RESUMO

Insulin Resistance syndromes (IR's), are a group of genetic disorders caused due a functional defect in chromosome 19p13. It is an autosomal recessive condition. Donohue Syndrome was initially described by Donohue and Uchida in 1948 and 1954, a case of sisters born to parents with a first-degree consanguineous marriage. Infants presented with typical facial features that resembled the Leprechaun elves of Irish fairy tales. The following is a report of a rare case of dental complications of Severe Insulin Resistance Syndrome. An eight year old female child, with characteristic features of severe insulin resistance syndrome, reported to the Department of Pediatric and Preventive Dentistry, Pravara Institute of Medical Sciences, Loni, presenting with cariously destructed molars and a previous history of dental treatment under local anaesthesia. Given her condition, it was decided to reduce the multiple appointments, to one appointment with all procedures done under general anaesthesia. The following case report discusses the advantages, disadvantages and post operative complications faced when forming a treatment strategy for Severe Insulin Resistance Syndrome.

INTRODUCTION: The incidence of childhood malignancy is greatest in the 1st year of life, with the second peak to be found at 2-3 years of age. The aim for the study was to investigate the incidence and nature of oral problems occurring in a population of child cancer patients; the other aim was to assess the dental management of the treatment undergoing children. MATERIALS AND METHODS: All child patients between 2 and 15 years old were admitted to the medical hospital for the treatment of newly diagnosed oral cancer over a period of 1 year were included in the study. Hundred children, 50 males and 50 females were examined in the study. Out of the 100 children, 65 children who confirmed; with the diagnosis of oral cancer were included in the study. RESULTS: Twenty children were found to have untreated dental caries at their initial dental screening; five children required dental extractions before chemotherapy. Fifty of the children in the study group did develop oral problems at some stage after their admission to the hospital. CONCLUSIONS: The delay before diagnosis varies from few weeks to approximately 10 months. Children seem particularly susceptible to the stomatotoxic effects of chemotherapy, and therefore, warrant special attention in preventing and treating these problems.

INTRODUCTION: Dentistry has found practically the best available aesthetic answer, is ceramic restoration. There are various factors that contribute to the success of ceramic veneers, like colour of underlying tooth, thickness if ceramics and the type of underlying luting cement. Shade selection and matching remains still challenge, however the shade of luting agent used for cementation of veneers produces a change in resultant shade of veneers. AIM: To compare and analyze the spectrophotometric effect of opaque and transparent luting agent on resultant shade of ceramic veneers made of 2L1.5 shade (Vitapan 3D-Masters) and B2 shade (Vitapan Classic). MATERIALS AND METHODS: Out of 15 ceramic veneers of 2L1.5 shade (VITAPAN 3D- Master), seven teeth cemented with opaque cement and eight teeth with transparent cement shade of dual cure resin cement (Variolink II(TM)). Out of 10 ceramic veneers of B2 shade (VITAPAN Classic), five teeth were cemented with opaque cement and other five teeth with transparent cement shade of dual cure resin cement (Variolink II(TM)). Spectrophotometric (Macbeth U.S.A.) analysis of all ceramic veneer crowns done with optiview software and readings were recorded in Commission Internationale de I' Eclairge {CIELAB} system and dE value was calculated. STATISTICAL ANALYSIS: Statistical analysis was done by using Paired t-test. RESULTS: Spectrophotometric analysis of all the veneers cemented with opaque luting agent were lighter in shade due to significant change in dL value. Veneers cemented with transparent luting agent were darker in shade due to significant change in the dL value. CONCLUSION: Opaque luting agent gives lighter shade and transparent luting agent gives darker shade to ceramic veneers fabricated with 2L1.5 and B2 shades.
