RESUMO

The objective of this systematic review was to assess the effectiveness, acceptability, and safety of systemic enzyme therapy, consisting of trypsin, bromelain, and rutoside trihydrate, as an anti-inflammatory agent, either when utilized independently or in conjunction with non-steroidal anti-inflammatory drugs (NSAIDs). This systematic review adhered to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Two studies met the inclusion criteria and were assessed in the review. The bias risk was evaluated using the risk-of-bias tool for randomized trials (RoB 2). Both studies revealed highly significant results for the study population. Individuals receiving oral enzymes and diclofenac sodium combination therapy showed a significant improvement in pain reduction, better eating, and mouth opening, as well as a decrease in joint noise and jerky mandibular motions. Patients receiving systemic enzyme therapy with diclofenac combinations performed better than those receiving NSAIDs alone, and the differences were quite substantial. For the treatment of internal derangement of the temporomandibular joint (TMJ), we recommend combining enzymes and diclofenac. Systemic enzyme therapy can be used in the treatment of TMJ osteoarthritis, as it shows a highly significant result in the study population.

RESUMO

Molluscum contagiosum (MC) is a common viral infection in children that affects the skin and oral mucous membranes. It is caused by the molluscum contagiosum virus (MCV), a double-stranded DNA virus in the Poxviridae family. Transmission takes place via direct skin contact, self-inoculation, and exposure to contaminated objects. Clinically, it is characterized by the presence of a single or multiple enlarged dome-shaped or doughnut-shaped flesh-colored papules with central umbilication, usually called "mollusca". The diagnosis of MC is based mainly on clinical observations, in addition to histopathological examinations to reveal characteristic molluscum bodies, also known as Henderson-Patterson bodies. Current treatment methods include mechanical, chemical, immune modulation, and antiviral treatments. In this context, we present a case involving a 42-year-old male infected with MC, outlining both the clinical and histopathological findings.

Oral Lichen Planus (OLP), an autoimmune disorder of unclear pathogenesis affects quality of life of affected individual. Intervention regimens are multiple and still evolving due to its resistance to recover and ability to recur. Platelet rich Plasma (PRP) is a newer, promising treatment modality tested by researchers because of its low cost and negligible adverse effects. Articles were retrieved from search engines of PubMed / Medline, Scopus and Web of Science which fulfilled the eligibility criteria. Cochrane risk of bias tool assessed quality of clinical studies and Joanna Briggs Institute for case reports. A total of 4 articles were included for the systematic review, of which 2 are clinical trials and 2 case reports. All cases were of erosive nature. PRP in case reports were administered when patients did not respond to conventional therapy. PRP demonstrated effective therapeutic benefit in regards to outcome of pain and lesion appearance. PRP can be considered as a potential alternative therapy in treating non-responsive OLP. Further studies are recommended to arrive at a definitive conclusion.

Temporomandibular disorders (TMD) are a common condition affecting the musculoskeletal group evoking clinical signs such as pain, restricted mouth opening, and disability in the temporomandibular joint (TMJ), masticatory musculature, and the osseous structures in the surroundings. Saliva is a strong proponent of a diagnostic and prognostic tool for TMDs. Hence, a systematic review was undertaken to answer the research question "What is the role of salivary biomarkers in the identification of TMD?" A thorough literature search was performed in databases of PubMed, Embase, and Google Scholar till February 2022. Every included study was characterized by Study ID, location, sample size, demographic information, biomarker analysis, assessment method, and results. Newcastle-Ottawa scale was used to assess the methodological quality of all qualifying research. A total of eight articles were included for the review after screening the titles, abstracts, and full-text articles. The review included articles of observational design with a control group. TMD disorders were confirmed both clinically and radiographically in the study of Shoukri et al. TMDs are commonly prevalent in maxillofacial conditions. Despite the availability of various diagnostic techniques, certain limitations are remarkable. The researchers are yet to ascertain a gold standard biomarker to identify TMD.

