Although the effectiveness of cardiac rehabilitation (CR) programs in secondary prevention is well-recognized, there is a lack of studies exploring the potential of mobile health to enhance educational interventions within CR. The objective is to assess the impact of a structured WhatsApp-assisted health educational intervention, in conjunction with the usual care, compared to the usual care alone among participants enrolled in a CR program. The trial will recruit 32 participants enrolled in a CR program, who will be randomly assigned to a structured WhatsApp-assisted health educational intervention plus usual care or usual care alone group. The intervention will span 4 weeks, with assessments at baseline, 4 weeks, and 3, 6, and 12 months. The primary outcome measure is the cardiovascular risk factors knowledge score. Secondary outcomes include physical activity levels, anxiety and depression, and quality of life. Expected results include improved knowledge of cardiovascular risk factors, increased physical activity levels, and better mental health outcomes in the intervention group. Additionally, an enhancement in the overall quality of life is anticipated. These findings are expected to underscore the value of integrating mHealth with traditional CR methods, potentially shaping future approaches in chronic disease management and prevention.

Electrostatic fermentation avoids the cellular redox imbalance of traditional fermentation, but knowledge gaps exist. This study explores the impact of electrostatic fermentation on the growth, volatile profile, and genetic response of Saccharomyces pastorianus Saflager S-23. The applied voltage (15 and 30 V) in the electrostatic fermentation system increased the growth and substrate utilization of S. pastorianus while decreasing ethanol production. The aromas typically associated with traditional fermentation, such as alcoholic, grape, apple, and sweet notes, were diminished, while aromas like roses, fruits, flowers, and bananas were augmented in electrostatic fermentation. RNA-seq analysis revealed upregulation of genes involved in cell wall structure, oxidoreductase activity, and iron ion binding, while genes associated with protein synthesis, growth control, homeostasis, and membrane function were downregulated under the influence of applied voltage. The electrostatic fermentation system modulates genetic responses and metabolic pathways in yeast, rendering it a promising method for tailored beer production. Demonstrating feasibility under industrial-scale and realistic conditions is crucial for advancing towards commercialization.

Introduction: Obesity is a chronic medical condition that affects, among others, the cardiovascular and respiratory systems. Interventions for its treatment focus on sustained weight reduction and general health improvement, leaving respiratory management aside. Our objective was to determine the effects of inspiratory muscle training (IMT) in patients with obesity. Methods: A systematic review was performed in Embase, Cochrane Library (CENTRAL), CINAHL, Web of Science, and PubMed/MEDLINE on June 26, 2023. Randomized clinical trials (RCTs), and quasi-randomized clinical trials investigating the effects of IMT in people with obesity were included. Selected studies were screened by two independent reviewers who extracted data and assessed the quality of the evidence. Results: The initial search returned 705 potential studies were included. Ultimately, eight studies met the criteria for eligibility and were included in the review. IMT improves physical capacity [6-minute walk test (6MWT): 44.5 m, 95% CI: 30.5 to 58.5; p < 0.0001] and the strength of the inspiratory muscles [maximal inspiratory pressure (MIP): -28.4 cm H2O, 95% CI: -41.9 to -14.8; p < 0.0001] compared to the controls, without differences in the pulmonary function, body mass index (BMI) and metabolic parameters. Conclusion: Inspiratory muscle training improves physical capacity and inspiratory muscle strength without significant changes in lung function, BMI, and metabolic parameters.Systematic review registration: PROSPERO, identifier CRD42023439625, https://www.crd.york.ac.uk/prospero/display_record.php?ID=CRD42023439625.

