additional orofacial features observed include gingivitis and periodontal disease, which was present in 90% (27 of 30 patients) of those assessed, followed by intraoral lesions (16 of 22 patients [73%]), angular cheilitis (24 of 33 patients [73%]), cheilitis (22 of 34 patients [65%]), gingival overgrowth (17 of 26 patients [65%]), microstomia (14 of 25 patients [56%]), and vestibular obliteration (8 of 16 patients [50%]). Other features included chronic lip ulcers (2 patients) and oral squamous cell carcinoma with lethal outcome (2 patients). Conclusions and Relevance: These findings suggest that hypoplastic pitted amelogenesis imperfecta is a feature of Kindler epidermolysis bullosa and underscore the extent and severity of oral manifestations in Kindler epidermolysis bullosa and the need for early and sustained dental care.

BACKGROUND: Real-world studies about the effectiveness and safety of vedolizumab (VDZ) in the treatment of inflammatory bowel disease (IBD) in Latin America are scarce. Our study describes the effectiveness and safety of VDZ in Colombian patients with IBD. METHODS: EXVEDOCOL (EXperience of VEDOlizumab in COLombia) was a retrospective, multicenter, observational study. Adults with IBD receiving a first dose of VDZ between July 2016 and October 2018 were included. The co-primary outcomes clinical response, and remission, were determined at week 14 and last visit during the maintenance phase (LVMP). The secondary outcomes, deep remission and loss of response were recorded at LVMP. RESULTS: Thirty-one patients (25 ulcerative colitis (UC), 6 Crohn's disease (CD)) were included. At week 14, clinical response was achieved by 87.1% (27/31) of the patients treated with VDZ, while loss of response was reported in 6.7% (2/30). The remission rate at week 14 was 65.5% (19/29) and 75.9% (22/29) at LVMP. Prior anti-TNF exposure was reported in 61.3% (19 patients) of whom 84.2% (16/19) achieved clinical response at week 14 and 89.5% (17/19) at LVMP. For anti-TNF naïve patients, clinical response was recorded in 91.7% (11/12) at week 14 and 100% (12/12) at LVMP. CONCLUSIONS: High clinical remission rates and safety profile highlight VDZ as a valuable treatment option for IBD patients. Anti-TNF naïve patients may derive greater benefit from therapy. Studies with larger cohorts could confirm these findings.

La hipertensión portal es una de las principales complicaciones de la cirrosis. El papel de la derivación portosistémica transyugular intrahepática (TIPS, por sus siglas en inglés), ha ganado aceptación como tratamiento efectivo en la hipertensión portal. En los últimos años su técnica se ha ido perfeccionando, disminuyendo la morbimortalidad relacionada con este procedimiento. Describimos un caso de un paciente masculino con cirrosis Child-Pugh 8 y MELD 16, con antecedente de descompensación por sangrado variceal recurrente y trombosis parcial de la vena porta, con un gradiente de presión venosa hepática (GPVH) de 20 mmHg, por lo que es llevado a TIPS como profilaxis secundaria, con un gradiente final post-TIPS de 6 mmHg. Posterior al procedimiento, presentó evolución tórpida con deterioro de las pruebas de bioquímica hepática. Se realizó una angiografía demostrando permeabilidad del TIPS sin progresión de la trombosis portal, y hallazgos anormales inespecíficos de la arteria hepática. Se decidió realizar una arteriografía selectiva, demostrando un pseudoaneurisma de la rama derecha de la arteria hepática y una fístula arteriovenosa de la arteria hepática a las colaterales portales. Se realizó embolización selectiva de la fístula con evolución satisfactoria del paciente.
Portal hypertension is a life-threatening complication of cirrhosis. The role of transyugular intrahepatic portosystemic shunt (TIPS) has gained acceptance as an effective treatment for portal hypertension. In the past few years, its technique has been improved, decreasing the mortality related with the procedure. We describe a case of a male with Child-Pugh 8 and MELD 16 cirrhosis, with previous decompensation of recurrent variceal bleeding and partial thrombosis of the portal vein. TIPS was performed due to a hepatic venous pressure gradient (HVPG) of 20 mmHg. The final measure showed HVPG of 6 mmHg. After the procedure, he presented a torpid evolution with deterioration of liver function tests. An angiography was performed demonstrating patency of the TIPS without progression of portal thrombosis and nonspecific abnormal findings of the hepatic artery. Selective arteriography was performed and revealed a pseudoaneurysm of the right branch of the hepatic artery and an arteriovenous fistula (AVF) from the hepatic artery to portal collaterals. Embolization was performed to treat the fistula with satisfactory evolution of the patient.

