RESUMO

OBJECTIVE To study the effects of Wubao capsule on airway inflammation in asthmatic model mice by regulating upstream and downstream cytokines of type Ⅱ innate lymphoid cells (ILC2s). METHODS Totally 40 female BABL/c mice were randomly divided into normal group, model group, positive control group (dexamethasone 1 mg/kg), Wubao capsule low-dose and high-dose groups (0.5, 1 g/kg), with 8 mice in each group. Asthma models were induced by ovalbumin (OVA) sensitization and nebulization. Each group was given normal saline or drug intragastrically for 7 consecutive days. The contents of IgE and OVA-IgE in serum, the contents of interleukin 5 (IL-5), IL-9, IL-13, IL-25, IL-33, thymic stromal lymphopoietin (TSLP) and mucin 5AC (MUC5AC) in bronchoalveolar lavage fluid (BALF) were determined by ELISA. HE staining was used to observe the pathological changes of lung tissues in mice. PAS staining was used to observe the changes of goblet cell proliferation in each group. The number of ILC2s in lung tissue was determined by flow cytometry (except for Wubao capsule low-dose group). RESULTS Compared with model group, the contents of IgE and OVA-IgE in serum and the contents of IL-5, IL-9, IL-13, IL-25, IL-33, TSLP and MUC5AC in BALF were significantly reduced in Wubao capsule high-dose and low-dose groups (P<0.01). The infiltration of inflammatory cells and the thickening of basement membrane in lung tissue was alleviated to varying degrees, and the proliferation of goblet cells was inhibited; the number of ILC2s in lung tissues of mice in Wubao capsule high-dose group was significantly reduced (P<0.01). CONCLUSIONS Wubao capsule could effectively reduce the number of ILC2s in lung tissue, the contents of upstream and downstream cytokines of ILC2s in BALF of asthmatic model mice, so as to inhibit the airway inflammation and improve asthma.

Serum antibodies IgM and IgG are elevated during COVID-19 to defend against viral attack. Atypical results such as negative and abnormally high antibody expression were frequently observed whereas the underlying molecular mechanisms are elusive. In our cohort of 144 COVID-19 patients, 3.5% were both IgM and IgG negative whereas 29.2% remained only IgM negative. The remaining patients exhibited positive IgM and IgG expression, with 9.3% of them exhibiting over 20-fold higher titers of IgM than the others at their plateau. IgG titers in all of them were significantly boosted after vaccination in the second year. To investigate the underlying molecular mechanisms, we classed the patients into four groups with diverse serological patterns and analyzed their two-year clinical indicators. Additionally, we collected 111 serum samples for TMTpro-based longitudinal proteomic profiling and characterized 1494 proteins in total. We found that the continuously negative IgM and IgG expression during COVID-19 were associated with mild inflammatory reactions and high T cell responses. Low levels of serum IgD, inferior complement 1 activation of complement cascades, and insufficient cellular immune responses might collectively lead to compensatory serological responses, causing overexpression of IgM. Serum CD163 was positively correlated with antibody titers during seroconversion. This study suggests that patients with negative serology still developed cellular immunity for viral defense, and that high titers of IgM might not be favorable to COVID-19 recovery.

Platinum anticancer drugs inhibit the division of cancer cells through a DNA binding mechanism. The bimetallic platinum compounds have a possibility for blocking DNA replication via the cross-linking of DNA functional groups at different distances. Many compounds with metals of the platinum group have been tested for possible antitumor activity. The main target of their biological action is a DNA molecule. A combined approach to the study of the interaction of DNA with biologically active compounds of this type is proposed. The capabilities of various methods (hydrodynamic, spectral, microscopy) in obtaining information on the type of binding of coordination compounds to DNA are compared. The analysis of DNA binding with platinum binuclear compounds containing pyrazine, tetrazole, 5- methyltetrazole, 3-propanediamine as bridging ligands in a solution was carried out with the methods of circular dichroism (CD), luminescent spectroscopy (LS), low gradient viscometry (LGV), flow birefringence (FB) and atomic force microscopy (AFM). The competitive binding of different platinum compounds to DNA and the analysis of platinum attachment to DNA after protonation of its nitrogen bases simply indicates the involvement of N7 guanine in binding. Fluorescent dye DAPI was also used to recognize the location of platinum compounds in DNA grooves. DNA conformational changes recorded by variations in persistent length, polyelectrolyte swelling, DNA secondary structure, and its stability clarify the molecular mechanism of the biological activity of platinum compounds.

