RESUMO

Medial quadriceps tendon-femoral ligament (MQTFL) reconstruction for prevention of recurrent patella dislocation is an alternative to medial patellofemoral ligament (MPFL) reconstruction. Because the reconstruction graft attaches to the quadriceps tendon, no patella drill hole is required, thereby eliminating iatrogenic fracture risk. The procedure remains anatomically accurate and early results are comparable to MPFL reconstruction for preventing patella dislocation. The MQTFL reconstruction graft is brought up under the vastus medialis obliquus distal to the patella apex, such that its orientation is directed toward the medial patellofemoral complex (MPFC) midpoint, also known as Tanaka's point. The graft is then secured by looping it around the vastus medialis and rectus femoris tendons, after which optimal graft length is easily established by cycling the knee, after which it is sutured securely into the deep quadriceps tendon precisely at the anatomic midpoint of the MPFC. Anatomic reconstruction of the MQTFL-in which graft orientation is crucial-confers reliable patellofemoral joint stability in the surgical treatment of patients with recurrent patella dislocations.

BACKGROUND: Surgeon- and patient-specific characteristics as they pertain to total shoulder arthroplasty (TSA) are limited in the literature. The influence of surgeon upper extremity dominance in TSA and whether outcomes vary among patients undergoing right or left TSA with respect to surgeon handedness have yet to be investigated. PURPOSE: To determine whether surgeon or patient upper extremity dominance has an effect on clinical outcomes after primary TSA at short-term follow-up. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A retrospective chart review was performed on prospectively collected data from an institutional shoulder registry. Patients who underwent primary TSA for glenohumeral osteoarthritis from June 2008 to August 2012 were included in the study. Preoperative and postoperative American Shoulder and Elbow Surgeons (ASES), Simple Shoulder Test (SST), and visual analog scale (VAS) pain scores were evaluated. To determine the clinical relevance of ASES scores, the minimal clinically important difference (MCID), the substantial clinical benefit (SCB), and the patient acceptable symptom state (PASS) were used. Active forward elevation, abduction, and external rotation were recorded for each patient. Glenoid version was also evaluated preoperatively on standard radiographs. RESULTS: Included in this study were 40 patients (n = 44 shoulders; mean age, 69.0 ± 7.3 years) with a mean follow-up of 36.5 ± 16.2 months. Final active range of motion between patients who underwent dominant versus nondominant and left versus right TSA by a right-handed surgeon was not significantly different. Clinical outcomes including the ASES, SST, and VAS pain scores were compared, and no statistical significance was identified between groups. With regard to the ASES score, 89% of patients achieved the MCID, 64% achieved the SCB, and 60% reached or exceeded the PASS. No significant difference in preoperative glenoid version between groups could be found. CONCLUSION: With the numbers available, neither patient nor surgeon upper extremity dominance had a significant influence on clinical outcomes after primary TSA at short-term follow-up. CLINICAL RELEVANCE: The influence of surgeon and patient upper extremity dominance on TSA outcomes is an important consideration, given the preferential use of the dominant extremity exhibited by most patients during activities of daily living. To this, operating on a right shoulder might be technically more demanding for a right-handed surgeon and vice versa, as it is considered in other subspecialties.

PURPOSE: To report the incidence of posterior medial meniscocapsular junction (PMCJ) separation in patients with anterior cruciate ligament (ACL) injury and to evaluate its biomechanical effect on the ACL. METHODS: Three hundred thirty-seven consecutive patients undergoing isolated primary ACL reconstruction were retrospectively analyzed for PMCJ lesion. Forty-four patients were identified with PMCJ lesion and studied. Eight cadaver knees underwent biomechanical testing to determine anterior tibial displacement and anteromedial bundle ACL strain in the intact, PMCJ lesion, and PMCJ repair states at 0°, 30°, 60°, and 90° of flexion. Mixed-effects linear regression with Bonferroni correction was used for statistical analysis. RESULTS: PMCJ tear incidence with ACL disruption was 13.1%. Specimen with PMCJ tears had statistically increased anterior tibial translation at 30° (1.2 mm; P < 0.01) and statistically increased ACL strain at 30° (24%; P < 0.01) and 90° (50%; P < 0.01). With PMCJ repair, translation reduced (P > 0.05) by 12%, 18%, and 10% at 0°, 30°, and 90° of flexion, respectively. PMCJ repair reduced (P < 0.05) ACL strain by 40%, 39%, 43%, and 31% at 0°, 30°, 60°, and 90° of flexion, respectively. CONCLUSIONS: A PMCJ lesion was observed in 13% of ACL injuries. This injury contributes to increased ACL strain, and PMCJ repair markedly reduces ACL strain to preinjury levels.

Treatment of proximal hamstring avulsions is challenging regarding both timing and choice of repair. In chronic situations, the tissue can become attenuated, bringing into question the need for adjunctive allograft. In general, proximal hamstring avulsions are affixed back to the ischial tuberosity with suture anchors, with allograft used as needed for chronic situations. This article reports on a novel technique of suspensory fixation with a unicortical button but without allograft to repair both acute and chronic proximal hamstring avulsions. Three patients, all of whom regained full strength and functionality 1 year postoperatively, are described. [Orthopedics. 2018; 41(2):116-119.].

Separation of the posteromedial meniscocapsular junction (PMC) is a unique injury seen in patients with disruption of the anterior cruciate ligament. PMC tears may go unrecognized despite preoperative magnetic resonance imaging and diagnostic arthroscopy of the medial compartment. Unrepaired lesions may lead to persistent laxity of the knee after anterior cruciate ligament reconstruction. Inside-out repair techniques risk iatrogenic injury to the articular cartilage during needle passage and require dissection of the posteromedial knee for suture retrieval. Previous all-inside techniques have required specialized implants and repaired PMC lesions with direct visualization of the tear. The presented all-inside technique is an easily reproducible, cost-effective means to anatomically repair separation of the PMC. The technique provides the surgeon direct visualization and full arthroscopic access to the lesion, making repair technically easy and efficient.

Finding discolored bone intraoperatively can be confusing and concerning to orthopedic surgeons. Multiple causes of pigmented bone exist, including ochronosis, metabolic bone diseases, metal deposits, sequestrum, metastatic disease, and minocycline use. Bone quality is an important consideration in intraoperative decision making with respect to components and fixation options in total joint arthroplasty. Abnormal bone encountered in routine arthroplasty can raise concerns over the integrity and healing potential of the bone when the etiology is uncertain.Minocycline is a drug routinely used for the treatment of acne, rosacea, and rheumatoid arthritis. Pigmentation is a commonly recognized adverse reaction associated with most of the drugs in the tetracycline family, affecting the skin, nails, teeth, oral mucosa, bones in the oral cavity, ocular structures, cartilage, thyroid, and other visceral structures.This article describes a case of pigmented bone secondary to minocycline use in a 55-year-old woman undergoing total knee arthroplasty. This entity has rarely been documented in the orthopedic literature; however, orthopedic surgeons should be aware of this side effect secondary to the widespread use of minocycline. Questions concerning the effect of minocycline on bone metabolism and structural integrity have yet to be fully answered, but an understanding and recognition of the entity will help guide surgeons with intraoperative decision making.
