BACKGROUND: Histiocytic sarcoma (HS) is defined as neoplasm resembling morphological and immunophenotypic characteristics of mature histiocytes. It is a rare form of lymphoid neoplasms. Despite advances in treatment and diagnosis of histiocytic sarcoma, majority of cases had poor prognosis due to progressive nature of the disease. In the following article, all reported cases of histiocytic sarcoma in renal transplant patients are reviewed. METHODS: In our literature review, all relevant reports were collected electronically by entering the necessary keywords. A Boolean approach using Medical Subject Heading (MeSH) keywords was implemented. After establishing the inclusion/exclusion criteria, article titles and abstracts were evaluated by Systematic Reviews and Meta-Analyses (PRISMA) standards for 2020. All cases of histiocytic sarcoma in renal transplant patients were included. RESULT: Based on our inclusion and exclusion criteria 4 case reports were yielded in this review. Two were males and 2 were females with the mean age of 42.25 years. Fever was the most common symptom. Although tumor originated from the native kidney on one patient, the site of the primary tumor was thorax, oropharynx, and transplanted kidney in the rest. Metastasis was detected in all cases. Prednisone was used for all cases. EBV was positive in 2 cases and negative in one of them. Histology was diagnostic and similar in all cases. Immunohistochemistry was done for 3 cases. Although chemotherapy was done for 3 patients, all 4 cases ended in mortality. CONCLUSION: Despite the fact that neoplasms are post renal transplant complications, histiocytic sarcoma is a scarce and fatal entity in such patients. Histological and immunohistochemistry tests are the corner stone in diagnosis of histiocytic sarcoma.

BACKGROUND: There is insufficient data on the prevalence and consequences of eating disorders in Type 2 diabetic patients. OBJECTIVE: To evaluate the presence of eating disorders (ED) and their association with glycaemic control and metabolic parameters in adult patients with type 2 diabetes mellitus (T2DM). MATERIALS AND METHODS: A cross-sectional study on 145 patients was conducted in the medicine outpatient unit of HAHC Hospital, Jamia Hamdard tertiary care center. The Eating Attitudes Test (EAT-26) was used to screen for ED in adults with T2DM. The Score of less than 20 and more than 30 on EAT-26 questionnaire was defined as control for participants and relevant medical details like duration of treatment, glycaemic control, complications were recorded. RESULTS: A total of 145 diabetic individuals participated in this study. Out of these, 17.3% of individuals with T2DM screened positive for ED on EAT-26 scale and had a significant positive correlation in <20 groups and a significant negative correlation in >30 groups. CONCLUSION: Our study reveals that eating disorders are not very common in our clinical population of T2DM, the prevalence rates of eating disorders are lower in patients with T2DM than those reported from developed western countries.

Carbamazepine is a commonly used drug in psychiatric settings used for indications such as partial seizures, generalized tonic-clonic seizures, mixed seizure patterns, pain associated with trigeminal neuralgia, acute manic, and mixed manic states. However, this drug comes with its own series of adverse effects such as nausea, vomiting, diarrhea, dizziness, hyponatremia, headaches, and dry mouth. One serious side effect which is rarely documented includes "carbamazepine-induced pancytopenia." In this case report, we wish to highlight a known case of schizophrenia, who was taking tablet carbamazepine as adjunctive treatment and subsequently developed pancytopenia. On removing the offending agent, her counts gradually returned to normal. By reporting this case, we wish to emphasize regarding the importance of regular monitoring of complete blood count, which often gets neglected. Furthermore, psychoeducating patients regarding regular blood monitoring and the importance of adherence holds considerable importance in enhancing their compliance. By ensuring such simple preventive measures, such serious and unusual side effects may be averted through early detection of warning signs.

RESUMO

The development of stoichiometric oxidant-free regioselective annulation protocol is a challenging aspect in organic synthesis. Herein, we disclose electricity as a greener oxidant for the C-H/N-H annulation to construct cinnolines using rhodium(III) catalyst under mild conditions. A detailed mechanistic investigation revealed the possibility of both Rh(III/I) and Rh(III/IV) catalytic cycles for the formation of annulated product. Exclusive regioselectivity, diverse substrate scope, and commercially available cheap graphite electrodes are key features of this protocol.