RESUMO

Central granular cell odontogenic tumor (CGCOT) is a rare, benign odontogenic tumor resulting from the jaw bone, especially the mandible or maxilla. It affects women of middle age and usually occurs as a painless swelling of the mandibular premolar-molar area. CGCOT is characterized by the presence of granular cells, which are large, eosinophilic, granular-looking cells found in the tumor tissue. We report an unusual CGCOT in a 38-year-old male patient's mandibular region. We also describe the clinical, radiological, and pathological characteristics along with the immunohistochemical investigation of the tumor.

Osteochondromas (OCs) are benign bone tumors characterized by their growth with a cartilage cap and typically occurring at the ends of long bones. Their occurrence in the head and neck region is infrequent, accounting for only around 1% of head and neck tumors. Notably, the mandibular coronoid process and the mandibular condyle are the primary sites where an OC is reported. Patients often exhibit facial asymmetry, limited mouth opening, and malocclusion. Possible treatment options depending on the condition include partial or total condylectomy, vertical ramus osteotomy, and supplementary orthognathic surgery. The recurrence rate of under 1%- 2% is reported after local resection.  In this case report, we present a unique case of an OC in a 27-year-old woman. It involved the mandibular condyle, resulting in a left-sided mouth deviation while opening and closing her mouth. The purpose of this article is to detail the clinical and radiographic features, histopathological aspects, and treatment strategies and differentiate potential diagnoses, for such OCs.

Background: The oral cavity is a unique environment where systemic maladies may be amplified by the oral mucosa. Oral diseases are usually local, but may also be the sign of systemic disease. Oral lesions are mostly one of the first indications of a systemic problem. Lichen planus (LP) is an inflammatory disease that involves the skin and mucous membrane. It is one of the most common oral diseases that manifest itself in the oral cavity. The exact cause of oral lichen planus (OLP) is unknown, but the immunologic system plays a leading role in the pathogenesis. It is well documented that OLP represents a cell-mediated immune response. Materials and Methods: The study population was drawn from the patients attending the outpatient Department of General Medicine and Department of Oral Medicine and Radiology with a total of 1000 subjects, out of which 500 subjects were clinically diagnosed with hypothyroidism and 500 subjects were without the history of hypothyroidism. The data collected was compiled and analysed to obtain the result. A Chi-square test was used to compare the categorical variables and the analysis was carried out on SPSS 16.0 version. Results: The study revealed an increased prevalence of OLP in hypothyroidism. Cases clearly had a predilection of 2.37 times more tendency to develop OLP as compared to the control. Conclusion: To conclude, it seems that OLP was more prevalent in hypothyroid individuals, especially in females in the third and fourth decades of life.

Angiogenesis is a complex process depending on the coordination of many regulators and there by activating angiogenic switch. Recent advances in understanding of angiogenic mechanism have lead to the development of several anti-angiogenic and anti-metastatic agents that use the strategy of regulation of angiogenic switch. Antiangiogenic therapy is a form of treatment not cure for cancer and represents a highly effective strategy for destroying tumour because vascular supply is the fundamental requirement for growth of tumour. Because of the quiescent nature of normal adult vasculature, angiogenic inhibitors are expected to confer a degree of specificity when compared to nonspecific modalities of chemo and radiotherapy, so it has the advantage of less toxicities, does not induce drug resistance and deliver a relatively non toxic, long term treatment of tumour.

Malignancy is characterized by anaplasia, invasiveness, and metastasis. Primary oral squamous cell carcinoma is the most prevalent oral malignancy, but secondary malignancy from distant sites have also been reported. Hepatocellular carcinoma (HCC) is a common primary liver malignancy that frequently metastasizes during the course of the disease, but < 1% of cases show oral involvement. Such secondary neoplasms do not have any pathognomonic clinical or radiologic findings, and therefore they pose a diagnostic challenge. Hence, in the differential diagnosis of malignant tumors of the oral cavity, it is essential to consider the occurrence of both primary as well as metastatic tumors despite the low incidence of the latter. A rare case of HCC metastasizing to both the maxilla and mandible is presented, in which the patient succumbed to the disease as a result of the delay in diagnosis.

