RESUMO

PATIENTS AND METHODS: We describe the outcome of the treatment of 1.000 consecutive patients treated since may 2000 to december 2003 with Siemens Lithostar Modularis to evaluate the efficacy of this new lithotripter RESULTS: 60.5% male and 39.5% female's patients were treated. Average age of 53.5 years old. 44.1% of the urinary stones were located on the right side and 55.9% on the left. The mean size of the stones was 1.22 +/- 0.74 cm with 59% lesser than 1 cm, 27.6% between 1 and 2 cm and 13.4% bigger than 2 cm. Mean number of sessions required were 1.6. There were differences in the number of sessions related to the size of the lithiasis, 1.3 session in the group of lithiasis smaller than 1 cm and 2.01 sessions in the group bigger than 1 cm. The stones that required less number of sessions were located in the ureter and upper calyx. 71.2% of the patients were stone free after one treatment. CONCLUSIONS: Siemens Lithostar Modularis has the adventages of a great confort of the patient during the procedure, better image because of a very good cuality of the fluoroscopy and the main point is a great comminution of the stone doing very easy the elimination of the fragments achieving a stone free rate of more than 90% in our experience, decreasing the need of auxiliary manoeuvres and the leng of the treatment.

RESUMO

OBJECTIVES: This study analyzed various factors that may affect social and personal consequences of disease in urological patients, as well as any possible relationships between these factors and disadvantageous situations they might produce. METHODS: A two-year retrospective study on patients of the University Hospital of Valladolid was conducted. The material used included a thorough analysis of case histories and questionnaires comprising all factors considered of interest by the most relevant Bodily Harm Valuers. The data obtained were processed and analyzed by the Statistics and Operations Research Departments of the University of Valladolid. RESULTS: In our study, sex and age were remarkably influential on sexual, economic and disablement damages. The different types of illnesses and the types of operations performed (non-surgical operations presented less consequences than surgical ones), as well as other factors such as distance from the patients' home to hospital, had a statistically significant correlation with the consequences of disease. CONCLUSIONS: We can detect certain disadvantageous situations through a previous study of the different variables that are collected in case histories, such as social, personal and economic circumstances. Thus, we can plan support programs in the future that will certainly reduce the consequences of disease.

RESUMO

We present the results achieved by catheter dilatation in 31 cases of ureteral strictures of different etiologies, using a large caliber (12 Fr) double pigtail catheter. The patients had a mean follow up of at least two years. The catheter was left indwelling for 2 1/2 months. The success rate was 74.2%, with few and unimportant complications. However, in 3 cases with strictures of a tuberculous etiology the long-term success rate dropped to 33.3%. The results indicate that ureteral stents of a caliber larger than that used normally, increase the success rate in dilatation of ureteral strictures.

RESUMO

Percutaneous endopyelotomy is becoming widely accepted in the treatment of primary or secondary stenosis of the ureteropelvic junction because it achieves excellent results and is less invasive. Herein we describe our experience with this technique, which we perform with the electric knife and large caliber ureteral stents. Seventeen endopyelotomy procedures were performed in 16 patients with a minimum follow-up of one year. This technique successfully resolved the stenosis in 14 cases. We consider it to be the procedure of choice due to the excellent results and the low complication rate.

RESUMO

Transurethral incision of the prostate (TUIP) or prostatotomy is a recently developed surgical technique whose favourable results have been reported elsewhere to range from 88% to 92%. It has a precise indication in small prostates and short urethras, and in well-selected cases its results are superior to that achieved by transurethral resection (TUR). Moreover, alterations of sexual function (impotence, retrograde ejaculation, etc.) are less and it affords the following advantages: the surgical technique is easy to perform, postoperative hospital stay is minimal, and patient comfort is enhanced. We have reviewed the first 100 prostatotomy procedures performed at the Urology Department of our hospital. Patient follow-up was 1 1/2 years. Good results were achieved in 65 (95.5%) of 68 patients with prostates less than 20 gms. In 20 patients with prostates between 20-30 gms., we achieved a success rate of 85% (3 poor results). The results were poor in 4 of 12 patients (60% success) with prostates more than 30 gms. The early complications were minimal and no late complications were observed. Thus, we believe this is a valid technique when performed in the appropriate cases.
