RESUMO

A wide variety of synthetic methods have been developed for the synthesis of functionalized aliphatic amines because of their broad utility in both synthetic and medicinal chemistry. The synthesis of functionalized aliphatic amines via direct C-H functionalization of readily available aliphatic amines, the majority of which rely on the use of metallic reagents/catalysts and hazardous oxidants, is advantageous in comparison to the classical multistep approaches. However, the scope to carry out such direct C-H functionalization of aliphatic amines under metal and oxidant-free conditions is being continuously explored. As a result, the examples of C-H functionalization of aliphatic amines employing iminium/azonium ions, which are formed via classical condensation of amines and carbonyl/nitroso compounds, are on the rise. This article summarizes the recent developments in the iminium and azonium-activated metal and oxidant-free C-H functionalization of aliphatic amines with the main focus on the intermolecular reactions of iminium/azonium ions, enamines, and zwitterions with suitable nucleophiles, electrophiles and dipolarophiles.

RESUMO

The kidney is an essential organ that removes waste products, balances the body's fluids, releases hormones that regulate blood pressure, produces an active form of vitamin D, promotes healthy bones, and controls the production of red blood cells. Structural and functional abnormalities occur in kidney with age. Alterations in kidney structure are based on physiological functions and environmental pressures. Variations in its structure across vertebrates are primarily due to the nature of alterations in number, complexity, arrangement, and location of the kidney tubules. Globally, individuals aged 65 and older are part of the fastest expanding population demographic, and as a result, a greater number of older patients are receiving a diagnosis of impaired renal function. The purpose of our mini-review is to summarize recent findings of the structural and functional differences between the normal and aging kidney, examine the evolutionary biology of the kidney across species, and demonstrate the role of aging in conditions such as diabetes, chronic kidney disease, and hypertension, along with their impact on SARS-CoV-2. Additional aims include discussing the potential therapeutic strategies to treat aged individuals with kidney health issues and how the impact of a healthy lifestyle, diet, and exercise can improve health conditions with aged kidneys.

Background and Objectives: Heart failure (HF) is a complex syndrome with multiple etiologies resulting in impaired ventricular filling or pumping of blood. HF is as a major public health concern that leads to significant morbidity and mortality resulting in an enormous financial burden on the healthcare system. The study objectives were to assess the 30-day hospital readmission rates and its financial impact on the hospital. Methods: The study was a retrospective single-center analysis of decoded data of all HF patients admitted to an outpatient diuretic infusion program. Adult patients who were readmitted to the hospital within 30 days despite guideline derive medical therapy were included if they were enrolled in the outpatient diuretic infusion clinic. Adult patients who were included in this study received a furosemide dose of 40 mg intravenously (infusion over 3 hours) at the clinic visit. Patients whose clinical signs/symptoms improved and remained stable in consequent visits were eventually discharged from the clinic. Financial impact was assessed using data obtained from the hospital administration on cost of HF readmissions. Results: The results show a 30-day hospital readmission rate at 6-9% in the years analyzed (n=56) with a net savings of $562,815 to $736,560 per year. Conclusions: This treatment strategy has no detrimental effects in addition to generating substantial financial savings. It appears to be a useful addition to the existing medical treatment regimens chronic HF patients.

Although children comprise the fewest cases of COVID-19 infection, symptoms, and complications among the various age groups affected, new long-term consequences are being reported. Here, we report a case of severe costochondritis unresponsive to traditional management in a child who had COVID-19 infection a few months earlier. To our knowledge, this is the first reported case of post-COVID-19 costochondritis (PCC) that has been successfully managed with colchicine. We recommend the consideration of colchicine as therapy for PCC in children presenting with severe musculoskeletal chest pain unresponsive to nonsteroidal anti-inflammatory drugs or steroids. Physicians should maintain a high clinical suspicion for PCC to prevent unnecessary steroid treatment, frequent emergency department visits, and potential for drug abuse in these patients with severe chest pain.

Individuals often experience ailments such as allergies, asthma and respiratory tract infections throughout the year. Weather reports often include estimations of common allergens that can affect these individuals. To describe the local 'atmospheric microbiome' in Lubbock, Texas, USA, we examined the culturable fungal and bacterial microbiome present in the air on calm and dust storm days using internal transcribed spacer (ITS)-1 and 16S rRNA amplicon sequencing, respectively. While some types of airborne fungi were frequently present throughout the year, distinct differences were also observed between calm and dust storm days. We also observed the influence of the origin of air parcels and wind elevation of the air trajectory. The most abundant genera of fungi identified during the study period were Cryptococcus, Aureobasidium, Alternaria, Cladosporium and Filobasidium. This observation was not surprising considering the agricultural intensive environment of West Texas. Interestingly, Cladosporium, a common allergenic mold, was increased during days with dust storm events. The predominant bacterial genera observed were Bacillus, Pseudomonas, Psychrobacter, Massilia and Exiguobacterium. The relative abundance of the psychrophiles, Psychrobacter and Exiguobacterium, was surprising, given the semi-aridity of West Texas. Coupling our observations with back trajectories of the wind (Hybrid Single-Particle Lagrangian Integrated Trajectory models) demonstrated that dust storms, regional anthropogenic activity and origin of air parcels are important influences on the diversity and temporal presence of the atmospheric microbiome.
