Introduction Auricular acupuncture (AA) can be used for both diagnosis and therapy. Diagnosis done with AA has become more prominent, with inspection by evaluating skin alterations considered the most important step. Literature on AA diagnosis in cancer patients is scarce. Globally, breast cancer (BC) is the most commonly diagnosed cancer in women. Materials and methods Subjects accessing the outpatient Breast Unit Clinic of Padua for BC were evaluated for auricle angiomas, with collected data including a number of angiomas, Romoli's Sectogram sector of identified angiomas, laterality of the auricle with the angioma, age, and laterality of BC. Results Of the 438 subjects evaluated, 129 had BC, and 64 had an identifiable auricle angioma. The odds of an auricular angioma were higher in subjects with BC diagnosis, mainly localized in tumor area II and predominantly ipsilateral to the side affected by BC. Conclusions AA auricle inspection is a simple, quick, and easy diagnostic tool. Screening for the presence and location of auricular angiomas may help health practitioners refer women for BC screening for early diagnosis.

Despite the vast potential for insights and value present in Electronic Health Records (EHRs), it is challenging to fully leverage all the available information, particularly that contained in the free-text data written by clinicians describing the health status of patients. The utilization of Named Entity Recognition and Linking tools allows not only for the structuring of information contained within free-text data, but also for the integration with medical ontologies, which may prove highly beneficial for the analysis of patient medical histories with the aim of forecasting future medical outcomes, such as the diagnosis of a new disorder. In this paper, we propose MedTKG, a Temporal Knowledge Graph (TKG) framework that incorporates both the dynamic information of patient clinical histories and the static information of medical ontologies. The TKG is used to model a medical history as a series of snapshots at different points in time, effectively capturing the dynamic nature of the patient's health status, while a static graph is used to model the hierarchies of concepts extracted from domain ontologies. The proposed method aims to predict future disorders by identifying missing objects in the quadruple 〈s, r, ?, t 〉, where s and r denote the patient and the disorder relation type, respectively, and t is the timestamp of the query. The method is evaluated on clinical notes extracted from MIMIC-III and demonstrates the effectiveness of the TKG framework in predicting future disorders and of medical ontologies in improving its performance.

Introduction: Speech to text (STT) technology has seen increased usage in recent years for automating transcription of spoken language. To choose the most suitable tool for a given task, it is essential to evaluate the performance and quality of both open source and paid STT services. Methods: In this paper, we conduct a benchmarking study of open source and paid STT services, with a specific focus on assessing their performance concerning the variety of input text. We utilizes ix datasets obtained from diverse sources, including interviews, lectures, and speeches, as input for the STT tools. The evaluation of the instruments employs the Word Error Rate (WER), a standard metric for STT evaluation. Results: Our analysis of the results demonstrates significant variations in the performance of the STT tools based on the input text. Certain tools exhibit superior performance on specific types of audio samples compared to others. Our study provides insights into STT tool performance when handling substantial data volumes, as well as the challenges and opportunities posed by the multimedia nature of the data. Discussion: Although paid services generally demonstrate better accuracy and speed compared to open source alternatives, their performance remains dependent on the input text. The study highlights the need for considering specific requirements and characteristics of the audio samples when selecting an appropriate STT tool.

In Biomedical Named Entity Recognition (BioNER), the use of current cutting-edge deep learning-based methods, such as deep bidirectional transformers (e.g. BERT, GPT-3), can be substantially hampered by the absence of publicly accessible annotated datasets. When the BioNER system is required to annotate multiple entity types, various challenges arise because the majority of current publicly available datasets contain annotations for just one entity type: for example, mentions of disease entities may not be annotated in a dataset specialized in the recognition of drugs, resulting in a poor ground truth when using the two datasets to train a single multi-task model. In this work, we propose TaughtNet, a knowledge distillation-based framework allowing us to fine-tune a single multi-task student model by leveraging both the ground truth and the knowledge of single-task teachers. Our experiments on the recognition of mentions of diseases, chemical compounds and genes show the appropriateness and relevance of our approach w.r.t. strong state-of-the-art baselines in terms of precision, recall and F1 scores. Moreover, TaughtNet allows us to train smaller and lighter student models, which may be easier to be used in real-world scenarios, where they have to be deployed on limited-memory hardware devices and guarantee fast inferences, and shows a high potential to provide explainability. We publicly release both our code on github1 and our multi-task model on the huggingface repository.2.

Active Points Test on auricle was renamed Needle Contact Test (or NCT), considered an extremely effective method to select most efficient auricle acupoint for Auricular Acupuncture (or AA) therapy. Our aims were to evaluate NCT efficiency as viable diagnostic option to identify the most appropriate AA acupoints to be selected for SupraSpinatus Tendinopathy (SST) associated shoulder pain treatment and to evaluate which auricle area has greater concentration of NCT positive acupoints in subjects with SST shoulder pain, comparing results with available AA ear maps. 45 subjects with SST diagnosis were enrolled. On 30 subjects in treatment group, NCT was performed on acupoints of the auricle shoulder representation areas (i.e., Scaphoid Fossa), while in 15 subjects of control group, NCT was performed on auricle areas not shoulder associated. Statistically significant lower Numeric Pain Rating Scale scores were seen for study group over time and compared to control. A small sample showed statistically significant increase of SST shoulder maximum abduction range of Movements. NCT identified best SST shoulder pain AA acupoints in Scaphoid Fossa (or SF1 and SF2). In conclusion NCT is a viable tool to efficiently identify the best AA acupoints in SST shoulder pain treatment.