Central ossifying fibroma (COF) is a rare benign fibro-osseous neoplasm which has a predilection for mandible and is encountered in middle aged women. It arises from mesenchymal blast cells of the periodontal ligament, and with a potential to form fibrous tissue, cementum and bone. It is a well circumscribed lesion mainly comprising of fibrous tissue with varying amounts of bone or cementum or osteo-cementum like tissue. We present an uncommon case of COF of the maxilla in a 35-year-old male who presented with a pus discharging sinus in the maxilla. Due to the bizarre size of the lesion we have termed as 'giant' COF. There is extensive facial asymmetry on the right side with obliteration of the maxillary antrum, deviation of the nose and upward displacement of the orbit.

A nasoalveolar cyst is a rare, non-odontogenic soft tissue cyst encountered in the anterior maxillary labial sulcus as an asymptomatic soft tissue swelling. Often, patients with these cysts report them to the dental clinic where they are mistaken for odontogenic lesions by the dental surgeon, especially if concomitant dental problems are present. They cannot be detected by routine conventional dental radiography as they are peripheral, lying within the mucosa thereby posing a diagnostic challenge. We document a case of a 47-year-old woman with a nasoalveolar cyst.

Dentistry is most importantly and foremost a healing profession. In today's world, it is very necessary to understand the importance of being eco-friendly in every facet of our lives, including dental practice which has a huge impact on the environment due to the large amount of metallic waste generated by various dental procedures along with excessive use of water and electricity, which specifically emphasis the thrust to move towards 'Green dentistry'. Green dentistry is an innovative way of dental practice which is environment friendly and at the same time conserves money and time by reducing waste, conserving energy and decreasing pollution with the use of latest techniques and procedures. Green dentistry therefore, protects the environment and mankind from the hazards of rapid urbanisation in developing countries. The authors wish to emphasize the practice of eco-friendly, green dentistry in a developing country like India which needs to conserve resources and curb environmental pollution.

Matrix metalloproteinases (MMPs) are a family of zinc dependent extracellular matrix (ECM) remodelling endopeptidases having the ability to degrade almost all components of extracellular matrix and implicated in various physiological as well as pathological processes. Carcinogenesis is a multistage process in which alteration of the microenvironment is required for conversion of normal tissue to a tumour. Extracellular matrix remodelling proteinases such as MMPs are principal mediators of alterations observed in the microenvironment during carcinogenesis and according to recent concepts not only have roles in invasion or late stages of cancer but also in regulating initial steps of carcinogenesis in a favourable or unfavourable manner. Establishment of relationships between MMP overproduction and cancer progression has stimulated the development of inhibitors that block proteolytic activity of these enzymes. In this review we discuss the MMP general structure, classification, regulation roles in relation to hallmarks of cancer and as targets for therapeutic intervention.

RESUMO

Emperipolesis is an uncommon biological process, in which a cell penetrates another living cell. Unlike in phagocytosis where the engulfed cell is killed by lysosomal enzymes of the macrophage, the cell exists as viable cell within another in emperipolesis and can exit at any time without any structural or functional abnormalities for either of them. This process can either be physiological or pathological and may be a pathognomonic feature of certain diseases. Histiocytes and Megakaryocytes are involved in Emperipolesis normally but tumour giant cells in Hodgkin's disease and Rosai-Dorfman disease are pathologic conditions in which this process is seen. We report a mini review as this process is rare and not much reported in the literature.