RESUMEN: Objetivo: Determinar parámetros visuales sencillos para la evaluación de características esqueletales relacionadas con el biotipo facial en una telerradiografía lateral. Material y método: Estudio transversal descriptivo. Se confeccionó una guía visual impresa acerca de ocho características morfológicas del esqueleto facial visibles en la telerradiografía de perfil. Ésta fue entregada a un grupo de 69 estudiantes de Odontología de 5º año sin estudios previos en cefalometría, quienes la analizaron durante 10 minutos. Inmediatamente se les mostraron una serie de telerradiografías, y se les pidió que evaluaran las 8 características a través de una serie de opciones presentadas en una escala visual que se correspondía con la guía estudiada. Las respuestas fueron registradas utilizando un sistema digital de cliqueras. Los datos fueron analizados con el método Rasch para determinar la dificultad de los ítems, estableciéndose el grado de dificultad a través del Logit. Resultados: Los estudiantes lograron identificar y graduar sin dificultad 5 de las 8 características presentadas. Éstas fueron: Proyección del mentón (-2,49), Divergencia facial (-2,85), Ancho de la rama mandibular (-1,97), Escotadura sigmoidea (-0,96) y Relación de la rama con el cuerpo mandibular (-0,06). Conclusión: Se determinó que hay 5 características de la morfología ósea facial relacionadas con el biotipo que se pueden identificar y graduar sin dificultad por estudiantes de pregrado de odontología. Ésta información podría ser utilizada para construir una escala sencilla de identificación del biotipo facial.
ABSTRACT: Aim: Determine simple visual parameters to evaluate skeletal characteristics related to facial types in lateral teleradiographs. Materials and Methods: Descriptive cross-sectional study. A printed visual guideline about the eight visible morphological characteristics of the facial skeleton in cephalometric x-rays was created. The guideline was given to 69 undergraduate dental students (fifth year) without previous studies in cephalometric to analyze it for 10 minutes. Immediately after, they were shown a series of teleradiographs and were asked to evaluate 8 characteristics through a series of options presented in a visual scale according to the guideline. The answers were recorded through a digital audience response system (clickers). The data obtained were analyzed with the Rasch method to assess each item level of difficulty, establishing the Logit number for each one. Results: The students identified and graded without difficulty 5 of the 8 characteristics. These were: Chin projection (-2.49), Facial divergence (-2.85), Mandibular ramus width (-1.97), Sigmoid notch (-0.96) and Mandibular body with ramus relationship (-0.06). Conclusions: Five facial osseous morphological characteristics related to the facial types can be identified and graded without difficulty by undergraduate dental students. This information may be used to create a facial type identification scale.

Biliary obstruction of different origin is a common clinical problem, with significant impact on the patients quality of life and poses a permanent risk of cholangitis. The management of these patients has evolved over time, makes collection of various technological developments and involve clinicians, surgeons, gastroenterologists, and interventional radiologists. Were port four cases of biliary obstruction that despite the significant demographic and clinical differences between them could be successfully managed approach in the biliary tract with the technique of radiological endoscopic Rendezvous.

