Organización del enfrentamiento a la COVID-19 en una institución hospitalaria. Provincia Cienfuegos, 2020-2021

Fundamento: las epidemias ponen a prueba la capacidad técnica y humana de los profesionales de la salud. Objetivo: describir la organización de los servicios de atención médica en el enfrentamiento a la COVID-19 en el Centro Especializado Ambulatorio Héroes de Paya Girón, en Cienfuegos. Métodos: se realizó una investigación en sistemas y servicios de salud que describe las etapas de reconversión hospitalaria para la atención de sospechosos y confirmados de COVID-19, el número de pacientes ingresados, la extensión a un centro escolar aledaño y el mantenimiento de otros servicios vitales, disponibilidad de recursos humanos y de camas. Se aplicó el método After Action Review (reflexión después de la acción) para el estudio de lecciones aprendidas. La información se obtuvo de las entrevistas a diez informantes clave y de los informes de los departamentos de estadística y de recursos humanos. Resultados: la organización de la atención médica estuvo relacionada con la evolución de la pandemia en la provincia. Se ingresaron 569 adultos y 91 niños sospechosos en la primera etapa. En la segunda se transformó la misión del centro hacia la atención de casos positivos, se incrementó el número de camas para cuidados intensivos, sobre todo a partir de finales del mes de junio, julio y agosto del 2021. Conclusiones: el centro demostró la capacidad de reordenarse para asumir la atención a enfermedades infecciosas emergentes con expresión epidémica. Las lecciones aprendidas mostraron la necesidad de incrementar la formación en Enfermería Intensiva, y de crear un Manual de Organización y Procedimientos en cada servicio.

ASTRACT Background: epidemics of unknown emerging diseases test not only the technical and human capacity of health professionals, but also of all those who make up the scientific community in society. Objective: to describe the organization of health care services in the fight against COVID-19 at the Héroes de Paya Girón Specialized Outpatient Center, in Cienfuegos. Methods a research was carried out on health systems and services that describe the stages of hospital reconversion for the care of suspected and confirmed patients, the number of admitted patients, the extension to a nearby school center and the maintenance of other vital services. Human resources and availability of beds for patient care were also taken into account. The After Action Review method was applied to study lessons learned. The information was obtained from interviews with ten key informants and from the statistics and human resources departments' reports. Results: the organization of medical care during the year 2020 was related to the evolution of the pandemic in the province. 569 adults and 91 pediatric patients were admitted. On January the 5th, 2021, the second stage of confronting the disease began and the mission of the center was transformed towards the care of positive cases of COVID-19, the number of intensive care beds was increased, a sign of the progressive worsening of patients especially from the end of June, July and August 2021. Conclusions: it can be affirmed that there is a center in the Cienfuegos Province with the capacity to rearrange itself in less than 24 hours to take care of patients with emerging infectious diseases. This shows the need to increase the training of Graduates in Intensive Nursing, as well as to create an Organization and Procedures Manual in each service, adjusted to the care protocols and asepsis, antisepsis and biosafety measures.

Primer Centro Especializado Ambulatorio en Cuba y sus resultados en tres años de trabajo / First Specialized Ambulatory Center in Cuba and its results in three years

Este trabajo tiene el propósito de exponer los resultados del trabajo del Centro Especializado Ambulatorio Héroes de Playa Girón adjunto al Hospital General Universitario "Dr. Gustavo Aldereguía Lima" tras tres años de su puesta en marcha. Estudio descriptivo para el cual se tuvieron en cuenta los principales indicadores del Hospital General desde el 2007 hasta el 2012. A partir de septiembre de 2009, apertura del Centro Especializado Ambulatorio, se incluyeron sus indicadores conjuntamente con los anteriores. Los datos se obtuvieron de los informes estadísticos anuales de ambos recintos hospitalarios. La apertura del Centro Especializado permitió un incremento en la cirugía mayor ambulatoria en el Hospital General; en el Centro Especializado, en el 2010, 2011 y 2012, se realizaron el 36,2; 43,9 y el 41,1 por ciento respectivamente del total de la cirugía mayor electiva ambulatoria realizada en el Hospital General. Las intervenciones realizadas con mayor frecuencia fueron: herniorrafías, hemorroidectomía, colecistectomía por cirugía mínimamente invasiva, extirpación de tumores de partes blandas, intervenciones ginecológicas del suelo pélvico, cirugías ortopédicas y entre otras. El Centro Especializado asumió el 20 por ciento del total de pacientes vistos en la consulta del Hospital General. En los tres años de trabajo se hospitalizaron 3 119 pacientes, la mayoría en el servicio de rehabilitación y la enfermedad más frecuente para rehabilitar fue la cerebrovascular. El Centro Especializado Ambulatorio, desde su apertura, contribuye a disminuir la carga asistencial del Hospital General, ofrece un servicio de alta calidad y juega un importante papel en el desarrollo de la cirugía ambulatoria en Cienfuegos(AU)

