OBJECTIVE: Treating complex posterior circulation aneurysms poses challenges, and extracranial to intracranial (EC-IC) bypass techniques are potential therapeutic options. However, the safety and efficacy of this approach for posterior circulation aneurysms remain unclear. The study's objective was to assess the safety and efficacy of EC-IC bypass in these aneurysms. METHODS: Adhering to Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines, we conducted a systematic review on EC-IC revascularization for posterior circulation aneurysms. Included studies had at least 4 patients and reported data on mortality, patency, complications, or clinical outcomes. Favorable clinical outcomes were defined as modified Rankin Scale below 3 or Glasgow Outcome Scale above 3, and complications were any issues related to the bypass procedure. RESULTS: From 3036 articles reviewed, 22 studies involving 196 patients who underwent 210 EC-IC bypass procedures for posterior circulation aneurysms were selected. The median follow-up period of 14 studies was 31.66 months (6-61 months). Final follow-up indicated a high bypass patency rate of 96% [95% confidence interval [CI]: 91%-100%; I2 = 12%], with a mortality rate of 5% [95% CI: 1%-9%; I2 = 0%]. Additionally, 83% [95% CI: 70%-96%; I2 = 77%] of patients showed good outcomes at the last follow-up. Complications were observed in 40% [95% CI: 26%-55%; I2 = 80%]. Heterogeneity was associated with specific studies. CONCLUSIONS: EC-IC bypass procedures are a viable treatment option for posterior circulation aneurysms, with high bypass patency rates and favorable clinical outcomes. However, complications, especially neurological deficits, exist. Open revascularization remains essential for neurovascular surgeons dealing with complex aneurysms.

Acute distal biceps injuries clinically present with sudden pain and acute loss of flexion and supination strength. The main injury mechanism occurs during the eccentric load of the biceps. The hook test is the most significant examination test, presenting the highest sensibility and specificity for this lesion. Magnetic resonance imaging, the gold standard imaging test, can provide information regarding integrity and identify partial and/or complete tears. The surgical treatment uses an anterior or double approach and several reattachment techniques. Although there is no clinical evidence to recommend one fixation method over the other, biomechanical studies show that the cortical button resists better to failure. Although surgical treatment led to an 89% rate of return to work in 14 weeks, the recovery of high sports performance occurred in 1 year, with unsustainable outcomes.

BACKGROUND: Ventriculoatrial shunt (VAS) is considered the second option for treating hydrocephalus, but there remains a lack of robust evidence regarding its complications profile. PURPOSE: Evaluate the complications associated with VAS. METHODS: Adhering to PRISMA guidelines, the authors searched Embase, PubMed, and Web of Science databases to identify articles reporting on the complications of VAS. Eligible articles were required to report whether complications occurred in a cohort of at least four patients. The extraction process focused on various outcome measures, including overall complications, pulmonary complications, cardiac complications, mortality, shunt revisions, glomerulonephritis, intracranial hemorrhage, and hygroma. Complications were defined as any adverse events related to the procedure, including revisions. RESULTS: After retrieving a total of 2828 articles, 53 studies met the predefined criteria, involving 2862 patients. The overall complication rate was estimated at 33 % (95 % CI: 25-42 %), while the long-term complications rate was found to be 49 % (95 % CI: 32-67 %). Among the pediatric population, the complication rate was calculated to be 53 % (95 % CI: 39-68 %). The overall revision rate was estimated at 32 % (95 % CI: 23-41 %), with an infection rate of 5 % (95 % CI: 3-7 %). Notably, in the pediatric group, the infection rate was 10 % (95 % CI: 6-13 %). The random analysis indicated an estimated risk of 0 % for glomerulonephritis, intracranial hemorrhage, hygroma, cardiac complications, pulmonary complications, and shunt-related mortality. CONCLUSION: VAS is a safe alternative when ventriculoperitoneal shunting is not feasible. Risk of classic severe complications is minimal. Nevertheless, caution is required when shunting critical patients. Further randomized studies are warranted to establish the ideal shunt for different patients.

Resumo As lesões agudas do tendão distal do bíceps se apresentam, clinicamente, com uma dor súbita associada a perda aguda de força de flexão e supinação. Seu principal mecanismo de lesão ocorre durante contração excêntrica do bíceps. O "Hook Test" é o principal teste semiológico, sendo o mais sensível e específico. A ressonância magnética, exame padrão ouro para o diagnóstico, pode fornecer informações sobre a integridade, identificando as lesões parciais e/ou completas. O tratamento cirúrgico pode ser realizado por duas vias principais: anterior e por dupla via porém as técnicas de reinserção tendínea são diversas não havendo evidência clínica que recomende um método de fixação em detrimento ao outro; embora o botão cortical apresente maior resistência a falha nos estudos biomecânicos. Com o tratamento cirúrgico o retorno as atividades laborais foi de 89% em 14 semanas (média) porém ao esporte de alto rendimento o prazo foi longo, média de 1 ano, e não duradouro.