The symptomatic metastasis of the colon from a pulmonary cancer is rare; however, the global incidence of pulmonary cancer is 12.9%. It is an infrequent site of metastasis, with a prevalence of less than 0.5% in patients with pulmonary cancer. One of the most common manifestation is intestinal obstruction. We present a case report of a patient with an acute lower intestinal bleeding from multiple metastasis lesion of the colon as the initial manifestation of a non-small cell lung carcinoma.

Las metástasis sintomáticas al colon de un carcinoma de pulmón son raras, a pesar de ser una neoplasia que representa el 12,9% de la incidencia mundial de cáncer. El colon es un sitio infrecuente de metástasis, con una prevalencia reportada menor al 0,5% en pacientes con carcinomas de pulmón, existiendo en la literatura mundial pocos casos. Es inusual que sea la forma de presentación de este tipo de neoplasia. Con respecto a las manifestaciones clínicas que se reportan pueden cursar con obstrucción intestinal como síntoma cardinal. Se presenta el caso de un paciente que curso con hemorragia de vías digestivas bajas como manifestación inicial de múltiples lesiones metastásicas a colon de un carcinoma de pulmón de célula no pequeña.

La obstrucción biliar de diverso origen constituye un problema clínico frecuente, con importante impacto sobre la calidad de vida de los pacientes y que plantea el riesgo permanente de colangitis. El manejo de estos pacientes ha evolucionado en el tiempo, haciendo acopio de diversos desarrollos tecnológicos e involucrando a clínicos, cirujanos, gastroenterólogos y radiólogos intervencionistas. Reportamos aquí cuatro casos de pacientes con obstrucción biliar que, a pesar de las importantes diferencias demográficas y etiológicas, pudieron ser exitosamente manejados, abordando la vía biliar con la técnica de Rendezvous radiológico endoscópico.

Antecedentes: el 90% de los pacientes cirróticos desarrollará várices esofágicas, y en algún momento de su vida, el 30% de ellos sangrará. Objetivo: identificar los factores pronósticos asociados con mortalidad en cirróticos con sangrado digestivo varicoso. Método: se presenta una cohorte retrospectiva de pacientes con sangrado digestivo varicoso en un período de 30 meses. Resultados: se presentan 63 pacientes (33 hombres y 30 mujeres), de 56 años (desviación estándar DE 16 años) de edad media. El 52,4% tenía una clase funcional Child-Pugh B. El promedio de hospitalización fue de 10 días. De estas, 12 (19%) fallecieron; el choque hipovolémico (p = 0,033) y la transfusión de glóbulo rojos (p = 0,05) estuvieron relacionados con la mortalidad. En el análisis bivariado, la variable más relacionada con la mortalidad fue el compromiso funcional hepático Child-Pugh C (p = 0,00). Las comparaciones de las variables numéricas encontraron que el valor de creatinina; media 1,74 mg/dL (p = 0,043); y la duración de la hospitalización; media de 10 días (p = 0,057); fueron superiores en los pacientes fallecidos. En los análisis bivariados, las variables Child-Pugh C (Exp(B) = 0,068; p = 0,002) y creatinina (Exp(B) = 0,094; p = 0,034) se mantuvieron estadísticamente relacionadas con el desenlace de interés. Conclusiones: la mortalidad del paciente con sangrado varicoso en este estudio es comparable con los estándares internacionales actuales. La enfermedad hepática avanzada y una función renal deteriorada están relacionadas con mayor mortalidad, razón por la cual los pacientes con predictores de mortalidad presentes durante la descompensación por sangrado varicoso ameritan una vigilancia estrecha e intervenciones tempranas para evitar desenlaces negativos
Background: Ninety percent of cirrhotic patients will develop esophageal varices, and bleeding will occur in 30% of these patients at some point in their lives. Objective: The objective of this study was to identify prognostic factors associated with mortality in cirrhotic patients with bleeding varices. Method: We present a retrospective cohort study of patients with bleeding digestive varices over a period of 30 months. Results: This study included 63 patients (33 men and 30 women) whose average age was 56 years (SD: 16 years). 52.4% of these patients Child-Pugh classification was B. The average stay in the hospital was 10 days. Twelve (19%) died. Hypovolemic shock (p = 0.033) and red blood cell transfusion (p = 0.05) were related to mortality. Bivariate analysis showed that the variable most closely related to mortality was hepatic impairment with Child-Pugh C classification (p = 0.00). Comparisons of numerical variables found that the creatinine value (Mean: 1.74 mg/dl, p = 0.043) and length of hospital stay (mean time:10 days, p = 0.057) were higher in patients who died. In the bivariate analysis, Child-Pugh C (Exp (B) = 0.068, p = 0.002) and creatinine (Exp (B) = 0.094, p = 0.034) remained statistically related to the outcome of interest. Conclusions: Mortality from bleeding varices in the patients in this study is comparable to current international standards. Because advanced liver disease and impaired renal function are associated with increased mortality, patients with bleeding varices who have predictors for mortality during decompensation due to bleeding warrant close monitoring and early interventions to avoid negative outcomes