Introduction: Radiographs are still widely used, underestimating the risks. This situation is frequent in neonatal care units, generating radiation doses than in adults. Objective: To quantify the received radiation doses when performing radiographs on neonates and the possible factors associated with higher doses. Materials and methods: We performed an observational study of 160 neonates from the newborn unit of the Hospital Universitario San Ignacio, Bogotá, Colombia. We considered the input dose of each radiograph as the dependent variable. Patients were characterized and a multivariate analysis with multiple linear regression was performed to identify associated factors. Results: We analyzed 160 newborns and 492 radiographs. The most frequent findings were male patients (n=87, 54.4%), cesarean delivery (n=122, 76.3%), and radiograph indication for respiratory distress (n=123, 24.9%). One-point eight percent of the patients (n=9) did not have radiograph indication. The most frequently taken radiograph was chest (322, 65.4%). Most radiographs were taken with a computerized equipment (n=352, 71.5%), compared to a digital one (n=140, 28.4%). The median input dose with computerized equipment was 0.112 mGy (0.022, 0.134 mGy), and with the digital equipment was 0.020 mGy (0.019, 0.022 mGy). Conclusions: The general and specific absorbed radiation doses were measured in neonates with a computerized and a digital equipment. We identified higher doses with the computerized equipment. In addition, it was recognized the correlation between computerized radiography equipment with lower corrected gestational ages as the main factor for dose increase.
Introducción: Las radiografías continúan usándose ampliamente, subestimando los riesgos. Esto sucede, especialmente, en las unidades de cuidado neonatal, lo que implica que los neonatos reciben una dosis de radiación ionizante mayor que los adultos. Objetivo: Cuantificar las dosis de radiación recibidas al tomar radiografías y evaluar los posibles factores asociados con el aumento de la dosis. Materiales y métodos: Se llevó a cabo un estudio observacional de 160 neonatos de la Unidad de Recién Nacidos del Hospital Universitario San Ignacio, Bogotá, Colombia. Se consideró como variable dependiente la dosis de entrada en piel por cada radiografía. Se hizo la caracterización de los pacientes, seguida de un análisis multivariado con regresión lineal múltiple para identificar factores asociados. Resultados: Se analizaron 160 pacientes y 492 radiografías en total. Entre los hallazgos más frecuentes, se encuentran: pacientes de sexo masculino (n=87; 54,4 %), nacimiento por cesárea (n=122; 76,3 %) e indicación de toma de radiografías por dificultad respiratoria (n=123; 24,9 %). El 1,8 % (n=9) de los pacientes no tenían una indicación para la toma de la radiografía. La radiografía más frecuente fue la de tórax (n=322; 65,4 %). La mayoría de las radiografías se tomaron con el equipo computarizado (n=352; 71,5 %) y no con el digital (n=140, 28,4 %). La mediana de la dosis de entrada en piel con el equipo computarizado fue de 0,112 mGy (0,022; 0,134 mGy) y, con el equipo digital, de 0,020 mGy (0,019, 0,022 mGy). Conclusiones: Se cuantificaron las dosis de radiación absorbida en neonatos, general y específica, con el equipo computarizado y el digital. Se identificaron mayores dosis con el equipo computarizado. Se reconoció la interacción entre el equipo computarizado con menores edades gestacionales corregidas como principal factor para el aumento de la dosis. Además, se reconoció la relación entre el equipo computarizado y una menor edad gestacional corregida, como principal factor para una mayor dosis.

Impaired functional capacity is one of the most commonly reported consequences among post-COVID-19 patients. This study aimed to analyse the clinical variables related to functional capacity and exertional desaturation in post-COVID-19 patients at the time of hospital discharge. A cross-sectional study was conducted on patients recovering from COVID-19 pneumonia. The main outcomes measures were functional capacity, assessed using the 1 min sit-to-stand test (1 min STST), and exertional desaturation, defined as a drop of ≥4% in the arterial oxygen saturation. Factors used to characterise the participant outcomes included the use of a high-flow nasal cannula (HFNC), prolonged hospitalisation, occurrence of pulmonary embolism during hospitalisation, and underlying comorbidities. A total of 381 participants (mean age = 53.7 ± 13.2 years, 65.6% men) were included. Participants completed a mean of 16.9 ± 6.2 repetitions in the 1 min STST. Exertional desaturation was observed in 51% of the patients. Higher odds of exertional desaturation were found in the participants who used a HFNC (OR = 3.6; 95%CI: 1.6 to 7.8), were admitted in the hospital >10 days (OR = 4.2; 95%CI: 2.6 to 6.8), and had a pulmonary embolism (OR = 3.5; 95%CI: 2.2. to 5.3). Use of a HFNC (ß = -3.4; 95%CI: -5.3 to -1.44), a hospital stay >10 days (ß = -2.2; 95%CI: -3.4 to -0.9), and a history of pulmonary embolism (ß = -1.4; 95%CI: -2.6 to -0.2) were also negatively associated with the 1 min STST. Most post-COVID-19 patients exhibited reduced functional capacity at the time of hospital discharge, and approximately half had exertional desaturation after the 1 min STST. The use of a HFNC, prolonged hospitalisation and pulmonary embolism were the main clinical variables associated with worse a 1 min STST performance and a higher likelihood of exertional desaturation.

