OBJECTIVE: To develop the second evidence-based Brazilian Society of Rheumatology consensus for diagnosis and treatment of lupus nephritis (LN). METHODS: Two methodologists and 20 rheumatologists from Lupus Comittee of Brazilian Society of Rheumatology participate in the development of this guideline. Fourteen PICO questions were defined and a systematic review was performed. Eligible randomized controlled trials were analyzed regarding complete renal remission, partial renal remission, serum creatinine, proteinuria, serum creatinine doubling, progression to end-stage renal disease, renal relapse, and severe adverse events (infections and mortality). The Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach was used to develop these recommendations. Recommendations required ≥82% of agreement among the voting members and were classified as strongly in favor, weakly in favor, conditional, weakly against or strongly against a particular intervention. Other aspects of LN management (diagnosis, general principles of treatment, treatment of comorbidities and refractory cases) were evaluated through literature review and expert opinion. RESULTS: All SLE patients should undergo creatinine and urinalysis tests to assess renal involvement. Kidney biopsy is considered the gold standard for diagnosing LN but, if it is not available or there is a contraindication to the procedure, therapeutic decisions should be based on clinical and laboratory parameters. Fourteen recommendations were developed. Target Renal response (TRR) was defined as improvement or maintenance of renal function (±10% at baseline of treatment) combined with a decrease in 24-h proteinuria or 24-h UPCR of 25% at 3 months, a decrease of 50% at 6 months, and proteinuria < 0.8 g/24 h at 12 months. Hydroxychloroquine should be prescribed to all SLE patients, except in cases of contraindication. Glucocorticoids should be used at the lowest dose and for the minimal necessary period. In class III or IV (±V), mycophenolate (MMF), cyclophosphamide, MMF plus tacrolimus (TAC), MMF plus belimumab or TAC can be used as induction therapy. For maintenance therapy, MMF or azathioprine (AZA) are the first choice and TAC or cyclosporin or leflunomide can be used in patients who cannot use MMF or AZA. Rituximab can be prescribed in cases of refractory disease. In cases of failure in achieving TRR, it is important to assess adherence, immunosuppressant dosage, adjuvant therapy, comorbidities, and consider biopsy/rebiopsy. CONCLUSION: This consensus provides evidence-based data to guide LN diagnosis and treatment, supporting the development of public and supplementary health policies in Brazil.

BACKGROUND: Patients using immunosuppressive drugs may have unfavorable results after infections. However, there is a lack of information regarding COVID-19 in these patients, especially in patients with rheumatoid arthritis (RA). Therefore, the aim of this study was to evaluate the risk factors associated with COVID-19 hospitalizations in patients with RA. METHODS: This multicenter, prospective cohort study is within the ReumaCoV Brazil registry and included 489 patients with RA. In this context, 269 patients who tested positive for COVID-19 were compared to 220 patients who tested negative for COVID-19 (control group). All patient data were collected from the Research Electronic Data Capture database. RESULTS: The participants were predominantly female (90.6%) with a mean age of 53 ± 12 years. Of the patients with COVID-19, 54 (20.1%) required hospitalization. After multiple adjustments, the final regression model showed that heart disease (OR = 4.61, 95% CI 1.06-20.02. P < 0.001) and current use of glucocorticoids (OR = 20.66, 95% CI 3.09-138. P < 0.002) were the risk factors associated with hospitalization. In addition, anosmia was associated with a lower chance of hospitalization (OR = 0.26; 95% CI 0.10-0.67, P < 0.005). CONCLUSION: Our results demonstrated that heart disease and the use of glucocorticoids were associated with a higher number of hospital admissions for COVID-19 in patients with RA. OBJECTIVES: To evaluate risk factors associated with unfavourable outcomes: emergency care, hospitalisation, admission to intensive care unit (ICU), mechanical ventilation and death in patients with immune-mediated rheumatic disease (IMRD) and COVID-19. METHODS: Analysis of the first 8 weeks of observational multicentre prospective cohort study (ReumaCoV Brasil register). Patients with IMRD and COVID-19 according to the Ministry of Health criteria were classified as eligible for the study. RESULTS: 334 participants were enrolled, a majority of them women, with a median age of 45 years; systemic lupus erythematosus (32.9%) was the most frequent IMRD. Emergency care was required in 160 patients, 33.0% were hospitalised, 15.0% were admitted to the ICU and 10.5% underwent mechanical ventilation; 28 patients (8.4%) died. In the multivariate adjustment model for emergency care, diabetes (prevalence ratio, PR 1.38; 95% CI 1.11 to 1.73; p=0.004), kidney disease (PR 1.36; 95% CI 1.05 to 1.77; p=0.020), oral glucocorticoids (GC) (PR 1.49; 95% CI 1.21 to 1.85; p<0.001) and pulse therapy with methylprednisolone (PR 1.38; 95% CI 1.14 to 1.67; p=0.001) remained significant; for hospitalisation, age >50 years (PR 1.89; 95% CI 1.26 to 2.85; p=0.002), no use of tumour necrosis factor inhibitor (TNFi) (PR 2.51;95% CI 1.16 to 5.45; p=0.004) and methylprednisolone pulse therapy (PR 2.50; 95% CI 1.59 to 3.92; p<0.001); for ICU admission, oral GC (PR 2.24; 95% CI 1.36 to 3.71; p<0.001) and pulse therapy with methylprednisolone (PR 1.65; 95% CI 1.00 to 2.68; p<0.043); the two variables associated with death were pulse therapy with methylprednisolone or cyclophosphamide (PR 2.86; 95% CI 1.59 to 5.14; p<0.018). CONCLUSIONS: Age >50 years and immunosuppression with GC and cyclophosphamide were associated with unfavourable outcomes of COVID-19. Treatment with TNFi may have been protective, perhaps leading to the COVID-19 inflammatory process.