Um sistema de saúde baseado em valor deverá entregar os melhores desfechos possíveis ao paciente, desfechos que realmente importam para ele, ao menor custo possível. O conceito é relativamente fácil de entender, mas a métrica para a sua demonstração tem sido um grande desafio dos sistemas de saúde. O presente artigo demostra a criação de um Escore de Valor em Saúde (EVS) utilizando Análise de Decisão por Multicritério, o qual pode ser aplicado em qualquer sistema de saúde para avaliar profissionais, serviços de saúde e linhas de cuidado de pacientes com condições clínicas específicas.


A value-based healthcare system must deliver the best possible patient outcomes, outcomes that really matter to the patient, at the lowest possible cost. The concept is relatively easy to understand, but the metric to demonstrate it has been a major challenge for healthcare systems. This article demonstrates the creation of a Health-Based Health Care Score (EVS) using Multi-Criteria Decision Analysis which can be applied in any health system to evaluate professionals, healthcare providers and care pathways for patients with a specific disease.

RESUMO

The treatment options for posterior instability associated with epilepsy includes grafts, osteotomies, arthrodesis and arthroplasty. The technique of reverse arthroscopic remplissage was described in 2006 as a method of filling the anterior humeral bone defect, associated with tenodesis of the subscapularis tendon. This case report presents the results of the reverse remplissage technique in relation to a patient who suffered a bilateral posterior glenohumeral dislocation with a reverse Hill-Sachs lesion.

RESUMO

Matching methods encompass non-parametric approaches to estimating counterfactual states through a rigorous selection of control units with similar characteristics to units submitted to an intervention. These methods enable comparisons between treated and control units in a way that facilitates understanding of causal relationships between interventions and outcomes. Matching methods have been used only recently in ecology and conservation biology, where such applications changed the way the field investigates causal questions, for example, in impact-evaluation studies. However, the strengths and limitations of matching methods are not well understood by most ecologists and environmental scientists. Herein, we review state-of-the-art matching methods aiming to help fill this gap in understanding. First, we present relevant theoretical concepts related to matching methods and related subjects such as counterfactual states and causation. Next, we propose guidelines and strategies for the application of matching methods in ecology and conservation biology. Finally, we discuss the possibilities for future applications of matching methods in the environmental sciences.

BACKGROUND: This study retrospectively evaluated the medium- and long-term results of patients submitted to double-bundle (DB) anterior cruciate ligament (ACL) reconstruction. METHODS: A retrospective study of case series at a single center. Cases submitted to isolated ACL reconstruction with at least five years of follow-up were included. The following data were collected: demographic data; practice of competitive sport before the injury; previous surgery; injury/surgery in the contralateral knee; return to the practices of sports and level; re-injury (postoperative time; mechanism; need for surgery); and symptoms at the last clinical follow-up visit. Descriptive and sub-group analyses were performed. RESULTS: Sixty-nine patients were included; 52 men (75%), 49 athletes (71%), 47 (68%) with primary injury, mean age of 30 years (SD 10). The patients were followed up for an average of 8.7 years (minimum 5, maximum 11.8) after surgery. After the reconstruction, 67 (97%) returned to the sport; 75% at the same level as before the injury. Ten patients (14%) suffered re-injury after an average of 32 months (between 9 and 50 months). Regarding the outcome of re-injury, no statistically significant differences were found between subgroups of athletes vs non-athletes or primary injury vs revision surgery, despite a significant tendency towards increased re-injury levels in athletes. However, this tendency was not statistically significant. CONCLUSION: In our series of patients operated on with the double-bundle technique and with a long follow-up time, 14% presented re-injury, with no differences between primary and revision cases, and with a trend towards higher re-injury levels among the athletes in relation to the non-athletes. The rate of return to sport was satisfactory, with 97%, of which 75% were playing at the same level as before the injury.