Introduction: Liver diseases have a significant impact on global morbidity and mortality rates, primarily attributed to cirrhosis and hepatocellular carcinoma. However, the true extent of their impact on patients, healthcare systems, and countries is often underestimated. Materials and methods: This descriptive, cross-sectional study aimed to determine the economic burden associated with premature deaths caused by cirrhosis and primary liver cancer. The economic assessment was conducted by analyzing potentially productive years of life lost (PPYLL) due to liver diseases in Colombia between 2009 and 2016. Results and conclusions: The total burden of liver disease accounted for 687,861 disability-adjusted life years (DALYs). Men experienced a higher number of years of life lost from mortality (YLL), while women had a greater number of years lived with a disability (YLD). The economic burden of deaths caused by cirrhosis and primary liver cancer exceeded USD 8.6 million, highlighting the urgency to enhance intervention strategies ranging from promotion and prevention to timely diagnosis and treatment.
Introducción: la enfermedad hepática representa una de las principales causas de morbimortalidad a nivel mundial, principalmente por cirrosis y hepatocarcinoma; sin embargo, se subestima su impacto para el paciente, sistema de salud y el país. Materiales y métodos: estudio descriptivo de corte transversal que determinó la carga económica asociada a las muertes prematuras por cirrosis y tumores primarios del hígado, mediante la valoración económica de los años productivos de vida potencialmente perdidos (APVPP) en Colombia y de enfermedad hepática en Colombia entre 2009 y 2016. Resultados y conclusiones: la carga total de enfermedad hepática representó 687,861 años de vida saludable perdidos ajustados por discapacidad (AVAD), los hombres con mayores años de vida perdidos por muerte prematura (APMP) y las mujeres con mayores años vividos con discapacidad (AVD). Las muertes por cirrosis y tumores primarios del hígado representan una carga económica que supera los 8,6 millones de dólares, lo cual refleja la necesidad de fortalecer las estrategias de intervención desde la promoción y prevención hasta el diagnóstico y tratamiento oportuno.

Up to 40% of rare disorders (RD) present facial dysmorphologies, and visual assessment is commonly used for clinical diagnosis. Quantitative approaches are more objective, but mostly rely on European descent populations, disregarding diverse population ancestry. Here, we assessed the facial phenotypes of Down (DS), Morquio (MS), Noonan (NS) and Neurofibromatosis type 1 (NF1) syndromes in a Latino-American population, recording the coordinates of 18 landmarks in 2D images from 79 controls and 51 patients. We quantified facial differences using Euclidean Distance Matrix Analysis, and assessed the diagnostic accuracy of Face2Gene, an automatic deep-learning algorithm. Individuals diagnosed with DS and MS presented severe phenotypes, with 58.2% and 65.4% of significantly different facial traits. The phenotype was milder in NS (47.7%) and non-significant in NF1 (11.4%). Each syndrome presented a characteristic dysmorphology pattern, supporting the diagnostic potential of facial biomarkers. However, population-specific traits were detected in the Colombian population. Diagnostic accuracy was 100% in DS, moderate in NS (66.7%) but lower in comparison to a European population (100%), and below 10% in MS and NF1. Moreover, admixed individuals showed lower facial gestalt similarities. Our results underscore that incorporating populations with Amerindian, African and European ancestry is crucial to improve diagnostic methods of rare disorders.