Oral soft tissues are affected by numerous pathologic conditions of variable etiology and hence their appropriate management relies on their accurate diagnosis. Clinical identification of intact vesicle and bulla in the oral cavity is really a challenge due to the regular irritation and the friable nature of oral mucosa. Rupture of these lesions leads to erosions or ulcerations on the surface, hence making the diagnosis of vesiculobullous (VB) lesions is even more difficult due to the fact that the differential diagnosis along with VB lesions will also include ulcerative, immunological-mediated diseases, and neoplasms and systemic diseases. Hence, knowledge of the clinical presentation of these disorders and the relevant diagnostic procedures is important not just for dermatologists, but also for general practitioners and dentists. In this article, the various procedures have been explained that can be used for the diagnostic purpose of VB lesions.

The significance of proper handling of biopsy specimens is important and obvious for any histopathological diagnosis. Accuracy of tissue diagnosis by the pathologists is heavily dependent upon the competence of histotechnologists. The aim of a good histopathological technique is to produce microscopic preparation of tissues, usually stained, that represents as closely as possible, their structures in life. But this is not always possible and some sort of tissue morphology is bound to be seen. The ability of oral pathologists in interpreting a biopsy correctly is directly proportional not only to the quality, but also to the quantity of the specimen. There are many ways through which the exact interpretation of tissue specimen becomes compromised and the major pitfall is the presence of artefacts. These artefacts may occur during surgical removal, fixation, processing, embedding, microtomy and staining procedures. It is therefore important to identify the common occurring artefacts during interpretation of oral biopsies.

OBJECTIVES: The study evaluated pathognomic histopathological features with the help of light microscopy for detecting the integration of human papillomavirus (HPV) (type 16 and 18) in oral squamous cell carcinoma (OSCC). MATERIALS AND METHODS: Forty-five histopathologically diagnosed cases of OSCC were evaluated for the presence of E6/E7 protein of HPV (16 + 18) with the help of nested multiplex polymerase chain reaction. Both HPV-positive and -negative cases were evaluated for four histological features: Koilocytes, dyskeratosis, invasion, and alteration of collagen. RESULTS: Fischer's exact test showed significant difference (P < 0.01%) for the presence of koilocytes and dyskeratosis, whereas no difference was observed for invasion and alteration in collagen between HPV-positive and -negative OSCC. CONCLUSION: The presence of koilocytes and dyskeratosis at light microscopic level can be used as a marker for the presence of HPV (type 16 and 18) in OSCC.

Odontogenic tumors develop in the jaw bones from the odontogenic tissue-oral epithelium in tooth germ, enamel organ, dental papilla, reduced enamel epithelium, remnants of Hertwig's root sheath or dental lamina, and so forth. Hence, a bewildering variety of tumors are encountered in the maxilla and mandible. Ameloblastoma is the second most common odontogenic neoplasm after odontomes, and it has numerous clinical and histologic variants. We report a very rare histologic variant: the papilliferous keratoameloblastoma which is the fifth reported case in the English literature.

Recent attention has been directed toward the role of mast cells in the pathogenesis of oral lichen planus. Mast cells are responsible for trafficking inflammatory cells into the connective tissue that in turn helps in progression and maintenance of chronicity of oral lichen planus (OLP). OLP is a T-cell-mediated chronic inflammatory oral mucosal disease of unknown etiology, and lesions contain few B-cells or plasma cells and minimal deposits of immunoglobulin or complement. Hence, OLP is ideally positioned for the study of human T-cell-mediated inflammation and autoimmunity. This study was done to evaluate the mast cell count using toluidine blue stain in OLP and compares it with oral lichenoid reaction (OLR), and to propose the possible role of mast cells in the pathogenesis of OLP and OLR. Ten cases each of OLP and OLR and five cases of normal buccal mucosa were taken from the archives of Department of Oral Pathology. The samples were stained with toluidine blue using standard toluidine blue method by Wolman 1971. An increase in mast cell count was observed in OLP and OLR in comparison to normal oral mucosa. However, no significant differences in mast cell count were noted between OLP and OLR.