Objective Arthroscopic Latarjet has been performed with the aim to be an accurate technique with a low incidence of complications. The aim of the present study was to briefly describe the technique and to evaluate the shot-term complications following arthroscopic Latarjet procedure to correct anterior shoulder dislocation with glenoid bone loss. Methods Retrospective study with 30 subjects with anterior shoulder instability, submitted to arthroscopic Latarjet. Intraoperative and short-term postoperative complications were recorded, as well as the rate of revision surgery. Results Five cases had complication (16.7%), and in the last 10 cases no complication occurred. In 1 case (3.3%), it was required to reverse for open surgery due to a fracture of the coracoid process during fixation in the glenoid. No other intraoperative complication occurred. No infection was observed. Two cases (6.7%) evolved with temporary neuropraxia of the musculocutaneous nerve, totally reversed with physiotherapy. With a follow-up from 6 to 26 months, 2 patients (6.7%) required a new intervention for graft/screws removal and release of the joint due to excessive limitation in external rotation. There was no case of recurrence. Conclusion Even in an initial learning curve, arthroscopic Latarjet demonstrated a low rate of short-tem complications and was a safe procedure for treating anterior dislocation of the shoulder with glenoid bone loss.

Resumo Objetivo A realização da cirurgia de Latarjet por via artroscópica tem sido possível pelo desenvolvimento de instrumentais adequados e um protocolo para cirurgia estabelecido e reprodutível, que promete uma técnica mais precisa e com menor incidência de complicações. O objetivo do presente estudo foi fazer uma breve descrição de técnica cirúrgica e avaliar as complicações de curto prazo após a cirurgia de Latarjet por via artroscópica para correção de luxação anterior do ombro com perda óssea glenoidal. Método Estudo retrospectivo com 30 pacientes com instabilidade anterior do ombro, operados com a técnica de Latarjet por via artroscópica. Foram feitas avaliação das complicações intraoperatórias e pós-operatório de curto prazo e documentação de necessidade de reintervenções. Resultados Cinco casos tiveram complicação (16,7%), sendo que nos últimos 10 casos nenhuma complicação foi observada. Em 1 caso (3,3%) foi necessária conversão para cirurgia aberta por fratura do processo coracoide no momento de fixação na glenoide. Nenhum outro caso apresentou complicação intraoperatória. Não houve infecção nos casos operados. Dois casos (6,7%) evoluíram com neuropraxia temporária do musculocutâneo, revertida com fisioterapia. Em tempo de seguimento de 6 a 26 meses, 2 pacientes (6,7%) necessitaram de nova intervenção para retirada do material de síntese e liberação articular por excessiva limitação da rotação externa. Não houve caso de recidiva. Conclusão O procedimento de Latarjet artroscópico se mostrou seguro e com baixa incidência de complicações de curto prazo para correção da luxação anterior no ombro com perda óssea na glenoide mesmo em curva inicial de aprendizagem.

RESUMO

Human cases of dengue virus based on the National Dengue Control Plan were compared with the molecular detection of the dengue virus in trapped mosquitoes, verifying the prediction and efficacy potentials of vector control between the two methodologies in a city with three endemic frontiers. Molecular detection of dengue virus in trapped mosquitoes was significantly higher than in human cases (p = 0.0435). Thus, molecular detection could be used as an early indicator to help prevent more human cases of dengue.

ABSTRACT Latin America embodies countries of special interest for ecological studies, given that areas with great value for biodiversity are located within their territories. This highlights the importance of an evaluation of ecological research in the Latin America region. We assessed the scientific participation of Latin American researchers in ecological journals, patterns of international collaboration, and defined the main characteristics of the articles. Although Latin American publications have increased in fourteen years, they accounted up to 9% of publications in Ecology. Brazil leaded the scientific production in Latin America, followed by Argentina and Mexico. In general, Latin American articles represented a low percentage of most journals total publication, with particularly low expression in high impact-factor journals. A half of the Latin American publications had international collaboration. Articles with more than five authors and with international collaboration were the most cited. Descriptive studies, mainly based in old theories, are still majority, suggesting that Ecology is in a developing stage in Latin America.

Objetivo: Verificar a correlação existente entre alterações da PCR e VHS com a atividade do Lúpus Eritematoso Sistêmico, medida pelo SLEDAI. Método: Estudo descritivo e transversal, utilizando-se 150 pacientes do ambulatório de reumatologia do Hospital Universitário Evangélico de Curitiba com diagnóstico de LES. Foi aplicado o questionário SLEDAI e coletados os valores de PCR e VHS, além de dados demográficos, principais manifestações clínicas já apresentadas, perfil sorológico e uso de medicamentos. Resultados: A média de idade foi de 39,6±41,3 anos e o tempo médio de doença foi de 8,29± 6,32 anos. A pontuação no SLEDAI ficou entre 0 e 44 pontos, Valores de VHS variaram de 0 a 130mm, com mediana de 24,00mm. Já os valores de PCR encontrados ficaram na faixa de 0 a 108 mg/dL (mediana de 1,62 mg/dL). Foi encontrada uma correlação positiva entre SLEDAI e PCR (p=0.001), SLEDAI e VHS (p= 0,0001). Conclusão: Existe uma correlação positiva entre SLEDAI, PCR e VHS. Porém em relação nenhuma dessas duas medidas de inflamação (VHS e PCR) demonstraram predileção por aumento em determinada forma de expressão clínica da atividade do lúpus