Background: Although the negative impact on fertility of men recovered from coronavirus disease 2019 (COVID-19) has been suggested, there is insufficient evidence, and the data are limited and contradictory. The present prospective study aimed to evaluate the sex-related hormones, semen parameters, erectile dysfunction (ED), and lower urinary tract symptoms (LUTS) in a cohort of men who recovered from COVID-19 and age-matched control men. Methods: Semen samples were collected from twenty-two men recovered from COVID-19 with a median time of 91.5 days and thirty-six control males. The semen parameters were evaluated according to the World Health Organization (WHO) laboratory manual to examine and process human semen. The blood samples were collected to assess the male hormone profile. ED and LUTS were evaluated with the International Index of Erectile Function 5 (IIEF-5) and the International Prostate Symptom Score (IPSS), respectively. Results: The follicle-stimulating hormone (FSH) (3.819±1.515 IU/L), luteinizing hormone (LH) (4.023±1.792 IU/L), prolactin (PRL) [12.60 (10.72-15.20) ng/mL], and testosterone (T) [4.345 (3.565-5.525) ng/mL] levels were at normal range in all males enrolled in the study. Levels of semen volume (control: 2.5 mL vs. COVID-19: 1.9 mL; P<0.05) and sperm concentration (control: 59×106/mL vs. COVID-19: 41.5×106/mL; P<0.005) were significantly lower in males recovered from COVID-19, but still technically well within normal regardless of WHO edition. All variables were examined through logistic regression analysis, demonstrating that only sperm concentration was an independent variable associated with men recovered from COVID-19 [odds ratio (OR) =1; 95% confidence interval (CI): 0.999-1.098; P=0.016]. According to correlation analysis, there was no correlation between sperm concentration and other semen parameters and sex-related hormone profiles. Furthermore, an absence of ED and LUTS in men who recovered from COVID-19 was evidenced using the IIEF-5 and IPSS, respectively. Conclusions: Reproductive-age males recovered from COVID-19 have normal sperm concentration. Sperm concentration did not correlate with other semen parameters, sex-related hormones, IIEF-5, and IPSS. Further studies should be performed to evaluate whether the lower sperm concentration and semen volume that were still within the normal range are a transient or prolonged downregulation resulting from the COVID-19 attack.

We aimed to describe the prevalence and factors associated with suicidal ideation in a sample of 1238 medical students from different medical schools in Peru based on question 9 of the Patient Health Questionnaire (PHQ-9). Our results revealed that 17.9% of the participants had suicidal ideation. Furthermore, using logistic regression, we found that not practicing any religion, the presence of clinically significant depression, and the presence of clinically significant anxiety were statistically related to the presence of suicidal ideation. Our results indicate that suicidal ideation was highly prevalent in the sample of medical students studied.

The pigments consumed in grazing give the milk from dual-purpose cows raised in tropical conditions a yellowish color, affecting the quality and price of the milk. This study aimed to develop an economical method with supplementary pectin to antagonize the availability of carotenes by designing microparticles with shellac and palm oil as a viable alternative to protect pectin degradation against rumen microbes. Three preparations of microparticles based on citrus pectin were synthesized: unprotected (PnP), protected with palm oil (PwP), and protected with palm oil and shellac (PwPL) microparticles. Samples were roughly characterized by spectroscopy and electron microscopy techniques. The effect of PnP, PwP, and PwPL on blood metabolites and physicochemical characteristics of the milk of grazing lactating cows was evaluated through in vivo assays. The release of citrus pectin from microparticles was determined as uronic acids using solutions with distinct pH, whereas its degradation was studied using in situ tests. Results revealed that PnP, PwP, and PwPL are amorphous structures with sizes that range from 60 to 265 nm or 750 to 3570 µm and have surface charges that range from -11.5 to -50.2 mV. Samples exhibited characteristic peaks during FTIR analyses that corresponded to O-H, C=O, and COOCH3 groups and bands within the UV-vis region that indicated the absorption of pectin. The EDS analysis revealed the presence of carbon, oxygen, or calcium in samples. The release of uronic acids was higher at pH 2-3 with PwPL. The in situ degradability of PnP, PwP, and PwPL was 99, 28.4, and 17.7%, respectively. Moreover, PwPL decreased the blood concentration of glucose, cholesterol, and lactate. In contrast, 100 g of pectin per animal daily during the feed process reduced yellow coloring. In conclusion, designing particles protected with lipids and polymers as shellac is an economical method that resists degradation at pH levels greater than five.