In recent years, we have witnessed an ever wider spread of multimedia streaming platforms (e.g., Netflix, Spotify, and Amazon). Hence, it has become more and more essential to provide such systems with advanced recommendation facilities, in order to support users in browsing these massive collections of multimedia data according to their preferences and needs. In this context, the modeling of entities and their complex relationships (e.g., users listening to topic-based songs or authors creating different releases of their lyrics) represents the key challenge to improve the recommendation and maximize the users' satisfaction. To this end, this is the first study to leverage the high representative power of hypergraph data structures in combination with modern graph machine learning techniques in the context of music recommendation. Specifically, we propose hypergraph embeddings for music recommendation (HEMR), a novel framework for song recommendation based on hypergraph embedding. The hypergraph data model allows us to represent seamlessly all the possible and complex interactions between users and songs with the related characteristics; meanwhile, embedding techniques provide a powerful way to infer the user-song similarities by vector mapping. We have experimented the effectiveness and efficiency of our approach with respect to the state-of-the-art most recent music recommender systems, exploiting the Million Song dataset. The results show that HEMR significantly outperforms other state-of-the-art techniques, especially in scenarios where the cold-start problem arises, thus making our system a suitable solution to embed within a music streaming platform.

Dry Eye Syndrome (DES) is an eye disorder caused by alteration of production or composition of tear film. Symptoms include dry eye dryness, burning, itching and pain, and often is associated with emotional stress, anxiety and depression. Evidence of the effectiveness of acupuncture to treat DES has recently appeared in literature but not on acupuncture treatment of DES related anxiety or depression symptoms.A 53-year-old male, good health, no serious medical conditions, with history of contact lenses wearing, anxious personality traits and anxiety symptoms, suffering from DES, was treated with acupuncture, twice a week for three weeks with somatic and ear acupoints selected from literature.Inferior marginal lacrimal meniscus, lipid layer interferometry, tear ferning test, psychiatric examination, State-Trait Inventory -Y (STAI-Y) questionnaire and Hamilton-Anxiety Rating Scale (HAM-A) scale was evaluated before and after treatment and at 15 , 30 , 60, and 90 days follow-up.Results show clear and lasting improvements with remission of eye symptoms, together with clear and stable improvements of anxiety symptoms.Acupuncture is an effective treatment also for DES related anxiety symptoms.

INTRODUCTION: The medial surface of the auricle (MSotA), as compared to the lateral, has been less studied and has limited consensus among schools of auricular acupuncture (AA) due to its small size, greater difficulty in carrying out an adequate physical examination on it, and less precise and limited agreement on its anatomical references as compared to the lateral surface. The thumb-index technique TIT is performed using a guiding finger (taking advantage of the anatomical conformation of the lateral surface) to guide the explorer finger (placed on the MSotA) to project the therapeutic areas and land marks on the MSotA. TIT could be considered useful and effective in AA to make the most of diagnostic and therapeutic MSotA potential. METHODS: An investigation was carried out on the impact of TIT in AA practice through a survey collected from former AA students. RESULTS: TIT showed a high consensus, and is used and appreciated by AA practitioners. Discussion/ Conclusions: To date, in AA, there is no thoroughly shared nomenclature for MSotA. TIT is simple and quick to project on to MSotA the well-coded lateral surface auricular maps from French or Chinese AA schools.

With the recent COVID-19 outbreak, we have assisted to the development of new epidemic models or the application of existing methodologies to predict the virus spread and to analyze how the different lock-down strategies can effectively influence the epidemic diffusion. In this paper, we propose a novel machine learning based framework able to estimate the parameters of any epidemiological model, such as contact rates and recovery rates, based on static and dynamic features of places. In particular, we model mobility data through a graph series whose spatial and temporal features are investigated by combining Graph Convolutional Neural Networks (GCNs) and Long short-term memories (LSTMs) in order to infer the parameters of SIR and SIRD models. We evaluate the proposed approach using data related to the COVID-19 dynamics in Italy and we compare the forecasts of the trained model with available data about the epidemic spread.

