BACKGROUND: Measurement of personal exposure to Der p 1 aeroallergen has previously been limited by the low quantity of material collected by sampling systems and the assay sensitivity. This has meant that exposure could only be detected if long sampling periods were used or reservoir dust was artificially disturbed. We have developed a sampling method to sample true personal exposure and combined it with a novel method which is sensitive enough to measure allergen exposure over shorter time frames. OBJECTIVE: To describe normal domestic exposure to dust mite allergen during a range of activities in houses in Sydney, Australia. METHODS: Inhaled particles containing mite allergen Der p 1 were collected using a nasal air sampler which impacts particles (> approximately 5 microm) onto a protein-binding membrane coated with a thin, porous, adhesive film. The allergen is bound to the membrane in the immediate vicinity of the particle and detected by immunostaining with monoclonal antibodies specific for Der p 1. In addition, samples were collected using a standard Institute of Occupational Medicine (IOM) personal air sampler and the amount of eluted Der p 1 was assayed by ELISA. RESULTS: The median number (range) of inhaled particles containing Der p 1 collected in each 10-min sampling period was: dust raising 5 (2-10); lying in bed, 0 (0-2); sitting on the bed, 1 (0-2); walking around the bedroom, 0 (0-2). This represented 0-5.1% of all particles captured. The Der p 1 concentration of floor and bed dust was 19.4 and 55.1 microg/g, respectively. The standard IOM personal sampler and ELISA were unable to detect Der p 1 for any of the activities performed. CONCLUSIONS: We were able to count individual allergen-carrying particles inhaled over short time periods, during different domestic exposure situations. This will offer new insight into several aspects of personal allergen exposure.

RESUMO

Lead exposure of the child population was studied in three different areas in Greece: Kalamata which is a rural area of Southern Greece; Tavros, a district of Athens with a considerable industrial activity; and Lavrion, a small city near Athens where a lead-zinc mining and smelting industrial complex has existed for more than 90 years. The results were evaluated with respect to a number of individual, social and environmental variables (i.e. smelter, occupation of the father) especially those concerning the area of Lavrion which is the most heavily polluted area in Greece. The results of this study can be considered as an index for the extent of the lead pollution problem in the named areas of Greece.

Blood lead (Pb-b) concentrations, hematocrit (Ht), and hemoglobin (Hb) were determined in three groups of men working in different parts of a cable-producing factory. All three parameters examined, i.e., Pb-b, Ht, and Hb, in the above groups were found to be within "normal" limits. However, within each group of men studied, there was a statistically significant negative correlation between Pb-b concentrations and Ht and Hb values. This finding indicates that any increase in Pb-b concentration, even within the "normal" values, may result in a decrease of hematocrit and hemoglobin values of the same subjects.

A retrospective study of 25 cases of cyanide poisoning has brought to light the following points: cardiorespiratory arrests are frequent (7/25) and often inaugural; in severe intoxications (7/25), deep metabolic acidosis is the rule, and cyanide poisoning should always be suspected in cases of coma with severe acidosis; mild intoxications are frequently symptomless. Anxiety and agitation should not be considered as evidence of cyanide poisoning; they are merely due to fear in most cases. The present treatment of acute cyanide poisoning relies basically on symptomatic measures: sodium bicarbonate, cardiac massage and, above all, assisted ventilation with 100% oxygen. Our experience does not support the concept of a lethal cyanide blood level when patients can rapidly be transferred by a medical team to an intensive care unit. Survival depends more on prompt medical care than on the accessibility to sophisticated antidotes.