La displasia fibrosa (DF) o displasia ósea es una ano- malía del desarrollo óseo rara, con una frecuencia de entre el 5% y 7% de los tumores óseos benignos, dentro de estas lesiones, 50-70% son monostóticas, 20-30% son poliostóticas y 3%-10% presentan un síndrome de McCune-Albright (DF, pigmentación cutánea y puber- tad precoz). Se caracteriza por el reemplazo del hueso normal y la médula ósea por tejido fibroso. Se presenta paciente masculino de 15 años de edad, mestizo con an- tecedente de fracturas a repetición y deformidad ósea, se establece diagnóstico mediante imagen en donde se observa lesiones líticas con biopsia positiva para DF. Resumen El objetivo es conocer acerca del efecto indirecto de la inteligencia emocional en la autoeficacia académica a través del compromiso académico en una muestra de estudiantes universitarios del Ecuador. Para ello se realizó un análisis de tipo descriptivo correlacional, de determinación, de mediación parcial a través de SEM y de corte transversal. El instrumental empleado corresponde a la Brief Emotional Intelligence Inventory for Senior Citizens, la Utrecht Work Engagement Scale y la Escala de Autoeficacia Percibida Específica de Situaciones Académicas. La muestra estuvo conformada por 500 estudiantes (57.5 % mujeres y 42.4 % hombres), entre 18 y 45 años (M = 24.5 años; SD = 6.9), procedentes de dos universidades de Ambato y Quito en Ecuador y de 11 carreras de pregrado. Entre los resultados se encontró que los niveles de las variables en análisis son normales entre los estudiantes, aunque un 30 % se encuentran en zona de riesgo por baja autoeficacia académica. Se confirma la relación entre inteligencia emocional (r = .522; p < .01) y el compromiso académico (r = .326; p < .01) en la autoeficacia académica. Estas variables forman un modelo explicativo del 30.6 % del cambio de la varianza de la autoeficacia académica. Además, la inteligencia emocional ejerce un efecto indirecto sobre la autoeficacia académica por medio del compromiso académico (ab = .031; p < .01). Como conclusión se establece que la inteligencia emocional y el compromiso académico son predictores de la autoeficacia académica. Además, que el compromiso académico es una variable de mediación parcial entre la inteligencia emocional y la autoeficacia académica en universitarios del Ecuador. La inteligencia emocional tiene como propiedad que ejerce un efecto directo en la autoeficacia académica e indirecto-parcial por medio del compromiso.
Abstract The intrapersonal and self-influence elements of individuals are necessary to know the impact they maintain in the educational context, because they are strong internal motivators that direct the behaviour of the individual to carry out various activities, as well as their achievement and success. There is evidence of the capacities that students have to monitor, control and regulate their own behaviours in the learning process. Although historically academic performance has been associated with intelligence in general, elements such as self-monitoring, self-control, self-regulation, self-motivation and others are key pieces to prevent school failure and maintain an adequate school performance. Within educational research, phenomena such as emotional intelligence, engagement and academic self-efficacy stand out due to their impact on academic performance. There are several studies that analyse the existing dynamics of these three variables, although separately, so a multifactorial study could expand the current conception of these internal elements and their effect on education. This investigation's objectives are to identify the current state of emotional intelligence, engagement and academic self-efficacy in a sample of university students from Ecuador, as well as the relationship between these variables and the indirect effect of emotional intelligence on academic self-efficacy through academic engagement. Thus, this study is a descriptive correlational, determination and partial mediation analysis, carried out through SEM using the Brief Emotional Intelligence Inventory for Senior Citizens, the Utrecht Work Engagement Scale and the Scale of Perceived Self-efficacy Specific to Academic Situations. The sample was made up of 500 students (57.5 % women and 42.4 % men), between 18 and 45 years old (M = 24.5 years; SD = 6.9), from two universities in Ambato and Quito in Ecuador and 11 undergraduate majors. The participants were selected through an intentional non-probabilistic sampling with inclusion criteria, which were: (a) be legally enrolled in one of the universities in this study; (b) voluntary participation in the study; (c) signed participation consent; and (c) be of legal age. In general, the levels of emotional intelligence, engagement and self-efficacy are normal among students; however, in academic engagement, 30 % are in a risk zone. The relationship between emotional intelligence (r = 0.522; p < .01) and academic engagement (r = 0.326; p < .01) in academic self-efficacy is confirmed. Together, these variables form an explanatory model that can predict changes in variance by 30.6 %. The theoretical model proposed by means of structural linear regression has an adequate adjustment indicator χ. = 252.9; p < .001; df = 116; χ./df = 2.1; CFI = .96; TLI = .95; RMSEA = .049 [.040 - .057]; SRMR = .042. In addition, emotional intelligence has an indirect effect on academic self-efficacy through academic engagement (ab = .0031; p < .01). Among the conclusions of the study, it is mentioned that emotional intelligence and academic engagement are predictors of academic self-efficacy. Therefore, their interest and management in the educational context is relevant. Moreover, the academic engagement is a partial mediation variable between emotional intelligence and academic self-efficacy in university students in Ecuador. Emotional intelligence has the ability to exert not only direct effects, but also partial indirect effects through a third variable. One of the implications of the study is that the use of more contemporary methods such as SEM allows a broader vision of the modelling of academic behaviour, which is an important contribution because it significantly reduces measurement bias. In addition, these results contribute to the future development of psychoeducational intervention processes that contribute to the improvement of the academic performance of students and to the teaching-learning processes in general. Finally, it is important to replicate similar studies in order to generalize these findings.

