RESUMO

The paper describes a new magnetic resonance imaging (MRI) phased-array receive-only (Rx) coil for studying decompression sickness and disorders of hyperbaricity, including nitrogen narcosis. Functional magnetic resonance imaging (fMRI) is noninvasive, is considered safe, and may allow studying the brain under hyperbaric conditions. All of the risks associated with simultaneous MRI and HBO2 therapy are described in detail, along with all of the mitigation strategies and regulatory testing. One of the most significant risks for this type of study is a fire in the hyperbaric chamber caused by the sparking of the MRI coils as a result of high-voltage RF arcs. RF pulses at 128 MHz elicit signals from human tissues, and RF sparking occurs commonly and is considered safe in normobaric conditions. We describe how we built a coil for HBO2-MRI studies by modifying an eight-channel phased-array MRI coil with all of the mitigation strategies discussed. The coil was fabricated and tested with a unique testing platform that simulated the worst-case RF field of a three-Tesla MRI in a Hyperlite hyperbaric chamber at 3 atm pressure. The coil was also tested in normobaric conditions for image quality in a 3 T scanner in volunteers and SNR measurement in phantoms. Further studies are necessary to characterize the coil safety in HBO2/MRI.

Locally contrasting objects, e.g. a red apple surrounded by green apples, attract attention. Does this generalize to differences in feature space? That is, do unique objects-regardless of their location-stand out from a collection of objects that are similar to one another, even when the unique object has lower local contrast with the background than the other objects? Behavioral data show indeed a preference for unique items but previous experiments enabled viewers to anticipate what response they were "supposed" to give. We developed a new experimental paradigm that minimizes such top-down effects. Pitting local contrast against global uniqueness, we show that unique stimuli attract attention even in not-anticipated, never-seen images, and even when the unique stimuli are faint (low contrast). A computational model explains how competition between objects in feature space favors dissimilar objects over those with similar features. The model explains how humans select unique objects, without a loss of performance on natural scenes.

BACKGROUND: Understanding human behavior under the effects of sleep deprivation allows for the mitigation of risk due to reduced performance. To further this goal, this study investigated the effects of short-term sleep deprivation using a tilt-based control device and examined whether existing user models accurately predict targeting performance. METHODS: A task in which the user tilts a surface to roll a ball into a target was developed to examine motor performance. A model was built to predict human performance for this task under various levels of sleep deprivation. Every 2 h, 10 subjects completed the task until they reached 24 h of wakefulness. Performance measurements of this task, which were based on Fitts' law, included movement time, task throughput, and time intercept. RESULTS: The model predicted significant performance decrements over the 24-h period with an increase in movement time (R2 = 0.61), a decrease in throughput (R2 = 0.57), and an increase in time intercept (R2 = 0.60). However, it was found that in experimental trials there was no significant change in movement time (R2 = 0.11), throughput (R2 = 0.15), or time intercept (R2 = 0.27). DISCUSSION: The results found were unexpected as performance decrement is frequently reported during sleep deprivation. These findings suggest a reexamination of the initial thought of sleep loss leading to a decrement in all aspects of performance.Bolkovsky JB, Ritter FE, Chon KH, Qin M. OBJECTIVE: The aim was to determine if indices of the autonomic nervous system (ANS), derived from the electrodermal activity (EDA) and electrocardiogram (ECG), could be used to detect deterioration in human cognitive performance on healthy participants during 24-hour sleep deprivation. BACKGROUND: The ANS is highly sensitive to sleep deprivation. METHODS: Twenty-five participants performed a desktop-computer-based version of the psychomotor vigilance task (PVT) every 2 hours. Simultaneously with reaction time (RT) and false starts from PVT, we measured EDA and ECG. We derived heart rate variability (HRV) measures from ECG recordings to assess dynamics of the ANS. Based on RT values, average reaction time (avRT), minor lapses (RT > 500 ms), and major lapses (RT > 1 s) were computed as indices of performance, along with the total number of false starts. RESULTS: Performance measurement results were consistent with the literature. The skin conductance level, the power spectral index, and the high-frequency components of HRV were not significantly correlated to the indices of performance. The nonspecific skin conductance responses, the time-varying index of EDA (TVSymp), and normalized low-frequency components of HRV were significantly correlated to indices of performance ( p < 0.05). TVSymp exhibited the highest correlation to avRT (-0.92), major lapses (-0.85), and minor lapses (-0.83). CONCLUSION: We conclude that indices that account for high-frequency dynamics in the EDA, specifically the time-varying approach, constitute a valuable tool for understanding the changes in the autonomic nervous system. APPLICATION: This can be used to detect the adverse effects of prolonged wakefulness on human performance.

