A case is described of a 59-year-old black man with massive chyluria, probably due to previous infection with Wuchereria bancrofti. Notably, no edema was present despite a urinary protein loss of 40 gm. per day resulting in a serum albumin level of 13 gm./l. Conservative treatment after lymphography, including prolonged bedrest and a medium chain triglyceride diet, was unsuccessful. Bilateral excision of the perinephric fascia and fat (Gerota's fasciectomy) in 2 separate operative sessions finally resulted in complete resolution of the chyluria.

PIP: During the period February 1975-November 1981, 86 men who had earlier undergone vasectomy, were subjected to bilateral macroscopical vasovasostomy with the use of a splint in the anastomosis in order to reverse the sterilization. The indications for the operation, its technique, and criteria, for evaluation of success are described. The results are judged on the basis of semen analysis of 57 men performed at least 9 months after the operation. Spermatozoa were found in the ejaculate of 95% of the men (patency rate); the semen was normal in 70% of the men (success rate). The pregnancy rate amounted to 61%. The results are compared with those obtained by other investigators as published in the literature. (author's)^ieng