Abstract: Introduction: Pulmonary thromboembolism (PTE) is a cardiovascular emergency threatening life. It is classified at low, intermediate and high risk of mortality. By the above, it allows to establishing conservative treatment for low-intermediate risk PTE and more intense treatment for high risk PTE. Objective: To report the number of cases of acute PTE, risk stratification and prognosis in those treated with adherence to clinical guidelines. Material and methods: Review of clinical records with a diagnosis of acute PTE by confirmatory diagnostic test. The type of treatment was determined by: oral anticoagulation (OAC) or parenteral anticoagulation (PAC), percutaneous thrombectomy, supraselective thrombolysis, systemic thrombolysis, surgical thrombectomy or combinations. The type of initial anticoagulation and maintenance therapy. Hard results: TIMI major bleeding, re-thrombosis, death and brain stroke (BS). Normality was verified by Kolmogorov-Smirnov test. Then was compared with Student t or U Mann-Whitney. Results: A sample of 36 patients was obtained, the mean age was 67.24 ± 18.83 years, 62.2% were females. The 29.7% were low-risk PTE, 51.4% were intermediate risk and 18.9% were high risk. The 70.3% received OAC, 8.1% percutaneous trombectomy, 8.1% systemic thrombolysis, 10.8% systemic thrombolysis + percutaneous thrombolysis, 2.7% percutaneous trombectomy + supraselective thrombolysis. There is increased risk of death in this group OR = 2.63 (95% CI 0.45-16.08) but not significant (p = 0.255). Conclusions: Lack of adherence to clinical guidelines confers increased risk of death in patients with acute PTE, this difference is not statistically significant.
Resumen: Introducción: La tromboembolia pulmonar (TEP) es una urgencia cardiovascular que pone en riesgo la vida. Se cataloga en riesgo bajo de mortalidad, intermedio y alto. Lo anterior permite establecer estrategias terapéuticas conservadoras para la TEP de riesgo bajo-intermedio y más intensas para alto riesgo. Objetivo: Reportar el número de casos de TEP aguda, la estratificación de riesgo y el pronóstico en quienes recibieron tratamiento con apego a guías clínicas. Material y métodos: Revisión de expedientes clínicos con diagnóstico de TEP aguda, mediante prueba diagnóstica confirmatoria. Se determinó el tipo de tratamiento: anticoagulación (ACO), oral o parenteral (ACP), trombectomía percutánea(TBTP), trombolisis supraselectiva (TBLSU), trombolisis sistémica (TBLSIS), trombectomía quirúrgica (TBTQ) o sus combinaciones. El tipo de anticoagulación inicial y de mantenimiento. Los resultados duros: sangrado TIMI mayor, retrombosis, muerte y evento vascular cerebral (EVC). Se verificó la normalidad mediante prueba de Kolmogorov-Smirnov. Posteriormente se comparó con t Student o U de Mann-Whitney. Resultados: Se obtuvo una muestra de 36 pacientes, la edad media fue 67.24 ± 18.83 años, el 62.2% fueron del género femenino. El 29.7% correspondía a TEP de riesgo bajo, 51.4% riesgo intermedio y 18.9% a riesgo alto. El 70.3% se dio ACO y ACP, 8.1% TBTP, 8.1% TBLSIS, 10.8% TBLSIS + TBTP y 2.7% TBTP + TBLSU. Existe mayor riesgo de muerte en el grupo de falta de apego a guías OR = 2.63 (IC 95% 0.45-16.08), sin embargo, no es significativo (p= 0.255). Conclusiones: La falta de apego a guías clínicas confiere mayor riesgo de muerte en pacientes con TEP aguda, esta diferencia no es estadísticamente significativa.