Finger pointing is a natural human behavior frequently used to draw attention to specific parts of sensory input. Since this pointing behavior is likely preceded and/or accompanied by the deployment of attention by the pointing person, we hypothesize that pointing can be used as a natural means of providing self-reports of attention and, in the case of visual input, visual salience. We here introduce a new method for assessing attentional choice by asking subjects to point to and tap the first place they look at on an image appearing on an electronic tablet screen. Our findings show that the tap data are well-correlated with other measures of attention, including eye fixations and selections of interesting image points, as well as with predictions of a saliency map model. We also develop an analysis method for comparing attentional maps (including fixations, reported points of interest, finger pointing, and computed salience) that takes into account the error in estimating those maps from a finite number of data points. This analysis strengthens our original findings by showing that the measured correlation between attentional maps drawn from identical underlying processes is systematically underestimated. The underestimation is strongest when the number of samples is small but it is always present. Our analysis method is not limited to data from attentional paradigms but, instead, it is broadly applicable to measures of similarity made between counts of multinomial data or probability distributions.

RATIONALE AND OBJECTIVES: To assess if ferumoxtran-10 (f-10) improves accuracy of MRI to detect lymph node (LN) metastasis in advanced cervical cancer. MATERIALS AND METHODS: F-10 MRI component of an IRB approved HIPAA compliant ACRIN/GOG trial was analyzed. Patients underwent f-10 MRI followed by extra-peritoneal or laparoscopic pelvic and abdominal lymphadenectomy. F-10-sensitive sequences were T2* GRE sequences with TE of 12 and 21. Seven independent blinded readers reviewed f-10-insensitive sequences and all sequences in different sessions. Region correlations were performed between pathology and MRI for eight abdomen and pelvis regions. Sensitivity and specificity were calculated at participant level. Reference standard is based on pathology result of surgically removed LNs. RESULTS: Among 43 women enrolled in the trial between September 2007 and November 2009, 33 women (mean age 49 ±11 years old) with advanced cervical cancer (12 IB2, 3 IIA, 15 IIB and 3 IIIB, 29 squamous cell carcinomas, 32 grade 2 or 3) were evaluable. Based on histopathology, LN metastasis was 39% in abdomen and 70% in pelvis. Sensitivity of all sequence review in pelvis, abdomen, and combined were 83%, 60%, and 86%, compared with 78%, 54%, and 80% for f-10 insensitive sequences (P: 0.24, 0.44 and 0.14, respectively). Mean diameter of the largest positive focus on histopathology was 13.7 mm in abdomen and 18.8 mm in pelvis (P = 0.018). Specificities of all sequence review in pelvis, abdomen, and combined were 48%, 75%, and 43%, compared with 75%, 83%, and 73% (P: 0.003, 0.14, 0.002 respectively) for f-10 insensitive sequences. CONCLUSION: Addition of f-10 increased MRI sensitivity to detect LN metastasis in advanced cervical cancer. Increased sensitivity did not reach statistical significance and was at the expense of lower specificity.

Hyperoxia during diving has been suggested to exacerbate hypercapnic narcosis and promote unconsciousness. We tested this hypothesis in male volunteers (12 at rest, 10 at 75 W cycle ergometer exercise) breathing each of four gases in a hyperbaric chamber. Inspired Po2 (PiO2 ) was 0.21 and 1.3 atmospheres (atm) without or with an individual subject's maximum tolerable inspired CO2 (PiO2 = 0.055-0.085 atm). Measurements included end-tidal CO2 partial pressure (PetCO2 ), rating of perceived discomfort (RPD), expired minute ventilation (V̇e), and cognitive function assessed by auditory n-back test. The most prominent finding was, irrespective of PetCO2 , that minute ventilation was 8-9 l/min greater for rest or exercise with a PiO2 of 1.3 atm compared with 0.21 atm (P < 0.0001). For hyperoxic gases, PetCO2 was consistently less than for normoxic gases (P < 0.01). For hyperoxic hypercapnic gases, n-back scores were higher than for normoxic gases (P < 0.01), and RPD was lower for exercise but not rest (P < 0.02). Subjects completed 66 hyperoxic hypercapnic trials without incident, but five stopped prematurely because of serious symptoms (tunnel vision, vision loss, dizziness, panic, exhaustion, or near syncope) during 69 normoxic hypercapnic trials (P = 0.0582). Serious symptoms during hypercapnic trials occurred only during normoxia. We conclude serious symptoms with hyperoxic hypercapnia were absent because of decreased PetCO2 consequent to increased ventilation.