Objetivo Elaborar recomendações para o diagnóstico, manejo e tratamento da nefrite lúpica no Brasil. Método Revisão extensa da literatura com seleção dos artigos com base na força de evidência científica e opinião dos membros da Comissão de Lúpus Eritematoso Sistêmico da Sociedade Brasileira de Reumatologia. Resultados e conclusões 1) A biópsia renal deve ser feita sempre que possível e houver indicação e quando não for possível, o tratamento deve ser orientado com base na inferência da clase histológica. 2) Devem ser implementados medidas e cuidados idealmente antes do início do tratamento, com ênfase na atenção ao risco de infecção. 3) Devem-se compartilhar riscos e benefícios do tratamento com pacientes e familiares. 4) O uso da hidroxicloroquina (preferencialmente) ou difosfato de cloroquina é recomendado para todos os pacientes (exceto contraindicação) durante as fases de indução e manutenção. 5) A avaliação da eficácia do tratamento deve ser feita com critérios objetivos de resposta (remissão completa/remissão parcial/refratariedade). 6) Os IECA e/ou BRA são recomendados como antiproteinúricos para todos os pacientes (exceto contraindicação). 7) A identificação de sinais clínicos e/ou laboratoriais sugestivos de GN laboratoriais sugestivos de glomerulonefrite proliferativa ou membranosa deve indicar início imediato de terapia específica incluindo corticosteroides e agente imunossupressor, mesmo que não seja possível comprovação histológica. 8) O tempo de uso dos imunossupressores deve ser no mínimo de 36 meses, mas eles podem ser mantidos por períodos mais longos. A sua suspensão só deve ser feita quando o paciente atingir e mantiver remissão completa sustentada. 9) Deve-se considerar nefrite lúpica refratária quando a remissão completa ou parcial não for alcançada após 12 meses de tratamento adequado, quando uma nova biópsia renal deve ser considerada para auxiliar na identificação da causa da refratariedade e decisão terapêutica. .