UNLABELLED: The evaluation of Left ventricular function has both prognostic as well as therapeutic implications in patients with heart disease. Non-invasive coronary angiography with computed tomography using 16 slices (MDCT-16) allows to obtain images of the coronary anatomy due to its high spatial and temporal resolution, and also, to obtain data regarding Left ventricular function. The objective of this study was to correlate the use of MDCT-16 for the evaluation of the Left ventricular parameters using MRI as the Standard of reference. METHODS: MRI: Se A 1.5 Tesla GE CvI Scanner optimized for cardiovascular applications was used. Using an ECG gated steady state fast precession sequence (SSFP, Thickness 10 mm, Flip Angle 45, FOV 36 cm. NEX 1, Frequency 256, Phase 128, Partial FOV 0.75, 16VPS), 6 to 8 short axis images of from base to apex of the left ventricle were obtained. Tomography: Using a 16 slice Multidetector tomograph (GE Lightspeed) and using ECG synchronization, images of the heart were obtained after the administration of 80 mls. of no-ionic contrast. The images were reconstructed off-line to obtain from 6 to 8 slices in a similar fashion to that of MR. Both studies were independently analyzed by 2 operators who obtained the ventricular function data. Linear correlation and a Paired T Student test was used to analyze the data and was considered significant when p < 0.05. RESULTS: 20 consecutive patients were evaluated with MDCT-16 and MRI, 18 males, mean age 52 +/- 15 years. There was no significant difference among the measurements for cardiac CT and MRI of the end-diastolic volume (EDV) and end-sistolic volume (ESV), stroke volume (SV), LV Mass or LV ejection fraction (LVEF). CONCLUSION: This results show a high correlation among the clinically relevant ventricular function parameters evaluated by cardiovascular CT and MRI. This findings suggest that ventricular function can be successfully evaluated along with the coronary anatomy using MDCT-16.