This paper was aimed at putting forward the work results of Heroes de Playa Giron specialized ambulatory center subordinated to ¿Dr Gustavo Aldereguia Lima¿ general university hospital after three years of operation. This descriptive study took into account the main indicators of the general hospital from 2007 through 2012. As from September 2009, the date of opening of the specialized ambulatory center, these indicators were jointly included with the above-mentioned. Data were collected from the yearly statistical reports of both hospital facilities. The opening of the Center allowed increasing the number of ambulatory major surgeries in the general hospital, since 36.2 percent, 43.9 percent and 41.1 percent of the total number of ambulatory elective major surgeries recorded in the hospital were performed in 2010, 2011 and 2012, respectively, in this specialized center. The most frequent were herniorraphies, hemorrhoidectomy, colecistectomy through minimally invasive technique, excision of soft tissue tumors, gynecological treatments of the pelvic floor, orthopedic surgeries and others. The Specialized Center gave service to 20 percent of the total number of patients seen at the outpatient service in the general hospital. In three years, 3 119 patients were admitted to the hospital, most of them in the rehabilitation service and the most treated disease was the cerebrovascular one. Since its opening, the Specialized Ambulatory Center has contributed to reduce the medical assistance burden of the general hospital, provides high quality service and plays a key role in the development of ambulatory surgery in Cienfuegos province(AU)

La superación profesional de los profesores de Medicina para el perfeccionamiento del proceso de enseñanza aprendizaje / Professional training of faculty medicals for improving the teaching-learning process

El perfeccionamiento del proceso de enseñanza aprendizaje de las ciencias médicas requiere de la superación profesional de los profesores. Este artículo tiene como objetivo reflexionar acerca del proceso de superación profesional de los profesores de la carrera de medicina en Cuba. Se describen las características de la superación profesional en el contexto de las ciencias médicas en el país (objetivos, contenidos, estrategias metodológicas, modalidades utilizadas, formas de superación); se hace referencia al caso particular de la superación profesional de los profesores de la carrera de medicina y se revela la ausencia de opciones con respecto a esta; se aborda la relación de esta problemática con el perfeccionamiento del proceso de enseñanza aprendizaje de una disciplina particular, como es el caso de la Farmacología. Finalmente, se ofrecen las condiciones que deben guiar el proceso de superación profesional orientado hacia ese perfeccionamiento(AU)

Improvement of the teaching-learning process of medical sciences requires the professional development of professors. This article aims to reflect on the professional development process of professors of medicine in Cuba. The characteristics of professional development in the context of medical sciences in the country (objectives, contents, methodological strategies, modalities, and forms of professional development) are described. The particular case of the professional development of professors of medicine is referred to and the lack of options is shown. The relation between this problem and the improvement of the teaching-learning process of a particular discipline, such as Pharmacology, is addressed. Finally, the conditions that should guide the process of professional development towards this improvement are provided(AU)

Comportamiento de la consulta para trabajadores en un centro especializado ambulatorio de Cienfuegos. Año 2011 / Behavior of the medical consultation for workers in a specialized ambulatory center or Cienfuegos city. 2011

Objetivo: Analizar el comportamiento de la consulta para trabajadores abierta en el centro especializado ambulatorio (CEA) Héroes de Playa Girón del hospital general universitario Dr. Gustavo Aldereguía Lima, de la ciudad de Cienfuegos en el año 2011. Método: En la consulta para trabajadores abierta en el CEA, se hizo un expediente clínico individual a cada trabajador para la realización del estudio; se confeccionó una encuesta, que se llenó con los datos del expediente clínico e interrogatorio de los trabajadores. Los datos obtenidos se procesaron con el programa SPSS, versión 12.0 para Windows. Los resultados se expresaron en tablas y gráficos para su mejor comprensión. Resultados: La edad media de los trabajadores del centro fue de 40,57 ± 12,71 años, predominando las mujeres. La categoría ocupacional más representada en la consulta fue la de obreros, con un 57,7 por ciento. Predominó también el grupo de enfermos con relación a la dispensarización. El 24,5 por ciento de los trabajadores eran fumadores; el riesgo ocupacional al que más expuestos estaban era el físico; el 16,8 por ciento eran hipertensos y a 71 trabajadores se les diagnosticaron enfermedades que desconocían padecer. Conclusiones: La consulta de trabajadores es una oportunidad para prevenir y disminuir los accidentes y enfermedades relacionadas con el trabajo; además, es una garantía para mejorar la calidad de vida y salud de los mismos, y de esta forma contribuir al incremento de la eficiencia económica(AU)

Objective: To analyze the behaviour of the medical consultation for workers open in the specialized ambulatory centre (SAC) Héroes de Playa Girón of the general university hospital Dr. Gustavo Aldereguía Lima of the city of Cienfuegos during 2011. Method: In the new consultation for workers of the SAC, an individual clinical record was made up for each worker, and these obtained data and those from a specific medical interrogation were processed with the program SPSS, version 12,0 for Windows. The results were expressed in tables and graphics for better understanding. Results: The half age of the workpeople studied in the centre was from 40,57 ± 12,71 years old, predominating the women. Occupational category most represented in the consultation was the relative to the workmen (57,7 percent). Sick person's group predominated; 24,5 percent of the workers were smokers, the occupational physical hazard were the more frequent; 16,80 percent were hypertensive; and 71 workers were ignorant of suffering diseases that were them diagnosed during the consultation. Conclusions: Worker's consultation is an opportunity to prevent and to decrease accidents and work-related diseases, besides a guaranty to upgrade life and health and contributing to the increment of economic efficiency(AU)