Resumen El divertículo de Meckel (DM) es el remanente del conducto onfalomesentérico y se considera la malformación congénita más común del tracto gastrointestinal. La mayoría de los pacientes son asintomáticos; sin embargo, aquellos en los que se desarrollan síntomas, estos pueden manifestarse como: dolor abdominal, hemorragia de tracto gastrointestinal, perforación u obstrucción intestinal. El objetivo de este reporte es presentar un caso de hernia interna secundaria a un divertículo de Meckel, siendo esta una causa poco frecuente de obstrucción intestinal.
Abstract Meckel's diverticulum is the remnant of the omphalomesenteric duct and is considered the most common congenital malformation of the gastrointestinal tract. Most patients are asymptomatic, however patients who develop symptoms can manifest: abdominal pain, gastrointestinal tract bleeding, intestinal perforation or obstruction. The objective of this report is to present a case of internal hernia secondary to a Meckel's diverticulum, a rare cause of bowel obstruction.

Resumen Presentar la experiencia en el diagnóstico clínico-radiológico y tratamiento de vólvulo sigmoideo. Se incluyeron casos con clínica sugestiva de obstrucción intestinal secundaria a vólvulo de sigmoides. Los factores de riesgo fueron: sexo masculino, mayor estancia hospitalaria, edad mayor a 70 años y la tríada clásica. Los estudios de imagen resultaron concluyentes, siendo la radiografía de abdomen y la tomografía simple de abdomen, los de elección para el abordaje diagnóstico. El vólvulo de sigmoides tiene un patrón clínico-radiológico predecible, un examen físico minucioso y estudios de gabinete de primera línea pueden ser concluyentes. La opción terapéutica para estos pacientes fue la cirugía.
Abstract To present the experience in clinical/radiological diagnosis and treatment of sigmoid volvulus. Cases with symptoms suggestive of intestinal obstruction secondary to sigmoid volvulus were included. The risk factors were: male sex, longer hospital stay, age over 70 years and the classic triad. Imaging studies were conclusive, with abdominal radiography and simple abdominal tomography being the tests of choice for the diagnostic approach. Sigmoid volvulus has a predictable clinical-radiological pattern, a thorough physical examination and first-line cabinet studies can be conclusive. The therapeutic option for these patients was surgery.

A viable alternative for the next generation of wound dressings is the preparation of electrospun fibers from biodegradable polymers in combination with inorganic nanoparticles. A poly(vinyl alcohol)-chitosan-silver nanoparticles (PVA-CTS-Ag NPs) system has been developed for antimicrobial and wound healing applications. Here, the preparation of PVA-CTS-Ag electrospun fibers using a two-step process is reported in order to analyze changes in the microstructural, mechanical, and antibacterial properties and confirm their potential application in the biomedical field. The Ag nanoparticles were well-dispersed into the chitosan matrix and their cubic structure after the electrospinning process was also retained. The Ag NPs displayed an average diameter of ~33 nm into the CTS matrix, while the size increased up to 213 nm in the PVA-CTS-Ag(NPs) fibers. It was observed that strong chemical interactions exist between organic (CTS) and inorganic phases through nitrogenous groups and the oxygen of the glycosidic bonds. A defect-free morphology was obtained in the PVA-CTS-Ag NPs final fibers with an important enhancement of the mechanical properties as well as of the antibacterial activity compared with pure PVA-CTS electrospun fibers. The results of antibacterial activity against E. coli and S. aureus confirmed that PVA-CTS-Ag(NPs) fibers can be potentially used as a material for biomedical applications.