La evaluación de parámetros funcionales del VI tienen implicaciones tanto pronosticas como terapéuticas en pacientes con cardiopatías. La angiotomografía coronaria con TCMD-16 permite obtener imágenes de la anatomía coronaria y por su alta resolución temporal y espacial, obtener datos de la función ventricular izquierda. El objetivo de este estudio fue correlacionar el uso de la TCMD-16 para la valoración de parámetros de función ventricular izquierda, utilizando la RM como estándar de referencia. Métodos: Resonancia: Se utilizó un resonador GE Cvl, de 1.5 Teslas optimizado para aplicaciones cardiovasculares. Mediante una secuencia de precesión rápida de estado fijo de manera sincronizada al ECG, (SSFP, Grosor de corte de 10 mm, Flip Angle 45, FOV 36 cm., NEX 1, Frecuencia 256, Fase 128, FOV parcial 0.75, 16 VPS), se obtuvieron de 6 a 8 imágenes en eje corto desde la base hasta el ápex del VI. Tomografía: Con un TCMD-16 GE Lightspeed sincronizado al ECG, se obtuvieron imágenes del corazón tras la administración de 80 mis. de contraste no-iónico. Las imágenes fueron reconstruidas para obtener de 6 a 8 cortes de manera similar a la RM. Ambos estudios fueron independientemente analizados por 2 operadores quienes obtuvieron los parámetros ventriculares. Las comparaciones fueron analizadas mediante la prueba de t de Student pareada y las correlaciones mediante regresión linear, considerando significativo cuando p<0.05. Resultados: Se evaluaron 20 pacientes consecutivos con TAC y RM cardiaca, 18 del sexo masculino, con una edad promedio de 52 ± 15 años. No hubo diferencia significativa entre las mediciones por TAC y por RM cardiaca del volumen telediastólico (VTD) y telesistólico (VTS) del VI, ni en el volumen latido (VL), la masa ventricular izquierda o la fracción de expulsión del VI (FEVI). Conclusión: Los resultados demuestran una alta correlación entre los parámetros de función ventricular sistólica clínicamente relevantes evaluados por TAC y por RM cardiaca. Esto sugiere que puede evaluarse la función ventricular en forma satisfactoria al mismo tiempo que se estudian estructuralmente las coronarias mediante el TCMD-16.
The evaluation of Left ventricular function has both prognostic as well as therapeutic implications in patients with heart disease. Non-invasive coronary angiography with computed tomography using 16 slices (MDCT-16) allows to obtain images of the coronary anatomy due to its high spatial and temporal resolution, and also, to obtain data regarding Left ventricular function. The objective of this study was to correlate the use of MDCT-16 forthe evaluation of the Left ventricular parameters using MRI as the Standard of reference Methods: MRI: Se A 1.5 Tesla GE Cvl Scanner optimized for cardiovascular applications was used. Using an ECG gated steady state fast precession sequence (SSFP, Thickness 10 mm, Flip Angle 45, FOV 36 cm. NEX 1, Frequency 256, Phase 128, Partial FOV 0.75, 16 VPS), 6 to 8 short axis images of from base to apex of the left ventricle were obtained. Tomography: Using a 16 slice Multidetector tomograph (GE Lightspeed) and using ECG synchronization, images of the heart were obtained after the administration of 80 mis. of no-ionic contrast. The images were reconstructed off-line to obtain from 6 to 8 slices in a similar fashion to that of MR. Both studies were independently analyzed by 2 operators who obtained the ventricular function data. Linear correlation and a Paired T Student test was used to analyze the data and was considered significant when p < 0.05. Results: 20 consecutive patients were evaluated with MDCT-16 and MRI, 18 males, mean age 52 ± 15 years. There was no significant difference among the measurements for cardiac CT and MRI of the end-diastolic volume (EDV) and end-sistolic volume (ESV), stroke volume (SV), LV Mass or LV ejection fraction (LVEF). Conclusion: This results show a high correlation among the clinically relevant ventricular function parameters evaluated by cardiovascular CT and MRI. This findings suggest that ventricular function can be successfully evaluated along with the coronary anatomy using MDCT-16.

Se presentan seis pacientes con diagnóstico inmuno-histoquímico de tumores estromales gastrointestinales y extragastrointestinales localizados en diferentes ór-ganos del tracto digestivo y otras áreas, estudiados e intervenidos en nuestra institución a partir del año 2000, con seguimiento en su evolución clínica hasta la fecha por los servicios de hemato- oncología y cirugía; a ex-cepción de una paciente. En su mayoría sin diagnósti-co claro en su preoperatorio o con alta sospecha de presentar tumores mesenquimales complicados por masa compresiva, oclusión intestinal o sangrado.Se plantean de manera concreta los aspectos rele-vantes en su diagnóstico y clasificación patológi-ca, manejo quirúrgico y tratamiento en relación a la literatura actual, exponiendo algunas de las con-troversias existentes. Además de fijar nuestra posi-ción como grupo terapéutico frente a esta neoplasia, con base en la revisión realizada y nuestra limitada experiencia