Objective: To investigate the correlation between changes in CRP and ESR with the activity of SLE, as measured by SLEDAI. Method: Descriptive and transversal, using 150 outpatients from the rheumatology at the Evangelic University Hospital of Curitiba diagnosed with SLE. SLEDAI questionnaire was applied and collected the values of CRP, ESR, demographic data, clinical manifestations presented, serologic profile and medication use. Results: The mean age was 39,6±41,3 and the mean disease duration was of 8,29± 6.32 years. The SLEDAI score was between 0 and 44 points, ESR values ranging from 0 to 130mm, with a median of 24.00 mm. The values of CRP were found in the range 0 to 108 mg / dL (median of 1.62 mg / dL). We found a positive correlation between SLEDAI and CRP (p=0.001), SLEDAI and ESR (p=0.0001). Conclusion: There is a positive correlation between SLEDAI, C-reactive protein and erythrocyte sedimentation rate. But these two measures of inflammation (VHS and the PCR) did not show preferencial increase in some forms of clinical manifestation of lupus

Objetivo: Analisar as solicitações de transfusões sanguíneas encaminhadas à central transfusional do Hospital Universitário Evangélico de Curitiba, buscando identificar o tipo e quantidade de hemocomponentes transfundidos nesse período, bem como o perfil dos pacientes receptores e as razões pelas quais eles foram submetidos a essa terapia. Material e Métodos: Foram analisadas todas as fichas de requerimento de transfusão encaminhadas à central transfusional do hospital no período de agosto a dezembro de 2013, sendo incluídas no estudo apenas aquelas que resultaram em transfusão efetiva. Foram obtidas informações referentes às características dos pacientes, aos hemocomponentes transfundidos e às indicações relativas a cada um deles. Resultados: A idade média foi de 50 anos, com predominância nos homens (58,7%). De forma geral, foi observado que a maioria dos procedimentos foi realizada na enfermaria (47,7%). O hemocomponente mais transfundido foi o concentrado de hemácias (74,1%). A principal indicação para a transfusão de concentrado de hemácias foi por anemia em paciente com doença cardíaca, pulmonar ou cerebrovascular (51,9%). O tipo sanguíneo predominante na nossa pesquisa foi o O positivo (43%). Conclusão: O fracionamento do sangue total em hemocomponentes levou a um maior benefício clínico para os pacientes que recebem as transfusões, acarretando em um maior número de pacientes atendidos pela terapêutica transfusional e menor desperdício desse material nobre. O hemocomponente mais transfundido foi o concentrado de hemácias, não sendo realizado nenhum procedimento utilizando-se de sangue total. Dentre os procedimentos, apenas 16,4% foram indicados devido a procedimento cirúrgico.


Objective: To analyze the requests for blood transfusions that were sent to the transfusion center of Hospital Universitário Evangélico de Curitiba, seeking to identify the type and amount of blood products transfused during this period as well as the profile of the recipient patients and the reasons why they were subjected to this therapy. Material and Methods: All records of transfusion requests sent to the hospital transfusion center from August 2013 to December of that same year were analyzed, but only those that resulted in effective transfusion were included in the study. Information regarding the characteristics of patients, transfused blood products and indications for each of them were obtained. Results: The mean age was 50 years, predominantly in men (58.7%). In general, we observe that most of the procedures was performed on the ward (47.7%). The most transfused blood component was the red blood cells (74.1%). The main indication for transfusion of red blood cells is anemia in patients with heart, lung or cerebrovascular disease (51.9%). The predominant blood type in our survey was the O positive (43%). Conclusion: The fractionation of whole blood into blood components led to a greater clinical benefit for patients receiving transfusions , resulting in a greater number of patients treated by transfusion therapy and less waste of this noble material. The most transfused blood component was the red blood cells , not being carried out no procedure using whole blood . Among the procedures , only 16.4 % were indicated due to a surgical procedure.

Descreve-se um caso de lúpus eritematoso sistêmico (LES) complicado com vasculite de SNC. Na investigação desta última encontraram-se aneurismas de carótida comum e interna bilateralmente - os quais foram tratados cirurgicamente após imunossupressão.