We have developed hydrophobic electrodes that provide all morphological waveforms without distortion of an ECG signal for both dry and water-immersed conditions. Our electrode is comprised of a mixture of carbon black powder (CB) and polydimethylsiloxane (PDMS). For feasibility testing of the CB/PDMS electrodes, various tests were performed. One of the tests included evaluation of the electrode-to-skin contact impedance for different diameters, thicknesses, and different pressure levels. As expected, the larger the diameter of the electrodes, the lower the impedance and the difference between the large sized CB/PDMS and the similarly-sized Ag/AgCl hydrogel electrodes was at most 200 kΩ, in favor of the latter. Performance comparison of CB/PDMS electrodes to Ag/AgCl hydrogel electrodes was carried out in three different scenarios: a dry surface, water immersion, and postwater immersion conditions. In the dry condition, no statistical differences were found for both the temporal and spectral indices of the heart rate variability analysis between the CB/PDMS and Ag/AgCl hydrogel (p > 0.05) electrodes. During water immersion, there was significant ECG amplitude reduction with CB/PDMS electrodes when compared to wet Ag/AgCl electrodes kept dry by their waterproof adhesive tape, but the reduction was not severe enough to obscure the readability of the recordings, and all morphological waveforms of the ECG signal were discernible even when motion artifacts were introduced. When water did not penetrate tape-wrapped Ag/AgCl electrodes, high fidelity ECG signals were observed. However, when water penetrated the Ag/AgCl electrodes, the signal quality degraded to the point where ECG morphological waveforms were not discernible.

This study investigated the benefits of adding unprocessed low-frequency information to acoustic simulations of cochlear-implant processing in normal-hearing listeners. Implant processing was simulated using an eight-channel noise-excited envelope vocoder, and low-frequency information was added by replacing the lower frequency channels of the processor with a low-pass-filtered version of the original stimulus. Experiment 1 measured sentence-level speech reception as a function of target-to-masker ratio, with either steady-state speech-shaped noise or single-talker maskers. Experiment 2 measured listeners' ability to identify two vowels presented simultaneously, as a function of the F0 difference between the two vowels. In both experiments low-frequency information was added below either 300 or 600 Hz. The introduction of the additional low-frequency information led to substantial and significant improvements in performance in both experiments, with a greater improvement observed for the higher (600 Hz) than for the lower (300 Hz) cutoff frequency. However, performance never equaled performance in the unprocessed conditions. The results confirm other recent demonstrations that added low-frequency information can provide significant benefits in intelligibility, which may at least in part be attributed to improvements in F0 representation. The findings provide further support for efforts to make use of residual acoustic hearing in cochlear-implant users.

OBJECTIVE: The aim of this study was to examine the effects of envelope-vocoder sound processing on listeners' ability to discriminate changes in fundamental frequency (F0) in anechoic and reverberant conditions and on their ability to identify concurrent vowels based on differences in F0. DESIGN: In the first experiment, F0 difference limens (F0DLs) were measured as a function of number of envelope-vocoder frequency channels (1, 4, 8, 24, and 40 channels, and unprocessed) in four normal-hearing listeners, with degree of simulated reverberation (no, mild, and severe reverberation) as a parameter. In the second experiment, vowel identification was measured as a function of the F0 difference between two simultaneous vowels in six normal-hearing listeners, with the number of vocoder channels (8 and 24 channels, and unprocessed) as a parameter. RESULTS: Reverberation was detrimental to F0 discrimination in conditions with fewer numbers of vocoder channels. Despite the reasonable F0DLs (<1 semitone) with 24- and 8-channel vocoder processing, listeners were unable to benefit from F0 differences between the competing vowels in the concurrent-vowel paradigm. CONCLUSIONS: The overall detrimental effects of vocoder processing are probably are due to the poor spectral representation of the lower-order harmonics. The F0 information carried in the temporal envelope is weak, susceptible to reverberation, and may not suffice for source segregation. To the extent that vocoder processing simulates cochlear implant processing, users of current implant processing schemes are unlikely to benefit from F0 differences between competing talkers when listening to speech in complex environments. The results provide further incentive for finding a way to make the information from low-order, resolved harmonics available to cochlear implant users.

This study investigated the effects of simulated cochlear-implant processing on speech reception in a variety of complex masking situations. Speech recognition was measured as a function of target-to-masker ratio, processing condition (4, 8, 24 channels, and unprocessed) and masker type (speech-shaped noise, amplitude-modulated speech-shaped noise, single male talker, and single female talker). The results showed that simulated implant processing was more detrimental to speech reception in fluctuating interference than in steady-state noise. Performance in the 24-channel processing condition was substantially poorer than in the unprocessed condition, despite the comparable representation of the spectral envelope. The detrimental effects of simulated implant processing in fluctuating maskers, even with large numbers of channels, may be due to the reduction in the pitch cues used in sound source segregation, which are normally carried by the peripherally resolved low-frequency harmonics and the temporal fine structure. The results suggest that using steady-state noise to test speech intelligibility may underestimate the difficulties experienced by cochlear-implant users in fluctuating acoustic backgrounds.