The molecular pathology of multi-organ injuries in COVID-19 patients remains unclear, preventing effective therapeutics development. Here, we report an in-depth multi-organ proteomic landscape of COVID-19 patient autopsy samples. By integrative analysis of proteomes of seven organs, namely lung, spleen, liver, heart, kidney, thyroid and testis, we characterized 11,394 proteins, in which 5336 were perturbed in COVID-19 patients compared to controls. Our data showed that CTSL, rather than ACE2, was significantly upregulated in the lung from COVID-19 patients. Dysregulation of protein translation, glucose metabolism, fatty acid metabolism was detected in multiple organs. Our data suggested upon SARS-CoV-2 infection, hyperinflammation might be triggered which in turn induces damage of gas exchange barrier in the lung, leading to hypoxia, angiogenesis, coagulation and fibrosis in the lung, kidney, spleen, liver, heart and thyroid. Evidence for testicular injuries included reduced Leydig cells, suppressed cholesterol biosynthesis and sperm mobility. In summary, this study depicts the multi-organ proteomic landscape of COVID-19 autopsies, and uncovered dysregulated proteins and biological processes, offering novel therapeutic clues. RESUMO

Severity prediction of COVID-19 remains one of the major clinical challenges for the ongoing pandemic. Here, we have recruited a 144 COVID-19 patient cohort consisting of training, validation, and internal test sets, longitudinally recorded 124 routine clinical and laboratory parameters, and built a machine learning model to predict the disease progression based on measurements from the first 12 days since the disease onset when no patient became severe. A panel of 11 routine clinical factors, including oxygenation index, basophil counts, aspartate aminotransferase, gender, magnesium, gamma glutamyl transpeptidase, platelet counts, activated partial thromboplastin time, oxygen saturation, body temperature and days after symptom onset, constructed a classifier for COVID-19 severity prediction, achieving accuracy of over 94%. Validation of the model in an independent cohort containing 25 patients achieved accuracy of 80%. The overall sensitivity, specificity, PPV and NPV were 0.70, 0.99, 0.93 and 0.93, respectively. Our model captured predictive dynamics of LDH and CK while their levels were in the normal range. RESUMO

Little is known regarding why a subset of COVID-19 patients exhibited prolonged positivity of SARS-CoV-2 infection. Here, we present a longitudinal sera proteomic resource for 37 COVID-19 patients over nine weeks, in which 2700 proteins were quantified with high quality. Remarkably, we found that during the first three weeks since disease onset, while clinical symptoms and outcome were indistinguishable, patients with prolonged disease course displayed characteristic immunological responses including enhanced Natural Killer (NK) cell-mediated innate immunity and regulatory T cell-mediated immunosuppression. We further showed that it is possible to predict the length of disease course using machine learning based on blood protein levels during the first three weeks. Validation in an independent cohort achieved an accuracy of 82%. In summary, this study presents a rich serum proteomic resource to understand host responses in COVID-19 patients and identifies characteristic Treg-mediated immunosuppression in LC patients, nominating new therapeutic target and diagnosis strategy.

The study of DNA interaction with the acetate bis(1,10-phenanthroline)silver(I) monohydrate in a solution is of interest both for understanding the mechanism of biological activity of silver compound and for forming ordered structures (DNA fibrils) that can be used to solve various problems in the field of nanotechnology. The analysis of changing the DNA conformation (secondary structure, persistent length and volume effects) during the interaction by the methods of UV spectroscopy with the analysis of DNA melting, circular dichroism, viscosity, flow birefringence, AFM (atomic force microscopy) and SEM (scanning electron microscopy) was performed. The formation of two types of complexes was observed. At lower concentration of compound in DNA solution, silver atoms form the coordination bonds with a macromolecule, while the released phenanthroline ligands intercalate between DNA bases. When the concentration of the compound increases, the phenanthroline ligands form an ordered "layer" around the helix. The excess of silver compounds in the DNA solution (with more than five silver atoms per base pair), DNA precipitation is observed with the formation of long fibrils. It was shown that the binding of silver to DNA during the formation of complexes provides further metallization of the resulting structures with the aid of reducing agents; phenanthroline ligands influence the result of such metallization.

