RESUMO

Using experimental and clinical methods, the authors studied the efficacy of combined use of anticonvulsants (phenobarbital, hexamidine, chloracon, trimetin, benzonal, diphenylhydantoin, carbamazepine) and tranquilizers (diazepam, chlordiazepoxide, meprotan, trioxazine, mebicar) in epilepsy treatment. The experimental findings showed that 13 combinations of the above drugs had a synergic effect. Five of these combinations (hexamidine-chlordiazepoxide, benzonal-chlordiazepoxide, phenobarbital-diazepam, phenobarbital-trioxazine, and phenobarbital-mebicar also proved clinically more effective than the use of anticonvulsants alone.

Using the method of concurrent radioisotope in-vitro analysis the authors examined the serum levels of thyroid hormones in 70 patients with temporal epilepsy. As a result of the examination two groups of the cases could be differentiated. In 53 patients the hormonal activity of the thyroid appeared to be lowered, while in 17 patients it was increased. A certain relationship between the clinical manifestations and course of the epilepsy on the one hand, and thyroid function on the other was revealed. More unfavourable variants of the epilepsy were found to be associated with subclinical hypothyrosis. The following phenomena are assumed to be criteria of the unfavourable course of epilepsy: a greater frequency of general convulsive paroxysms and epileptic states, early development and steady progress of the intellectual and emotional impairments; more marked and diffuse disorganization of the brain bioelectrical activity, and loss of the elastic properties by cerebral vessels.

RESUMO

Corrective therapy (by thyroidin, di-iodo-thyrosine) of disturbed hormonal activity of the thyroid gland was carried out in 30 patients with temporal epilepsy. There was a favourable immediate (1,5 years) therapeutical effect: the epileptic paroxysms either disappeared or their frequency was reduced, significant normalization of the EEG and iodine metabolism, and improvement in the mental state of the patients were shown. It is recommended to include hormonal correction of the activity of the thyroid gland in the comprehensive treatment of patients with temporal epilepsy.

Using a radioisotope concurrent analysis the authors studied the concentration of general thyroxine (T4), the coefficient of effective thyroxine, the equivalent content of free thyroxine in the blood serum and the fixating capabilities of plasma protein in relation to T3 in patients with temporal epilepsy. The study depicted complex changes of the hormonal activity of the thyroid gland. Considering the amount of the index of free thyroxine and coefficient of effective thyroxine, the state of the thyroid gland in cases of dissociated indices was qualified as hypo-, hyper- and euthyroid (respectively in 3, 8 and 10 patients). The authors discuss the possible pathogenetical significance of changes in the hormonal activity of the thyroid gland.