(7) The identification of clinical and/or laboratory signs suggestive of proliferative or membranous glomerulonephritis should indicate an immediate implementation of specific therapy, including corticosteroids and an immunosuppressive agent, even though histological confirmation is not possible. (8) Immunosuppressives must be used during at least 36 months, but these medications can be kept for longer periods. Its discontinuation should only be done when the patient could achieve and maintain a sustained and complete remission. OBJECTIVE: Data about the survival rate of lupus patients on dialysis is controversial. The objective of this controlled prospective study was to report the survival of SLE patients undergoing dialysis and to analyse factors associated with mortality. METHODS: Twenty dialysis centres were visited between May 2003 and February 2004. Disease activity was evaluated using the SLEDAI without the renal-related parameters [non-renal (nr)SLEDAI] data collection was performed at entrance to the study. The control group consisted of haemodialysis patients without SLE matched for gender, age, dialysis modality, time on dialysis and dialysis-associated HCV infection. Patients were prospectively followed for 60 months. The Kaplan-Meier method was used to estimate survival. Associations of risk factors with mortality were tested using the Cox proportional hazards model. RESULTS: Each group was composed of 57 patients. In the multivariate analysis, only nrSLEDAI > 8 [hazard ratio (HR) 6.368; 95% CI 1.798, 22.548; P = 0.004], and urea reduction ratio (HR 0.953; 95% CI 0.917, 0.990; P = 0.014) were independently associated with mortality in lupus patients. The 5-year survival rate of the control group and the one from SLE patients with nrSLEDAI ≤ 8 (n = 51) were similar (83% and 73%, respectively) but significantly better than the one for SLE patients with nrSLEDAI > 8 (n = 6, 17%), P < 0.001. CONCLUSION: A high nrSLEDAI was strongly associated with 5-year mortality in lupus patients on dialysis.

A sobrevida dos pacientes com lúpus eritematoso sistêmico (LES) em diálise ainda é assunto controverso. O presente estudo controlado e prospectivo tem como objetivo estudar a sobrevida dos pacientes com LES em diálise e os fatores relacionados com a mortalidade. Vinte centros de diálise foram visitados entre maio de 2003 a fevereiro de 2004. A atividade da doença foi avaliada através do índice SLEDAI sem os parâmetros renais (SLEDAInr). Os dados foram coletados na entrada do estudo. O grupo controle foi composto por pacientes sem LES em diálise pareados por gênero, idade, tipo de diálise e infecção por vírus C da hepatite (VCH). Todos os pacientes foram acompanhados prospectivamente durante 60 meses. O método de Kaplan-Meier foi usado na estimativa de sobrevida e a associação de fatores de risco com mortalidade foi testada com o modelo de proporção de risco de Cox. Cada grupo foi composto por 57 pacientes. Na análise multivariada, apenas o SLEDAInr > 8 (HR 6368, 95% C1.798-22.548, P=0,004) e o índice de redução de ureia, IRU, (HR 0,953, 95%C1 0,917-0,990, P=0,014) foram fatores independentes associados com mortalidade nos pacientes com LES. A taxa de sobrevida em 5 anos do grupo controle e dos pacientes com LES e SLEDAInr < ou = a 8 foram semelhantes (83% e 73%, repectivamente), mas significativamente superior à dos pacietnes com LES e SLEDAInr > 8 (17%), P ,0,00. O IRU se mostrou um fator protetor contra a mortalidade. No entanto, a presença de um SLEDAInr > 8 conferiu um risco extrememamente elevado de mortalidade em cinco anos na população de pacientes com LES em diálise.

Objetivo: Determinar a frequência e causas de internaçao de pacientes com lúpus eritematoso sistêmico (LES). Métodos: Estudo retrospectivo de 277 casos confirmados de LES, do sexo feminino, diagnosticados no período 1952-1992 no Hospital dos Servidores do Estado - Rio de Janeiro. Resultados: Dos 277 casos de LES, 235 já haviam sido internados. Em 157 destes, as causas da internaçao foram determinadas: 105 por ativaçao da doença; 29 por associaçao de atividade e infecçao intercorrente; dez por quadro infeccioso com doença inativa; e 13 com diversas causas. Os achados laboratoriais dos 235 pacientes, comparados com os de 26 outros jamais internados, evidenciaram reduçao de C3 e da albumina sérica no primeiro grupo. Conclusao: A frequência de internaçao hospitalar é elevada nos pacientes com LES e sua principal causa é a atividade da doença, por vezes associada a infecçao intercorrente