Objective To study the impact of lung volume reduction surgery on inflammatory factors, pulmonary function and quality of life in patients with severe chronic obstructive pulmonary emphysema. Methods 57 cases patients with severe chronic obstructive pulmonary emphysema received lung volume reduction surgery from May 2009 to December 2013 were divided into observation group 32 cases and control group 25 cases, the control group were given open chest surgery, the observation group received video-assisted thoracoscopic surgery. Then compare the operation indicator, serum inflammatory factor content, pulmonary function and life quality score between the two groups. Results Operation indicators: Observation group: Intraoperative blood loss, thoracic drainage, hospital stay were significantly lower than that in control group (P0.05); SGRQ score: 12 weeks after the surgery, observation group respiratory symptoms, activity ability, disease im﹣pact, SGRQ total score were significantly lower than the control group (P< 0.05). Conclusion Video assisted tho﹣racic surgery helps to reduce surgical trauma, and alleviate inflammatory reaction, then improve the quality of life.

Objective: Design a test phantom with a full function, simple structure and high accuracy which is used for MRI’s Quality Control. Methods: Through a intensive study in imaging principle of MRI and parameters which effect the image quality ,first come up with the phantom’s performance data, and then determine the technical solutions including the choosing material, processing technology, the machining precision, at last develop a trial set of MRI test phantom using the method of numerical control machining. Make a comparative experiment between this phantom and the similar imported one to analyze the consistency of the results by the two phantoms. Results: The result of all the technical parameters of this phantom is consistent with the result of the imported one. There are some extra test components which the imported phantom doesn’t contain. Some performance data like the low contrast resolution are superior to the imported phantom. Conclusion:This MRI test phantom can satisfy the requirement of the daily quality control work in hospital.

The negative experience such as burden and positive experience such as benefits may engender in caregivers in the process of caring for stroke patients,which present both mutual independence and coexistence.Negative experience was paid more attention,but positive experience received less.The article analyzed the current situation of assessment in care burden of stroke patients' caregivers,summarized the shortcomings,made suggestions.Based on these summaries,the care benefits were suggested to do research on three levels (individual,family and social).

Objective To investigate the restricting factors of long-term care of stroke survivors in community.Methods The phenomenological methodology was adopted in the study.Sixteen stroke survivors were interviewed.Data were analyzed by Colaizzi's method.Results Three themes were found,including physical barriers,incorrect attitude towards disease,and defects of community health service.Conclusions On the basis of improving technology and ability,medical workers in community should consider of supporting the patients with information of disease and useful resources in a variety of ways,and improving the patients health literacy.

Objective To analyse the adherence of community-dwelling stroke survivors to functional exercise regimens and explore the influencing factors.Then to establish measures to improve the situation.Methods A total of 208 stroke survivors from 10 communities were randomly recruited.Four questionnaires-a general information questionnaire,an exercise adherence questionnaire,a self-efficacy perceptions scale and a perceived social support scale-were administered.Results The mean adherence score was 34.71 ± 8.45 and the mean adherence rate was 61.98%.The compliance to physical exercise,seeking advice positively and exercise effectiveness monitoring scores ranged from highest to lowest in order.Self-efficacy perceptions and perceived social support were both positively correlated with the adherence rate.Multiple linear regression showed that the influencing factors included self efficacy perceptions,social support perceptions,self-care ability and age.Conclusions The community stroke survivors' exercise adherence was moderate in general,but it could be improved by increasing their self-efficacy and social support perceptions and their self-care ability.

