RESUMO

OBJECTIVE: High-grade lymphoepithelioma-like urothelial carcinoma is a rare neoplasia in the bladder, and its relationship with Epstein-Barr virus has not been demonstrated. METHODS: We report the case of an old male presenting with recurrent hematuria diagnosed of this tumor. RESULTS: These are neoplasias with a great lymphocyte component, sometimes predominant, which may lead to histological confusion with inflammatory processes like chronic cystitis or even lymphomas. CONCLUSIONS: Despite their infiltrative character and high cytological and architectural grade their prognosis is better than urothelial carcinomas of the same grade and stage, but this only applies for pure forms and those with predominant lymphoepithelioma-like pattern.

RESUMO

Objetivo: El carcinoma urotelial de alto grado con patrón de linfoepitelioma es una neoplasia infrecuente a nivel vesical en la que no se ha logrado demostrar relación con la infección por el virus de Epstein-Barr. Métodos: Presentamos el caso de un paciente de edad avanzada que tras presentar episodios de hematuria recurrente fue diagnosticado de esta singular entidad. Resultados: Se trata de neoplasias con gran componente linfocitario, en ocasiones mayoritario, que pueden prestarse a confusión histológica con procesos inflamatorios como cistitis crónicas e incluso con linfomas. Conclusiones: Pese a su carácter infiltrante y a su alto grado citológico y arquitectural su pronóstico resulta ser mejor que el de los carcinomas uroteliales del mismo grado y estadio si bien ello sólo es aplicable a las formas puras y aquellas en las que el patrón de linfoepitelioma es predominante (AU)

RESUMO

An unusual case of synchronous gastric carcinomas occurred in a 28-year-old man with a family history of gastric disease. Two tumor foci were identified: a well-differentiated advanced carcinoma with the phenotypic properties of complete intestinal metaplasia and an early intestinal-type carcinoma. Histochemical and immunohistochemical stains to demonstrate complete intestinal metaplasia, ie, Alcian blue pH 2.5/periodic acid-Schiff, high iron diamine/Alcian blue pH 2.5, CD10, and MUC2, were all positive in the advanced adenocarcinoma. Of all markers used, only high iron diamine/Alcian blue pH 2.5 and Alcian blue pH 0.5 were positive in the early carcinoma. In these cases, mistakes frequently are made during examination of endoscopic biopsies. Fortunately, the advanced adenocarcinoma was low grade (the patient has shown no signs of disease at 6 years postsurgery). Histopathologic, histochemical, and immunohistochemical findings suggest that an extensive substrate of complete intestinal metaplasia (corpus) and of complete and incomplete intestinal metaplasia (antrum) can be associated with two independent tumors with different phenotypes.

RESUMO

We report 4 new cases of breast carcinoma in teenage girls diagnosed by use of histochemical and immunohistochemical methods. These cases of breast carcinoma in women under 20 years of age were found in the files of our department in the last 24 years (1976-2000). The patients had operable breast carcinomas corresponding to various histologic types (1 invasive ductal carcinoma associated with fibroadenoma, 2 secretory carcinomas, and 1 invasive lobular-type carcinoma). Simple mastectomy with low axillary lymph node dissection was performed in 2 postpubertal patients. Only 1 patient received adjuvant chemotherapy (case 4). After follow-up ranging from 76 to 126 months, 3 patients are alive and disease-free and 1 has disseminated metastatic disease. The correlations with prognosis-related risk factors (stage, age, previous benign lesions, family history, histologic types, hormonal receptors, and pregnancy) were examined.