BACKGROUND/OBJECTIVES: Considering the high number of deaths from coronavirus disease 2019 (COVID-19) in Latin American countries, together with multiple factors that increase the prevalence of vitamin D deficiency, we aimed to determine 25-hydroxyvitamin D (25[OH]D) levels and its association with mortality in patients with critical COVID-19. SUBJECTS/METHODS: This was a prospective observational study including adult patients with critical COVID-19. Data, including clinical characteristics and 25(OH)D levels measured at the time of intensive care unit admission, were collected. All patients were followed until hospital discharge or in-hospital death. The patients were divided into those surviving and deceased patient groups, and univariate and multivariate logistic regression analyses were performed to determine independent predictors of in hospital mortality. RESULTS: The entire cohort comprised 94 patients with critical COVID-19 (males, 59.6%; median age, 61.5 years). The median 25(OH)D level was 12.7 ng/mL, and 15 (16%) and 79 (84%) patients had vitamin D insufficiency and vitamin D deficiency, respectively. The median serum 25(OH)D level was significantly lower in deceased patients compared with surviving (12.1 vs. 18.7 ng/mL, P < 0.001). Vitamin D deficiency was present in 100% of the deceased patients. Multivariate logistic regression analysis revealed that age, body mass index, other risk factors, and 25(OH)D level were independent predictors of mortality. CONCLUSIONS: Vitamin D deficiency was present in 84% of critical COVID-19 patients. Serum 25(OH)D was independently associated with mortality in critical patients with COVID-19.

INTRODUCTION: Goiter is very common in patients with acromegaly; its development is correlated to the duration of the disease. Thyroid cells express the IGF-1 receptor and the TSH/IGF-1 interaction has been demonstrated to have a synergistic effect in thyroid cell growth. There is a correlation between IGF-1 levels and the thyroid volume of patients with acromegaly. The aim of this study was to evaluate, in a retrospective case-cohort study of patients with acromegaly, the associated risk factors for thyroid nodules disease in this population. METHODS: This was a case-cohort study matched by age, gender, and growth hormone at diagnosis. Cases consisted of acromegalic patients that developed thyroid nodules during the follow up, and controls consisted in acromegalic patients without thyroid nodules. A Cox proportional hazard estimation was carried out for measure the associated risk factors for thyroid nodules disease in acromegalic patients. A nodular thyroid disease-free survival analysis was estimated using the Kaplan-Meier analysis. RESULTS: We recruited 49 cases and 56 controls. In a multivariate Cox proportional hazard analysis age and IGF-1â¯≥â¯2.2 x ULN were significantly related with the presence of thyroid nodules [HR of 2.21 (95% CI; 1.15-4.25, pâ¯=â¯0.01)]. Nodularity-free survival rates in patients who had an IGF-1 X ULNâ¯≥â¯2.2 was found to be lower in comparison to those who had IGF-1 X ULNâ¯<â¯2.2, according to a Kaplan-Meier survival analysis. CONCLUSIONS: Our findings support that exist more probability to develop thyroid nodular disease in patients with acromegaly that present IGF-1 X ULNâ¯≥â¯2.2, suggesting a possible direct effect between the time of exposure to the IGF-1 axis hyperactivity and the genesis of thyroid nodules.