ObjectiveTo investigate the effect of the displacement of the selected silver clip in the different respiratory state achieved by active breathing control ( ABC ) system on the displacement of the geometry constituted by all of the silver clips at the border of the cavity in external-beam partial breast irradiation (EB-PBI).MethodsTwo sets of CT images in state of moderate deep inspiratory breathing hold (mDIBH),deep expiratory breathing hold (DEBH),and free breath (FB) were acquired in the same CT simulation assisted by ABC system for each of the 27 patients after breast conservative surgery.All silver clips in the cavity were delineated based on each set of CT images.Thereafter,the irregular geometry based on the silver clips as the vertices was automatically formed.Four selected clips located at the top,bottom,lateral border and medial border of the cavity were correspondingly manually registered based on automatic registration of the CT images acquired in the same or different state of respiration including mDIBH,FB,and DEBH.The displacement of center of the geometry in the direction of right-left (RL),anterior-posterior (AP),and superior-inferior (SI) separately based on automatic registration and manual registration was evaluated.The difference of the displacement was analyzed by Kruskal-Wallis H-test and Kolmogorov-Smirnov Z-test.Results When registered between mDIBH and mDIBH,FB and FB,DEBH and DEBH,the differences of the displacement of the center of geometry were not statistically significant (H =0.00 - 1.76,P=0.184-0.954). When registered between mDIBH and DEBH,the differences were statistically significant ( Z =11.31 - 23.00,P =0.000 - 0.001 ).There were statistically significant differences in the displacement of geometry center based on the selected silver clip between different registration forms in AP and SI directions (Z=4.76-25.54,P=0.000-0.029).ConclusionsThe difference of intrafraction displacement of the geometry constituted by the clips between the same respiratory states in the three dimensional direction is not statistically significant,but the difference is statistically significant between the different respiratory states in AP and SI directions.

Objective To study the causes of conversion to open surgery in patients receiving gynecological laparoscopic operations. Methods From January 2002 to December 2007,totally 2630 patients received gynecological laparoscopy in our hospital; 45 of them were converted to open surgery. The data of the 45 cases were analyzed. Results The causes of conversion to open surgery included uterus myoma located at specific locations (25 cases,55.6%),severe pelvic-abdominal adhesion (10 cases,22.2%),uterine horn pregnancy (3 cases,6.7%),ovarian tumors (3 cases,6.7%),surgical injury (3 cases,6.7%) ,and adenomyoma with requirement for uterus conservation (1 case,2.2%). Conclusions Peculiar locations of uterine myoma and severe pelvic-abdominal adhesion are the main causes of conversion to open surgery during gynecological laparoscopy. Detailed preoperative evaluation can decrease the rate of the conversion rate. Right timing of the conversion may reduce complication rate.

RESUMO

This is an in vitro study designed to assess the effect of temperature variation on the heart function of the five-strip eyebrow squirrel--amammal of the hibernating kind. A heart model of exsomatizing action and for determining the physiological parameters with four physiological recorders was used to measure the LVSP, LVEDP and (+/- dP/dtmax), respectively. It was found that the curves of the squirrel's heart mechanics property in relation to temperature variation are similar to the curves of the non-hibernating animals'. But, the squirrels' heart contraction function changes with in a much wider range, suggesting that their hearts are more tolerant of temperature variation.

Purpose:To study the distribution of blood pressure in the cardiovascular system of rat. Method:The experimental animal is Wistar rat (n=lS). The blood pressure in different parts of the cardiovascular system were recorded with a 4-channel polygraph system under closing condition. This method is more exact than that of manometer. Results:Our experiments demonstrated the following distribution of blood pressure in the cardiovascular system of rat. (1) The left ventricular systolic pressure(LVSP ) was 145.00+15.16 mmHg and the aortal systolic pressure(ASP ) was 126.87+11.16 mmHg. The LVSP was higher than ASP. The right ventricular systolic pressure(RVSP ) was 35.04+3.95 mrnHg and the pulmonary arterial systolic pressure(PASP) was 27.0313.46 mmHg. The RVSP was higher than PASP. This distribution of blood pressure was different with human. (2) On Value and range, the blood pressure in vascular system of rat (including A SP、A DP、MA P、PA SP、PA DP、MPA P、CVP、 WP)were clese to human. Conclusion: (1) The LVSPwas higher than ASP and the RVSP was higher than PASP. This distribution of blood pressure of rat was different with human. (2) The LVSP and RVSP of rat were higher when compared with human and the blood pressure distribution of other parts in the vascular system of rat was close to human
