POST-SURGICAL NEUROSENSORY DYSFUNCTION OF INFERIOR ALVEOLAR NERVE IN BILATERAL SAGITTAL SPILT OSTEOTOMY OF THE MANDIBLE USING SAW VERSUS PIEZOTOME: A SYSTEMATIC REVIEW AND META-ANALYSIS.

PURPOSE: The purpose of this systematic review was to compare neuro-sensory dysfunction incidence between saw and piezotomes in patients requiring bilateral sagittal split osteotomy. METHODS: Searches were performed electronically in 4 databases (PubMed, LILACS, Cochrane Library, and grey literature) up to September 2020 and manually to identify studies addressing the subject. Randomized and non-randomized clinical trials were included. RESULTS: Six studies met the eligibility criteria with a total number of 284 participants. The risk of bias assessment for randomized clinical trials was high, and for non-randomized clinical trials was critical and serious. Regarding inferior alveolar neuro-sensory dysfunction, the meta-analysis showed no significant difference between saw and piezotome 1 week [RR = 0.99, 95% CI (0.90, 1.08) P = 0.79], and 3 months [RR = 0.39, 95% CI (0.09, 1.75) P = 0.22], post-operatively. CONCLUSIONS: For patients requiring bilateral sagittal split osteotomy, piezotomes seem to offer no advantage over conventional saws regarding the incidence of neurosensory disturbance. Follow-up periods longer than 3 months may reveal faster physiologic regain of sensations. They seem to be safer than conventional saws regarding blood loss. However, proper training for using the device is mandatory, while considering the longer operating time required. Further RCTs are still recommended to improve the level of evidence.

A technique for retrieving cement-retained implant prostheses.

Lack of retrievability is one of the most important disadvantages of cement-retained implant restorations. A straightforward and cost-effective technique for accessing the abutment screw channel and unscrewing the abutment and cement-retained prosthesis as 1 unit is described. The technique does not jeopardize the integrity of the restoration, screw, abutment, or implant. A guiding acrylic resin index is fabricated for locating and guiding the access to the screw channel of the abutment, provided that the patient's cast with the implant analog is available. The procedure could be extrapolated to computer-aided designed and computer-aided manufactured (CAD-CAM) implant prostheses, whereby a CAD-CAM index could be fabricated on the digital model.

Effect of two different soft liners and thicknesses mediating stress transfer for immediately loaded 2-implant supported mandibular overdentures: A finite element analysis study.

STATEMENT OF PROBLEM: The use of soft liners as female receptacles for ball attachments retaining immediately loaded implant overdenture has been recommended to enhance osseointegration and provide a shock absorbing effect. However, which liner and which thickness is still unclear. PURPOSE: The purpose of this finite element analysis (FEA) study was to evaluate the effect of a 2- and 4-mm thickness of thermoplastic resin (TRL) and silicone-based liners (SBL) on the displacement and stresses transmitted to immediately loaded implants retaining a mandibular overdenture. MATERIAL AND METHODS: Four 3-dimensional (3D) FEA models of a mandibular implant overdenture retained by 2 ball attachments (2 models for each lining material with 2- or 4-mm liner thicknesses) were developed. Implants were placed in the canine regions and surrounded by a 1-mm cylinder of immature bone simulating immediate loading. A vertical and an oblique load of 150 N were applied in the right premolar molar regions. Stresses and displacement were set as output variables. RESULTS: Replacing the TRL by the SBL was associated with a decrease in stresses by 73% and in displacement by at least 46%. Increasing the thickness of any liner decreases stresses by 45% during vertical loading and by 25% during oblique loading and decreases displacement by 61.5% during vertical loading and by an average of 32.5% during oblique loading. None of the liners exceeded the experimental risk limits for micromotion at the bone-implant interface (50 µm). CONCLUSION: In immediately loaded mandibular implant overdentures, both SBL and TRL decrease the micromotion of implants and the stresses at the bone-implant interface. However, SBL is more effective. The thickness of both liners seems to play a major role in decreasing the stresses and displacement of periimplant tissues.