A case of systemic lupus erythematosus (SLE) complicated by CNS vasculitis is described. In the investigation of the latter we found common and internal carotid aneurysms bilaterally - which were treated surgically after immunosuppression.

Objetivo: Relatar, informar e atualizar a comunidade médica do diagnóstico e das opções terapêuticas da FN, bem como, alertar sobre sua rápida progressão e alta morbimortalidade. Relato do Caso: Paciente feminina, 41 anos, com diversas comorbidades, evoluiu quadro de 10 dias de evolução de FN de flanco abdominal. Foi realizada hidratação vigorosa, antibioticoterapia empírica e debridamento cirúrgico agressivo. Conclusão: A FN está associada a altos índices de morbimortalidade. Para seu manejo é essencial um diagnóstico precoce, medidas de reanimação, com hidratação vigorosa, antibioticoterapia empírica e um debridamento cirúrgico agressivo.


Objective: Report, inform and update the medical community of diagnostic and therapeutic options of FN, as well as warn of its rapid progression and high mortality. Case report: Female, 41, with several comorbidities, with 10 days of evolution of NF in abdominal side. Vigorous hydration, empirical antibiotic therapy and aggressive surgical debridement was performed. Conclusion: NF is associated with high rates of morbidity and mortality. For their management is essential early diagnosis, resuscitation measures, vigorous hydration, empiric antibiotic therapy and aggressive surgical debridement.

Objetivo: Verificar se o nível de IgA sérica reflete a atividade inflamatória de pacientes com EAP medido pelo VHS, proteína C reativa e BASDAI ( Bath Ankylosing Spondylitis Disease activity índex) ou com o dano cumulativo da mesma medido pelo BASFI ( Bath Ankylosing Spondylitis Functional Index). Metodologia: Catorze pacientes com EAP foram entrevistados para medida do BASDAI e BASFI e foram submetidos a dosagem de proteína C reativa, VHS e IgA sérica. Resultados: Não se observou correlação entre níveis de Ig A sérica e VHS (p= 0,98), BASDAI (p=0,41) e BASFI (p=0,24 ) uma tendência para associação com proteína C reativa foi observada (p=0,06). Conclusões: Os níveis de Ig A sérica em pacientes com EAP não refletem atividade da doença medida pelo VHS ou pelo BASDAI. UMA tendência é observada com relação a proteína C reativa.


Objective: To verify if the serum IgA level reflects the inflammatory activity of patients with PAD measured by HSV, C-reactive Protein and BASDAI (BathAnkylosing Spondylitis Disease activity index) or with cumulative damage measured by BASFI (Bath Ankylosing Spondylitis Functional Index). Methods: Fourteen patients with PAD were interviewed for BASDAI and BASFI measurements and were submitted to C-reactive protein, VHS and serum IgA. Results: There was no correlation between serum IgA levels and HSV (p = 0.98), BASDAI (p = 0.41) and BASFI (p = 0.24) a trend towards association with C-reactive protein was observed P = 0.06). Conclusions: Serum IgA levels in patients with PAD do not reflect disease activity measured by HSV or BASDAI. A trend is observed with respect to C-reactive protein.

Introduction:Down Syndrome (DS) patients have increased susceptibility to the development of periodontal diseases by the occurrence of several factors, such as inadequate hygiene, mouth breathing, dental morphology, leukocyte reduction and increased inflammatory mediators. Objective:This study aimed to review the literature on the main aspects of DS and present a clinical case of a DS patient treated with basic periodontal therapy and free gingival graft surgery. Case report: DS patient, leucoderma, aged 26 years showed gingival recession and little amount of keratinized tissue in the area of teeth #31 and #41. After surgery, there was an increase in the keratinized tissue band. Conclusion: The free gingival graft surgery performed in DS patients was effective, as the increase of keratinized tissue band occurred.

É apresentado relato de caso de uma pré-escolar grande queimada encaminhada a um serviço hospitalar de referência, a qual evoluiu com óbito devido à sepse subsequente à infecção de cateter, a despeito da utilização de terapia antimicrobiana guiada por antibiograma. O objetivo desse estudo é alertar a equipe pediátrica quanto às particularidades da criança grande queimada, principalmente quando em vigência de infecção, e levantar discussões quanto ao uso precoce, mas não profilático, de antibióticos.


It is presented a case report of a major burn in a preschool referred to a hospital service reference, which evolved to death due to subsequent sepsis infection catheter, despite the use of antimicrobial therapy guided by antibiogram. The aim of this study is to alert the pediatric team and the particularities of major burned kid, especially when in presence of infection, and raise discussions on the early use, but not prophylactic, antibiotics.