RESUMO

Several new heterocyclic systems based on a hydroxybenzofuro[2,3-b]pyridine building block were prepared. This benzofuropyridine is easily available from the Meerwein reaction of benzoquinone and a heterocyclic diazonium salt, followed by reduction and cyclization. Electrophilic substitution and further condensations give polycyclic systems, including oxazolo- and chromeno-fused analogues.

RESUMO

A ruthenium(ii)-catalyzed coupling reaction of isatoic anhydrides and salicylaldehydes has been developed for the synthesis of 2-aminobenzoates. This reaction proceeds through metal-catalyzed decarbonylation and decarboxylation to afford good yields of aryl 2-aminobenzoates.

RESUMO

The development of metal-free syntheses toward 1,2,3-triazoles has been a burgeoning research area throughout the past decade. Despite the numerous advances, the scarceness of methods for the preparation of 1,5-disubstituted 1,2,3-triazoles from readily available substrates remained a challenge that was addressed by our group in 2016. A metal-free three-component reaction, which we have dubbed the triazolization reaction, was established for the rapid synthesis of 1,5-disubstituted, fully functionalized and NH-1,2,3-triazoles. This novel approach stands out because it utilizes widely available starting materials, namely primary amines and enolizable ketones. Furthermore, the broad substrate scope is a major advantage, and was further expanded by the number of modified protocols that have been reported. Triazolization products have successfully found utility as intermediates in various synthetic transformations, and were the subject of a few interesting biological activity studies.

Objective: To assess the pattern of serum folate and vitamin B12 levels in psychiatric inpatients compared with nonpsychiatric controls. Methods: An observational cross-sectional study was conducted with 100 psychiatric inpatients diagnosed with psychiatric illness for the first time per ICD-10 criteria and their age-matched caregivers at a super-specialty center in northern India (from January 1, 2012, to December 31, 2012). Complete blood counts and serum levels of vitamin B12, folate, and homocysteine were measured in all patients and caregivers, who were sharing the same kitchen as that of the patients. Results: Twenty-five percent of the patients were found to have low levels of serum vitamin B12, which was significant compared with healthy controls (P < .001). Similarly, the difference in homocysteine levels between the patient and control groups was significant (35% vs 13%, P = .012). Conclusions: A significant proportion of psychiatric patients were found to be vitamin B12 deficient. In-depth studies are required to establish the cause-effect relationship between vitamin B12 deficiency and psychiatric illness and the effect of vitamin B12 replacement.

RESUMO

An unprecedented Ru(II)-catalyzed C-H activation and annulation reaction, which proceeds via C-C triple bond cleavage, is reported. This reaction of 2-phenyldihydrophthalazinediones with alkynes, which works most efficiently in the presence of bidented ligand 1,3-bis(diphenylphosphino)propane, affords good yields of substituted quinazolines.

RESUMO

Valproate (VPA) is a well-tolerated and commonly used drug to treat variety of psychiatric and neurological disorders. VPA-induced hyperammonemic encephalopathy is a rare adverse effect which can commonly occur in the background of normal liver function and therapeutic serum levels. Any delay in treatment of VPA-induced hyperammonemic encephalopathy can lead to life-threatening coma thus a strong clinical suspicion, fair understanding of the pathophysiology, and management of this drug-related complication can prevent fatal outcome. We hereby report a series of cases admitted to a tertiary care center that developed hyperammonemia and all patients recovered on stopping VPA. This case series cautions the clinicians about hyperammonemia as an uncommon but highly plausible life-threatening side effect, emphasizing astute observation, and high degree of clinical suspicion to prevent mortality and limit morbidity. Early recognition of subtle gastrointestinal, cognitive, and behavioral signs can lead to immediate intervention with satisfying results.