The objective of the study is to evaluate the effectiveness of Radial Extracorporeal Shock-wave Therapy (RESWT) compared with High Power LASER Therapy (HPLT) for the treatment of patients with Rotator Cuff Calcific Tendinosis (RCCT). RCCT is widely diffused, it is painful and invalidating. It is an important public health problem with social and economic implications. The most common therapeutic approach is a physiotherapic one. Both HPLT and RESWT give positive results. There is a debate on which is to be preferred. Therefore there is need to obtain scientific evidence to support either case. An observational study was carried out in the period between October 2008 and September 2009 in our outpatient clinic with 62 patients, divided into 3 groups: group A 36 patients treated only with RESWT, group B 26 patients treated only with HPLT and group C 16 patients with only short term improvement with HPLT retreated with RESWT. Patients were evaluated with Constant-Murley scale before and after treatment (immediately, 1 month and 3 months) for mean constant score, pain and range of movement. Data were examined statistically with SPSS. Criteria for inclusion and exclusion were defined. Patients treated with HPLT have shown good clinical results but have returned to original syndrome 1 month after treatment. RESWT has given improvement after treatment extended in time (3 months) in terms of pain and recover of functionality with a limited number of applications. The evidence collected indicates that RESWT is the method of choice.

Osteoporosis is a systemic disease with reduced bone mass and qualitative alterations of the bone, associated to increased risk of fracture. Pathogenesis of osteoporosis fractures is multifactorial. Main risk factor is falls (except for vertebral fragility fractures which occurs often in absence of trauma). Aging by itself produces physiological changes: muscular hypotrophy with asthenia, deficit of visus and hearing together with associated pathologies and multi-drug therapies. In osteoporosis patients with vertebral fractures posture change occurs which reduces balance. After clinical postural evaluation it is possible to carry out instrumental evaluation of posture with computerized methods such as stabilometry, baropodometry, dynanometry and gait analysis. Examination carried out with use of stabilometric computerized platform allows stabilometric (body sway assessment) as well as posturometric examination (center of pressure assessment during quiet standing). Fundamental parameters obtained are: position of the body center of gravity, area and shape of sway density curve and velocity variables. Protocol of evaluation includes assessment of examination in standard condition and in condition of temporary sensorial deprivation (to investigate the influence of various afferent systems on the maintenance of posture and balance). Accurate evaluation of postural control in osteoporosis patients constitutes a fundamental tool in fracture risk evaluation due to fall and in identification and correction of modifiable factors responsible for balance defect. This approach, together with adequate drug therapy, may lead to significant reduction of fractures in osteoporosis patients with subsequent reduction of hospitalization and residual consequent disabilities.

RESUMO

This paper is a short review of the available information on peripheral non femoral fractures (PNFF) which are strictly related to osteoporosis. Particular attention is focused on wrist fractures in view of their frequent occurrence as indicated by world wide statistics. Of special interest is the definition of risk groups (old age), risk areas (developed countries) and risk factors. Reference is made to 2008 WHO guidelines for fracture risk assessment. PNFF are a serious concern to health authorities because their high incidence in constant growth, causes a serious burden on the health budget. The pathway of patient care is described, from initial evaluation (including bone fragility, multi-morbidity and risk factors) to specific diagnosis and treatment. The multiplicity of etiological factors involved requires multidisciplinary approach. This aspect justifies the importance given to rehabilitation, which is the longest part of patient care and is strictly connected to preventive measures. There is ample reference to falls, to exercises, to appropriate sports, to complications and to active and passive mobilization. The paper suggests research in areas related to PNFF, to health economics, epidemiology, prevention, health education, training and multidisciplinary coordination.

Tuberculosis is encountering a renewal attention due to emergent causes, linked primarily to migration from high-endemic countries of many subjects who present important risk factors as poverty, homelessness, drug abuse, HIV infection. The purpose of the present study is to evaluate the spread of Mycobacterium tuberculosis in social groups traditionally at higher risk of developing the disease and often difficult to detect. Tuberculin screening with Mantoux intradermal reaction has been carried out on 2014 subjects with one or more risk conditions. We have considered the following risk conditions: extra European community (e-EC), immigrants, nomads, drug dependants, immunodeficients, alcohol dependants, prison inmates living in protected community or home, subjects known to have contact with a TB case with also one or more of the above conditions. A logistic-regression model has been applied to evaluate the association between several social and/or pathological risk factors and Mantoux responsiveness. The population groups examined show higher rates than the Italian population. The majority of risk factors have a significant association (positive or negative) with Mantoux positivity. In particular, if we add to the condition of e-EC (coded as reference category) other conditions (community, contact, prison), the probability of positive Mantoux increases. In fact, the OR values are 2.92 (C.I. 95%=2.23-3.82) for "e-EC migrants + protected communities", 2.35 (C.I. 95%=1.62-3.40) for "e-EC migrants + contact" and 2.53 (C.I. 95%=1.24-5.14) for "e-EC migrants + prison". On the other hand the condition of "e-EC migrants + nomad" appears to be significantly "protective" in relation to Mantoux positivity (OR=0.23; C.I. 95%=0.16-0.33), and the same is true for "protected community + contact", "prison", "drug dependence", "drug dependence + immunodeficiency" with respective OR values of 0.37 (C.I. 95%=0.15-0.93), 0.29 (C.I. 95%=0.14-0.58), 0.20 (C.I. 95%=0.14-0.28) and 0.12 (C.I. 95%=0.04-0.41). There is a sharp difference between those socially organized and those without social reference, who seek occasional and temporary hospitality in available structures.