El mapeo participativo es una modalidad de interacción horizontal entre los actores implicados en procesos de toma de decisiones, que tiene como finalidad la representación gráfica del territorio. Para ello, se combina la percepción local con herramientas modernas de cartografía. Con la finalidad de documentar la percepción espacial de los guardarecursos del Parque Nacional Laguna Lachuá (PNLL) con relación a la dinámica de la biodiversidad actual en la Eco-región Lachuá, se utilizó la herramienta del mapa hablado durante un taller con los 11 guardarecursos del parque en 2018. Los mapas de la Eco-región se dividieron en cuatro zonas: Noreste (NE), Sureste (SE), Suroeste (SO) y Noroeste (NO), dentro del PNLL como en la zona de amortiguamiento. Se contabilizaron los elementos favorables y desfavorables para la biodiversidad reportadas para cada zona. Se comparó la información proporcionada por los guardarecursos con las imágenes satelitales de la Eco-región mediante un análisis de Sistemas de Información Geográfica (SIG). De esta manera, se brinda un enfoque integral para comprender los patrones del paisaje actual, ya que estos son resultado de procesos socio-económicos y políticos que culminaron en conflictos del manejo de la tierra y los recursos. Se identificó que en el PNLL los elementos desfavorables más reportados fueron los cultivos, carreteras y asentamientos humanos. En la zona de amortiguamiento los elementos desfavorables corresponden a los cultivos, (p.ej.: palma africana) y los asentamientos humanos. Además, las áreas con mayor cobertura boscosa son las zonas NE y NO en el PNLL.
Participatory mapping facilitates horizontal interaction among interested parties in decision-making processes, which pursues the graphic representation of the territory. This technique unifies local perception with modern cartography. It eases communication and has been used to document spatial representations and resource management thus, integrating tools such as a Geographic Information System (GIS) might facilitate solutions in land conflicts and community governance. In this article, we share the findings regarding the spatial perception biodiversity dynamics of park rangers in National Park Laguna Lachuá (PNLL) during a workshop in 2018, documented through the "talking maps" tool. To analyze each map, we used GIS sofware, by dividing the PNLL and its buffer zone into four quadrants: Northeast (NE), Southeast (SE), Southwest (SO), and Northwest (NO). We registered every record reported by the park rangers from a list of positive and negative elements that have an effect on biodiversity. We then compared the information reported with spatial layers in GIS to evaluate which elements were detectable and viceversa. This is relevant since participatory mapping allows a more integral understanding of landscape patterns than GIS on its own and given the fact that the actual landscape is the result of several socio-economic and political processes that have created land and resource management conflicts. We identified that the unfavorable elements inside the PNLL are agriculture, roads, and human settlements, and in the buffer zone were agriculture, African palm, and human settlements. The quadrants with more forest cover corresponded to NE and NO inside the park.

Abstract Background: Unplanned extubations are safety events relatively frequent in the neonatal intensive care units (NICU). This study aimed to describe the frequency and characteristics of unplanned extubations in a NICU. Methods: We conducted a retrospective observational study of unplanned extubations in the NICU of a tertiary regional referral hospital. We reviewed medical records for data collection and performed the statistical analysis, comparing the cases of unplanned extubations with those in which it did not occur among all the cases that received intubation and invasive neonatal mechanical ventilation. Results: A total of 958 newborns were admitted to the NICU, of which 174 required assistance with invasive mechanical ventilation (18.1%) and 28 experienced unplanned extubations (16.1%): 25 patients with one episode, one with two episodes, and two with three episodes. The rate was 2.93 unplanned extubations for every 100 days of invasive mechanical ventilation in 5 years, with a significant decrease in the last three years (p = 0.0158). We found a statistically significant correlation between a weight < 1500 g and unplanned extubation in the multivariate analysis, although sedation appears to affect its interaction. Conclusions: Unplanned extubations are a relatively frequent problem, although with a tendency to decrease in recent years. The weight of the patients at birth and sedation during ventilation are important factors in this safety problem.
Resumen Introducción: La extubación no programada es un evento de seguridad relativamente frecuente en las unidades de cuidados intensivos neonatales (UCIN). El objetivo de este estudio fue conocer la frecuencia y las características de las extubaciones no programadas en una UCIN. Métodos: Se realizó un estudio observacional retrospectivo de las extubaciones no programadas en la UCIN de un hospital regional de referencia de tercer nivel. Se revisaron las historias clínicas y se realizó el análisis estadístico de los datos, comparando los casos de extubación no programada con aquellos en los que no sucedió entre todos los pacientes que recibieron intubación y ventilación mecánica invasiva neonatal. Resultados: Ingresaron en la UCIN 958 neonatos, de los cuales 174 precisaron asistencia con ventilación mecánica invasiva (18.1%) y 28 tuvieron una extubación no programada (16.1%). De estos, 25 presentaron un episodio, uno presentó dos episodios y dos presentaron tres episodios de extubación no programada. La tasa fue de 2.93 extubaciones no programadas por cada 100 días de ventilación mecánica invasiva en 5 años, con una importante disminución en los últimos 3 años (p = 0.0158). El peso < 1500 g se relacionó con las extubaciones no programadas de forma estadísticamente significativa en el análisis multivariante, aunque la sedación aparece como un factor modificador de efecto a través de su interacción. Conclusiones: Las extubaciones no programadas son un problema relativamente frecuente, aunque con tendencia a disminuir en los últimos años. El peso de los pacientes al nacimiento y la sedación son factores importantes en este problema de seguridad.
